Use of methylprednisolone in scoliosis surgery with high neurological risk

Abstract:Objetive:To evaluate the clinical benefits and potential adverse effect of the use of methylprednisolone during scoliosis surgery with high neurological risk.Method:Retrospective study were carried out on 86 cases with spinal deformity treated from July 2001 to May 2005.Potential high neurological complications were estimated in these patients due to severe spinal deformity including severe scoliosis,severe kyphosis,combined spinal cord malformation,rotational dislocation or high risk maneuver including thoracic pedicle screw placement,360 degree spine osteotomy,posterior hemivertebraectomy and so on.Patients were divided into Group A(with neurologically intact) and Group B(with neurological compromise) according to the presence or absent of preoperative neurological deficit.Intraoperative and posterative MP were administrated according to authors′ defined protocol.Neurological function evaluation were performed preoperatively,immediate postoperatively and 6 months postoperatively according to international standards for neurological classification of spinal cord injury.Result:Neurological complication were observed in 3 cases in Group A which resolved within 6 months postoperatively after MP treatment.No significant improvement of neurological function were found in Group B after using MP.No adverse effect due to the use of MP were recorded in this series.Conclusion:Methylprednisolone is effective for the recovery of neurological deficit due to acute spinal cord injury during scoliosis surgery with high neurological risk.
