Abstract:In order to provide a new way and method for safety evaluation of Chinese materia medica (CMM) and also to provide a reference for conventional animal experiments, computer toxicity prediction technique and method were established to predict the cardiotoxicity of CMM. Mold2 software (version 2.0.0) was used to calculate molecular descriptors of 1034 chemical components. Then, the random forest (RF) method and the support vector machine (SVM) method were used to screen the descriptors. After that, boosting trees method, SVM, regularized discriminant analysis method and RF method were used to build up prediction model, respectively. Finally, the cardiotoxicity of chemical components was predicted by the quantitative structure-activity relationship (QSAR) model with the best accuracy and Kappa value. The results showed that by comparing the accuracy and Kappa value of prediction model, it was found that the RF model was the optimal algorithm model with 86.3%accuracy and the Kappa value of 0. 725. Through the prediction research on chemical components of Chinese herbs with toxicity recorded in the Pharmacopoeia of People’s Republic of China (version2010),suchasEvodia rutaecarpa,North bean root,Murraya incense,some meaningful results had been received. It was concluded that QSAR model on prediction research of chemical components of Chinese herbs provided important references for further experimental studies and clinical researches.

Abstract:The toxicity of Chinese materia medica is the popular topic at home and abroad, and is the main barrier of Chinese materia medica in its internationalization. The safety of Chinese materia medica is the common focus in society. So, emphasis should be payed to the application and study of toxicant Chinese materia medica, correct recognition of its effct and bias is the basis of exerting its therapeutic effect thoroughly. On basis of expounding the concept and classifi cation of toxicant Chinese materia medica, this article classifyed and analyzed the heavy toxicant Chinese materia medica in government standard according to its application, and proposed thinking on study of heavy toxicant Chinese materia medica.

Abstract:Background: Chinese patent medicines are specialty preparations in China that are produced using traditional prescriptions processed by modern pharmaceutical technology. They contain complex ingredients and much attention is paid to their clinical safety. Demonstrating the clinical safety of Chinese patent medicines containing toxic ingredients in modern pharmacological studies has become one of the urgent issues to be solved for the safe use of clinical medicines. Objectives: The aim of this research is to evaluate the safety of Chinese patent medicines containing toxic ingredients by applying the risk-benefit assessment method. Additionally, a database of 'toxic ingredients-toxic Chinese herbal medicines-adverse reactions' will be established to explore the relationship between toxic ingredients and adverse reactions. This will lay the foundation for the rational clinical use of Chinese patent medicines containing toxic ingredients. Methods: 1) Establish a database of 'toxic Chinese herbal medicines-toxic ingredients-toxic Chinese patent medicines' to count the Chinese patent medicines containing toxic ingredients in the 2020 edition of Chinese Pharmacopoeia. 2) Filtered the clinical studies, extracted the drug-related ADEs, and analyzed the characteristics and correlations of these ADEs. 3) Finally, this section summarizes the causes of ADEs related to Chinese patent medicines containing toxic ingredients and extracts the main risk factors to provide a reference for further study. Outcomes: 1) There are four main types of Chinese patent medicines containing toxic ingredients. These include medicines with diester aconitine metabolites, mineral composition, Araceae metabolites, and hydrogen cyanide. 2) Digestive system, skin and its appendages, and allergic reactions were the main types of ADEs related to four types of Chinese patent medicines containing toxic ingredients. 3) There are four primary risk factors associated with the clinical use of Chinese patent medicines containing toxic ingredients: medicine, medication, individual and regulatory factors.

Abstract:At the end of 2017, research revealed that herbal remedies containing aristolochic acid may cause liver cancer. This triggered a wide debate on ‘poison’ in traditional Chinese medicine. The popular TV show A Bite of China 3 also put the safety issue of traditional Chinese medicines as tonic food in the spotlight of public opinion. In fact, there are many misunderstandings about poison in descriptions of food safety and medicine safety on the internet and WeChat. From the perspective of science communication, our understanding of poison and its uses should transcend the monism of Western science.

Abstract:It is estimated that three quarters of the world population rely on herbal and traditional medicine as a basis for primary health care. Therefore, it is one of the most important and challenging tasks for scientists working in drug research to investigate the efficacy of herbal medicine, to dissect favorable from adverse effects, to identify active principles in medicinal plants and to ban poisonous plants or contaminations from herbal mixtures. In the present review, some problems are critically discussed. Botanical misidentification or mislabeling of plant material can play a role for toxic reactions in humans. Some plant descriptions in traditional herbal medicine (e.g. traditional Chinese medicine) have changed over time, which may lead to unintended intoxication by using wrong plants. A problem is also the contamination of herbals with microorganisms, fungal toxins such as aflatoxin, with pesticides and heavy metals. Unprofessional processing, which differs from safe traditional preparation represents another potential source for herbal poisoning. Unwanted effects of herbal products may also develop by the interaction of herbs with conventional drugs upon concomitant intake. The art of herbal medicine is to dissect pharmacologically and therapeutically valuable herbal drugs from harmful and toxic ones and to develop combinations of medicinal plants as safe and efficient herbal remedies. Standardization and strict control measures are necessary to monitor sustainable high quality of herbal products and to exclude contaminations that badly affect patients consuming herbal medicine.

Abstract:Objective:To analyze the adverse reaction of Chinese herbs and Chinese patent medicines with antirheumatic activity and discuss the regulation and characteristic of adverse reaction to provide the reference for using herbs and Chinese patent medicine rationally.Methods:Search and analyze full text for China National Knowledge Infrastructure (CNKI),by using herbs and medicines names as headings,and adverse reaction,side effect,toxicity,contraindication,allergy and intoxication as subheadings.Results:There are seven Chinese herbs including Thunder God Vine,Nux Vomica,Radix Aconiti Lateralis Praeparata,Tnpterygium Hypoglaucum Hutch,Angelica Pubescens,Long-noded Pit Viper,Radix Clematidis,which reported adverse reaction.There are five varieties of Chinese patent medicines,except Qianggu Capsule,happening adverse reaction.Conclusion:Because the adverse reactions of parts of Chinese herbs and patent medicines with antirheumatic activity are common.Rheumatologists must use them legitimately and value the adverse reactions.

Abstract:Rational drug use for children is the focus of physicians,pharmacists and researchers because of characteristics of growth and development of children.This paper tried to analyze the current problems in padiatric use of Chinese patent medicines and proposed a primary solution from the safety aspect,then the criteria and compromise of this issue were given.Currently,the pediatric use of Chinese patent medicines was relatively common,with much unreasonable uses and off-label uses recorded in some literatures.However,it is believed in the traditional Chinese medicine that some mineral medicines like calomel and cinnabar,some toxic medicines like croton and musk and some cold bitter medicines like bezoar and rhinoceros horn,should be strictly limited when they were used for children.The dosage and administration route were also elucidated from the traditional aspect.Besides,modem medicine had more specific guidelines for how to choose and use medicines for children,which gave references for the administration of Chinese patent medicine.Furthermore,we need to construct a comprehensive safety medication system for children and strengthen the scientific research of developmental toxicology.At last,the principles of prescription review for children were proposed and three important items including medicine choice,usage adjustment and treatment course were elucidated.

Abstract:Toxicity and adverse drug reactions (events) related to Chinese herbal medicines (CHMs) have been reported increasingly and stirred discussion within the public, media, government and academia, of which hepatotoxicity has become prominent problems in recent years, especially for those CHMs not conventionally considered of toxicity, for instance, Polygonum Multiflorum Thunb. (He shou wu in Chinese). Nowadays, clinical diagnosis for drug-induced liver injury (DILI) mainly adopts the Roussel Uclaf Causality Assessment Method (RUCAM) and the structured expert opinion process (SEOP). However, the accurate diagnosis of DILI is still the international challenge due to a lack of specific diagnosis indicators. Herb-induced liver injury (HILI), nevertheless, is more difficult to diagnosis and sometimes exists misdiagnosis, by reason of the complexity of both the composition of CHMs and combinational prescription of CHMs and western medicines in clinic.

Abstract:Over the years, the safety of traditional Chinese medicine (TCM) has received widespread attention, especially the central nervous system‐related adverse reactions. Indeed, the complexity of traditional Chinese medicine has limited the widespread application of TCM. The article summarizes the main components associated with neurotoxicity, including alkaloids, terpenes, flavonoids, saponins, proteins, and heavy metals, by reviewing the literature on the neurotoxicity of traditional Chinese medicine. It has been established that the neurotoxicity mechanisms mainly include mitochondrial damage, oxidative damage, inhibition of cell proliferation (including transcriptional and DNA damage), changes in cell membrane permeability, and apoptosis. By reviewing the latest literature, this paper provides the foothold for follow‐up studies and can assist clinicians in preventing neurotoxicity via rational and safe TCM drug use.

Abstract:Abstract Aconitum carmichaeli Debx.- derived herbal medicine has been used for anti-inflammation and anti-arrhythmia purpose for more than two thousand years. It is processed into Chuanwu ( Radix Aconiti praeparata ) and Fuzi ( Radix Aconiti lateralis praeparata ) in Traditional Chinese Medicine, which are two useful drugs but with toxic properties. There have been patients poisoned by accidental ingestion of Aconitum plants or misuse of the herbal drug, and this is of great concern to study in-depth. In this review, we provided the traditional and contemporary practice of using Aconitum herbs as medicine, from functions, processing methods to toxicity in ethnomedicine aspects to discuss the underlying connections of traditional and modern understanding on the toxicity of Aconitum plants. We summarized the functions and toxicology of the herbal drugs are analyzed from chemical and clinical aspects, with the help of traditional and modern knowledge of medicine. The medicinal doses and lethal doses determined by researches are summarized, and the usage and processing methods are updated and reviewed in the modern view. In addition, clinical management of poisoned cases using western medicine is discussed. This review provides insights and awareness of safety when using Aconitum -derived herbal medicine, and the application of modern scientific knowledge to optimize the detoxification processes. We suggest the possibility to renew the current standard processing method from the official Pharmacopoeia all over the world .

Abstract:AbstractTraditional Chinese medicine (TCM) is a medical system that has collected and summarized abundant clinical experience over its long history of more than 2000 years. However, the frequent occurrence of TCM-induced adverse reactions has hindered the modernization and internationalization of TCM, while attracting increasing attention from around the world. Unlike chemical drugs and biological agents, the difficulties involved in research on the toxicity and safety of TCM mainly include the complexity of its components and the unpredictability of drug–body interactions. Much of TCM, which has overall therapeutic effects, has the typical mechanisms of multiple components, multiple pathways, and multiple targets. While considering the gradualness and unpredictability of TCM toxicity, the ambiguity of toxicants and safe dosage, and individual differences during long-term TCM administration, we have systematically established key techniques for the toxicity assessment of TCM. These techniques mainly include TCM toxicity discovery in an early phase, based on a combination of drug toxicology genomics and metabolomics; methods to identify dose–toxicity relationships in TCM; and integrated techniques for the exploration of TCM interactions, such as fast-screening tests based on drug-metabolizing enzymes and receptor pathways. In particular, we have developed a new technical system for TCM safety evaluation using molecular toxicology, which has been validated well in research on TCM compatibility contraindication, quality control, and allergen discovery. The application of this key technical platform is introduced here in detail. This application includes model organisms, toxicant biomarkers, a magnetic suspension technique, and the application of network toxicology and computational toxicology in research on the toxicity of Fructus toosendan, Semen cassiae, Polygonum multiflorum, and Fructus psoraleae.

Abstract:To discuss the non-rational use of cough remedy and expectorants by analysis of the specific case.Analysing non-rational use of the drug based on:1) The prescription does not fit for the disease,especially the etiology of cough and characteristics of cough and sputum ;2) The prescription does not fit for the physiological and pathological features of patients ;3) The prescription does not fit for the characters of drugs such as mechanisms,pharmacodynamics,pharmacokinetics,efficacy,adverse reactions and drug interactions.Physicians and pharmacists should consider more about the factors above mentioned so as to improve the level for rational drug use.

Abstract:Objective: To define the present situation of adverse reactions of traditional Chinese medicine(TCM) decoction based on references in recent 30 years.Method: The case reports of adverse reaction caused by TCM decoction were collected from medical literature.Result: The case reports were increased in recent years.Thirty one cases were reported in 25 literatures.In the all cases about 32.25% were severe.The main adverse reaction was allergic reaction.The adverse reaction also happened in digestive system,nervous system and the hematological system.All the patients recovered by withdrawing the decoction and taking symptomatic treatment.Conclusion: TCM decoction can cause adverse reaction,even sometimes cause the severe reaction.Mueti-means and comprehensive analysis can help us to judge the right medicine which bring the adverse reaction and make the judgment more scientifically and accurately.

Abstract:OBJECTIVE:To discuss the dispensing of prescriptions of traditional Chinese medicine.METHODS:According to the dispensing procedure of Chinese traditional medicine and the principle of drug use,the problems occurred during Chinese traditional medicine dispensing were pointed out and analyzed.RESULTS:The problems occurred in the dispensing of prescriptions of traditional Chinese medicine were mainly resulted from doctors,pharmacists and nurses' lack of basic rationale or they had no intimate knowledge of operation regulations.These problems can be avoided if with efforts.CONCLUSION:Only through strictly following the prescription of Chinese traditional medicine can the quality and efficacy of Chinese drugs dispensing be guaranteed.
