A Risk Analysis Based on a Two-Stage Delayed Diagnosis Regression Model with Application to Chronic Disease Progression

Abstract:This paper presents a regression model based on multi-stage disease progression with delayed diagnosis time to quantify the stages of the progression and to identify the risk factors associated with each stage. The conventional chronic disease progression study usually relied on the assumption that the time of the confirmation of the disease by diagnosis is the time of the observation of the start of a particular disease state. Clearly this will lead to biased estimates of progression since the disease state should have already occurred before the diagnosis, but is hidden. We call this as the delayed diagnosis and develop a modelling procedure for the identification of the hidden stages of progression. A hazard-based multi-stage regression model is also proposed for a further risk analysis. We apply the developed methods to hepatitis C data and the analysis shows that taking the delayed diagnosis into account significantly improved the model fit in comparison with the conventional model. We also find that the risk factors associated with each stage are more significant, particularly in the later stage of progression, than those in the conventional model. We conclude that such delayed phenomena in diagnosis must be taken into account when modelling the chronic disease progression process and conducting related risk analysis.
