Abstract:Objective To investigate the short-term curative effects of thoracic endovascular repair and laser fenestration in situ for the treatment of aortic diseases involving aortic arch and even ascending aorta. Methods Twelve patients with aortic disease involving the aortic arch and even ascending aorta were treated with transluminal covered stent -graft placement under laser fenestration in situ from November 2016 to January 2017.There were 7 cases of Stanford type B aortic dissection,2 cases of Stanford type A aortic dissection,1 case of ascending aorta and aortic arch aneurysm, and 2 cases of ascending aorta penetrating ulcer with intracranial hematoma.During the surgery,there were 7 cases of left subclavian artery fenestration,1 case of left common carotid artery and left subclavian artery fenestration, and 4 cases of cephalic brachial trunk and left common carotid artery and left subclavian artery fenestration.The surgery was performed in acute stage in 5 cases and in chronic stage in 7 cases. Results A total of 12 conical type covered artery stents were implanted and 21 Fluency plus straight stent grafts were implanted.There were two cases(16.7%,2/12) of perioperative death,one of which died from the ascending aorta rupture(8.3%,1/12)and the other died from massive hemorrhage of digestive tract and multiple organ failure.The incidence of early postoperative complications was 16.7%(2/12),including 1 case of transient neurological dysfunction and 1 case of respiratory insufficiency and pulmonary infection, all of which were cured after treatment.Three-month follow-up results showed good isolation of the entry of tear(or ulcer or aneurysm).No endoleak was seen.The fenestration vessel was unobstructed. Conclusions Thoracic endovascular repair combined with laser fenestration in situ is safe and effective for the treatment of aortic diseases involving the aortic arch and even ascending aorta.It has advantages of less trauma,quick recovery,high intraoperative success rate,low incidence of endoleak,and good short-term efficacy.
