Prognostic factors of locally persistent and recurrent nasopharyngeal carcinoma treated with ster-eotactic radiotherapy

Abstract:Objective To analyze the prognostic factors of locally persistent and recurrent nasopha-ryngeal carcinoma ( NPC) treated with fractionated stereotactic radiation therapy ( FSRT). Methods From Sep. 1999 to Dec. 2005, 90 patients with locally persistent or recurrent NPC were treated with FSRT. Except for 3 patients who had distant metastases before FSRT, 87 patients (34 for persistence and 53 for recurrence) were included in this study. Among them, 44 had tumor confined to nasopharynx and 44 had tumor extended beyond nasopharynx. Median tumor volume was 5. 7 cm3 (0. 8-24. 7 cm3 ). Median FSRT dose was 18 Gy in 3 fractions for persistent disease and 48 Gy in 6 fractions for recurrent disease. Local failure-free survival (LFFS) and disease-specific survival (DSS) were estimated by the Kaplan-Meier method. Univariate and multivariate analysis were performed by the Logrank test and Cox model, respectively. Results Median follow-up time after FSRT was 24.9 months (3. 3-86. 3 months ). The 1-, 2-, 3- and 4-year LFFS and DSS were 90% ,83% ,81% ,75% and 84,77% ,61% ,56% , respectively. Univariate analysis showed that tumor volume (≥5 cm3 vs. > 5 cm3 ) was a significant factor for DSS (P = 0. 015 ). Multivariate analysis showed that disease type (persistence vs. recurrence) and tumor volume were significant independent factors for DSS ( P < 0. 05). Late side-response rates were 9% and 26% in patients with local persistence and recurrence, respectivly. Conclusions FSRT provides better local control and survival of patients with persistent or recurrent NPC. Recurrent disease and large tumor volume are independent poor prognostic factors.
