Abstract:Objective:To study on effects of Chinese herbs for removing toxic substances,nourishing Yin and removing blood stasis on activity of disease in the patient of systemic lupus erythematosus(SLE).Methods:65 cases of SLE were divided into a Chinese medicine group and a control group,and their cumulative scores of SLE disease activity index and changes of doses of prednisone were observed before and after treatment of 2 courses,respectively.Results:At the end of the second treatment course,there was significant difference in the number of case at the active stage and the resting stage between the two groups(P<0.05),and the daily withdrawing amount of prednisone was superior to that in the control group(P<0.01).Conclusion:The Chinese herbs for removing toxic substances,nourishing Yin and removing blood stasis can control activity of disease and also can reduce dose of prednisone for the patient of systemic lupus erythematosus.

Abstract:Systemic lupus erythematosis (SLE) is a complex and clinically heterogeneous autoimmune disease. A variety of immunological defects contribute to SLE, including dysregulated innate and adaptive immune response. A clearer understanding of the mechanisms driving disease pathogenesis combined with recent advances in medical science is predicted to enable accelerated progress towards improved SLE-personalized approaches to treatment. The aim of this review was to clarify the immunological pathogenesis and treatment of SLE. Literature reviews and original research articles were collected from database, including PubMed and Wanfang. Relevant articles about SLE were included. Breakdown of self-tolerance is the main pathogenesis of SLE. The innate and adaptive immune networks are interlinked with each other through cytokines, complements, immune complexes and kinases of the intracellular machinery. Treatments targeted at possible targets of immunity have been assessed in clinical trials. Most of them did not show better safety and efficacy than traditional treatments. However, novel targeting treatments are still being explored. Dysregulated immune response plays a critical role in SLE, including innate immunity and adaptive immunity. Biologic agents that aim to specifically target abnormal immune processes were assessing and may bring new hope to SLE patients.

Abstract:OBJECTIVE: To evaluate the curative effect of integrated Traditional Chinese and Western Medicine on gout, and to investigate the therapy timing and exact treatment options of integrated medicine. METHODS: Totally 860 patients were enrolled, including 460 patients with intermittent gout, 200 patients with active Traditional Chinese Medicine (TCM) syndrome (TCM syndrome score >= 6) and 200 patients with stable TCM syndrome (score < 6). They were randomly divided into intervention and control groups. The control group was treated according to Western Medicine guidelines. The intervention group was treated with integrated Traditional Chinese and Western Medicine. The efficacy of TCM syndrome, joint pain score, joint swelling score, ESR, C-reactive protein, serum uric acid, liver and kidney function, and the duration of remission of TCM syndrome were compared between the two groups before and after treatments. RESULTS: For the patients with stable TCM syndrome, there was no significant difference in the effective rate and inefficiency between the intervention group and the control group. For the active type, the effective rate of the intervention group is better than the control group significantly. For the stable type, there was no significant difference between the intervention group and the control group in improving the scores of joint pain and swelling, reducing the level of ESR, C-reactive protein, serum uric acid and improving liver and kidney function. For the active type, the differences between the two groups were significant. The stable stage of gout in the intervention group was longer than the control group. CONCLUSION: For the gout patients with stable TCM syndrome in the acute stage of gout, we can use TCM treatment or Western Medicine alternatively; for the patients with active TCM syndrome, the scheme of combination of Traditional Chinese and Western Medicine can be applied, with the better curative effect than any medicine alone. Abstract:We read with great interest the article by Muller-Calleja et al 1 in which a novel mechanism of hydroxychloroquine (HCQ) was discovered, which might explain well-established anti-inflammatory effects of antimalarials.2–4 HCQ and chloroquine (CQ), both known as antimalarial drugs, have become fundamental therapeutic elements in systemic lupus erythematosus (SLE) in these decades. Abstract:Objectives:To research the clinical effect and regulation of nourishing yin,replenishing qi and detoxicating on systemic lupus erythematosus(SLE)with qi-yin deficiency.Methods:Nourishing yin,replenishing qi and detoxicating combined with glucocorticosteriod were used to treat SLE.The clinical symptoms,laboratory index and partial immune items before and after treatment were observed and compared with those of healthy people. Results:52 cases were included,the symptoms of statistically significant improvement were oral ulcer,fatigue, hectic fever,insomnia,palpitation(P0.001).The SLEDAI decreased obviously after therapy(P0.001).WBC,LY and ESR were improved statistically(P0.01or P0.001).Titers of ANA and anti-dsDNA antiboby of the patients decreased after therapy(P0.01).C 3 of the patients notably increased after therapy(P0.001).The research showed that TNF-α(P0.001),IL-6(P0.05),β 2 -MG(P0.001)in the patients'serum were much more higher than those of healthy volunteers,after the therapy of traditional Chinese medicines,TNF-α(P0.001)and IL-6(P0.05)decreased, butβ 2 -MG had not changed.Conclusions:Nourishing yin,replenishing qi and detoxicating method could decrease the patients'titers of ANA and anti-dsDNA antibody,increase C3,adjust the unbalance of cytokines,show positive conform functions and thus make the immune system balance again.

Abstract:Lupus nephritis (LN) is the most serious and common complication of systemic lupus erythematosus (SLE), which is mainly treated with glucocorticoids combined with immunosuppressive drugs, but its long-term use in large quantities can produce many toxic side effects, which seriously affect the continued application of therapeutic drugs and affect the prognosis and quality of life of patients. TCM can effectively reduce the toxic side effects of western drugs, improve clinical efficacy, reduce comorbidities, ensure the continued use of western drugs, reduce the relapse rate after discontinuation of drugs, and improve the prognosis of patients through evidence-based treatment. The combination of Chinese and Western medicine can effectively control LN activity, relieve symptoms, improve renal function, and reduce the side effects of Western medicine.

Abstract:Lupus nephritis (LN) is an autoimmune disease with multiple system involvement and is also one of the most serious forms of organ damage in systemic lupus erythematosus (SLE), which is mainly caused by the formation and deposition of immune complexes in glomeruli. More than 50% of SLE patients have clinical manifestations of renal damage. At present, the treatment of lupus nephritis is mainly based on glucocorticoids and immunosuppressants. However, due to adverse drug reactions and frequent recurrence or aggravation after drug reduction or withdrawal, the prognosis remains poor; thus, it is still one of the most important causes of end-stage renal failure. Therefore, new treatment strategies are urgently needed. This article aims to review the application of traditional Chinese medicine and natural extracts in the treatment of lupus nephritis to provide the basic mechanisms of treatment and a new treatment strategy with clear effects and high safety performance.

Abstract:Objective:To observe the curative effect and safety of integrated traditional Chinese and western medicine in treating systemic lupus erythematosus (SLE)complicated with herpes zoster (HZ).Methods:A total of 56 cases of patients with SLE were randomly divided into treatment group and control group with 28 patients in each.Both groups were treated wiht conventional treatment and acyclovir orally.Additionally,heat-clearing,blood-cooling antitoxicant herbs and helium-neon laser therapy were given to the treatment group.The treatment lasts for 1 4 days.Results:After treatment,the results of medicinal diagnosis in treat-ment group showed better than the control group in the clinical manifestation of total effective rate,pain outcome,the duration of herpes regression,and the duration of crusts form and fall.The results is of statistical significance with P <0.05.Adverse reaction has not been seen in both groups.Conclusion:Integrated traditional Chinese and western medicine is an effective and safe way to treat SLE complicated with HZ,and worth spreading for clinical use.
Chinese Herbal Medicine for Systemic Lupus Erythematosus: A Systematic Review and Meta-analysis of Randomized, Placebo-Controlled Trials

Abstract:ObjectiveTo provide evidence on the efficacy and safety of Chinese herbal medicine (CHM) as interventions for systemic lupus erythematosus (SLE).MethodsSeven electronic databases, including the Cochrane Library, Chinese National Knowledge Infrastructure (CNKI), Chinese Scientific Journal Database (VIP), Chinese Biomedical Literature Service System (SinoMed), Wanfang, Embase, and PubMed, were comprehensively searched, from their inception to August 16, 2020, for all randomized controlled trials (RCTs) that focused on CHM used alone or in combination with conventional medicine for SLE. Outcomes were SLE activity index (SLEDAI), traditional Chinese medicine symptom/syndrome score (TCMSS), dosage of glucocorticoids, main serological testing, and incidence of adverse events. Data were extracted and pooled using Review Manager 5.3 software.ResultsA total of 13 RCTs enrolling 856 participants met our inclusion criteria. Meta-analyses showed that, compared to placebo, CHM had statistically significant effect on reducing SLEDAI score (MD=−1.74, 95% CI: −2.29 to −1.18), diminishing TCMSS (SMD=−0.89, 95% CI: −1.16 to −0.62), decreasing dosage of glucocorticoids (MD=−2.41 mg/d, 95% CI: −3.34 to −1.48), lowering erythrocyte sedimentation rate (MD=−4.78 mm/h, 95% CI: −8.86 to −0.71), and increasing serum complement C4 level (MD=0.03 mg/dL, 95% CI: 0.00 to 0.06). No significant difference was found between CHM and placebo on adverse events.ConclusionsCHM provided significant beneficial effect on controlling disease activity and reducing dose of glucocorticoids used among SLE patients. Future advanced designed RCTs for CHM treating moderate to severe SLE with multicenter and longer follow-up are urgently needed.

Abstract:Systemic lupus erythematosus (SLE) is a chronic autoimmune disorder affecting multiple systems, characterized by the development of harmful autoantibodies and immune complexes that lead to damage in organs and tissues. Chinese medicine (CM) plays a role in mitigating complications, enhancing treatment effectiveness, and reducing toxicity of concurrent medications, and ensuring a safe pregnancy. However, CM mainly solves the disease comprehensively through multi-target and multi-channel regulation process, therefore, its treatment mechanism is often complicated, involving many molecular links. This review introduces the research progress of pathogenesis of SLE from the aspects of genetics, epigenetics, innate immunity and acquired immunity, and then discusses the molecular mechanism and target of single Chinese herbal medicine and prescription that are commonly used and effective in clinic to treat SLE.

Abstract:After a thorough search through the database as CNKI database, VIP database, Wanfang database, PubMed, and Cochrane Library, the clinical experimental articles have been selected out on the effects of Integrated Traditional Chinese and Western Medicine on the treatment of lupus nephritis. A meta-analysis was carried out in terms of clinical efficacy criteria and safety criteria by RevMan 5.3 software. Based on the results, we cautiously conclude that Integrated Traditional Chinese and Western Medicine used for lupus nephritis could improve the clinical efficacy while at same time lower the 24-hour urine protein, serum creatinine, and adverse drug reactions.
Integrative therapy decreases the risk of lupus nephritis in patients with systemic lupus erythematosus: A population-based retrospective cohort study

Abstract:Ethnopharmacological relevance: Evidence on alleviating the risk of lupus nephritis by integrative therapy with conventional medicine (CM) and herbal medicine (HM) had not been addressed. Aim of the study: We investigated the integrative effect associated the risk by a retrospective Systemic Lupus Erythematosus (SLE) cohort from Taiwan National Health Insurance Research Database (NHIRD). Materials and methods: SLE patients with a catastrophic illness certificate (CIC) were retrospectively enrolled from the SLE cohort of the Taiwan NHIRD between 1997 and 2011. The patients were divided into an integrative medicine (IM: integrated CM plus HM) and a non-IM (CM only) group with 1:1 propensity score matching. Cox proportional regression model and the Kaplan-Meier method were conducted to estimate the hazard ratio (HR) for lupus nephritis in the cohort. Results: Among 16,645 newly diagnosed SLE patients holding a CIC (SLE/CIC), 1933 had received HM and 1571 had received no HM treatment. After propensity score matching, there were 273 patients with lupus nephritis-120 in the IM group and 153 in the non-IM group. The adjusted HR (0.68, 95% confidence interval [CI]: 0.54-0.87, p<0.01) for lupus nephritis was lower in the IM group than in the non-IM group. The adjusted HR (0.69, 95% CI: 0.54-0.88, p<0.001) for lupus nephritis was also lower in the group of patients who had received CM plus HM than in the group that received CM only. The core pattern of HM prescriptions, which were integrated with CM for preventing lupus nephritis, was "Sheng-Di-Huang" (raw Rehmannia glutinosa Libosch.), "Mu-Dan-Pi" (Paeonia suffruticosa Andr.), "Dan-Shan" (Salvia miltiorrhiza Bge.), "Zhi-Bo-Di-Huang-Wan.", and "Chi-Shao" (Paeoniae lactiflorae Rubra). Conclusion: Integrative therapy decreased the risk of lupus nephritis among SLE patients in Taiwan. Further investigation of the pharmacological mechanism and clinical efficacy are warranted.

Abstract:Systemic lupus erythematosus (SLE) is a chronic and autoimmunity-mediated diffuse connective tissue disease. The mainstay of treatments for SLE mainly relies on corticosteroids and immunosuppressants, which have a series of unavoidable side effects. Therefore, it is of fundamental importance to search novel therapeutic targets for better treatment with favorable efficacy and minor side effects. Recent studies shed light on potential therapeutic targets for SLE, mainly covering the followings: B-cell/plasmocyte-related targets [B cell activating factor (BAFF), a proliferation-inducing ligand (APRIL), CD20, CD22, CD19/FcγRIIb, Bruton tyrosine kinase (Btk), and proteasome], T cell-related targets [calcineurin, mammalian target of rapamycin (mTOR), regulatory factor X1 (RFX1), and Rho kinase], macrophage-related targets (macrophage migration inhibitory factor), intracellular signaling molecules, cytokines (cereblon, histone deacetylase 6, Janus activated kinase/signal transducer and activator of transcription), co-stimulating factors (CD28/B7, CD40/CD154), IgE autoantibody, and gut microbiome. Among them, belimumab (a humanized monoclonal antibody against B-lymphocyte stimulator) and telitacicept (a recombinant human B-lymphocyte stimulator receptor-antibody fusion protein) have been sequentially approved for the clinical treatment of SLE in China. A variety of new targeted-therapy drugs are in the Phase 2 or Phase 3 clinical trials,among which anifrolumab (a human monoclonal antibody against type I interferon receptor subunit 1) has completed a Phase 3 clinical trial with good responses achieved, although its incidence of herpes zoster is higher than that in the control group. The research progress in both molecular mechanisms and new drug development for different therapeutic targets have greatly promoted our better and in-depth understanding of the pathogenesis of SLE, and have also reflected the complexity and heterogeneity of the disease. Successful development and clinical application of more novel therapies would no doubt usher in a new era of individualized treatment for SLE in the future.

Abstract:Gut commensals help shape and mold host immune system and deeply influence human health. The disease spectrum of mankind that gut microbiome may associate with is ever-growing, but the mechanisms are still enigmas. Characterized by loss of self-tolerance and sustained self-attack, systemic lupus erythematosus (SLE) is labeled with chronic inflammation, production of autoantibodies and multisystem injury, which so far are mostly incurable. Gut microbiota and their metabolites, now known as important environmental triggers of local/systemic immune responses, have been proposed to be involved in SLE development and progression probably through the following mechanisms: translocation beyond their niches; molecular mimicry to cross-activate immune response targeting self-antigens; epitope spreading to expand autoantibodies spectrum; and bystander activation to promote systemic inflammation. Gut microbiota which varies between individuals may also influence the metabolism and bio-transformation of disease-modifying anti-rheumatic drugs, thus associated with the efficacy and toxicity of these drugs, adding another explanation for heterogenic therapeutic responses. Modulation of gut microbiota via diet, probiotics/prebiotics, antibiotics/phages, fecal microbiota transplantation, or helminth to restore immune tolerance and homeostasis is expected to be a promising neoadjuvant therapy for SLE. We reviewed the advances in this territory and discussed the application prospect of modulating gut microbiota in controlling SLE.

Abstract:Background For the large number of systemic lupus erythematosus (SLE) patients in China, it is critical to carry out effective disease management to improve the treatment effect and reduce disease burden. A pharmacist-led multidisciplinary care model has not been reported in Chinese SLE patients before. Objective To assess the effect of patient-centered, pharmacist-led, multidisciplinary care on clinical outcomes and satisfaction with health care in Chinese SLE patients. Setting: The South Campus, Renji Hospital, School of Medicine, Shanghai Jiao Tong University, Shanghai, China. Method Participants were 143 systemic lupus erythematosus patients randomly assigned to either the intervention group (multidisciplinary care: physician, pharmacist and nurse) or the control group (usual care only). Main outcome measures The primary outcome was scores on the systemic lupus erythematosus disease activity index-2000, the satisfaction with information about medicines scale, and the EuroQol five-dimension questionnaire, assessed at baseline and 12 months. Results Between October 1, 2017 and October 1, 2018, 42 participants were included in the intervention group and 40 in the control group. At 12 months, results for the systemic lupus erythematosus disease activity index-2000 differed significantly between the intervention group and the control group (0 vs. 2, P = 0.027). Patient satisfaction with health care was also significantly greater in the intervention group than in the control group (92.9 vs. 0%, P = 0.000). According to the EuroQol five-dimension questionnaire, health quality was also improved (0.94 vs. 0.85, P = 0.006). Conclusion Our multidisciplinary care team significantly improved clinical outcomes and satisfaction with drug information in Chinese systemic lupus erythematosus patients.

Abstract:Objective To study the effect and safety of optimum proposal of integration of disease and TCM syndrome for the treatment of rheumatoidarthritis.Methods Seventy-six rheumatoidarthritis patients were randomized into Chinese herbal group(22 cases),Chinese herbal plus western medicine group(29 cases),and western medicine group(25 cases).The Chinese herbal group was treated with the optimum proposal of integration of disease and TCM syndrome,based on which,the Chinese herbal plus western medicine group was treated with loxoprofen sodium tablets(60mg,twice a day)and leflunomide(20mg,once a day).The western medicine group was treated with western medicines alone.Three months constitue once course,3 courses in succession were fufilled.Three and nine months after treatment,the scores of visual analogue scale(VAS),number of painful joints,number of swelling joints,mean grip strength of hands,morning stiffness time,erythrocyte sedimentation rate(ESR),C-reactive protein(CRP),rheumatoid factors(RF),and untoward effect of 3 groups were observed.Results Three and nine months after treatment,there was significant difference in all 3 groups in VAS,number of painful joints,number of swelling joints,mean grip strength of hands,morning stiffness time,and ESR as compared with those before treatment(P<0.05 or P<0.01),and the improvement of VAS score in Chinese herbal plus western medicine group and Chinese herbal group was more obvious than that of western medicine group(P<0.05).The general effect of 3 groups was without significant difference(P>0.05).The incidence of untoward effect in the Chinese herbal group was less than that of the western medicine group and the Chinese herbal plus western medicine group(P<0.05).Conclusion The effect of optimum proposal of integration of disease and TCM syndrome in the treatment of rheumatoidarthritis is definit,especially in analgesia,and with less untoward effect.

Abstract:Systemic lupus erythematosus(SLE)is an autoimmune disease,which can be diagnosed based on its clinical manifestations and a few immunological index,especially antinuclear antibodies.There is not specific method for management of the disease.With the development of immunology,genetics,molecular biology,molecular pharmacology and cytobiology,new drug and strategies have been introduced in the treatment of SLE.The article summarized their pathways,animal experiment,clinic experiment and so on.

Abstract:Objective To observe the efficacy of integrated traditional Chinese and Western medicine in the treatment of chronic heart failure.Methods 100 patients with chronic heart failure were selected and randomized into the therapeutic group(Wen Yang Fu Mai decoction group) and the control group(conventional western medicine group).The treatment course was two months.The symptoms were observed and heart B ultrasonic detection was done before and after the treatment.Results After two months' treatment,symptoms and LVEF,LVESVI,and LVEDVI were improved markedly.The total effective rate of the therapeutic group was 98%,and that of the control group was 82%.The difference was significant(P<0.05).Conclusion: Integrated traditional and western medicine can improve the curative rate and recurrence rate of chronic heart failure.
Effect of Detoxification, Removing Stasis and Nourishing Yin Method on Corticosteroid-Induced Hyperlipidemia in Patients with Systemic Lupus Rythematosus

Abstract:OBJECTIVE:To observe the effect of TCM therapy for detoxification, removing stasis, and nourishing yin on corticosteroid-induced hyperlipemia in patients with systemic lupus erythematosus (SLE), and to investigate its mechanism.METHODS:One hundred and seventy patients with SLE were randomly assigned to the integrative medicine group (IM group) and the Western medicine group (WM group), 85 in each group. Also, 30 healthy subjects selected from blood donors were enrolled in the normal control (NC) group. All patients were treated mainly with prednisone, while those in the IM group were given TCM therapy additionally, and the therapeutic course for both groups was 6 successive months. The changes of serum total cholesterol (TC), triglyceride (TG), high density lipoprotein cholesterol (HDL-C), low density lipoprotein cholesterol (LDL-C), very low density lipoprotein cholesterol (VLDL-C) and apolipoprotein A (ApoA) were determined and observed. A 2-year follow-up study was carried out in 16 patients of the WM group and 25 of the IM group.RESULTS:Before treatment, no significant difference had been found among the three groups in the serum levels of lipids and lipoproteins. After the 6-month treatment, as compared with the WM group, the IM group showed lower levels of TC, TG, LDL-C, and VLDL-C (P<0.05 or P<0.01) and higher levels of HDL-C and ApoA (P<0.05). A similar effect was also shown by the follow-up study in the IM group (P<0.05 or P<0.01).CONCLUSION:TCM therapy for detoxification, removing stasis, and nourishing yin can effectively regulate the levels of serum lipids and lipoproteins in preventing and treating SLE patients with corticosteroid-induced hyperlipemia.
