Abstract:Dr.CAO considers that cause of adenomysis is the combination of blood stasis.And its location is the inner area including Chong meridian, Ren meridian and the uterus.The liver-diagnosed treatment should be taken to dredge Chong channel and Ren channel by Qi and blood.The syndrome differentiations are divided in to three kinds: Firstly, the pattern of liver-qi stagnation is due to Qi-blood stasis.Its location is channels to be cured by dredging Qi and adjusting Qi-blood in Chong channel and Ren channel.And treatment should be taken by Xiaochaihu Tang with proper adjustment.Secondly, the pattern of liver-blood stasis is due to Ren channel while Qi is combined with pathogenic qi in the disease that lasts for a long time.The prescription contains Shuizhi[Whitmania pigra Whitman], Dibiechong[Polyphaga plancyi Bol.], Quanxie[Buthus martensii Karsch].The third pattern of insufficient Qi and blood stasis is due to recurrence disease with liver-qi deficiency.The prescription could take Buzhong Yiqi Tang with proper adjustment because of sinking Qi in the liver and spleen.

Abstract:Objectives Adenomyosis is a common and refractory disease in gynecology. Preserving the uterus during treatment for adenomyosis remains a problem. High-intensity focused ultrasound (HIFU) is widely used in treatment of solid tumors. This study aimed to analyze patients with adenomyosis who were treated by HIFU and to preliminarily examine the characteristics of patients who are more suitable for HIFU to treat adenomyosis with reliable efficacy. Methods Over 2 years, 67 women who were diagnosed with adenomyosis and treated with HIFU at our gynecology department were included in this study. We investigated outcomes of their symptoms (dysmenorrhea and hypermenorrhea) and the volume of their uterine lesions. We also compared the patients’ clinical profiles. Results The women had a mean follow-up duration of 11.6 ± 0.46 months. In the numerical rating scale, used to assess the degree of dysmenorrhea, the score was significantly lower (mean difference: −1.94, 95% confidence interval: −2.704 to −1.176) 3 months after HIFU treatment compared with before treatment, then it remained stable for 3 to 12 months. Hypermenorrhea was reduced to a certain degree, with a mean difference of −0.54 (−1.01–0.02). Conclusions HIFU is a new noninvasive treatment method for adenomyosis that may help relieve dysmenorrhea.

Abstract:In recent years, surgical and non-surgical excision and drug therapy have replaced hysterectomy as the main therapeutic modalities for adenomyosis. It is suggested that the precise clinical diagnosis should be based on the reconstruction of digitized three-dimensional model with original image data of adenomyosis. Patients' age and clinical manifestations should also be considered, and the patients should be stratified according to reproductive requirements, so as to determine the best treatment. In view of the infiltration and diffuse growth of adenomyosis lesions in the myometrium of the uterus, it is suggested that long-term drug management should be adopted after surgical or non-surgical lesion resection.Gonadotropin releasing hormone agonists, levonorgestrel-releasing intrauterine system, dienogest and short-acting oral contraceptives should be recommended to consolidate the curative effect in order to delay the progress of the disease and prevent recurrence.

Abstract:ABSTRACT:The paper introduces professor He Xiaohui's experience in treating chronic atrophic gastritis(CAG).In his view, this disease is caused by deficiency in origin and excess in superficiality,and the basic pathogenesis of CAG is the deficiency of both qi and yin,the dampness and heat in the stomach,and the stasis of the collaterals of the stomach meridian.The site of CAG is stomach,and related closely with spleen,liver,gallbladder and kidney.Following the principle of the combination of disease differentiation and syndrome differentiation,he puts forward a three-stage partition method,which is”cure the disease according to the syndrome,treat the disease combined with syndrome,treat the disease without syndrome directly”,advocates the application of balanced method,and creates the medical treatment of the syndrome differentiation”regulating gastritis with eight prescriptions” and the disease differentiation”shuang pu powder”,which can effectively reverse the pathological changes of chronic atrophic gastritis,and prevent the incidence of gastric cancer.

Abstract:Objective:To discuss the clinical rule of TCM syndrome differentiation,syndrome characteristics and relationship with reproductive endocrinology based on Professor Cao Lingxian's academic thoughts in the syndrome differentiation and treatment of polycystic ovary syndrome (PCOS).Methods:The PCOS data of medical cases history on clinical diagnosis and treatment by Professor Cao were collected,and the symptoms,signs and syndrome differentiation and classification were recorded in detail The studies on TCM syndrome of PCOS were performed by Logistic regression analysis,Fuzzy c-means clustering algorithm and Fisher discriminant analysis,and the correlations between different syndrome types and sex hormones were analyzed.Results:127 PCOS patients were classified according to Professor Cao's clinical syndrome differentiation,among 42 cases with syndrome of kidney deficiency,47 cases with syndrome of kidney deficiency and phlegm obstruction,38 cases with syndrome of kidney deficiency and liver-qi stagnation.The secondary classification on TCM syndrome of PCOS was performed by Logistic regression analysis,the syndrome of kidney deficiency was separated first and then the remaining concurrent syndromes were reclassified.The accuracy on classification of kidney deficiency syndrome and concurrent syndromes was 90.5％,and the accuracy on classification of different concurrent syndromes was 92.9％.According to Fuzzy C-means clustering algorithm,the total accuracy on classification of different concurrent syndromes was 90.6％.According to Fisher discriminant analysis,the total accuracy on classification of different concurrent syndromes was 87.1％.For the comparison of sex hormones levels in different syndrome types,the serum level of prolactin(PRL) in syndrome of kidney deficiency and liver-qi stagnation was significantly higher than that in syndrome of kidney deficiency.Conclusion:In clinical differentiation and treatment of PCOS,the concurrent syndromes are common and kidney deficiency,phlegm dampness and liver-qi stagnation are basic syndromes.There is a certain correlation between PRL level increase and liver-qi stagnation.

Abstract:ObjectiveTo investigate the relationship between the magnetic resonance imaging (MRI) classification of different clinical symptoms and corresponding therapeutic efficacy in adenomyosis patients. MethodsFrom January 2015 to October 2020, a total of 468 patients diagnosed with adenomyosis through MRI examination at Peking University Third Hospital were included in this retrospective cohort study. Totals of 184 (39.3%), 208 (44.4%), 17 (3.6%), and 59 (12.6%) patients were categorized into Subtypes I (intrinsic), II (extrinsic), III (intramural), and IV (penetrating), respectively. Clinical information such as age, dysmenorrhea, menorrhagia, infertility, assisted reproduction, and drug treatment and its efficacy were analyzed. By comparing the clinical information of different adenomyosis subtypes, we intend to provide better fertility guidance and find better treatment strategies for these patients. ResultsThe proportion of dysmenorrhea increased in intrinsic, extrinsic, intramural, and penetrating subtypes (74.5% vs 82.7% vs 94.1% vs 94.9%, respectively, P = 0.002). The proportion of menorrhagia in the intrinsic subtype (53.3%) was significantly higher than that in the extrinsic (28.4%) and intramural (29.4%) subtypes (P < 0.001). The effective rate of progesterone in the intrinsic subtype was significantly lower than that in the extrinsic subtype (52.0% vs 86.5%, P < 0.001). The infertility rates of adenomyosis patients with different subtypes were relatively high (17.6%-41.3%), and that of the extrinsic subtype was the highest among all the subtype groups (41.3%, P < 0.001). ConclusionsSignificant differences in age, dysmenorrhea, menorrhagia, and infertility were found among patients with different subtypes of adenomyosis. A novel classification of adenomyosis was proposed to provide a theoretical basis for the treatment of adenomyosis patients with infertility.

Abstract:Famous traditional Chinese medicine professor Wang Aihua of Hunan province advocates that functional con-stipation is often cold and heat syndromes located mainly in the large intestine. The pathyogenesis of functional constipation is deficiency of both spleen and body fluid, and unsmooth defecating. That is closely associated with lung, spleen, liver and kidney. Foundation treatment is invigorating the spleen and promoting the production of body fluid, and ventilating lung Qi for lowering adverse Qi. Clinical treatment of functional constipation is divided into five types. The excessive heat of intestine syndrome was treated by clearing heat and moistening intestine with modified Tongmai prescription. Liver depression and Qi stagnation was treated by dispersing liver and relieving depression and regulating Qi-flowing for activating stagnancy with modified Simo decoction. The Qi deficiency of lung-spleen syndrome was treated by tonifying Qi and moistening intestine with Yiqi Ziyin decoction. The Yang deficiency of spleen-kidney syndrome was treated by warming to relaxing bowels with Jichuan decoction. The intestine dryness due todeficiency of fluid syndrome was treated by benefiting Qi and nourishing Yin and moistening dryness for relaxing bowels by Zengye decoction.

Abstract:Objectives:To discuss the clinical manifestations and laparoscopic management of cystic adenomyosis.Methods:The data of 5 patients with cystic adenomyosis were analyzed retrospectively.Results:All patients had dysmenorrhea.With an average age of 28.2 years old,all the patients underwent laparoscopic enucleation of the lesion.The pathogenesis of all the patients showed adenomyosis.All the patients had clinically significant reduction in dysmenorrhea and there were no recurrence.One patient conceived naturally after surgery and had term vaginal delivery.Conclusion:Laparoscopic enucleation of the lesion is a reliable procedure to treat cystic adenomyosis.

Abstract:Background Adenomyosis is a common gynecological disease in women. A relevant literature search found that approximately 82% of patients with adenomyosis chose to undergo hysterectomy. However, women of childbearing age are more likely to undergo surgery to preserve the uterus. Because it is difficult to determine the extent of adenomyosis, it is almost impossible to resect adenomyotic tissue and retain the uterus at the same time. Materials and methods Following ethics approval and patient consent, tissue samples were resected and prepared to create frozen slices for analysis. One slice was subjected to H&E staining while the remaining slices were photographed with Coherent Anti-Stokes Raman Scattering (CARS), Second-Harmonic Generation (SHG) microscopy, and Raman spectroscopy. Comparative observations and analyses at the same positions were carried out to explore the diagnostic ability of CARS, SHG, and Raman spectroscopy for adenomyosis. Results In adenomyotic tissue, we found two characteristic peaks at 1,155 and 1,519 cm –1 in the Raman spectrum, which were significantly different from normal tissue. The substances shown in the CARS spectrum were represented by peaks of 1,519 cm –1 . SHG microscopy showed a distribution of collagen at the focus of the adenomyosis. Conclusion This study represents a novel analysis of Raman microscopy, CARS, and SHG in the analysis of adenomyotic lesions. We found the diffraction spectrum useful in determining the focal boundary and the diagnosis of adenomyosis in the tested samples.

Abstract:Adenomyosis is a diffuse or localized organic disease caused by benign invasion of endometrial glands and stroma into the myometrium. It is a common disease that seriously affects reproductive health of women in childbearing age. Due to the unknown etiology and pathophysiological mechanism, and the lack of unified diagnostic criteria and effective treatment methods, total or subtotal hysterectomy has become a radical treatment for adenomyosis, which will lead to the complete loss of fertility. With the continuous exploration of the treatment to adenomyotic patients who have infertility or fertility intentions, new drugs, surgical methods and treating concepts appears. Adopt individualized conservative therapeutic strategies for patients with different conditions, preserve the uterus as much as possible and protect the patient's fertility, which will play an important role on the follow-up assisted reproductive treatment and long-term management of adenomyosis.

Abstract:Objective: Adenomyosis is characterized by the presence of endometrium or endometrium-like glands and stroma within the myometrium. In this study, we aimed to investigate whether the cGAS-STING pathway was activated and correlated with clinical outcomes in adenomyosis patients. Materials and methods: Twenty patients diagnosed with adenomyosis and 10 patients diagnosed with cervical intraepithelial neoplasia grade 3 (CIN-3) but no adenomyosis were enrolled in this study. Specimens were collected during surgery from August 2017 to December 2017 at Third Xiangya Hospital. The messenger RNA (mRNA) and protein levels of key cGAS-STING pathway factors in uterine tissue were detected by real-time reverse-transcription polymerase chain reaction and immunohistochemistry, respectively. The correlations of gene expression and clinical outcomes, including dysmenorrhea and uterine volume, were analyzed. Results: The cGAS, STING, TANK-binding kinase 1 (TBK-1), interferon-α (IFN-α), IFN-β, and tumor necrosis factor-α (TNF-α) mRNA and protein levels in the ectopic endometrial tissue from adenomyosis patients were significantly higher compared with that from the controls in endometrium (p < .05). cGAS and STING gene expression were correlated with TBK-1, IFN-β, and TNF-α expression (p < .05). Importantly, TBK-1 and TNF-α expression were correlated with the clinical outcome of dysmenorrhea (p < .05). Conclusion: Our study reveals that the cGAS-STING pathway is activated in adenomyosis patients and its activation is subsequently correlated with clinical outcomes, which suggests that the cGAS-STING pathway may contribute to adenomyosis pathogenesis.

Abstract:Objective:To investigate the clinical significance of laparoscopic cytoreduction of adenomyosis and laparoscopic uterus nerve ablation in the treatment of adenomyosis.Methods:15 patients with adenomyosis in this hospital from May 2004 to August 2005 which received adenomyosis cytoreduction plus uterosacral ligament amputation were selected.Operation condition,effectiveness of releasing pain,postoperation follow-up were investigated.Results:After 3~15 months' observation,these patients got great alleviation from severe dysmenorrhea and chronic abdominal pain.Conclusion:Laparoscopic cytoreduction of adenomyosis and laparoscopic uterus nerve ablation is effective in the treatment of young patients suffering from adenomyosis.

Abstract:Hunan's famous traditional Chinese physician Wang Aihua thinks that the deficiency of stomach Qi is the primary factor of ulcerative colitis, while damp-heat was the symptomatic factor, heat-toxicity causing bloody stool, blood stasis is the local pathological change, intestine sores is local pathological characteristics. The long illness will damage kidney, caus-ing the dificiency of both spleen and kidney. She proposes that invigorating spleen for digestion is the prima-ry method, harmonizing Qi and blood is the auxiliary method, while clearing heat and promoting diuresis in the whole course of ulcerative colitis.
