The Relationship Between Ambulatory Pulse Pressure Coherent Index and Early Renal Injury in Elderly Patients with Primary Hypertension

Abstract:Objective To study the predictive value of the ambulatory pulse pressure related indexes,derived from ambulatory blood pressure monitoring on the renal impairment in elderly patients with essential hypertention.Methods One hundred and four patients(male 56 and female 48)with essential hypertention were accessed according to 24hPP(≥55 or 55),PPI(≥0.36 or 0.36) and AASI(≥0.30 or 0.30) respectively in relation to differences of glomerular filtration rate(GFR).Microalbuminuria,plasma cysteine.The prediction importance of 24hPP,PPI and AASI for early renal impairment was accessed by ROC cureve.The related risk factors for renal function impairment was tested by multiple stepwise regression analysis.Results GFR were significantly lower in those with 24hPP≥55 mm Hg,PPI≥0.36 and AASI≥0.30(P0.05).AUC of ROC curve of 24hPP,PPI and AASI were 0.636,0.644 and 0.656 respectively.Multiple linear regression analysis showed a inverse correlation in GFR with age(r=-0.367,P=0.001),course of hypertension(r=-0.371,P=0.001),24hPP(-0.254,P=0.009)、PPI(-0.262,P=0.007)and AASI(-0.316,P=0.001) Conclusions With the elevation of 24hPP,PPI and AASI,GFR is significantly decreased.Age and hypertension duration are closely related to impairment of renal function.
