Osteoid osteoma of the cervical spine:diagnosis and surgical strategy

Abstract:Objective:To discuss the diagnosis and surgical strategy of the osteoid osteoma in cervical spine. Method:A retrospective review of patients with osteoid osteoma in cervical spine operated in our hospital from December 1999 to June 2008 was performed.6 patients including 3 male and 3 female had a mean age of 17.8 years-old(range,7-37 years-old) .All patients complained of long-term neck pain.Three patients had pain relieved after non-steroidal anti-inflammatory medicine.Two patients presented with marked torticollis. Three patients had arm pain and numbness.Tumors were identified by X ray film in only 2 cases.All lesions were revealed by computed tomography(CT) which revealed a typical well-demarcated lytic lesion surrounded by a distinct zone of sclerosis.The lesions had different degrees of radiographic density.Bone scan were taken in 4 patients,of which all lesions showed high fluorine 18 fluorodeoxyglucose(FDG) uptake.Lesion located in C1 lateral mass in one patient,while the others located in lower cervical spine including C5 1 case,C6 3 cases and C7 1 case.Five lesions had posterior structures and one had vertebral body involved.Five tumors in lower cervical spine were excised marginally,which included four posterior approach plus lateral mass fixation and one anterior approach plus interbody fusion and fixation.The tumor in C1 lateral mass was curetted in-tralesionally and grafted fusion in the cavity.Result:The mean surgical time was 2.8h(range,1.5~4.5h) ,the av-erage intraoperative blood loss volume was 233ml(range,50~400ml) . All patients were relieved from symptoms after surgery.Torticollis was corrected and the neurological defect disappeared.No surgery related complications occurred.Tumor recurrence was screened by CT scan in all cases.No tumor recurrence occurred during the follow-up(mean 42.5 months ranging from 12 to 108 months) ,no fixation failure occurred.One removed the posterior fixation 2 years after the surgery without neck pain and kyphosis.Two gafts were fused in the 6-month follow-up.Conclusion:Osteoid osteoma in cervical spine is prevalent in youth.Neck pain is the mostcommon symptom.Torticollis and neurological defect can also be found in some cases.CT combined with bone scan is important for the diagnosis.Surgery is indicated if one has long term neck pain,neurological defect and torticollis.Marginal resection of the tumor is an effective procedure.
