Abstract:Wilms' tumour is the most frequent renal tumour in children. Based on the SIOP strategy, children with Wilms' tumour may benefit from preoperative chemotherapy, but few publications address the effect of preoperative transcatheter arterial chemoembolization (TACE) on patients with Wilms' tumours. The aims of this study were to investigate the prognostic relevance of preoperative TACE followed by tumour resection, proliferating cell nuclear antigen (PCNA) and vascular endothelial growth factor (VEGF) expression in patients with Wilms' tumours. Two therapeutic strategies including tumour resection only and TACE, followed by tumour resection were conducted in a cohort of 44 patients with Wilms' tumours. Clinical and follow-up data was analysed. Immunohistochemistry staining was used to explore PCNA and VEGF expression in the Wilms' tumour. Two years tumour-free survival of the patients in the TACE group was significantly higher than that of the patients in the control group (P < 0.001) and recurrence and cases of death within one year in the TACE group was markedly lower than that in the control group (P < 0.001). Fifty-five percent of patients in the control group were PCNA-positive vs. 4.17% of patients in the TACE group (P < 0.001). Fifty percent of patients in the control group were VEGF-positive vs. 29.17% of patients in the TACE group (P > 0.05). Patients with Wilms' tumours benefited from preoperative TACE treatment. PCNA expression was significantly lower in patients in the TACE group than those in the control group. There was no significant difference on VEGF expression between the patients in TACE and control groups. Eur J Clin Invest 2008; 38 (12): 931-938.
Assessment of Hepatocellular Carcinoma Undergoing Trans-Catheter Arterial Chemoembolization with Functional Magnetic Resonance Imaging

Abstract:Objective To analyze the feasibility of functional magnetic resonance imaging index in evaluating the effec tiveness of the trans-catheter arterial chemoembolization(TACE) for hepatocellular carcinoma.Methods Thirty patients with primary hepatocellular carcinoma were undergoing functional magnetic resonance imaging before and after hepatic artery chemoembolization therapy.Finally,we compared the parameters before and after TACE.Results There was a significant difference (P ＜ 0.05) in Cho to lipid (Cho/Lip) ratio which was obviously decreased after TACE.The volume transfer constant (K) value 、interstitium-to-plasma rate constant (kep) value and hepatic arterial perfusion index (HPI)were lower and the apparent diffusion coefficient (ADC) value was higher after treatment,there was a significant difference before and after TACE (P ＜ 0.05).The Kt reduction demonstrated a positive correlation with tumor necrosis (r =0.751,P ＜ 0.01).ADC demonstrated a negative correlation with Cho/Lip (r =-0.386、-0.407,P ＜ 0.05) before and after hepatic artery chemoembolization therapy.Conclusion The fMRI indexes of ADC、Ktrans 、kep 、HPI values and Cho/ Lip ratio had high value in assessing the effectiveness of hepatocellular carcinoma by transcatheter arterial chemoemboliza tion,and great significance in predicting the curative effect.
Effect of transcatheter arterial chemoembolization on angiogenesis and expression of vascular endothelial growth factor in cervical squamous cell carcinoma

Abstract:Objective: To investigate the clinical significance of the expression of vascular endothelial growth factor (VEGF) and intratumoral microvessel density (MVD) in survival cervical squamous cell carcinoma tissues after transcatheter arterial chemoembolization (TACE). Methods: The surgical specimens from 46 cervical squamous carcinoma patients' were stained immunohistochemically for VEGF and MVD expression, which included 24 cases treated with surgical resection alone (control group) and 22 cases undergoing 1-2 times TACE therapy prior to II-phase surgical resection (TACE group), MVD was calculated by labing the endothelial cells of the blood vessels within the tumor by F VII RAb. Results: The positive expression of VEGF is higher in TACE group (90.9%) than that in control group (66.7%). There was a positive correlation between them (P<0.05). The MVD (>30) was 90.9% in TACE group and 50.0% in control group. There was significant difference between the two groups. Conclusion: VEGF was highly expressed in survival cervical squamous cell carcinoma tissues after TACE, and the MVD also enhanced. VEGF may play an important role in angiogenesis to the survival of cervical squamous cell carcinoma tissues.
[Clinical Effect of Terminal Branches Portal Vein Embolization Combined with Transcatheter Arterial Chemoembolization on Liver Neoplasms].

Abstract:Objective: To analyze the efficacy of branches portal vein embolization (TBPVE) combined with transcatheter arterial chemoembolization (TACE) on liver neoplasms. Methods: From August 2016 to May 2017, there were 13 patients including 11 males and 2 females with primary hepatocellular carcinoma who underwent TBPVE+ TACE , among whom there were 11 cases with a history of HBV infection.Average age of the 13 patients was (60.8±6.2)years. The live function of all patients were Child-Pugh A classification.The CT or MRI images of each patient was reconstructed and the standard liver volume(SLV) before TBPVE+ TACE was (1 181.2±49.3)ml, estimated future liver remnant(FLR) was (326.1±72.1)ml and FLR/SLV was (27.6±6.0)%.The puncture site for TBPVE was determined by the three-dimensional reconstruction of portal vein.CT scan or MRI, AFP and liver function test were repeated after one and two weeks after TBPVE+ TACE.FLR and FLR/SLV were calculated respectively.Hepatectomy would be performed if the patients agreed.The postoperative complications were analyzed. Results: On the 7thday after TBPVE+ TACE, the FLR/SLV was(42.6±8.0)% and the FLR increasement was(56.0±24.6)%.The level of AFP decreased from(87.9±81.8)μg/L to (29.7±20.9)μg/L.On the 14thday after TBPVE+ TACE, the FLR/SLV was(45.8±6.2)% and the FLR increasement was(71.8±29.0)%.Four patients underwent surgery which including 2 right hepatectomies and 2 right trisegmentectomies in 2 weeks after TBPVE+ TACE.Nine patients were performed with targeting intratumoral lactic acidosis TACE (TILA-TACE). No severe complication occurred in all patients. Conclusions: TBPVE could induce a rapid growth of the liver remnant but still with the concern of inducing the growth of neoplasms at the same time.To combine TACE in TBPVE therapy not also can the growth of neoplasms be prevented but also inducing its shrinking.This method might be a new mode for the treatment of hepatocellular carcinoma.

Abstract:From 2003 to 2013, 55 patients (median age 3.3 years; 29 males, 26 females) with unresectable, metastatic, or diffuse anaplastic histology (AH) Wilms tumor were treated with neoadjuvant transcatheter arterial chemoembolization (TACE) and systemic chemotherapy. Characteristics of patients were maximal tumor diameter greater than 10 cm, involvement of periaortic lymph nodes, tumor thrombus in inferior vena cava/right atrium, distal metastasis, or diffuse AH. The chemoembolic emulsion for TACE consisted of pirarubicin, vindesine, and iodized oil. For the tumor with distal metastasis or diffuse AH, cisplatin was added in the chemoembolic emulsion. Intravenous chemotherapy with vindesine and actinomycin D was administered 2-3 weeks after TACE. For the patients with distal metastasis or diffuse AH Wilms tumor, intravenous chemotherapy consisted of ifosfamide and etoposide. Nephrectomy was performed 2-3 weeks after preoperative combination therapy. Surgical stage was assigned according to local operative findings in terms of the National Wilms Tumor Study (NWTS) Group combined with the pretherapeutic imaging to define metastatic disease. Postoperative treatment was based on tumor histology and surgical stage. All patients were followed up for 17-141 months (median: 82 months). No cardiotoxicity, renal insufficiency, and hepatic dysfunction after neoadjuvant TACE and systemic chemotherapy were found. Oral mucositis developed in 5 patients, grade I-II marrow suppression developed in 12 patients, and 19 patients became moderately febrile. In terms of response evaluation criteria in solid tumors, partial response (PR) in 34 (61.8%), stable disease (SD) in 19 (34.5%), and progressive disease (PD) in 2 (3.6%) patients were observed. Four of five patients had complete regression of inferior vena cava tumor thrombus. Atrial tumor thrombus retreated to inferior vena cava in one of two patients. Distant metastasis disappeared in four of six cases. Fifty patients (90.1%) underwent complete tumor resection. Tumor spillage occurred in 3 patients (5.5%). Two patients (3.6%) had microscopic residual disease. Surgical stages were stage II in 25, stage III in 24, and stage IV in 6 patients. On pathologic examination, tumor necrosis was >90% in 14 (25.5%), 50%-90% in 23 (41.8%), and <50% in 18 cases (32.7%). The 5-year event-free survival was 92.7%, and the overall survival was 94.5%. These preliminary results suggest that the use of neoadjuvant TACE and systemic chemotherapy may provide a promising choice in the treatment of unresectable, metastatic, or diffuse AH Wilms tumor in children. Further investigations are necessary.
Evaluation of the Curative Effect of Transcatheter Arterial Chemoembolization Via Hepatic Artery Combined with Percutaneous Ethanol Injection for the Treatment of Primary Hepatocellular Carcinoma

Abstract:Objective To investigate the curative effect of transcatheter arterial chemoembolization (TACE) combined with percutaneous ethanol injection (PEI) for the treatment of primary hepatocellular carcinoma.Methods Sixty-one cases with primary hepatocellular carcinoma were divided into group A and group B. In group A, 34 cases were treated with TACE, and 27 cases were treated with TACE and PEI in group B.Results The complete necrosis rate of tumor, the reduction rate of tumor, and one-year death rate were 26.5%, 61.8% and 35.3%, respectively in group A, and 92.6%, 92.6% and 3.7%, respectively in group B. There were significant statistical differences between the two groups (P value were 0.000, 0.005 and 0.004, respectively).Conclusion The curative effect of TACE combined with PEI shows better results than that of TACE alone.
Comparative Evaluation of Preoperation Transcatheter Arterial Chemoembolization in Children with Advanced Wilms Tumor

Abstract:Objective: To evaluate the therapeutic effect and prognosis of preoperative transcatheter arterial chemoembolization (TACE), transcatheter arterial chemoembolization (TAIC), preoperative intravenous chemotherapy for children with advanced stage nephroblastoma. Methods: From January 2007 to December 2018, according to different treatment protocols before surgery, children with nephroblastoma were divided into 3 groups, which were TACE group (44 cases), TAIC group (7 cases) and intravenous chemotherapy group (9 cases) in Children's Hospital, Zhejiang University School of Medicine. The imaging examination, treatment safety and long-term efficacy of these three groups before and after treatment were compared. Observed indicators include tumor debulking rate, envelope integrity rate, necrosis rate, postoperative adverse reactions and follow-up. Results: Tumor debulking rate: tumor of TACE group decreased 46.5%, TAIC group shrinked 28.3%, and intravenous chemotherapy group reduced by 23.3%, the difference of tumor shrinkage between TACE group and intravenous chemotherapy group was statistically significant (P<0.05). Tumor necrosis rate: necrotic area in TACE group was about 46.0%-95.4%, average 75.1%±12.5%, while in intravenous chemotherapy group was 65.8%±8.7%, the difference was statistically significant (P<0.01). The tumor membrane integrity rate of these three groups were 86.4%(38/44),5/7 and 6/9 respectively, and difference between TACE group and intravenous chemotherapy group was significant (P<0.05). Patients who had myelosuppression each was 5 in TACE group (5/44), 4 in TAIC group (4/7), and 8 in intravenous chemotherapy group (8/9), and there were significant differences. Follow-up time and tumor-free survival rate in each group were respectively 20 to 92 months (median time 64 months) and 95% in TACE group, 12 to 69 months (median time 30 months) and 43.0% in TAIC group, and 16 to 72 months (median 28 months) and 56.0% in intravenous chemotherapy group. Conclusions: Preoperative TACE can lead to tumor shrinkage and necrosis more obviously, systemic adverse reactions are small, also the tumor complete resection rate is higher and the operation is safer.The survival rate can be effectively improved and more suitable for clinical treatment.

Abstract:Purpose: To evaluate the therapeutic effect of preoperative transcatheter arterial chemoembolization (TACE) combined with short-term systematic chemotherapy in the treatment of advanced Wilms turner.Materials and Methods: This was a retrospective study on 66 patients with unilateral advanced Wilms tumor, age 5 months to 11 years (median, 2.9 years; 30 boys and 36 girls), treated at our institution between 1995 and 2007. Characteristics of the patient population were maximal tumor diameter > 10 cm, or involvement of periaortic lymph nodes, or inferior vena cava invasion, or distal metastasis, or tumor with anaplastic histology. Patients were divided into three groups. Twenty patients were treated with conventional preoperative chemotherapy (PC group) using vindesine, actinomycin D, and pirarubicin for 4 weeks; 21 patients were treated in the TACE group with preoperative renal arterial chemoembolization using Lipiodol-pirarubicin-vindesine emulsion; and 25 patients were treated with preoperative chemoembolization combined with short-term systematic chemotherapy (T+S) for 2 weeks.Results: No drug-induced cardiotoxicity, nephrotoxicity, or hepatic dysfunction was observed. Complete surgical removal of the tumor was achieved in 12 (65.0%), 17 (80.9%), and 22 (88.0%) patients in the PC, TACE, and T+S groups, respectively (T+S group vs PC group, P = .030). The 2-year relapse-free survival rates were 65.0%, 80.9%, and 100.0% in the PC, TACE, and T+S groups, respectively (T+S vs PC, P = .001).Conclusions: From our experience, preoperative chemoembolization combined with short-term systematic chemotherapy is able to achieve higher rates of complete tumor resection and relapse-free survival in the treatment of advanced Wilms tumor.

Abstract:OBJECTIVE:To improve prognosis of the patients with advanced Wilms' tumor, the authors compared different therapeutic strategies including preoperative transcatheter arterial chemoembolization (TACE), conventional preoperative chemotherapy and initial surgery.METHODS:Sixty-two patients aged from 5 months to 10 years (mean 3.2 years) were identified from medical records to have histologically confirmed advanced Wilms' tumor during the period from January 1993 to December 2002. The criteria for choice were huge tumor size with a volume more than 550 ml or the mass extending beyond the midline, involvement of vital structures, inferior vena cava invasion, distal metastasis or bilateral Wilms' tumor judged by imaging examination. All cases were divided into 3 groups according to the treatment received: 31 cases in group TACE received preoperative transcatheter arterial chemoembolization with Lipiodol-Epirubicin (EPI)-Vincristine emulsion. One week after TACE, systemic chemotherapy with Actinomycin D (ACTD) was administered and tumor resected at two weeks after TACE. 20 cases in group PC received conventional preoperative chemotherapy with VCR, ACTD plus EPI for 4-5 weeks, and 11 cases in group IS underwent initial surgery. Postoperative treatment for all patients was based on the postoperative staging and tumor histology.RESULTS:In the patients treated with TACE, no drug-induced complications such as cardiotoxicity, nephrotoxicity, hepatic dysfunction or bone marrow suppression were observed except for mild fever due to tumor necrosis. The percentages of tumor size shrinkage were 32.4% and 20.3% in group TACE and group PC, respectively (P < 0.05). Complete surgical removal of the tumor was achieved in 27 patients (87.1%) in group TACE, significantly higher in comparison with 14 in group PC (70.0%, P < 0.05) and 2 in group IS (18.2%, P < 0.01). Event-free survival (EFS) at 2 years was 87.1% (27/ 31), 60.0% (12/20) and 18.2% (2/11), respectivrely. EFS at 4 years was 84.6% (11/13), 56.3% (9/16 ) and 18.2% (2/11) in groups TACE, PC and IS, respectively.CONCLUSION:The present study has shown that both preoperative TACE and conventional preoperative chemotherapy can be applied to the patients with advanced Wilms' tumor who are not candidates for immediately surgical resection. The survival is significantly increased in the patients undergoing preoperativeTACE when compared with conventional preoperative chemotherapy and initial surgery.
Effect of preoperative adjuvant transcatheter hepatic arterial chemoembolization on prognosis of resectable huge hepatocellular carcinoma

Abstract:OBJECTIVE: To assess the effect of preoperative adjuvant transcatheter hepatic arterial chemoembolization on prognosis for resectable huge hepatocellular carcinoma. METHODS: The clinical and pathologic data of 176 patients with huge hepatocellular carcinoma who had undergone surgical resection were retrospectively reviewed, and all the patients were assigned into a TACE group (n=51) or non-TACE group (n=125) according to whether preoperative adjuvant TACE had been performed or not. The subgroup was defined by using risk factors for residual tumor such as portal vein branches tumor thrombus and satellite nodules, and the overall survival rates between the different groups were compared. RESULTS: The 1-, 3-, 5-year overall survival rates of the TACE group and non-TACE group were 65.60%, 43.00%, 30.70% and 52.90%, 30.60%, 25.10%, respectively, and there was no statistically significant difference bwteen two groups (P=0.214), but for those patients with risk factors for residual tumor, the 1-,3-year overall survival rates of the TACE group and non-TACE group were 58.00%, 21.70% and 35.90%, 9.10%, respectively, and there was a statistically significant difference between two groups (P=0.027). CONCLUSIONS: For the patients with resectable huge hepatocellular carcinoma, the preoperative adjuvant TACE should be applied selectively. Only for those patients with portal vein branches tumor thrombus or satellite nodules, the preoperative adjuvant TACE can help prolong the survival time after resection.
[Effect of Postoperative Transcatheter Arterial Chemoembolization on Hepatocellular Carcinoma Patients with Residual Tumor].

Abstract:OBJECTIVE:To evaluate the effect of postoperative adjuvant transcatheter arterial chemoembolization (TACE) on hepatocellular carcinoma (HCC) patients with residual tumor.METHODS:The patients were classified into intervention group (with adjuvant TACE) and control group (without adjuvant TACE) who were further stratified to those with high risk (patients with single tumor > 5 cm in diameter, or with multiple tumors, invasion to blood vessels), and low risk factors. Univariate analysis and Cox model were used to analyse prognostic factors.RESULTS:In low risk patients with residual tumor, the 1-, 2-, 3-, 4-year survival rate was 97.2%, 78.0%, 66.5% and 66.5% in the intervention group, and 91.2%, 81.4%, 70.3% and 54.4% in the control group, respectively. There was no statistical difference between the two groups in survival (log-rank P = 0.7667). Comparing with the control group, the 1-, 2-, 3-, 4-year survival rate was 89.5%, 73.4%, 59.2% and 53.8% in the intervention group, and 70.5%, 61.9%, 46.8% and 46.8% in the control group, respectively. Postoperative adjuvant TACE significantly prolonged the survival in high risk patients with residual tumor (P = 0.0029). Cox model revealed that the benefit of adjuvant TACE was significantly increased by the high risk factors in HCC patients with residual tumor.CONCLUSION:The beneficial effect of postoperative TACE was only observed in high risk patients with residual tumor but not in the low risk patients with residual tumor.
Effect of chemotherapeutics and embolization on tumor inhibition and liver injury in TACE treatment of hepatic carcinoma

Abstract:Objective To discuss the effect of chemotherapeutics and embolization on tumor inhibition and liver injury in TACE treatment of hepatic carcinoma.Methods AFP and liver function were investigated and evaluated in 152 HCC patients who had been treated by TAC(group A,n=30) or TAE(group B,n=29) or TACE(group C,n=93) without extra-liver metastasis.Results At the 1std and 4thd after TACE,(1)the rate of AFP decrease were higher in group B(56.5%,78.3%) and group C(67%,80.7%) than that in group A(25%,25%),P<0.01.(2)the abnormal rate amplification,bilirubin total was higher in group A(119.54%,116.1%) and group C(157.8%,140%) than that in group B(41.3%,28%),P<0.001.ALT and ALB were higher both in group B and group C than that in group A,P<0.005.Pre-albumin was higher in group C(17.5%,114.4%)than that in group A(3.3%,0) and group B(3.5%,29.4%),P<0.005.Conclusion It is chemoembolization that play an important role in tumor inhibitory action in TACE treatment.The increase of chemotherapeutics dosage accompanied exacerbation of liver injury.It suggested that the control of best combining site both increase tumor inhibitory action and decrease liver injury is key point to evaluate therapeutic effect of TACE.
Pathological Alterations of Wilm′s Tumor after Interventional Chemotherapy

Abstract:Purpose To discuss the effect of interventional chemotherapy on the pathological change of Wilm's tumor. Methods A comparative analysis of pathological findings was made in 30 cases of Wilm's tumor subjected to interventional chemotherapy before operation, 16 Wilm's tumors with chemical therapy before operation, 17 Wilm's tumor tissues without any therapy before operation and 20 Wilm's tumor tissues obtained by needle biopsy. TUNEL staining and SP immunohistochemical staining were used to detect the tumor cell apoptosis and the expression of PCNA in every 15 tissues of the former three groups. Results In the interventional therapy group, the average thickness of pseudo fibrous capsule was 2.4±0.7 mm, the ratio of the average necrosis area to the whole tumor area was 77.5%, and 93.3 per cent of middle vessels became occlusion. In addition, yellowy lipiodol emulsion left and lots of foaming histiocytes infiltration were observed in all the tumor cases; in the chemical therapy group, the average thickness of pseudo fibrous capsule was 2.1±0.9 mm, the ratio of the average necrosis area to the whole tumor area was 51.2%, 43.8 per cent of middle vessels became occlusion; in the only operation group, the average thickness of pseudo fibrous capsule was 1.1±0.4 mm, the ratio of the average necrosis area to the whole tumor area was 14%, and no middle-sized vessels became occlusion. Compared with the histological changes of needle biopsy in interventional group before and after operations, necrosis was seldom observed in mesenchymal type but prominent in other type, especially in embryonic type. The tumor proliferating activity decreased significantly in the interventional therapy group than in the only operation group. On the other side, the apoptotic cells increased significantly but no difference from that in chemical therapy group. Conclusions Interventional chemotherapy before operation can promote vessel necrosis and occlusion, massive tumor necrosis with fibrous tissue proliferation and pseudo fibrous capsule formation; Yellowy lipiodol emulsion left and a lot of foaming histiocytes infiltration can help kill tumor cells; Histologic type of tumors is related to therapy sensitivity; Interventional therapy can inhibit tumor development by suppressing tumor cell proliferation and inducing tumor cell apoptosis.
Apoptosis, proliferation and angiogenesis of tumor cells after TACE treatment with As2O3 and Lipiodol

Abstract:Objective To investigate the effect of transcatheter arterial chemoembolization (TACE) with As_2O_3 and Lipiodol on apoptosis, proliferating cell nuclear antigen (PCNA) expression and angiogenesis of hepatic tumor. Methods Thirty-two New Zealand White rabbits were randomly divided into four groups and VX2 carcinoma was implanted in the left lobes of the livers. Two weeks later, a catheter was inserted into the hepatic artery of rabbit with VX2 hepatic tumor and infusion was performed via the hepatic artery with physiological saline, Lipiodol, Lipiodol-ADM and Lipiodol-As_2O_3 emulsion. The apoptotic index, proliferation index and values of MVD in survival tumor tissue were examined 1 week after treatment by TUNEL (in situ nick end-labeling) method and immunohistochemistry with anti-CD31, anti-PCNA monoclonal antibody respectively. Results Apoptotic index of survival tumor cells was 1.53±0.42, 2.66±0.54, 2.91±0.32 and 3.44±0.65, proliferation index was 60.8±15.5, 42.4±11.2, 40.6±8.8 and 28.5±5.7 in each group respectively. There is a negative correlation between apoptotic index and proliferation index. MVD was slightly higher in Lipiodol embolization group (23.4±4.7) and ADM plus Lipiodol treatment group (22.4±5.3) versus physiological saline group (21.8±6.3, P0.05), MVD was significantly lower in As_2O_3 plus Lipiodol embolization group (15.1±3.2, P0.05). Conclusion TACE using As_2O_3 and Lipiodol can induce apoptosis, decrease proliferation of residual tumor cells and inhibit the development of neovascularization.
Preoperative Interventional Therapy for Children with Advanced Wilms' Tumor

Abstract:Objective To study the effects of preoperative interventionaltherapy on children with large or advanced Wilms' tumor.Methods Ten patients with large or advanced Wilms' tumor were performed on preoperative interventional chemoembolization with adriamycin, iodized oil and gelatin sponge, and underwent operation two weeks later.Results The patients had fever, but no bone marrow depression or hepatic, renal and cardiac dysfunction after interventional therapy. The size of tumor was shrinkage. It was less bleeding and easier to achieve a complete resection. "Downstaging" and necrosis of the tumor were found in some cases.Conclusions The advantages of preoperational interventional therapy are lower dosage of chemotherapy, less side effects and obvious tumor necrosis. It could be applied as preoperative therapy or adjuvant chemotherapy making surgical resection possible. Because of the "Downstaging" expression, it is difficult in a histodiagnosis in some cases. Percutaneous fine needle biopsy may be helpful to a definite histology.
Expression of PCNA and Nm23-H1/ndpk in Hepatocellular Carcinoma Following Transcatheter Arterial Chemoembolization Therapy

Abstract:AIM:To study the effect of transcatheter arterial Chemoembolization (TACE) on the proliferation and metastasis of the hepatocellular carcinoma (HCC). METHODS:Seventy-two patients with inoperable huge HCC administered from 1992 to 2000 were treated with TACE initially, and then operated following the shrinkage of the tumor. During same periods of treatment, there were comparable 487 HCC patients who were not treated with TACE, of which 32 pairs of patients who met with the criteria were selected for this study based on the 10 items screening criteria including gender, age, history of the disease, BUS examination. Expression of proliferating of cell nuclear antigen (PCNA) and nm23-H1 were studied by immunohistochemistry. RESULTS:PCNA label index (PCNA LI) of HCC following TACE was elevated. The distribution of PCNA LI in the range 25 %, 26-50 %, 51-75 % and 75 % is 75 %, 14.3 %, 10.7 % and 0 % in control group, and 35.7 %, 21.4 %, 28.6 % and 14.3 %, in TACE group (P <0.01, n =28), respectively. The expression of nm23-H1 gene exists in both cytoplasm and nuclear with no significant difference between treatment and control group. CONCLUSION:The proliferative activity of residual tumor of HCC following TACE causes the elevation of PCNA LI implying the poor prognosis of long-term effect of TACE, and therefore, it is necessary to perform two stage-resection after TACE of HCC.
Influence of Chemotherapy on Apoptosis and Necrosis of Wilms' Tumor Cells

Abstract:Objective The aim of study is to investigate the influence of chemotherapy on apoptosis and necrosis in Wilms' tumor. Methods From 1990 to 1998, 44 children with Wilms' tumor were randomized into two groups. Group A ( n =26) underwent preoperative chemotherapy with Dacinomycine, Vincrinstine and Cyclophosphamide. Eight of them had additional chemo-embolization with iodized oil and Adramycin via renal artery. Group B ( n =18) underwent nephrectomy followed by chemotherapy. Utilizing the technique of TUNEL and routine pathological methods, the status of apoptosis and necrosis were determined in all specimens. Results The Apoptotic Index (AI) of group A was significantly higher than that of group B ( P 0.05 ). Within group-A patients, AIs of those who had more than 14 days of chemotherapy were higher than the AI of those less than 14 days. The AIs between those who died and those who survived for two or more years, between those who had additional chemo-embolization and those who did not, between those diagnosed as FH and as UH, were not statistically significant. The difference in rates of complete tumor necrosis were statistically significant between group A and group B patients ( P 0.05 ). The difference between those who had additional chemo-embolization and those who had not was also statistically significant ( P 0.05 ). Conclusions Chemotherapy can produce significant apoptosis and necrosis in Wilms' tumors. Preoperative chemotherapy and additional chemo-embolization can lead to more necrosis. AI of Wilms' tumor may not be as a prognostic factor but can be used as an indicator of the efficacy of chemotherapy.
Effects of transarterial chemoembolization on the immunological function of patients with hepatocellular carcinoma

Abstract:The present study aimed to investigate the effects of transarterial chemoembolization (TACE) on the immune function of patients with hepatocellular carcinoma (HCC). A total of 114 patients with HCC were selected and their peripheral blood was collected before and 1 month after TACE treatment. Flow cytometry and reverse transcription-quantitative PCR were performed to analyze the changes in immune function in patients before and after treatment. Kaplan-Meier curves were plotted for survival analysis. The programmed cell death ligand 1 (PD-L1) and programmed cell death protein 1 (PD1) expression before TACE treatment were significantly higher in patients with poor TACE response compared with those patients with well response. Higher PD-L1 mRNA expression in the peripheral blood mononuclear cells after TACE predicted a superior prognosis. After TACE treatment, the proportion of CD4+/CD8+ cells were decreased while the expression levels of programmed cell death protein 1 (PD1) were significantly increased. To conclude, TACE could reduce the proportion of CD4+/CD8+ cells and improve the mRNA expression levels of PD1 in patients with HCC. The expression levels of PD1 and PD-L1 were closely related to the therapeutic effect of TACE and the prognosis of patients with HCC. TACE combined with immunotherapy may have potential clinical value for patients with HCC.

Abstract:PURPOSE:To explore the influence of transcatheter hepatic arterial chemoembolization (TACE) combined with microwave ablation on T lymphocyte subsets and prognosis in patients with liver cancer and analyze the safety. METHODS:The clinical data of 160 patients were retrospectively analyzed. The patients in the control group underwent TACE, while those in the observation group were treated with microwave ablation in addition to TACE. Then, the changes in the levels of T lymphocyte subsets, liver function indexes and alpha-fetoprotein (AFP) before and after treatment were compared between the two groups. RESULTS:After treatment, the levels of cluster of differentiation 3+ (CD3+), CD4+ and CD4+/CD8+ of patients were significantly increased in both groups, while the levels of CD8+ and serum alanine aminotransferase (ALT), aspartate aminotransferase (AST) and AFP were overtly lowered, while the observation group had more obvious changes in the above-mentioned levels (p<0.05). The response rate and prognostic survival rate were evidently higher in the observation group than in the control group (p<0.05). The complications after treatment showed no significant difference between the two groups (p>0.05). Child-Pugh, tumor diameter and number of lesions were independent risk factors for the prognosis of survival of liver cancer patients undergoing TACE combined with microwave ablation (p<0.05). CONCLUSIONS:TACE combined with microwave ablation has relatively high efficacy and safety in the treatment of liver cancer, which can improve liver function, immune function and prognostic survival.

Abstract:Objective: To compare the effect of the frequency of transcatheter arterial chemoembolization(TACE) on preventing tumor recurrence after hepatectomy.Methods: A total of 45 post-operative patients who had received prophylactic TACE once or thrice were retrospectively examined between January 2008 and June 2009.Of the 45 patients,23 underwent TACE once,and the others underwent it thrice.TACE was administered to all patients via the hepatic artery one to two months after operation and was repeated every two to four months with patients who underwent TACE three times.All cases were followed up for 36 to 40 months after surgery.The rates of cumulative recurrence between the two groups were compared.Results: In the group that underwent TACE once,the 1-,2-and 3-year cumulative recurrence rates were 30.43%,47.83%,and 47.83%,respectively.In the group that underwent TACE thrice,the 1-,2and 3-year cumulative recurrence rates were 4.55%,27.27%,and 36.36%,respectively.Statistical analysis showed that the relapse rate within one year was lower in the group that underwent TACE thrice than in the group that underwent TACE only once(P=0.022).However,no significant difference in the cumulative recurrence rate was found between the two groups in two and three years(P=0.086,0.225).Conclusion: Hepatocellular carcinoma patients who undergo preventive TACE three times after hepatectomy exhibit reduced recurrence rates during the peak time of tumor recurrence and extended disease-free survival intervals.
