Abstract:【Objectives】To analyze the potential advantages of harmonic scapel focus on laryngeal neuroprotection in complex thyroid operation.【Methods】A retrospective study was conducted to analyze patients underwent complex thyroid operation,from September 2009 to December 2012,who were divided into group focus and electric scapel according to auxiliary device.The primary endpoint was laryngeal nerve impairment including superior laryngeal nerve and recurrent laryngeal nerve.【Results】Two groups of patients including retrosternal thyroid operation 54 cases,re-thyroid operation 48 cases,thyroidectomy for upper nodular goitre 88 cases,were 190 patients in group electric scapel and 125 patients in group Focus harmonic scapel.Compared to group electric scapel,the rate of superior laryngeal nerve impairmentrate of re-thyroid operation was 6.7%(2/30) vs.10.4%(5/48),and thyroidectomy for upper nodular goitre was 5.8%(3/52) vs.18.6%(16/88),respectively(P <0.05).Four patients of permanent impairment were in group electric scapel,and one was in group focus harmonic scapel.The Focus was less tempary recurrent laryngeal nerve impairment rate than electric scapel,4%(5/125) vs.10.53%(20/190)(P <0.05).Four patients was forever hoarseness in group electric scapel,and none was in group harmonic scapel.【Conclusions】In contrast to electric scapel,focus adds further benefits to laryngeal neuroprotection in complex thyroid operation achieved by Harmonic technology.Harmonic scapel focus is an useful operation instrument,which has great advantage on laryn geal neuroprotection.
