Clinical Analysis of 27 Cases with Cholesteatmas in the Cerebellopontine Angle Region Presenting as Trigeminal Neuralgia

Abstract:BACKGROUND & OBJECTIVE : Cholesteatomas is the common tumor found in the cerebellopontine angle and is the most common cause of secondary trigeminal neuralgia. In this article, we explored the methods of diagnosis and treatment for cholesteatomas in cerebellopontine angle region presenting as trigeminal neuralgia. METHODS: Clinical data of 27 cases with cerebellopontine angle cholesteatoma that primarily manifested with trigeminal neuralgia were analyzed retrospectively. RESULT: The preoperative CT or (and)MRI diagnosis was confirmed by the surgery. Total resection was achieved in 19 cases, while subtotal in 8 cases.Postoperatively, the symptoms of trigeminal neuralgia and/or hemifacial spasm disapeared in all the cases,and disturbance of hearing was improved obviously.In one case,the trigeminal neuralgia reoccurred one year after the surgery. CONCLUSIONS : Trigeminal neuralgia is often an initial symptom of cholesteatoma in cerebellopontine angle region.CT and MRI are the main means of diagnosis for the disease,and total resection can be usually achieved by microsurgery.Special attention should be paid to preserve the surrounding important structures.
