Minimally invasive surgery for the intra-and extra-spinal tumors

Abstract:Objective To investigate the minimally invasive surgery for the intraand extra-spinal tumors (IEST).Methods Clinical data of 164 patients with IEST were analyzed retrospectively.The tumors were divided into four types according to their location on MRI,including intervertebral foramen and subdural tumorin 54 cases,paravertebral foraminal and subdural tumorin 33,paravertebral foraminal and extradural tumor in 61 and extensive and huge tumor in 16.The tumor removal was performed via posterior median approach in 131 cases,lateral approach in 29 and transthoracic approach in 4.The reconstruction of spinal stabilization was carried out afterthe removal of extensive and huge tumor.Results Total tumorremoval was achieved in 155 cases and subtotal removal in 9.The tumors were diagnosed by pathological analysis,including neurilemoma in 153 cases,neurofibroma in 9,ganglioneuroma in 1 and meningioma in 1.Postoperative low-grade infection occurred in 4 cases,cerebrospinal fluid leak in 3,partial extradural hydrops in the neck in 1.One hundred and thirty-three patients were followed up formedian period of 29 months,ranged from 3.5 months to 7 years.The patients recovered well in 94,had no obvious improvement in 26 and the symptoms were aggravated in 13.MRI reexamination showed that the tumors recurred in 6 of 131 cases.Conclusions Tumor typing and the corresponding surgical approaches have been outlined.To master minimally invasive surgical technique,one-stage total removal of partial tumors can be achieved via posterior median and lateral approach without spinal fixation,including intervertebral foramen and subdural tumor,paravertebral foraminal and subdural tumor,paravertebral foraminal and extradural tumor.
