Analysis of Compensatory Patterns and Efficacy of Endovascular Stent Implant Treatment of 5 Subclavian Steal Syndrome Cases

Abstract:Aim:To observe the compensatory collateral circulation patterns and efficacy of endovascular stent implant treatment by cerebral angiography in patients with subciavian steal syndrome. Methods:Compensatory collateral circulation patterns were observed and analyzed by cerebral angiography in 5 patients with subclavian artery steal syndrome. All the patients underwent endovascular stent implant treatment and were followed up 2 months to 13 months. The curative effect was observed and analyzed. Results:6 stents were implanted in 5 patients. In the extracranial collateral circulation compensatory pathways,anastomosis of occipital artery and muscular branches of vertebral artery was observed open in 4 cases. Blood flowed into skull in one case. Blood flowed bidirectionally in one case. Blood flowed out of skull in 2 cases,accompanied by posterior communicating artery being open and blood flowing from anterior circulation to posterior circulation in one case. The same patient was complicated by non-infectious endarteritis because of repetitive balloon expansion during the surgery. Conclusion:Intravascular stent implant treatment is safe and effective for subclavian steal syndrome patients. Anastomosis of occipital artery and muscular branches of vertebral artery plays an important role in extracranial collateral circulation compensatory pathways. When performing angioplasty on patients with severe vascular stenosis,the occurrence of non-infectious endarteritis should be avoided.
