Effect of HPV Multiplex PCR Fluorescence Test on Cervical Cancer Screening

Abstract:[Purpose] To evaluate the effect of high-risk human papillomavirus (HPV) multiplex PCR fluorescence test on cervical cancer screening. [Methods] From October 2007 to June 2008, a cervi-cal cancer screening program was conducted on the eligible women aged 30 to 59 from two communi-ties (Shang Meiling Community & Zi Wei Community) and two hospitals (Beijing University Shenzhen Hospital & Maternity and Child Health Care Hospital) in Shenzhen. Three exfoliated cervical cell samples were collected from each woman, which were used for Hybrid Capture Ⅱ(HC-Ⅱ), HPV mul-tiplex PCR fluorescence test, and cytology examine, respectively. The women with either positive HPV DNA test or abnormal cytology were called back about two weeks later, and biopsied under col-poscopy. Pathologic result was used as the gold standard to evaluate the accuracy of the multiplex PCR fluorescence test for detecting high-grade cervical squamous intraepithelial lesions (CINⅡ/Ⅲ) or worse, and to access the agreement between this new test and HC-Ⅱ. [Results] CINⅡ/Ⅲ and worse accounted for 4.0% in the 1988 eligible women. The HPV prevalence detected by the multiplex PCR fluorescence test and HC-Ⅱ was 19.8% and 17.8%, respectively. The sensitivity, specificity, Yonden’s index, positive predictive value and negative predictive value of the multiplex PCR fluorescence test were 92.3%, 83.1%, 75.4%, 18.3% and 99.6%, respectively. Those of HC-Ⅱ were 97.4%、85.5%、 82.9%、21.5% and 99.9%, respectively. The consistency rate was 87.8%, and Kappa value was 0.60 between the two HPV DNA tests. The viral loads of HPV DNA from the two HPV tests all increased with the severity of cervical neoplasia (P<0.0001). [Conclusion] Multiplex PCR fluorescence test has the high sensitivity and specificity for detecting CINⅡ/Ⅲ or worse. There is a better agreement with HC-Ⅱ test, so it will have a better prospect in the clinical practice.
