Abstract:China is gradually taking its place as one of the world's economic giants and concurrently learning to cope with the burden of diseases that are more common in the developed world, such as paediatric type 2 diabetes mellitus. The prevalence of type 2 diabetes has been recently observed among children and adolescents in China; hence, there is a lack of information about the incidence, prevalence, pathogenesis, and pathophysiology of the disease. Diagnosis, treatment, and management have been standardized to a large degree, but there is still a need for data regarding optimal management protocols and how to achieve the best control over current state of the disease. The objective of this review is to consolidate the available information about paediatric diabetes, with a focus on the increasing prevalence of type 2 diabetes in Chinese youth. Here we emphasize the prevention strategies and have included literature with respect to pathogenesis, diagnosis, and treatment published in English and Chinese within the past 10 years.

Abstract:Aim. To investigate the metabolic profiles of newly diagnosed type 2 diabetes (NEW2D) in Chinese older adults (≥65 years) and assess the proportion of patients who achieved the targeted goals of blood glucose, blood pressure, and blood lipid. Methods. NEW2D study was an observational, longitudinal, prospective cohort study involving patients who were diagnosed with type 2 diabetes (T2D) within the past 6 months and had a follow-up of 12 months. Participants were divided into younger NEW2D group (aged 20-65 years old) and older NEW2D group (aged ≥65 years old) according to age of diabetes onset. The baseline metabolic profiles were compared and the proportion of patients achieving adequate control of blood glucose, blood pressure, and blood lipids in reference to target goals were assessed during treatment. Results. The older NEW2D (n=1362) had a lower BMI, HbA1c%, diastolic blood pressure, triglyceride, low-density lipoprotein cholesterol (LDL-C), and total cholesterol, higher systolic blood pressure, and high-density lipoprotein cholesterol levels at baseline. 47.8%, 66.7%, and 39.4% reached the target of HbA1c<7.0%, BP<140/90 mmHg, and LDL−C<2.6 mmol/L, respectively. After 12 months, the proportion achieving above three targets increased to 70.2%, 76.1%, and 47.5%, respectively. The proportions of patients achieving three combined therapeutic targets doubled from 13.5% to 26.7%. Conclusion. The older NEW2D patients have special metabolic profiles compared with younger patients. The control of cardiovascular disease risk factors was suboptimal in older adults with type 2 diabetes.

Abstract:The features of the pathogenesis and course of type 2 diabetes mellitus and diseases of the cardiovascular system comorbid with it are considered in patients of elderly and old age – coronary artery disease, arterial hypertension, chronic heart failure. The leading role of insulin resistance, hyperglycemia and dyslipidemia in the development of metabolic, homeostatic disorders, the formation of oxidative stress and endothelial dysfunction, which, together with age-related changes in the body, contribute to the progression of type 2 diabetes mellitus and microvascular complications, primarily diabetic cardiomyopathy. Particular attention is paid to the relationship between cognitive impairment, type 2 diabetes mellitus and chronic heart failure. The main factors that worsen the course and prognosis of type 2 diabetes mellitus in elderly patients, in particular, hypertension, atrial fibrillation, diabetic polyneuropathy, nephropathy, and other concomitant diseases, have been identified. Lack of compensation for type 2 diabetes due to metabolic disorders leads to the development of diabetic cardiovascular autonomic neuropathy, diabetic cardiomyopathy along with the progression of atherosclerotic lesions of different localization. The course of type 2 diabetes in these patients is often complicated by geriatric syndrome, which contains a set of cognitive impairment, senile weakness, depression, functional disorders, polymorbidity. Cognitive disorders negatively affect the course of type 2 diabetes and its complications, significantly disrupting the process of teaching patients the methods of self-control, following the advice of a doctor. It is noted that the management of this category of patients should be individual and include adequate correction of hyperglycemia to prevent microvascular complications and hypoglycemic conditions, as well as reduce cardiovascular mortality and maintain quality of life. Rational selection of drugs, taking into account the factors that determine their impact on the body of elderly patients with type 2 diabetes mellitus and possible adverse drug reactions, will increase the effectiveness and safety of drug therapy in such patients. Optimizing therapeutic approaches for elderly patients with type 2 diabetes requires effective changes in the health care system to provide them with comprehensive medical and social care according to their special needs

Abstract:Objective Provide the basis for early diagnosis,treatment and prevention of type 2 diabetes patients with complications.Methods The clinical data of 330 cases diagnosed as type 2 diabetes mellitus including clinical information and findings of blood specimens were collected for this study.Age,gender,body mass index(BMI),some clinical and biochemical indicators of 330 patients were analyzed for deduction of relationship between complications and risk factors.Results The occurrence of complications in patients with type 2 diabetes had no association with gender,body mass index and blood level of glucose,but they had close association with age.Prevalence rates were increased with the increase of age in these five complications.In age group of over 60 years,hypertension was the highest(47.5%) in various complications,the second was peripheral vascular disease(43.33%) and the third one was peripheral neuropathy(40.83%).Abnormal lipid metabolism was the risk factor of peripheral vascular disease.Hypertension was usually associated with diabetic nephropathy,neuropathy,peripheral vascular lesions and ocular diseases.These patients with complication of peripheral vascular disease were more vulnerable to have peripheral neuropathy(50.94%).Conclusion Old age is a risk factor of complications in patients with type 2 diabetes.Patients with type 2 diabetes complicated with hypertension are more likely to lead other complications of diabetes,and the occurrence of peripheral vascular disease is more easily associated with peripheral neuropathy.

Abstract:Diabetes mellitus is a chronic multifaceted disease with multiple potential complications, the treatment of which can only delay and prolong the terminal stage of the disease, i.e., type 2 diabetes mellitus (T2DM). The World Health Organization predicts that diabetes will be the seventh leading cause of death by 2030. Although many antidiabetic medicines have been successfully developed in recent years, such as GLP-1 receptor agonists and SGLT-2 inhibitors, single-target drugs are gradually failing to meet the therapeutic requirements owing to the individual variability, diversity of pathogenesis, and organismal resistance. Therefore, there remains a need to investigate the pathogenesis of T2DM in more depth, identify multiple therapeutic targets, and provide improved glycemic control solutions. This review presents an overview of the mechanisms of action and the development of the latest therapeutic agents targeting T2DM in recent years. It also discusses emerging target-based therapies and new potential therapeutic targets that have emerged within the last three years. The aim of our review is to provide a theoretical basis for further advancement in targeted therapies for T2DM.

Abstract:Diabetes mellitus has become a major public health issue globally, putting an enormous burden on global health systems and people. Among all diseased groups, a considerable part of patients are elderly, while their clinical features, pathogenic processes, and medication regimens are different from patients of other ages. Despite the availability of multiple therapies and techniques, there are still numerous elderly diabetes patients suffering from poor blood glucose control, severe complications, and drug adverse effects, which negatively affect the quality of life in their golden years. Traditional Chinese Medicine (TCM) has been widely used in the treatment of diabetes for several decades, and its relevant clinical practice has confirmed that it has a satisfactory effect on alleviating clinical symptoms and mitigating the progression of complications. Chinese herbal medicine and its active components were used widely with obvious clinical advantages by multiple targets and signaling pathways. However, due to the particular features of elderly diabetes, few studies were conducted to explore Traditional Chinese Medicine intervention on elderly diabetic patients. This study reviews the research on clinical features, pathogenic processes, treatment principles, and TCM treatments, hoping to provide fresh perspectives on the prevention and management strategies for elderly diabetes.

Clinical analysis of glucose and lipid metabolism in elderly hospitalized patients with type 2 diabetes

Abstract:Objective To explore characteristics of blood pressure ,blood glucose ,lipids ,drug use and the risk factors of chronic diabetic complications in the elderly type 2 diabetic patients. Methods We retrospectively analyzed the clinical data of 447 patients above age of 60 years with T2DM consecutively admitted to the Department of Endocrinology. The non‐diabetic population aged over 60 years and the T2DM patients under the age of 60 years were recruited as controls.Furthermore ,we investigated the differences in the metabolic indicators among groups classified by BMI. Results HbA1c and 2 hPG were significantly higher in the elderly diabetic patients than in the elderly non‐diabetic population and in patients under the age of 60 with T2DM ,while HDL‐C was lower in elderly diabetic patient than in the controls(P< 0.05). A proportion of patients with overweight or obese were 52.8% .Along with the increase of BMI ,the values of blood pressure ,uric acid (UA) and urine Alb/Cr were increased(P<0.05) , LDL‐C had an increasing trend and eGFR had a decreasing trend (P>0.05).98.0% of the patients with T2DM were suffering from diabetic complications. Logistic regression analysis showed that the age , duration of disease ,female ,SBP and UA were risk factors for diabetic chronic complications. Conclusion In this study ,T2DM are diagnosed mainly by health examination and finding diabetic complications ,and most of the patients suffer from overweight or obesity. The chronic diabetic complications are highly prevalent and the risk factors include the age ,the duration of disease ,female ,SBP and UA.
The Achievement of Comprehensive Control Targets among Type 2 Diabetes Mellitus Patients of Different Ages

Abstract:OBJECTIVE:To evaluate achievement of comprehensive controls among patients with type 2 diabetes mellitus (T2DM) in different age groups.RESULTS:The elderly patients had higher control rates for BMI (44.36%), TC (50.83%) and LDL-C (48.27%) than those aged 60-80 years and younger patients (all P <0.05). Multiple logistic regression revealed that elderly patients were more likely to achieve control targets for HbA1c (odd ratio (OR) = 2.19), TC (OR = 1.32), HDL-C (OR = 1.35), and TG (OR = 1.74) than younger patients. This effect was stronger in males (ORHbA1c = 2.27; ORTC = 1.41; ORHDL-C = 1.51; ORTG = 1.80). By contrast, elderly females were only more likely to achieve HbA1c < 7.0% (OR=1.88).CONCLUSIONS:Our findings suggest that comprehensive control strategies still should be strengthened.METHODS:A total of 3126 T2DM patients were included, and detected blood pressure (BP), body mass index (BMI), glycosylated hemoglobin (HbA1c), fasting plasma glucose (FPG), postprandial plasma glucose (PPG), total cholesterol (TC), low density lipoprotein cholesterol (LDL-C), triglycerides (TG), and high-density lipoprotein cholesterol (HDL-C). We divided patients into three age groups (<60, 60-80 and ≥ 80 years), to assess the differences in achieving the control targets.

Abstract:Diabetes is a widespread chronic disease that occurs mainly in the elderly population. Due to the difference in pathophysiology between elderly and young patients, the current clinical practice to treat elderly patients with anti-diabetes medications still faces some challenges and dilemmas, such as the urgent need for early diagnosis and prevention, and an imbalance between restricted dietary intake and the risk of undernutrition. Traditional Chinese medicine (TCM) offers various treatment regimens that are actively utilized in the field of diabetes management. Through multiple targets and multiple pathways, TCM formulas, medicinal herbs, and active natural products enhance the efficacy of diabetes prevention and diabetes control measures, simplify complex medication management, and improve common symptoms and common diabetic complications in elderly people. Historically, natural products have played a key role in material composition analysis of TCM and mechanism interpretation to enable drug discovery. However, there have been few conclusions on this topic. This review summarizes the development of TCM for the prevention and management of diabetes in elderly people, existing evidence-based clinical practices, and prospects for future development.

Abstract:Objective To investigate the effect of central obesity on the glucose, blood lipid, FFA, blood pressure, insulin resistance and function of islet β cell in newly diagnosed elderly patients with type 2 diabetes. Methods Ninety newly diagnosed elderly patients with type 2 diabetes were selected randomly. They were divided into central obesity group and control group, and all the subjects received oral glucose tolerant test. During the test, the levels of insulin, C-peptide, FFA at the time of 0, 30, 60, 120, 180 min were detected. All patients received the measurement of blood lipid. HOMA-IR was applied to assess the status of insulin resistance, and HOMA-β was used to evaluate the basic function of islet β cells respectively. Results The levels of fasting blood glucose, FFA and HOMA-IR were higher in central obesity group than those in control group. Conclusions Central obesity has adverse effect on glucose and lipid metabolism in newly diagnosed elderly patients with type 2 diabetes.

Abstract:During the past 40 years, the prevalence of diabetes in China has increased from less than 1·0% in 1980 to 12·4% in 2018, an increase in line with the rapid growth of the nation's economy. To address such a burden, the Healthy China 2030 initiative and subsequent Action Plan, including a diabetes prevention and control campaign, were launched. A shift from a disease-centred approach to a health-centred approach and from treatment to prevention is the core of the Action Plan and diabetes management in China. In this Review, we discuss the challenges of diabetes prevention in China, including unhealthy lifestyle, increasing young-onset type 2 diabetes, and substantial diabetes care disparities. To address such challenges, countermeasures across different stages of diabetes prevention and targeted at different populations, are needed. Such countermeasures include primordial prevention of risk factors in the general population, primary prevention of diabetes onset in high-risk populations, and secondary prevention of cardiovascular complications for individuals with diabetes. We reflect on China's current progress, strategies, and achievements.
Clinical Analysis of Senile Type 2 Diabetes Complicated with Hypoglycemia in the Suburbs of Shanghai

Abstract:Objective To explore the characteristics of elderly type 2 diabetes (T2DM) complicated with hypoglycemia in the sub-urbs of Shanghai and seek measures to prevent and reduce it.Methods 116 cases of senile T2DM were random sampled so as to investigate and analyze the risk factors of hypoglycemia.Further more,72 h continuous glucose monitoring was performed during blood sugar stable peri-od.Results Compared with the control group, indexes were lower including triglyceride, total cholesterol, low density lipoprotein, body mass index ( BMI) and glycosylated hemoglobin, but blood creatinine and standard deviation increased significantly.In observation group, patients combined with hypertension, diabetic nephropathy, renal insufficiency had markedly increased.Conclusions Elderly T2DM pa-tients are prone to fall to hypoglycemia, which results from multiple chronic complications, weak insulin resistance, fluctuation blood glu-cose, low educational level, cognitive ability and other relevant factors.For those elder patients who have the severe hypoglycemia, with ma-crovascular complications, should follow the principle of individualized target to control HbA1c, generally not less than 7%.These patients should be informed of the rational use of drugs.Blood glucose testing should become a routine testing content in coma patients.

Abstract:Diabetes mellitus is one of the most serious diseases threatening human health.The role of trace elements in the etiology of Type 2 Diabetes(T2D) is not well established but several studies have shown iron may play a key role.Iron is a strong oxidant and high levels of body iron may produce oxidative stress that increases the risk of T2D.Several epidemiological studies have reported a positive association between high levels of circulating ferritin and the prevalence of T2D as well as other insulin resistant conditions such as obesity,metabolic syndrome,and gestational diabetes.Dietary iron,especially heme iron,is positively associated with high risk of T2D.Catalytic iron that is available to participate in free radical reactions may play a role in the pathogenesis of iron metabolic disorder-mediated diabetes and its complications.Reducing body iron level by iron chelate can ameliorate insulin resistance,suggesting a novel approach for the treatment of T2D.This review summarizes recent studies of iron metabolism and T2D.

Abstract:The past 50 years have seen a growing ageing population with an increasing prevalence of type 2 diabetes mellitus (T2DM); now, nearly half of all individuals with diabetes mellitus are older adults (aged ≥65 years). Older adults with T2DM present particularly difficult challenges. For example, the accentuated heterogeneity of these patients, the potential presence of multiple comorbidities, the increased susceptibility to hypoglycaemia, the increased dependence on care and the effect of frailty all add to the complexity of managing diabetes mellitus in this age group. In this Review, we offer an update on the key pathophysiological mechanisms associated with T2DM in older people. We then evaluate new evidence relating particularly to the effects of frailty and sarcopenia, the clinical difficulties of age-associated comorbidities, and the implications for existing guidelines and therapeutic options. Our conclusions will focus on the effect of T2DM on an ageing society.

Abstract:Diabetes mellitus and dementia, two of the most common disorders in the elderly, are posing a tremendous burden on public health in the society. Moreover, the absolute number and the proportion of older people who are affected by diabetes, borderline diabetes and dementia are expected to further increase over the next few decades. The aim of this study was to determine if a relationship existed between type 2 diabetes and dementia by performing a meta-analysis of published cross-sectional and prospective studies.Comprehensive literature search was performed and the extracted data was analyzed.Our findings highlight the need to detect borderline diabetes and undiagnosed diabetes in order to effectively prevent dementia, as previous studies have shown that prediabetes and diabetes could be improved by interventions addressed towards lifestyle changes and antidiabetic treatment.As far as clinical practice is concerned, it is important to bear in mind that prevention, timely diagnosis, and the optimum treatment of diabetes and borderline diabetes may help to reduce the occurrence of dementia.

Abstract:Abstract Type 2 diabetes is a frequent chronic disease. Given its strong positive association with older age, it is a significant public health issue in elderly populations. Furthermore, the aging of the population, driven by increasing life expectancy in high and middle-income countries leads to an increasing prevalence of diabetes. Although the same diagnostic criteria apply to the elderly and to younger people, there are unique aspects to the care for elderly type 2 diabetes patients. Both treatment goals and preferred medications, as well as non-pharmacological approaches should be adjusted in the elderly. For example, increasing the amount of physical activity may encounter difficulties, while introducing an appropriate diet may be more challenging. The patients' therapeutic adherence requires special attention due to cognitive and physical limitations. The most important treatment goal is to avoid hypoglycemia. Frailty, social and economic issues, comorbidities and the consequent polypharmacy frequently causing drug-drug interactions, as well as the increased danger of drug toxicity due to renal failure are only some of the problems that make the health care for old diabetes patients extremely difficult. Adequate care requires cooperation from a multidisciplinary team of health care professionals. Acute diabetes complications have a higher mortality in the elderly, thus close attention must be paid to avoid them. Family members should be involved in the care of elderly diabetes patients, and it is recommended to educate them on clinical signs of complications. Regular care for the patients including feedback on quality of life and early signs of health issues are essential.

Advances in the management of type 2 diabetes in adults

Abstract:Type 2 diabetes is a chronic and progressive cardiometabolic disorder that affects more than 10% of adults worldwide and is a major cause of morbidity, mortality, disability, and high costs. Over the past decade, the pattern of management of diabetes has shifted from a predominantly glucose centric approach, focused on lowering levels of haemoglobin A 1c (HbA 1c ), to a directed complications centric approach, aimed at preventing short term and long term complications of diabetes, and a pathogenesis centric approach, which looks at the underlying metabolic dysfunction of excess adiposity that both causes and complicates the management of diabetes. In this review, we discuss the latest advances in patient centred care for type 2 diabetes, focusing on drug and non-drug approaches to reducing the risks of complications of diabetes in adults. We also discuss the effects of social determinants of health on the management of diabetes, particularly as they affect the treatment of hyperglycaemia in type 2 diabetes.
