[Application of Benchmark Dose (BMD) in a Bone-Effect Study on a General Population Environmentally Exposed to Cadmium].

Abstract:[Objective] An epidemiological study was conducted to estimate the benchmark dose of a population exposed to environmental fluoride in order to evaluate critical exposure level of fluoride and to find an easier biomarker for its bone damage. [Methods] Urine fluorine(UF)、urine creatine(UCr)、blood fluorine(BF)were used as exposure biomarkers, urinary hydroxyproline(HYP)、serum osteocalcin(BGP)、serum alkaline phosphatase(AKP)、serum alkaline phosphatase (bone isoenzyme)(BAKP)and bone mineral density were mensured as effect biomarkers. Z scores were calculated. [Results] Comparing with the control group,it has been concluded that UF,BF,BGP,HYP,BAKP and bone mineral density of the exposure group were significantly different,P0.05. The bone mineral density declines significantly but BGP and HYP ascend significantly following the raise of urine fluorine,P0.01. The BMDLs of UF calculated basing on BGP,HYP,Z score,BAKP and AKP were 0.31,2.75,4.36,4.06 and 5.09 mg/L respectively. [Conclusion] The internal dose could reflect fluorine exposure level better. Z score was identified as a specific and sensitive biomarker. BGP and HYP can be used as highly sensitive biomarkers to screen the high risk population of bone fluorosis.
