Diagnosis and surgical treatment of temporomandibular joint ankylosis resulting from intracapsular condylar fractures

Abstract: PURPOSE: This retrospective study was aimed to present our treatment protocol for temporomandibular joint ankylosis resulting from intracapsular mandibular condylar fractures. METHODS: The study group included 19 cases (23 joints), 8 male (10 joints) and 11 female (13 joints), with an average age of 12.68 years at operation and 7.32 years at injury, and the average preoperative maximum mouth opening(MMO) was 5.74mm. They were further divided into 4 types (Ⅰ, Ⅱ, Ⅲ and Ⅳ). The surgical procedures comprised lateral gap arthroplasty (LGA) for typeⅠand Ⅱ, resection of ankylotic bone and costochondral grafting (CCG) for Ⅲ and Ⅳ. Follow- up was carried out at regular intervals. RESULTS: 16 cases (19 joints) underwent LGA, CCG for 3 patients(4 joints), and postoperative healing was uneventful. The mean follow- up period was 11.37 months and the mean MMO was 27.89mm by the last visit. CONCLUSION: LGA can release ankylosis, and in some sense, maintain the potential of growth and development of the condyle. However, there are still limited indications for this procedure, sometimes CCG is indicated. Supported by Shanghai Leading Academic Discipline Project (Y0203).
