Analysis of Phenylketonuria in 39 Children

Abstract:Objective To investigate the clinical diagnosis,therapy and prognosis of children with phenylketonuria(PKU),and to analyze the possible causes of misdiagnosis.Methods The detailed data of 39 cases with PKU[male 21 cases,femal 18 cases;nosopoietic age was(8.79±11.28) months;diagnostic age was (2.48±2.72) years] admitted from Jan.1993 to Oct.2008 were analyzed retrospectively,including clinical manifestation,final diagnosis,misdiagnosis analysis,perspective therapy.Results The clinical manifestations of PKU in this group included mental retardation in all 39 cases,epilepsy in 26 cases,abnormal rat-like odor in 21 cases,language delay in 11 cases and dyskinesia in 2 cases.The electroencephalogram was abnormal in 29 cases.The magnetic resonance imaging showed abnormalities in 5 cases.All children had been regarded as mental retardation or congenital developmental problems,the courses of the disease before final diagnosis were(1.75±2.55) years,15 cases of them had been misdiagnosed as cerebral palsy or atelencephalia.Three cases manifested as movement disorders or epileptic seizures,hypopigmentation or rat-like odor.They were identified as tetrahydrobiopterin deficiency through urine pterin analysis and tetrahydrobiopterin tolerance test.Conclusions The clinical manifestations of PKU mainly include mental retardation,developmental delay,epileptic seizures,and movement disorders.Early diagnosis and prompt intervention are the key for a satisfactory long-term prognosis.It is not easy,however,to make a correct diagnosis for children with PKU missing the neonatal routine screening test.Tetrahydrobiopterin tolerance test is of value for the identification of atypical PKU.
