Abstract:The occurrence of free-floating pigmented cysts in anterior chamber is very unusual (Shields 1981), and this type of cysts was rarely treated by operation other than periodic observation, because most of them were asymptomatic (Shields et al. 1996). But when it cause visual disturbance, it may need to be removed. A 25-year-old Han Chinese man noticed a black spot in his left eye during a routine examination. No ocular disease history except myopia of −5.00DS for his both eyes with the best-corrected visual acuity of 20/20. There was no ocular injury and surgery, nor any topical medications administration and evidence of ocular inflammation. Slit lamp and fundus examination of the right eye was normal. However, the left eye showed a pigmented brownish round-shaped cyst located at 6 o'clock position in the anterior chamber. The cyst was about 3.0 mm in diameter and was apparently filled with transparent fluid, but no other contents could be observed. Its surface was sprinkled with small dots of black iris pigment (Fig. 1A). The cyst was found to be free-floating, as it could move freely when the patient tilted his head (Fig. 1B). The cyst appeared to be in contact with the corneal endothelium surface, whereas the cornea was still clear. Ultrasound biomicroscopic (UBM) examination revealed a free-floating pigmented cyst with a thin wall and no internal reflectivity. The angle structures on gonioscopy and UBM examination were normal in both eyes, and no other cysts were observed in the ciliary sulcus (Fig. 1C). No other abnormalities were detected. As his visual acuity was 20/20 without any other ocular disturbance, therefore, a routine observation was recommended. After 2 months, he noticed in mirror that the mass seemed to be slightly enlarged. He also saw a ‘cloud’ in his left eye's visual field, particularly when he looked downward. He then returned to our clinic and wanted the mass removed. In view of the presented symptoms and risk of corneal endothelium injury due to the continuously touch with the cyst, surgical removal was suggested. Prior to surgery, 2% pilocarpine drops (tid) was administered to keep the pupil constricted. Surgery was performed under topical anesthesia. A limbal paracentesis of 1.2 mm was performed at 2 o'clock position, and a small volume of viscoelastic was injected into the anterior chamber. Then, a major limbal incision of 2.8 mm was made at 12 o'clock position. The iris cyst moved through the major incision with aqueous right away, when viscoelastic was injected slowly through the major incision. The sample of cyst was collected immediately and fixed in 10% formaldehyde solution. The incisions were then sealed by hydration. The surgery was completed in 10 min. There was minimal inflammation in the anterior chamber for a few days and had no signs of any lenticular changes. The best-corrected visual acuity was 20/20 in day 1 after the operation. The pathohistological examination of the cyst demonstrated a folded epithelium lining composed of mature pigment epithelium (Fig. 1D). This was consistent with the clinical diagnosis of a cyst of the iris pigment epithelium. No recurrence of cyst and any other ocular complications were observed during the succeeding 6-month follow-up. Our case suggested that surgical removal of free-floating iris cyst in anterior chamber could be an effective choice of treatment.
