Abstract:Malignant hyperthermia has rarely been reported in China. We report the first case of malignant hyperthermia, verified by caffeine-halothane contracture test and genetic testing, in a Chinese patient.

Abstract:OBJECTIVE:To explore the application of caffeine-halothane contracture test (CHCT) in the confirmation of malignant hyperthermia (MH).METHODS:One patient who underwent radical gastrectomy presented with clinical manifestations of MH during routine intravenous-inhalation anesthesia process. Isoflurane inhalation and the operation were ceased immediately and emergency management approaches such as physical cooling therapy were taken. Meanwhile, the levels of serum creatine kinase (CK), serum myoglobin, and urinary myoglobin were examined and rectus abdominis was taken and then CHCT was performed to confirm the clinical diagnosis. Total genome was extracted from the patient and then exons 2-18, 39-46, and 90-104 of ryanodine receptor 1 (RYR1) gene were screened to detect mutations using DNA sequencing technique.RESULTS:The patient was diagnosed as MH episode by clinical characteristics and postoperatively continuous elevation of the levels of CK, serum myoglobin, and urinary myoglobin (30 times higher than normal level). Despite halothane test was negative, the diagnosis of MH was verified by the positive result of caffeine test. DNA sequencing of RYR1 gene of the patient revealed c. 6724C > T (p. T 2 206M).CONCLUSION:CHCT can be used to confirm the diagnosis of MH.

Abstract:Journal of International Medical Research, Volume 52, Issue 6, June 2024. Malignant hyperthermia (MH), characterized by severe myoclonus, pyrexia, tachycardia, hypertension, elevated muscle enzymes, and hypercapnia, often occurs in patients with congenital deformities or genetic disorders. Although the reported incidence rate is as low as 1:5000 to 1:100,000, patients with MH exhibit rapid aggravation and an elevated mortality rate. Thus, MH is associated with substantial perioperative risk. Successful treatment of patients with MH largely depends on early diagnosis and timely effective treatment. This clinical report provides a detailed description of a patient with newly diagnosed MH who developed a rapid rise in body temperature, end-tidal carbon dioxide, and heart rate during maxillary osteotomy. After successful rescue, the patient recovered smoothly during the postoperative period, indicating the importance of intraoperative monitoring, early diagnosis, effective treatment, and postoperative monitoring. This case is expected to serve as a reference for future interventions and healthcare practices in managing other patients with MH.

Abstract:Malignant hyperthermia (MH) is a potentially fatal inherited pharmacogenetic disorder related to pathogenic variants in the RYR1, CACNA1S, or STAC3 genes. Early recognition of the occurrence of MH and prompt medical treatment are indispensable to ensure a positive outcome. The purpose of this study was to provide valuable information for the early identification of MH by summarizing epidemiological and clinical features of MH. This scoping review followed the methodological framework recommended by Arksey and O'Malley. PubMed, Embase, and Web of science databases were searched for studies that evaluated the epidemical and clinical characteristics of MH. A total of 37 studies were included in this review, of which 26 were related to epidemiology and 24 were associated with clinical characteristics. The morbidity of MH varied from 0.18 per 100 000 to 3.9 per 100 000. The mortality was within the range of 0%-18.2%. Identified risk factors included sex, age, disorders associated with MH, and others. The most frequent initial clinical signs included hyperthermia, sinus tachycardia, and hypercarbia. The occurrence of certain signs, such as hypercapnia, delayed first temperature measurement, and peak temperature were associated with poor outcomes. The epidemiological and clinical features of MH varied considerably and some risk factors and typical clinical signs were identified. The main limitation of this review is that the treatment and management strategies were not assessed sufficiently due to limited information.
Congenital Hyperinsulinism in China: A Review of Chinese Literature over the Past 15 Years

Abstract:Congenital hyperinsulinism (CHI) is a rare but severe cause of hypoglycemia. The present study investigates the clinical presentation, therapeutic outcomes and genetic mutations of CHI in Chinese individuals over the past 15 years.The authors retrospectively reviewed one case in their department and 206 cases reported from January 2002 to October 2016 in China. PubMed, Ovid Medline, Springer and Wanfang Database, CBMD database, and CKNI database were the sources used to collect the data.In total, 207 cases were recruited. Of these, the ages of 100 (48.3%) were within the 4th week after birth. Seventy-seven cases (37.2%) were born large for gestational age (LGA). Seizures occurred in 140 cases (67.6%). Among 140 cases (67.6%) who were administered diazoxide treatment, 90 (64.3%) were responsive. Seven cases (3.4%) received octreotide treatment and 19 cases (9.2%) underwent surgery. 63/129 cases (48.8%) were detected to have gene mutations, including ABCC8 (69.8%), KCNJ11 (12.7%), GLUD1, GCK, HADH, and HNF4A. Among the diazoxide-unresponsive cases, gene mutations were detected in 20/36 (55.6%) cases with ABCC8 and in 2 (5.6%) cases with KCNJ11. Among the diazoxide-responsive cases, gene mutations were detected in 8 patients with ABCC8, 4 with KCNJ11, 5 with GLUD1, and 1 with GCK.The present study indicates that most CHI cases occurred in neonates and that 1/3 of the cases were born LGA. ABCC8 and KCNJ11 are the most common gene mutations. More than half of the diazoxide-unresponsive CHI detected mutations are in ABCC8 and KCNJ11 genes. The GLUD1 gene mutations cause diazoxide-responsive CHI. Identifying the gene mutations can assist in the diagnosis and treatment of CHI.

Early detection and prompt intravenous administration of dantrolene are crucial for effective management of MH. However, there is currently a lack of comprehensive nationwide surveys on the availability of dantrolene and anesthesiologists' understanding of MH in China. Methods: A nationwide survey was conducted between January 2022 and June 2022. Online questionnaires on the cognition of MH among anesthesiologists in China were sent through social platforms to anesthesiologists in mainland China. Data regarding participants' perception of MH-related knowledge, availability of domestic dantrolene, and reported MH cases were collected in this study. Results: Responses were collected from a total of 11,354 anesthesiologists representing 31 provinces across the Chinese mainland. Among the 11 scoring questions, the highest accuracy rates were observed for the question regarding therapeutic drugs for MH (99.3%) and the characteristics of MH (98.0%). Conversely, the question pertaining to the earliest clinical signs of MH had the lowest accuracy rate (23.5%). Significant variations were observed in the scores among different professional titles ( P =0.003), academic degree ( P < 0.001), hospital classification ( P < 0.001), and urban hierarchy ( P < 0.001). Of the respondents, 919 (8.1%) anesthesiologists reported dantrolene availability in their hospitals, and 631 (5.6%) indicated unclear. A total of 136 hospitals in this survey reported at least one previous case of MH. Conclusion: Mainland China faces challenges such as insufficient experience in diagnosing and treating MH, as well as difficulty in obtaining dantrolene. To improve the public awareness of MH, it is imperative to establish and promote a refined MH training system. Additionally, a streamlined and rapid dantrolene linkage emergency system should be implemented to ensure prompt access to the drug. It often occurs after exposure to volatile anesthetics and/or depolarized muscle relaxant succinylcholine, and rarely by stress and heat. The estimated incidence of MH varies between 1/10,000 and 1/250,000 anesthetic procedures. 1–3 With an increasing understanding of MH and introduction of intravenous dantrolene in 1979, the mortality rates has decreased from 80.0% to 1.4% in developed countries. 4 The mortality rates associated with MH in China was reported as high as 73.5% in the early 21 st century. 5 A retrospective analysis of the suspected MH patients from 2015 to 2020 reported a mortality rate of 53.4%. 6 Lack of awareness concerning the availability and accessibility of the orphan drug dantrolene was considered to attribute to the high mortality rates with MH. Comparison of malignant hyperthermia cases in Chinese mainland and China's Taiwan region

Abstract:Objective:To comparatively analyze the clinical data of malignant hyperthermia (MH) cases in Chinese mainland and China's Taiwan region, in order to explore the early diagnosis, treatment measures and effectiveness of MH, and the use of dantrolene.Methods:All MH cases reported from January 1976 to April 2022 were collected by searching Wanfang Data, SinoMed, CNKI, and Taiwan Scholar Journal Database. The epidemiological characteristics, the characteristics of onset, clinical manifestations, treatment measures, dantrolene usage and outcomes of MH cases in Chinese mainland and China's Taiwan region were analyzed.Results:There were 140 MH cases reported in Chinese mainland and 12 cases in China's Taiwan region, where most of the patients were adolescents, with more boys than girls, and the disease was often developed during oral surgery (cleft lip and palate repair), and orthopedic surgery (orthopedic surgery for congenital scoliosis). The typical clinical manifestations of MH patients included rapid increases in body temperature, abnormal increases in heart rate, muscular tension or muscle rigidity, and abnormal high end-tidal carbon dioxide partial pressure (P ETCO 2). The patients were mainly clinically diagnosed, while muscle biopsy or genetic testing was performed in several cases. Treatment measures were mainly cooling, respiratory and circulatory support, diuresis, correction of acidosis, and alkalinization of the urine. In Chinese mainland, when dantrolene was not available, a few patients underwent blood purification treatment, and the overall survival rate of MH patients was 54.3%. The application rate of dantrolene in China's Taiwan region was 83.3%, with a MH survival rate of 100%. Conclusions:Compared with patients in China's Taiwan region, dantrolene is less commonly used for MH patients in Chinese mainland, with a lower survival rate. With the increasing attention and understanding of MH, the proposal of a new version of the consensus of China, and the application of domestic dantrolene sodium for injection use, the treatment effectiveness of MH cases in Chinese mainland is expected to be improved.

Abstract:目的分析我国大陆恶性高热(MH)病例的临床资料,探讨诊断和治疗中应注意的问题.方法检索中国生物医学文献数据库,收集1978年1月至2004年3月报道的MH病例34例,分析其流行病学特征、发病特点、临床表现、治疗及转归.结果我国大陆报道的MH病例以青少年居多,男性多于女性,集中在口腔科手术(尤其是唇腭裂修补术)、骨科手术和小儿外科手术(尤其是先天畸形矫治术),典型的临床表现为突然发生的高碳酸血症、体温急剧升高和骨骼肌僵直等.诊断上主要是根据临床表现,尚未建立骨骼肌体外收缩试验方法,治疗上主要是对症处理,缺乏特效药物(丹曲林).结论近几年我国大陆MH病例逐渐增加,应加强对MH发病机制的研究,尽快建立MH的实验诊断标准,增强预防、诊断和治疗MH的能力。

Abstract:Objectives: A narrative expert review aiming to summarize the clinical epidemiology and management of critically ill patients with malignant hyperthermia (MH). Data sources: Medline searches were conducted to identify relevant articles describing the epidemiology, pathophysiology, and management of MH. Guidelines from key MH organizations were also incorporated into this review. Study selection: Relevant studies regarding MH in both ICU and perioperative settings were reviewed. Data extraction: Data from relevant studies were summarized and qualitatively assessed. Data synthesis: MH is a severe reaction triggered by inhalational volatile anesthetics and succinylcholine in genetically susceptible patients. The condition is characterized by an early onset (min to hr) rise in temperature, hypercarbia, and muscular rigidity following exposure to triggering medications with potential complications of coagulopathy, rhabdomyolysis, and acute kidney injury. Acute management necessitates a coordinated multidisciplinary team approach with specific management using dantrolene, active cooling, and hyperventilation. A suspected MH reaction has important implications for future anesthetic exposure for both the patient and their family. All suspected reactions should be followed up at a specialized MH testing center using muscle contracture and genetic testing. Conclusions: Increasing use of inhalational anesthetics in the ICU underscores the need for enhanced education on the diagnosis and management of MH to ensure optimal patient sedation care and safety.

Abstract:To the EditorWe present a case report of a 63-year-old man with ocular trauma admitted to the operating room for vitrectomy. He had medical history of hypertension, dyslipidemia, alcohol abuse, and chronic obstructive pulmonary disease Gold IIIC. He had been submitted to gastric suture under general anesthesia, 30 years ago with no known complications. He denied familial history of anesthetic complications. Pulmonary wheezing and rumbles were evident at auscultation. Standard American Society of Anesthesiologists monitorization was ensured and rapid sequence induction with propofol, fentanyl, and rocuronium was achieved. Sevoflurane was used for maintenance. Ninety minutes after induction, EtCO2 started to rise (60 mm Hg) with an obstructive capnography curve, increased airway pressure, and worsening of pulmonary wheezing at auscultation. Bronchospasm was assumed and treated with salbutamol, ipratropium bromide, hydrocortisone, magnesium sulfate, and ketamine. Despite treatment and ventilation adjustment, EtCO2 continued to rise until a maximum of 100 mm Hg. An arterial blood gas (ABG) analysis was obtained, showing acidosis (pH = 7.16) with hypercapnia (pCO2 = 88.7 mm Hg) and desaturation (SatO2 = 93.9%, with FiO2 = 60%, and pO2 = 88.6 mm Hg). Muscular rigidity became evident and temperature increased to a maximum of 40.5°C. The diagnosis of malignant hyperthermia was assumed. Sevoflurane was discontinued. High flow ventilation (FiO2 100%) and propofol perfusion were initiated. A dantrolene intravenous (iv) bolus (2.5 mg/kg) and 100 mL of 8.4% sodium bicarbonate were administered and active cooling measures initiated. An arterial line, central venous catheter, and urinary catheter were promptly placed. Venous blood samples were obtained to evaluate muscular enzymes, hepatic, and renal function. Intravenous crystalloids and 1 mg/kg furosemide were administered to maintain urine output and prevent renal injury. Noradrenaline infusion was started to maintain adequate median arterial pressures. ABG after starting therapeutic measures showed significant respiratory acidosis with severe hypercapnia (pH = 7.0, pCO2 = 156.9 mm Hg), without hypoxemia (PaO2 = 380 mm Hg with FiO2 = 100%), and a mild increase in lactate (2.7 mmol/L). Another iv bolus of dantrolene was given (total dose of 5 mg/kg). After a few minutes, temperature and EtCO2 started to drop. Forty-five minutes later, patient stabilized registering a core temperature of 37.2°C. Serialized ABG showed a progressive improvement in acidosis (pH = 7.355, pCO2 = 67.9 mm Hg). After stabilizing, he was taken to the intensive care unit (ICU) sedated with propofol and fentanyl, intubated and with a 24-hour iv infusion of dantrolene (0.25 mg/kg/h). In the ICU, muscular and hepatic enzymes gradually normalized, and renal function remained preserved. Patient was extubated on the third day, discharged from ICU on the fourth day and from the hospital 12 days after the procedure. Patient was referenced to follow-up anesthesiology and genetic consultations, where in vitro caffeine-halothane contracture and DNA tests confirmed the diagnosis of malignant hyperthermia (MH).
Emergency treatment of malignant hyperthermia during oral and maxillofacial surgery:Report of 2 consecutive cases

Abstract:PURPOSE: To investigate the emergency treatment of malignant hyperthermia during oral and maxillofacial surgery. METHODS: Two patients with oral and maxillofacial surgery developed malignant hyperthermia during operation. It was detected in time and emergency treatment was given promptly. Continuous renal replacement therapy and plasmapheresis were used when the CPK,Mb and LDH increased significantly. RESULTS: After emergency treatment and continuous renal replacement therapy,CPK,Mb and LDH in the blood plasma decreased significantly and the complication of DIC,ARDS and ARF were resolved .Two patients were successfully cured. CONCLUSIONS: We emphasize early recognition and appropriate therapy are the keys of curing MH successfully.
Diagnosis and rescue of malignant hyperthermia induced by anesthesia during radical surgery in a cervical cancer patient using the National Remote Emergency System: A case report

Abstract:Rationale: Malignant hyperthermia (MH) is a rare yet serious medical complication that typically arises following general anesthesia or the administration of specific anesthetics. Due to the infrequency of MH, anesthesiologists often lack sufficient expertise in identifying and managing it, leading to misdiagnosis and inappropriate treatment. There is an urgent need to enhance the diagnosis and management of MH through the utilization of relevant tools. Patient concerns: In this case, a 52-year-old woman underwent radical cervical cancer surgery under general anesthesia, with no family or significant medical history. She experienced a gradual increase in end-tidal carbon dioxide (ETCO 2 ) to a maximum of 75 mm Hg and a rise in body temperature from 36.5 to 37.5 °C in a very short period, as well as a blood gas analysis showing a pH of 7.217. Diagnosis: The anesthesiologist immediately used The WeChat applet-based National Remote Emergency System for Malignant Hyperthermia (MH-NRES), and the score was 40, which indicated that the patient was very likely to have MH. Interventions: We immediately discontinued sevoflurane and switched total intravenous anesthesia to maintain general anesthesia, with a rapid intravenous infusion of dantrolene sodium. Outcomes: The ETCO2 and the temperature quickly dropped to normal, followed by successful completion of the surgery, and the patient was discharged 8 days after surgery. Lessons: The experience can provide a basis use of MH-NRES and improve the ability of anesthesiologists to deal with intraoperative MH as well as increase the survival probability of patients.

The caffeine test of isolated human muscle in relation to malignant hyperthermia

Abstract:Summaryo| li]1.|Pharmacological tests of isolated fibers of skeletal muscle proved to be a means of clarifying the occurrence of certain genetic defects. li]2.|Muscle specimens from 74 subjects were investigated. Of these, 14 had recovered from an episode of malignant hyperthermia (MH), an often fatal complication of general anaesthesia which is known to occur on the basis of a genetic predisposition. The others were 25 relatives of MH patients, and 35 subjects unrelated to malignant hyperthermia. li]3.|The tests consisted in measuring the contracture tensions of isolated fibres of skeletal muscle when exposed to various concentrations of caffeine, once in the presence, once in the absence of the anaesthetic agent halothane. The method is described in detail. li]4.|Muscle specimens which in the presence of halothane required more than 1.3 millimolar concentration of caffeine to produce a tension increase of 1 gram were defined as controls. The 45 control data thus defined were normally distributed. Age, sex, type of muscle, and size of the specimen did not account for individual difference within this control population. The control group did not contain any MH patient who had developed rigidity during the attack. On the other hand, it did contain all but one of the specimens from MH-unrelated subjects. li]5.|The acceptance of the control data as a standard left a deviant group of 29 subjects. This group could be subdivided according to the caffeine-contracture in the absence of halothane.All those MH patients who had displayed some rigidity during their attack were in this deviant group; they were characterized by high sensitivity to caffeine. The deviant group included two subjects who had suffered an MH episode without rigidity; they differed from all others by a high resistance to caffeine contracture and thereby seemed to represent a separate category of MH which could not be further analyzed during the studies reported here.The deviant group contained 10 relatives of MH patients who differed in their caffeine response from the patients themselves. Hence there are at least three kinds of relatives of MH patients: those that are indistinguishable from patients, those that fall into the control group, and an intermediate group. One of the MH-unrelated subjects reacted like an intermediate, suggesting that this represents a defect which is not rare in the general population. li]6.|When these data were seen in the context of family groups, there was evidence of a recessively inherited predisposition to malignant hyperthermia. However, part of the data were also compatible with the repeatedly published evidence of dominant inheritance of this predisposition. Since multigenic inheritance appears to be excluded, the simplest genetic models which can reasonably account for the observations are either a two-gene case or the assumption of one gene locus with three alleles. Improvement of the test methods or an increase of the data base should allow a choice between genetic theories.
Pharmacokinetics of Dantrolene in the CVVH/PE Treatment of Malignant Hyperthermia in a 14-year-old Chinese Boy: A Case Report and Literature Review

Abstract:Malignant hyperthermia (MH) is a rare life-threatening response that triggered by exposure to specific anesthetics commonly used during surgery interventions. Dantrolene is a well-known drug which used as first-line therapy for MH. A 14-year-old Chinese boy with a mutation in RYR1 whose muscle biopsy diagnosis was central core disease (CCD) occurred MH after cervical spine surgery underwent general anesthesia without volatile anesthetics or succinylcholine. The MH crisis treatment workflow was started and intravenous dantrolene was used soon which combined with sequent continuous veno-venous hemofiltration (CVVH) and plasma exchange (PE) therapy. We explored the pharmacokinetics of dantrolene during the treatment and the results showed that a one-compartment model with first-order absorption and elimination was sufficient to characterize dantrolene pharmacokinetics. The clearance estimate for dantrolene was 0.43 mL/(min*kg) and volume of distribution was 0.61 L/kg. The clearance of dantrolene was significantly affected by CVVH and PE.

Abstract:Malignant hyperthermia is a life-threatening disease caused by derangement of the autonomic nerve system and hypermetabolism of the peripheral musculature. Commonly body core temperatures of more than 40 °C will be found in this disease which is caused mostly by psychopharmacological drugs like antidepressants, neuroleptics but also antibiotics, pain killers, anti-Parkinson drugs, and volatile anesthetics. The inducers of malignant hyperthermia interact with postsynaptic receptors (serotonin, anticholinergics) or muscular intracellular structures responsible for calcium utilization (volatile anesthetics, succinylcholine). Rarely malignant hyperthermia is a consequence of mental stress or vigorous exercise and or heat. Malignant hyperthermic syndromes lead to a severe dysbalance of the autonomic nerve system accompanied by rhabdomyolysis, disseminated intravascular coagulopathy, and finally multi-organ failure. Accordingly, medical management is primarily directed to stabilize vital functions, withdrawal of the causing drug, and if possible antagonizing toxic substances. The leading symptom hyperthermia needs to be treated physically with available cooling systems.
Mutation Analysis of the Small Heat Shock Protein 27 Gene in Chinese Patients with Charcot-Marie-Tooth Disease

Abstract:Background: Charcot-Marie-Tooth (CMT) disease, the most common hereditary peripheral neuropathy, is highly clinically and genetically heterogeneous, and mutations in at least 18 genes have been identified. Recently, mutations in small heat shock protein 27 (Hsp27) were reported to cause CMT disease type 2F and distal hereditary motor neuropathy.Objective: To investigate the frequency and phenotypic features of an Hsp27 mutation in Chinese patients with CMT disease.Design: DNA samples from 114 unrelated patients with CMT disease were screened for mutations in Hsp27 by polymerase chain reaction and direct sequencing. A cosegregated study was performed using the Mbil restriction endonuclease, and 50 healthy control subjects were analyzed. Haplotype analysis was performed using 5 short tandem repeat markers to analyze whether the families with the same mutation probably had a common ancestor.Results: One missense mutation, C379T, was detected in 4 autosomal dominant families with CMT'disease type 2, and haplotype analysis indicated that the 4 families probably had a common founder. The frequency of the Hsp27 mutation is 0.9% (1/111) in Chinese patients with CMT disease in our study, and the phenotypes were characterized by later onset (age, 35-60 years) and mild sensory impairments. Electrophysiological findings showed moderately to severely slowed nerve conduction velocities in lower limb nerves but normal or mildly reduced velocities in upper limb nerves.Conclusions: To our knowledge, this is the first report of an Hsp27 mutation in the People's Republic of China. The C379T mutation in Hsp27 also causes CMT disease type 2, except for distal hereditary motor neuropathy, and the phenotypes are distinct from the family with CMT disease type 2F described previously. A mutation of Hsp27 maybe uncommon in Chinese patients with CMT disease.
Malignant Hyperthermia in Bariatric Surgery: A Case Study With Clinical, Pathophysiological, Biochemical and Biophysical Correlations

Abstract:Malignant hyperthermia (MH) is an acute pharmacogenetic disorder, which while uncommon is potentially fatal. MH is a calcium channelopathy of skeletal muscle in which a constant increase of intracytoplasmic Ca2+ concentration occurs causing a change in cellular metabolism. A hypermetabolic state develops when susceptible patients are exposed to halogenated volatile inhalational anesthetic agents and depolarizing muscle relaxants and/or extreme physical activity in hot environments. MH presents variable clinical expression. During an episode of MH, the patient may present clinical signs and laboratory findings including masseter muscle spasm, tachycardia, rise in end-tidal CO2 (EtCO2), tachypnea, hyperthermia, cyanosis, metabolic acidosis, rhabdomyolysis, hyperkalemia, myoglobinuria, hyperlactacidemia, and acute renal failure. The aim of this case report is to describe an episode of MH associated with the use of halogenated anesthetic during bariatric surgery. A 29-year-old Brazilian man was admitted to the hospital to undergo a bariatric surgery. The patient's relevant medical history included morbid obesity grade III, allergy to sulfa-based drugs and severe obstructive sleep apnea. Preoperative evaluations with cardiopulmonary exercise testing, echocardiogram and electrocardiogram showed no anatomical and functional changes of the patient's heart. Surgical procedures lasted for 4 h, without complications, but the evolution of the patient's condition indicated oliguria and acute breathing insufficiency. Five hours after the patient was placed under mechanical ventilation in the intensive care unit, he started to present clinical signs of hypermetabolic state, with tremors, excessive sweating, and rapid body temperature increases. In the postoperative period, the patient had hyperglycemia, hypocalcemia, hypernatremia, hyperkalemia, changes in creatine phosphokinase (CPK), aspartate transaminase (AST), alanine aminotransferase (ALT), urea and creatine concentrations, and metabolic and respiratory acidosis. Urinalysis showed traces of proteinuria, presence of ketones, leukocytes, red blood cells, and urobilinogen. In our case report, MH crisis was diagnosed late; the dantrolene was not administrated because it was not available and the patient died. This detailed case report of MH episode triggered by isoflurane anesthetic during bariatric surgery allowed us to describe the severity and lethality of this hypermetabolic syndrome. Dantrolene should be mandatory in all operating rooms. Knowledge of the symptoms, an early diagnosis and an adequate treatment can prevent the death of patients in MH crisis.

Abstract:Bibliometric analysis is a widely used method to identify and evaluate the trends and characteristics of the scientific publications in a specific research field. A large volume of literature has been published in the field of malignant hyperthermia (MH). However, no bibliometric studies have been conducted to describe the characteristics of highly cited articles on MH. Therefore, this study aims to identify the 100 most frequently cited articles about malignant hyperthermia, describe their characteristics, and investigate research trends. Searches were performed in Web of Science Core Collection, Google Scholar, and Scopus from January 1900 to March 2023, with no language limitation. The 100 most frequently cited articles were selected and analyzed. Characteristics including publication year, journal, impact factor, authors, authors countries, citation number, journal citation indicator, study design, and topics were analyzed. For the 100 most cited articles, the number of times each article was cited ranged from 84 to 1254, with a median of 133. The number of articles published peaked in the decade of 1990s (n = 30). The articles were published in 39 journals, with Anesthesiology leading with 13 publications and then the British Journal of Anaesthesia with 12 publications. The authors were from 21 countries, led by the United States (n = 37). Forty articles focused on genetic susceptibility screening, 27 articles were about MH etiology, and 11 articles were about diagnosis and treatment. The mortality rate of MH and dantrolene availability are known to be uneven worldwide. This may partly be explained by the fact that scientific publications primarily report by authors from developed countries. This bibliometric analysis highlights the characteristics of the most influential research related to malignant hyperthermia, providing a reference for conducting future research.
[Malignant hyperthermia in a child with acute lymphatic leukemia]

Abstract:A 5-year-old boy with acute lymphatic leukemia in remission developed signs of malignant hyperthermia (MH) during general anesthesia for removal of a central venous access port. The anesthetic procedure for implantation of the port 17 months before had been uneventful despite use of the same triggering agents, halothane and succinylcholine. Meanwhile, the patient had received chemotherapy (COALL-03-85). The first sign of MH was masseter spasm following succinylcholine; then tachycardia, acidosis, myoglobinuria, and CPK elevation (8953 IU) appeared. There was only moderate temperature elevation to 37.8 degree C. Rapid improvement and complete recovery occurred after dantrolene i.v. The patient's father was found to have undiagnosed muscle pain and an elevated CPK level. An in vitro contracture test with halothane and caffeine revealed susceptibility to MH and supported the patient's diagnosis and genetic predisposition. Referring to several other cases in the literature concerning MH in patients with lymphomas and leukemias, a possible correlation between the two diseases is discussed. As the MH crisis in our patient was most probably genetic in origin, a common acquired cause such as a viral infection seems less probable. We do not believe the chemotherapy our patient received between the two anesthetics was the cause since about one-half of the patients in the literature had not had chemotherapeutic pretreatment at the time of the MH crisis. We believe that a common genetic predisposition is the most likely link between the two diseases. In any case, patients with leukemias and lymphomas should be monitored very carefully for symptoms of MH.(ABSTRACT TRUNCATED AT 250 WORDS)
Spinal cord injury-related thermoregulatory impairment masks a fatal malignant hyperthermia crisis: a case report

Abstract:Purpose: Malignant hyperthermia (MH) is a hypermetabolic disorder that can occur in genetically susceptible individuals exposed to halogenated anesthetics and succinylcholine. Spinal cord injury (SCI) above the sixth thoracic vertebra is associated with dysfunction of the sympathetic/parasympathetic nervous pathways, including thermoregulatory dysfunction, presenting as hypothermia in cold environments because of vasodilation and heat loss. This effect could mitigate or obscure an MH episode. Here, we describe development of a fatal MH crisis in a patient with SCI. Clinical features: A 27-yr-old male patient with an SCI after fracture of the sixth cervical vertebra was admitted for spinal arthrodesis. Anesthetic medications included remifentanil, propofol, succinylcholine, rocuronium, and isoflurane. After the start of the surgery, muscular contractures resembling myoclonus were noted, which resolved with pancuronium administration. Four hours after the start of anesthesia, the patient presented with hyperthermia, hypercarbia, hypotension, muscle rigidity, arrhythmia, and cardiogenic shock, with metabolic/respiratory acidosis. Malignant hyperthermia was suspected and the treatment was started, but he developed cardiopulmonary arrest and died an hour and a half after the first cardiac arrest. Both parents were investigated and were found to have normal creatine kinase levels and positive in vitro contracture tests. His mother carried a variant in the ryanodine receptor type 1 (RYR1) gene (c.14918C>T), which is associated with MH. Conclusion: Spinal cord injury-induced thermoregulatory dysfunction may obscure the early diagnosis of MH and lead to fatal outcome.

Abstract:Malignant hyperthermia is defined in the International Classification of Diseases as a progressive life-threatening hyperthermic reaction occurring during general anaesthesia. Malignant hyperthermia has an underlying genetic basis, and genetically susceptible individuals are at risk of developing malignant hyperthermia if they are exposed to any of the potent inhalational anaesthetics or suxamethonium. It can also be described as a malignant hypermetabolic syndrome. There are no specific clinical features of malignant hyperthermia and the condition may prove fatal unless it is recognised in its early stages and treatment is promptly and aggressively implemented. The Association of Anaesthetists has previously produced crisis management guidelines intended to be displayed in all anaesthetic rooms as an aide memoire should a malignant hyperthermia reaction occur. The last iteration was produced in 2011 and since then there have been some developments requiring an update. In these guidelines we will provide background information that has been used in updating the crisis management recommendations but will also provide more detailed guidance on the clinical diagnosis of malignant hyperthermia. The scope of these guidelines is extended to include practical guidance for anaesthetists dealing with a case of suspected malignant hyperthermia once the acute reaction has been reversed. This includes information on care and monitoring during and after the event; appropriate equipment and resuscitative measures within the operating theatre and ICU; the importance of communication and teamwork; guidance on counselling of the patient and their family; and how to make a referral of the patient for confirmation of the diagnosis. We also review which patients presenting for surgery may be at increased risk of developing malignant hyperthermia under anaesthesia and what precautions should be taken during the peri-operative management of the patients.
