Application of medical images in localization of epileptic foci

Abstract:AIM: To compare the results of positron emission tomography (PET), scalp video electroencephalography(VEEG), intracranial EEG and magnetic resonance imaging (MRI) in patients with intractable epilepsy and to explore their application values in the localization of epileptic foci. METHODS: PET, MRI and EEG were performed in 75 patients (52 males, 23 females, 30.6±18.3 years ) with intractable epilepsy; 51 patients received surgical treatment and 14 of them had intracranial electrodes implanted.All the patients who had undergone surgery had MRI examination 1 month after surgery. Seizure control was also evaluated according to Engel’s classification of surgical outcome.The efficacy of PET for the localization of epileptogenic focus was compared with that of VEEG, MRI and intracranial EEG. RESULTS: Seventy patients showed interictal focal hypometabolism, and 5 patients were norma1. PET localized the seizure foci in the patients whose VEEG showed bilateral or no definite epileptoform discharge. Surgical outcome of the patients with abnormal MRI images was better than that with normal MRI. CONCLUSION: The sensitivity and accuracy of the interictal PET imaging in localizing the seizure focus are significantly higher than those of MRI and scalp VEEG.The PET metabolic images combined with MRI and scalp VEEG can help the neurosurgeons to localize the epileptogenic foci, confirm the operative indications and predict the surgical outcomes.
