Abstract:Fetal brain MRI has become an important tool for in utero assessment of brain development and disorders. However, quantitative analysis of fetal brain MRI remains difficult, partially due to the limited tools for automated preprocessing and the lack of normative brain templates. In this paper, we proposed an automated pipeline for fetal brain extraction, super-resolution reconstruction, and fetal brain atlasing to quantitatively map in utero fetal brain development during mid-to-late gestation in a Chinese population. First, we designed a U-net convolutional neural network for automated fetal brain extraction, which achieved an average accuracy of 97%. We then generated a developing fetal brain atlas, using an iterative linear and nonlinear registration approach. Based on the 4D spatiotemporal atlas, we quantified the morphological development of the fetal brain between 23 and 36 weeks of gestation. The proposed pipeline enabled the fully automated volumetric reconstruction for clinically available fetal brain MRI data, and the 4D fetal brain atlas provided normative templates for the quantitative characterization of fetal brain development, especially in the Chinese population.
Automated 3D Fetal Brain Segmentation Using an Optimized Deep Learning Approach

Abstract:BACKGROUND AND PURPOSE: MR imaging provides critical information about fetal brain growth and development. Currently, morphologic analysis primarily relies on manual segmentation, which is time-intensive and has limited repeatability. This work aimed to develop a deep learning?based automatic fetal brain segmentation method that provides improved accuracy and robustness compared with atlas-based methods. MATERIALS AND METHODS: A total of 106 fetal MR imaging studies were acquired prospectively from fetuses between 23 and 39?weeks of gestation. We trained a deep learning model on the MR imaging scans of 65 healthy fetuses and compared its performance with a 4D atlas-based segmentation method using the Wilcoxon signed-rank test. The trained model was also evaluated on data from 41 fetuses diagnosed with congenital heart disease. RESULTS: The proposed method showed high consistency with the manual segmentation, with an average Dice score of 0.897. It also demonstrated significantly improved performance (P?<?.001) based on the Dice score and 95% Hausdorff distance in all brain regions compared with the atlas-based method. The performance of the proposed method was consistent across gestational ages. The segmentations of the brains of fetuses with high-risk congenital heart disease were also highly consistent with the manual segmentation, though the Dice score was 7% lower than that of healthy fetuses. CONCLUSIONS: The proposed deep learning method provides an efficient and reliable approach for fetal brain segmentation, which outperformed segmentation based on a 4D atlas and has been used in clinical and research settings.

Abstract:Diffusion MRI (dMRI) is known to be sensitive to hypoxic-ischemic encephalopathy (HIE) , however, the existing dMRI studies only used diffusion tensor metrics in a few selected brain regions and primarily focus on severe-to-moderate HIE. Here we investigated microstructural alterations using multi-shell dMRI across the whole-brain in severe, moderate, and mild HIE babies (n=5/13/13) in comparison with control neonates (n=11) with region-of-interest-based, tract-based, and fixel-based analysis. We found microstructural alternations in projection and association fibers, especially the inferior fronto-occipital fasciculus and inferior longitudinal fasciculus that are important for visual functions.

Abstract:Background: The 20th week gestational age (GA) is at mid-gestation and corresponds to the age at which the termination of pregnancy in several countries and the first Magnetic Resonance Imaging (MRI) can be performed, and at which the premature babies may survive. However, at present, very little is known about the exact anatomical character at this GA.Objective: To delineate the developing fetal brain of 20 weeks GA and obtain the three dimensional visualization model. Materials and methods: 20 fetal specimens were scanned by 3.0 T and 7.0 T post-mortem MRI, and the three dimensional visualization model was obtained with Amira 4.1.Results: Most of the sulci or their anlage, except the postcentral sulcus and intraparietal sulcus, were present. The laminar organization, described as layers with different signal intensities, was most clearly distinguished at the parieto-occipital lobe and peripheral regions of the hippocampus. The basal nuclei could be clearly visualized, and the brain stem and cerebellum had formed their common shape. On the visualization model, the shape and relative relationship of the structures could be appropriately delineated. The ranges of normal values of the brain structures were obtained, but no sexual dimorphisms or cerebral asymmetries were found.Conclusions: The developing fetal brain of 20 weeks GA can be clearly delineated on 3.0 T and 7.0 T postmortem MRIs, and the three dimensional visualization model supplies great help in precise cognition of the immature brain. Development of Layered Structures of Normal Fetal Cerebral Cortex: A Study Using MR Imaging

Abstract:PurposeTo analyze the developmental changes of the layered structures of normal fetal cerebral cortex using 3.0T MRI. Materials and Methods: Head of 30 fetal specimens of 14‐35 weeks gestational (GA) were scanned, and developmental changes of the layered structures were observed.ResultsCoronal T1 images of parieto‐occipital part of the brain are chosen to analyze. At 14 GA, 3 layers can be recognized. During 18‐23 GA, when the layered structures are typical, 7 layers of different signals including continuing periventricular fiber‐rich zone and external capsule fibers can be differentiated, which correspond to 5 zones in the histological staining slices. During 24‐27 GA, layered structures remains the same appearance as 18‐23 GA, but the borderlines among them become obscured and periventricular fiber‐rich zone and the external capsule fibers become discontinuous. After 30 GA, layered structures disappear and turn into mature‐like. The migrating thalamocortical fibers appeare as a high signal layer located at the lower, middle, and upper part of the subplate zone respectively at different GA.ConclusionThe development of layered structures of normal fetal cerebral cortex follows a specific spatio‐temporal law, and T1 images of parieto‐occipital part of the fetal brain obtained using 3.0T MRI are the best to show the developmental changes of layered structures of normal fetal cerebral cortex.

Abstract:Objective To study the capability of high field MRI in demonstrating the post-mortem fetal brains at different gestational age(GA).Methods One hundred and eight post-mortem fetal brains of 14-40 weeks GA were evaluated by 3.0 T MRI. Eleven brains of 14 to 27 weeks GA with good 3.0 T MRI images were chosen and scanned by 7.0 T MRI. The developing sulci, layered structures of fetal cerebral cortex and basal nuclei were evaluated on MRI of different Tesla(3.0 T and 7.0 T)and their results analyzed. Results On T_1 WI of 3.0 T MRI, the layered structures of fetal cerebral cortex were present at 14 weeks GA, the sulci were more accurately identified after 16 weeks GA. The basal nuclei were clearly distinguishable after 20 weeks GA. and these structures were better visualized as the GA increased. On T_2WI of 7.0 T MRI, the sulei, layered structures of fetal cerebral cortex and basal nuclei were shown more clearly at the same GA when compared to 3.0 T, especially the sulci at the early developmental stages. Conclusions T_1 WI of 3.0 T MRI could show the developing structures of post-mortem fetal brain well, but the T_2 WI of 7.0 T MRI were comparatively better.

Abstract:Objective To study the developmental process of the region of basal nuclei of postmortem fetuses by 3.0 T and 7.0 T MRI.Methods One hundred and thirty-one postmortem fetuses of 14 to 40 weeks of gestational age(GA)were scanned by 3.0 T MR,of which 11 fetuses of 14-27 weeks of GA were chosen and scanned by 7.0 T MR. The time when the structures in the region of basal nuclei could be detected and the changes of MR signal intensity were analyzed for MRI of different Tesla.Results On 3.0 T MRI.the dorsal thalamus could be delineated as early as 14 weeks of GA. The germinal matrix, caudate nucleus,and putamen could be visualized as early as 15 weeks of GA. The globus pallidus could be described as early as 18 weeks of GA.and the internal capsule and external capsule could be shown as early as 20 weeks of GA. The signal of the caudate nucleus during 15-30 weeks of GA was relatively hypointense on T1WI and hyperintense on T2WI.but during 31-40 weeks of GA, it was relatively hyperintense on T1WI and hypointense on T2WI. The putamen had a relatively high signal intensity on T1WI and low signal intemity on T1WI during 15-17 weeks of GA, and it appeared patchy during 18-25 weeks of GA,then it had a relatively low signal intensity on T1WI and high signal intensity on T2WI during 26-30 weeks of GA, and during 31-40 weeks of GA,its signal intensity was relatively high on T1WI and low on T2WI.The globus pallidus had a relatively high signal intensity on T1WI and low signal intensity on T2WI during 20-40 weeks of GA Compared to the 3.0 T MRI,the T2 images of 7.0 T MRl were more clear,and most structures in the region of basal nuclei could be clearly displayed as early as 16 weeks of GA.such as the germinal matrix,caudate nucleus,dorsal thalamus,putamen,globus pallidus,internal capsule,and external capsule.The claustrum could be delineated as early as 18 weeks of GA on 7.0 T MRI. Conclusions 3.0 T MRI could show the developmental process of the region of basal nuclei well,but the T2 images of 7.0 TMRl were comparatively better.

Abstract:Objective:This study aimed to delineate the fetal specimens brain structures at 10-weeks gestational age (GA) using 9.4 T ultra-high field magnetic resonance imaging (MRI) and analyze its relationship with corresponding histopathological sections.Methods:Six fetal specimens at 10 weeks GA with normal brain development were included in this study. The specimens were obtained because of spontaneous abortion and provided by the Research Center for Sectional and Imaging Anatomy of Shandong University Cheeloo College of Medicine. They were scanned by using 9.4 T MRI, and then sectioned and dyed to obtain the corresponding histopathological sections. The fetal brain structures were comparatively observed on the MRIs and histopathological sections, and 3D reconstruction models were obtained.Results:Four zones of the laminar organization were found in 9.4 T MRIs, whereas six zones were delineated in the corresponding histopathological sections. The ventricular zone, cortical plate, and marginal zone were delineated clearly on MRIs and histopathological sections. The mixed periventricular zone, subventricular zone, intermediate zone, and subplate zone were found on MRIs, whereas the periventricular zone, mixed subventricular zone, intermediate zone, and subplate zone were delineated on histopathological sections. Three zones of the choroid plexus in the lateral ventricles were observed on MRIs, with low signal intensity around the edges, slightly lower signal in the middle, and considerably lower signal intensity in the center, and on histopathological sections with epithelial cells on the edge, associated fiber matrix in the middle, and vascular tissue in the center. Good consistencies were also observed between the MRIs and histopathological sections. The surface of the cerebellum was smooth with low signal on MRIs, and each zone could not be found on histopathological sections, including the intermediate zone, molecular zone, and granular precursor cell zone from the inside to the outside. MRI 3D reconstruction models could directly delineate the shape, position, and adjacency of fetal brain structures. The fetal brain volume at 10 weeks GA was small. The choroid plexus was located in the lateral ventricles, while the laminar organization and cerebral hemisphere were located outside of the lateral ventricles. The cerebellum was crawled behind the midbrain and locally connected with the midbrain. The proportion of the lateral ventricles and choroid plexus was large in the brain, with the choroid plexus accounting for 40.4%±1.3% of the lateral ventricles and the lateral ventricles accounting for 46.1%±4.7% of the cerebral hemisphere.Conclusion:The fetal brain at 10 weeks GA is still at an early stage of development. In addition, good consistence is observed between the 9.4 T MRI and histopathological sections in delineating the fetal brain structures. The combination of the 9.4 T MRI and 3D reconstruction can better reveal the characteristics of the fetal brain structure at 10 weeks GA, which can provide a reference for clinicians to identify early diseases related to fetal development.

Abstract:OBJECTIVE:To obtain the three dimensional visualization model with normal measurements of fetal brain in the second trimester and analyze the developmental changes with gestational age (GA), sexual dimorphisms and cerebral asymmetries.METHODS:The brains of 69 fetal specimens of 12 - 22 weeks GA were scanned by 7.0T magnetic resonance imaging (MRI). The developing structures were analyzed and a three dimensional visualization model was rebuilt with Amira 4.1 software.RESULTS:Most sulci, except for postcentral and intraparietal sulcus, were present until 22 weeks GA. And none developed secondary branches. Laminar organization, described as early as 12 weeks GA, was delineated as layers with different signal intensities and became typical after 16 weeks GA. Basal nuclei was distinctly visible. Brains had different growth rates linearly increasing with GA. But neither sexual dimorphisms nor cerebral asymmetries was detected.CONCLUSIONS:The initial developmental stage of fetal brain occurs at 12 - 22 weeks GA. The developing structures may be distinctly visualized on 7.0T post-mortem MRI. And the three dimensional visualization model aids greatly in the precise cognition of immature brain.

Abstract:The cerebellum is one of the most important structures in the posterior cranial fossa, but the characterization of its development by magnetic resonance imaging (MRI) is incomplete. We scanned 40 fetuses that had no morphological brain disorder at 14-22 weeks of gestation using 7.0 T MRI. Amira 4.1 software was used to determine morphological parameters of the fetal cerebellum, which included the cerebellar volume (CV), transverse cerebellar diameter (TCD), and the length and width of the vermis. The relationship between these measurements and gestational age (GA) was analysed. We found that the primary fissure was visible at week 14 of gestation. From week 16, the prepyramidal fissure, the secondary fissure and the dentate nucleus could be identified. The posterolateral fissure and the fourth ventricle were recognized at week 17, whereas the tentorium of the cerebellum was visible at week 20. The relationships between GA and CV, TCD, and the width and length of the vermis were described adequately by second-order polynomial regression curves. The ratios between TCD and vermis length and between TCD and vermis width decreased with GA. These results show that 7.0 T MRI can show the trajectory of cerebellar development clearly. They increase our understanding of normal cerebellar development in the fetus, and will facilitate the diagnosis of pathological intrauterine changes in the cerebellum.
Development of fetal sulci of 20~30 weeks gestational age: a comparative demonstration between post-mortem and in vivo fetal MRI

Abstract:Objective To study fetal sulcal development with post-mortem and in vivo fetal MRI,and then to analyze the differences and relationships between the two kinds of images.Methods 20 fetal specimens of 20~30 weeks gestational age were scanned by 3.0 T MRI with common sequence.100 in vivo fetuses,which had doubtable abnormalities after ultrasonograph examination,were scanned by 1.5T MRI with HASTE sequence,then 30 fetuses,which did not have abnormalities during the follow-up investigation after birth,were selected.The post-mortem and in vivo fetal MRIs of the same age were analyzed on the corresponding transverse,sagittal and coronal directions,and then the developmental and demonstrational condition of the fetal sulci at different gestational age were obtained.Results There were good consistencies between the post-mortem and in vivo fetal MRI in demonstrating the sulcal development.The fissures and sulci could all be delineated on both MRIs especially after 24 weeks gestational age,and the post-mortem MRI was better than the in vivo fetal MRI in supplying information of more distinct,various and accurate.Conclusion Post-mortem and in vivo fetal MRI could well delineate fetal sulcal development,and it was significant to study post-mortem MRI for the analysis and diagnosis of in vivo fetal MRI.
Development of fetal cerebral cortex: assessment of the folding conditions with post-mortem Magnetic Resonance Imaging

Abstract:Quantitative data of fetal cortical folding and its developmental changes supply important information in the estimation of fetal age and assessment of brain maturation, so the increasing tendencies of cortical growth and its folding conditions at the beginning of the second and third trimesters with post-mortem Magnetic Resonance Imaging (MRI) were analyzed. 131 fetal specimens of 14–40 weeks gestational age (GA) were selected and scanned with 3.0T MR. Then the length of folded cortical margin (LFCM) and length of unfolded cortical margin (LUCM) were measured by Photoshop and ZoomMagic software. Degrees of cortical folding (DCF) were calculated by means of (LFCM−LUCM)/LFCM. Growth curves were obtained between the 3 above values and GA, and significant differences in age stages, hemispheres and genders were analyzed. The relationship between LFCM in centimeters, DCF and GA in weeks was described by two exponential growth curves [LFCM=5.325exp(0.079GA); DCF=11.890exp(0.043GA)]. The curves increased rapidly after 26 weeks GA, which could be recognized as a cut-off point of fetal cortical and sulcal development. LUCM and GA were described by a logarithmic growth curve which slowed down after 26 weeks GA [LUCM=30.580Ln(GA)−72.490]. Significant differences of the 3 values before and after 26 weeks GA (p<0.01), but not any in hemispheres and genders were noticed. These results, which may be valuable in assessing normal brain development and can serve as a model in clinical settings, indicate that the cerebral volume first increases and is then followed by increases of the surface area.

Abstract:BACKGROUND:The fetal brain undergoes a dynamic process of development during gestation, marked by well-orchestrated events such as neuronal proliferation, migration, axonal outgrowth, and dendritic arborization, mainly elucidated through histological studies. Ex vivo magnetic resonance imaging (MRI) has emerged as a useful tool for 3D visualization of the developing fetal brain, serving as a complementary tool to traditional histology. SUMMARY:In this review, we summarized the commonly employed ex vivo MRI techniques and their advances in fetal brain imaging, and proposed a standard protocol for postmortem fetal brain specimen collection and fixation. We then provided an overview of ex vivo MRI-based studies on the fetal brain. KEY MESSAGES:According to our review, ex vivo T1- or T2-weighted structural MRI has contributed to the characterization of the anatomy of transient neuronal proliferative zones, the basal ganglia, and the cortex. Diffusion MRI-related techniques, such as diffusion tensor imaging and tractography, have helped investigate the microstructural patterns of fetal brain tissue, as well as the early emergence and development of neuronal migration pathways and white matter bundles. Ex vivo MRI findings have shown strong histological correlations, supporting the potential of MRI in evaluating the developmental events in the fetal brain. Postmortem MRI examinations have also demonstrated comparable, and in certain cases, superior performance to traditional autopsy in revealing fetal brain abnormalities. In conclusion, ex vivo fetal brain MRI is an invaluable tool that provides unique insights into the early stages of brain development.

Abstract:In this study, scalar values for the fetal brain from 12 to 20 weeks gestational age were obtained. Fifty-two fetal specimens of 12-20 weeks gestational age with an anatomically normal and developmentally appropriate central nervous system (CNS) were scanned using a 7.0T magnetic resonance imaging (MRI) scanner. The linear biometric measurements of the brain were then determined. All the measurements (except for the interhemispheric distance) were found to increase linearly with gestational age, although each increased at a different growth rates. The 95% confidence interval for each value was obtained. These data may be considered to be a valuable reference for the assessment of normal fetal brain development in clinical settings and as a supplement to post-mortem MRI or anatomical investigations. Methods 87 fetal specimens without CNS diseases aged from 14-40 gestational weeks were scanned under 3.0T MRI.The cerebellar transverse diameter (TCD), anteroposterior diameter (A-PD), vermis length and cerebellar volume were then calculated by Amira software. Lastly the relationship between these values with fetal age(FA) was analyzed statistically. Results The TCD, A-PD and vermis length increased linearitily following the growth of the fetuses [TCD=0.165FA -1.410; A-PD=0.091FA-0.717; vermis length=0.108FA-0.832]. The relationship between cerebellar volume (in cubic centimeters) and FA (in weeks) was well described by a single exponential function [CV=0.034 exp (FA/5.918)]. The doubling time of CV was about 4.1 weeks. Conclusions The development of fetal cerebellum could be displayed and quantified by 3.0T MRI, which is valuable for clinical diagnosis and evaluation of fetal development in vivo.

Abstract:Abstract Magnetic resonance imaging (MRI) is widely used to provide detailed information regarding fetal brain development in utero. Conventional T1‐and T2‐weighted sequences provide anatomical details of the normal brain and demonstrate brain lesions. In addition to providing highly detailed qualitative assessments of fetal brain development, advanced MRI methods such as three‐dimensional high‐resolution MRI, diffusion MRI, magnetic resonance spectroscopy, and functional MRI can provide quantitative morphologic assessments of tissue microstructure and functional activity. This review aims to describe normal fetal brain development and highlight current state‐of‐the‐art MRI sequences for fetal neuroimaging. We focus on current clinical applications which can provide a better understanding of in utero impairments in fetal brain development.
Development of the subcortical brain structures in the second trimester: assessment with 7.0-T MRI

Abstract:Introduction This study aims to obtain the signal intensity changes and quantitative measurements of the subcortical brain structures of 12–22 weeks gestational age (GA). Methods Sixty-nine fetal specimens were selected and scanned by 7.0-T MR. The signal intensity changes of the subcortical brain structures were analyzed. The three-dimensional visualization models of the germinal matrix, caudate nucleus, lentiform nucleus, and dorsal thalamus were rebuilt with Amira 4.1, and the developmental trends between the measurements and GA were analyzed. Results The germinal matrix was delineated on 7.0-T MR images at 12 weeks GA, with high signals on T1-weighted images (WI). While at 16 weeks GA, the caudate nucleus, lentiform nucleus, and internal and external capsules could be distinguished. The caudate nucleus was high signal intensity on T1WI. The signal intensity of the putamen was high on T1WI during 15–17 weeks GA and was delineated as an area with uneven signal intensities. The signal intensity of the peripheral area of the putamen became higher after 18 weeks GA. The signal intensity of the globus pallidus was high on T1WI and low on T2WI after 20 weeks GA. At 18 weeks GA, the claustrum was delineated with low signals on T2WI. Measurements of the germinal matrix, caudate nucleus, lentiform nucleus, and dorsal thalamus linearly increased with the GA. Conclusion Development of the subcortical brain structures during 12–22 weeks GA could be displayed with 7.0-T MRI. The measurement provides significant reference beneficial to the clinical evaluation of fetal brain development.
The ultrastructural changes of cerebral cortex during fetal development

Abstract:Objective To disclose the mechanism of neuron development by investigating ultrastructural changes of cerebral cortex during fetal development,then to explore the correlation between neuron development and congenital nervous system diseases.Methods The cerebral tissue samples of 3,4,5,6 and 7-monthold fetuses were collected,whose nervous systems showed no lesion and deformity.The samples were gained from motor area which limited by precentral gyrus.Electronic microscope was used to observe the ultrastructure of cerebral neurons and synapse.Results With the increase in month,cellular volume of neurons was increased,with more and longer spines.Noticeable morphism variations in the nucleolus and organelle in some neurons could be observed under electron microscope;with the increase in month,the structure and function of synapse were gradually completed.Conclusion The 6 and 7-month stage of fetus is vital for the fetal development.In this period congenital malformations are taken place quite often.

Abstract:OBJECTIVE:To provide sectional anatomic data for the precise localization of developmental malformation of fetal brain in sagittal magnetic resonance imaging (MRI).METHOD:After abdominal and pelvic MRI scanning, the gravid specimen was cut into serial sagittal slices in correspondence with MRI in a low temperature laboratory to demonstrate the structures of fetal brain.RESULT:(1) Directional determination of the sloping and rotating fetal head. From the serial sagittal sections of pregnant cadaver at term, we concluded that, the longitudinal lying and cephalic presentation fetal had run into maternal pelvis, and rotated and sloped to right. Anteroposterior position and median sagittal plane of the fetal was in correspondence with his mother's. (2) Seven serial sagittal sections of the fetal brain were obtained through lateral surface of the right cerebral hemisphere, lateral sulcus, internal capsule, median sagittal plane, middle cerebellar peduncle, brainstem, and lateral surface of the left cerebral hemisphere.CONCLUSION:Through the comparison study between sagittal sections and corresponding MRI of fetal brain at term, we could obtain morphological anatomic structures and MRI of fetal brain, providing morphological demonstration of the intrauterine development of fetal brain and auxiliary diagnosis of ultrasound and MRI in pregnant woman.
Development of the Fetal Brain Cortico-Cortical Structural Network During the Second-to-third Trimester Based on Diffusion MRI

Abstract:During the second-to-third trimester, the neuronal pathways of the fetal brain experience rapid development, resulting in the complex architecture of the interwired network at birth. While diffusion MRI-based tractography has been employed to study the prenatal development of structural connectivity network (SCN) in preterm neonatal and postmortem fetal brains, the in utero development of SCN in the normal fetal brain remains largely unknown. In this study, we utilized in utero dMRI data from human fetuses of both sexes between 26 and 38 gestational weeks to investigate the developmental trajectories of the fetal brain SCN, focusing on intrahemispheric connections. Our analysis revealed significant increases in global efficiency, mean local efficiency, and clustering coefficient, along with significant decrease in shortest path length, while small-worldness persisted during the studied period, revealing balanced network integration and segregation. Widespread short-ranged connectivity strengthened significantly. The nodal strength developed in a posterior-to-anterior and medial-to-lateral order, reflecting a spatiotemporal gradient in cortical network connectivity development. Moreover, we observed distinct lateralization patterns in the fetal brain SCN. Globally, there was a leftward lateralization in network efficiency, clustering coefficient, and small-worldness. The regional lateralization patterns in most language, motor, and visual-related areas were consistent with prior knowledge, except for Wernicke's area, indicating lateralized brain wiring is an innate property of the human brain starting from the fetal period. Our findings provided a comprehensive view of the development of the fetal brain SCN and its lateralization, as a normative template that may be used to characterize atypical development.
