Abstract:Objective: The aim of this study was to investigate the mechanism to reverse the drug resistance of leukemia cells in Cyclosporine A(CsA), tetrandrine (Tet) alone or their combination by using empirical methods, and to provide the basic evidence for further study, and lay the theoretical basis for the clinical applications. Method: By MTT assay, the IC50(the concentration causing 50% inhibitor of cell growth) of DNR was determined;By flow cytomet ry (FCM) assay, the intracellular DNR concentration and the expression of P-glyco-protein (P-gp) were examined, and the apoptotic changes were observed by Anexin-V. By fluorescent semi-quantitative reverse transcriptase polymerase chain reaction (RT-PCR), the mRNA expression of multidrug resistance (MDR) was measured. Results: The IC50 of DNR for K562 and K562/A02 cells were 0.26mg/l and 21.22mg/l respectively. Treating K562/A02 cells with CsA(1mg/L), TTD(0.1mg/L)and both for 48 hours partially restored the sensitivity of K562/A02 cells to DNR(IC50 were 7.83 mg/L,5.32 mg/L,2.33 mg/L respectively.) but had not effect on K562 cells; K562/A02 cells showed apoptotic characteristics after treated with CsA, and both for 48 hours(Apoptosis rate was 10.27%,17.64%,52.79% respectively); The fluorescence intensity of intracellelar DNR in K562 and K562/A02 cells treated with DNR(1mg/L)was 522,173 respectively. CsA and TTD (alone or combination) elevated the intracellular DNR concentration in K562/A02 cells(the fluorescence intensity of intracellelar DNR in K562/A02 cells was 188,339,515 respectively); The fluorescence intensity of P-gp in K562 and K562/A02 cells was 0.5,97.97. The P-gp expression was down after treated with CsA, TTD and both(73.7,65.05,58.1); mdr1 mRNA was also down regulated, and the effect of their combination was greater. Date was analyzed by SPSS 13.0 software and expressed as mean ± SD. Conclusions: High expression of P-gp may be involved in the mechanism of multidrug resistance (MDR) of K562/A02 cells line; MDR can be partially reversed by CsA or TTD, of which the combination shows a great synergistic reversal effect.

Abstract:Objective:To summarize the progress of the targets and resistance to molecular targeted anti-tumor drugs.Methods:Papers were searched with tumor, resistance, targeted drugs and reversal of drug resistance as key words, and 32 papers were finally screened out according to the folowing inclusion criteria:①Molecular targets of targeted drugs;② Molecular mechanism of targeted resistance;③ Reversing the targeted drugs resistance.Results:The targets of targeted drugs are based on cell membrane receptor and intracellular kinases. The molecular mechanisms of targeted drugs resistance include several factors such as cel signal transduction, genetic mutation, tumor stem cel, tumor microenvironment, tumor cel metabolism and epithelial-mesenchymal transition, etc. Individualized treatment, combination treatment, improving tumor microenvironment and new drugs development may help to revise drug resistance.Conclusion:The targets of targeted drugs are varied, and the mechanisms of drug resistance are complicated. A series of measures should be taken to prevent and reverse targeted drug resistance.
Rational combinations of targeted cancer therapies: background, advances and challenges

Abstract:Over the past two decades, elucidation of the genetic defects that underlie cancer has resulted in a plethora of novel targeted cancer drugs. Although these agents can initially be highly effective, resistance to single-agent therapies remains a major challenge. Combining drugs can help avoid resistance, but the number of possible drug combinations vastly exceeds what can be tested clinically, both financially and in terms of patient availability. Rational drug combinations based on a deep understanding of the underlying molecular mechanisms associated with therapy resistance are potentially powerful in the treatment of cancer. Here, we discuss the mechanisms of resistance to targeted therapies and how effective drug combinations can be identified to combat resistance. The challenges in clinically developing these combinations and future perspectives are considered.

Abstract:Multidrug resistance (MDR) is one of the main reasons leading to the failure of tumor chemotherapy and affecting the therapeutic effect. There are two main causes of MDR: The first refers to the activation of efflux pumps for drugs, which can transport intracellular drug molecules from cells; Another mechanism is associated with non-efflux pumps, e.g., apoptosis prevention, strengthened DNA repair, and strong oxidation resistance. In recent years, nanodrug delivery systems have attracted wide attentions, showing some unparallel advantages such as drug targeting and reduced drug efflux, drug toxicity and side effects in reversing MDR. It is worth mentioning that, based on the drug delivery platform of nanotechnology, multiple therapeutic strategies are integrated into one system, which can compensate their personal shortcomings. In this review, the mechanisms of tumor MDR, common vectors and nanocarriers-combined therapy strategies to reverse MDR were summarized to promote the understanding of the latest progress in improving the efficiency of chemotherapy and synergistic strategies. Especially, the adopted nanotechnology was highlighted, and the the underlying principles were shed light on, which are expected to provide guidances to promote the development of more effective anticancer strategies.

Abstract:Resistance to anticancer drugs is a major problem in oncology, which causes the failure of antitumor treatment. A variety of factors contribute to drug resistance, including drug efflux and metabolism, tumor cell heterogeneity, tumor microenvironment stress-induced genetic or epigenetic alterations in the cancer cells and so on. However, how to circumvent this resistance to improve anticancer efficacy remains to be determined. To circumvent chemotherapeutic resistance, many reversal agents have been developed, but most of them fail in clinical trials due to severe adverse effects. Recently, several natural products have been reported to augment sensitivity or overcome resistance of anticancer chemotherapeutic drugs, including elemene, curcumin, Shenqi Fuzheng Injection (参芪扶正注射液), PHY906, etc. Thus, understanding the novel function of Chinese medicine may allow us to develop a promising therapeutic approach to enhance the effects of anticancer strategies and prevent or overcome their resistance in the treatment of cancer patients.

Abstract:Chemotherapy is the mainstay in the treatment of breast cancer. However, many drugs that are commonly used in clinical practice have a high incidence of side effects and multidrug resistance (MDR), which is mainly caused by overexpression of drug transporters and related enzymes in breast cancer cells. In recent years, researchers have been working hard to find newer and safer drugs to overcome MDR in breast cancer. In this review, we provide the molecule mechanism of MDR in breast cancer, categorize potential lead compounds that inhibit single or multiple drug transporter proteins, as well as related enzymes. Additionally, we have summarized the structure-activity relationship (SAR) based on potential breast cancer MDR modulators with lower side effects. The development of novel approaches to suppress MDR is also addressed. These lead compounds hold great promise for exploring effective chemotherapy agents to overcome MDR, providing opportunities for curing breast cancer in the future.

Abstract:Chemotherapy has been widely applied in oncotherapy. However, the development of multidrug resistance (MDR) has diminished the effectiveness of anticancer drugs against tumor cells. Such resistance often results in tumor recurrence, metastasis, and patient death. Fortunately, nanoparticle-based drug delivery systems provide a promising strategy by codelivery of multiple drugs and MDR reversal agents and the skillful, flexible, smart modification of drug targets. Such systems have demonstrated the ability to bypass the ABC transporter biological efflux mechanisms due to drug resistance. Hence, how to deliver drugs and exert potential antitumor effects have been successfully explored, applied, and developed. Furthermore, to overcome multidrug resistance, nanoparticle-based systems have been developed due to their good therapeutic effect, low side effects, and high tumor metastasis inhibition. In view of this, we systematically discuss the molecular mechanisms and therapeutic strategies of MDR from nanotherapeutics. Finally, we summarize intriguing ideas and future trends for further research in overcoming MDR.

Breath-by-breath VCO2 and VO2 required compensation for transport delay and dynamic response.

Abstract:Both transport delay (DELAY) and dynamic response (RESPONSE) of a mass spectrometer would theoretically result in considerable errors in the breath-by-breath calculation of VCO2 and VO2. However, curiously, the contribution of RESPONSE has been ignored. The purpose of this study is to quantify the error caused by RESPONSE. We found that RESPONSE of a mass spectrometer was regarded as a first-order response. We determined DELAY and time constant (T) of RESPONSE and compensated the on-line calculation for both DELAY and RESPONSE and for DELAY only. With T of 150 and 100 ms, deviations of VCO2 from the gas-collection method were 8 +/- 6 and 8 +/- 6 ml/min with compensation for both DELAY and RESPONSE, and 69 +/- 10 and 50 +/- 5 ml/min with compensation for DELAY only, respectively (mean +/- SD). Similar results were obtained with VO2. A computer simulation of error caused by RESPONSE disclosed that the error linearly increased with increasing T. We conclude that to be accurate within +/- 5% of the exact value, compensation should be made when T exceeds 25 ms.

Abstract:Overexpression of ATP-binding cassette (ABC) transporters in cancer cells results in multidrug resistance (MDR), which is one of the major obstacles in the treatment of cancer patients. None of the strategies to overcome MDR has been successfully applied in the clinic until now. Plenty of evidence shows that some triterpenoids function as reversal agents of MDR for anticancer drug resistance treatment. Here, we review the latest findings of reversing cancer MDR with triterpenoids. Findings are summarized showing that triterpenoids are MDR modulators and potential chemosensitizers. Finally, we contemplate future prospects of modulating MDR in the clinic.

Anti-Cancer Agents in Medicinal Chemistry

Abstract:Multidrug resistance occurs when a tumor develops resistance to multiple chemotherapeutic drugs, which may include antitumor drugs with different chemical structures and mechanisms. Multidrug resistance limits the treatment effects of antitumor drugs, and is the main cause of chemotherapy failure. Multidrug resistance is caused by numerous factors including changes in ATP-binding cassette transporters, target proteins, detoxification, deoxyribonucleic acid repair, drug metabolic enzymes, and signal pathways of apoptosis. Clinical research indicates that natural products have great potential to treat tumors and reverse multidrug resistance. Natural products, which often have multiple targets, could play an important role in tumor treatment, have beneficial effects on tumor inhibition, improve symptoms, reduce radiotherapy and chemotherapy side effects, enhance immunity, and prolong survival. Because natural products often have few adverse reactions and less drug resistance, the antitumor activities of natural products have attracted extensive research. We aimed to review the basic research and clinical application of natural products in the reversal of multidrug resistance.

Abstract:Multidrug resistance is a major cause that leads to the refractory of cancers after a combination strategy by chemotherapy, radiation and surgical treatment. Among the comprehensive treatment, chemotherapy still plays a crucial role in eliminating malignant cells. The objectives of present review were to elucidate the research advances in the mechanism of cancer multidrug resistance and the liposomes-based treatment strategy. The drug resistance could be related to cancer cells, heterogeneity, microenvironment, and physiological barriers. Drug resistance mechanisms, which involved in adenosine triphosphate (ATP)-binding cassette (ABC) transporters, cytoplasm, nucleus, apoptotic proteins, cancer stem cells, side-population cancer cells, angiogenesis, vasculogenic mimicry channels, extracellular matrix, and blood–brain barrier, were discussed. Latest advances in the nanostructured liposome treatment strategy were summarized, including regulating the overexpression of ABC transporters, and targeting treatments on mitochondria, apoptotic genes, cancer stem cells, cancer side-population cells, new blood vessels, vascular mimicry channels, extracellular matrix, and blood–brain barrier. These translational investigations provide new insights into current cancer therapy, and useful considerations for further developments of the liposomes-based treatment strategy.

Abstract:Chemotherapy for tumors occasionally results in drug resistance, which is the major reason for the treatment failure. Higher drug doses could improve the therapeutic effect, but higher toxicity limits the further treatment. For overcoming drug resistance, functional nano-drug delivery system (NDDS) has been explored to sensitize the anticancer drugs and decrease its side effects, which are applied in combating multidrug resistance (MDR) via a variety of mechanisms including bypassing drug efflux, controlling drug release, and disturbing metabolism. This review starts with a brief report on the major MDR causes. Furthermore, we searched the papers from NDDS and introduced the recent advances in sensitizing the chemotherapeutic drugs against MDR tumors. Finally, we concluded that the NDDS was based on several mechanisms, and we looked forward to the future in this field.

Abstract:Chemotherapy is one of the important methods to treat cancer, and the emergence of multidrug resistance (MDR) is one major cause for the failure of cancer chemotherapy. Almost all anti-tumor drugs develop drug resistance over a period of time of application in cancer patients, reducing their effects on killing cancer cells. Chemo resistance can lead to a rapid recurrence of cancers and ultimately patient death. MDR may be induced by multiple mechanisms, which are associated with a complex process of multiple genes, factors, pathways and multiple steps, and today the MDR associated mechanisms are largely unknown. In this paper, from the aspects of protein–protein interactions, alternative splicing (AS) in pre-mRNA, non-coding RNA (ncRNA) mediation, genome mutations, variance in cell functions, and influence from the tumor microenvironment, we summarize the molecular mechanisms associated with MDR in cancers. In the end, prospects for exploration of antitumor drugs that can reverse MDR are briefly discussed from the angle of drug systems with improved targeting properties, biocompatibility, availability, and other advantages.

Abstract:Anti-angiogenesis therapy, a promising strategy against cancer progression, is limited by drug-resistance, which could be attributed to changes within the tumor microenvironment. Studies have increasingly shown that combining anti-angiogenesis drugs with immunotherapy synergistically inhibits tumor growth and progression. Combination of anti-angiogenesis therapy and immunotherapy are well-established therapeutic options among solid tumors, such as non-small cell lung cancer, hepatic cell carcinoma, and renal cell carcinoma. However, this combination has achieved an unsatisfactory effect among some tumors, such as breast cancer, glioblastoma, and pancreatic ductal adenocarcinoma. Therefore, resistance to anti-angiogenesis agents, as well as a lack of biomarkers, remains a challenge. In this review, the current anti-angiogenesis therapies and corresponding drug-resistance, the relationship between tumor microenvironment and immunotherapy, and the latest progress on the combination of both therapeutic modalities are discussed. The aim of this review is to discuss whether the combination of anti-angiogenesis therapy and immunotherapy can exert synergistic antitumor effects, which can provide a basis to exploring new targets and developing more advanced strategies.

Abstract:Combination therapy is one of the important treatment strategies for cancer at present. However, the outcome of current combination therapy based on the co-administration of conventional dosage forms is suboptimal, due to the short half-lives of chemodrugs, their deficient tumor selectivity and so forth. Nanotechnology-based targeted delivery systems show great promise in addressing the associated problems and providing superior therapeutic benefits. In this review, we focus on the combination of therapeutic strategies between different nanomedicines or drug-loaded nanocarriers, rather than the co-delivery of different drugs via a single nanocarrier. We introduce the general concept of various targeting strategies of nanomedicines, present the principles of combination antitumor therapy with dual-nanomedicines, analyze their advantages and limitations compared with co-delivery strategies, and overview the recent advances of combination therapy based on targeted nanomedicines. Finally, we reviewed the challenges and future perspectives regarding the selection of therapeutic agents, targeting efficiency and the gap between the preclinical and clinical outcome.

Abstract:Chemotherapy is one of the most commonly used clinical treatments among the currently available cancer therapies. However, the phenomenon of Multidrug resistance (MDR) has become a challenge in the treatment process, weakening the impact of chemotherapy. Extensive research on elucidating the development of cancer MDR has identified the following mechanisms that play a critical role in the development of several MDR reversal agents: abnormal expression of cell membrane transporters, adaptation of cancer cells to the microenvironment, regulation of hypoxia, repair of DNA damage and reduction of apoptosis, the enhancement of the EMT process, the existence of cancer stem cells (CSCs), and the abnormal activation of key signaling pathways. However, they failed to demonstrate significant efficacy due to severe side effects during their clinical trials. Traditional Chinese medicines (TCMs) are known to play an important anti-cancer role since they have low toxicity, high efficacy, and safety and can reverse MDR. TCMs reversal agents can be divided into Chinese medicine monomers, synthetic monomers, analogs, or derivatives. Several studies have shown that TCMs can effectively overcome cancer MDR and can be effectively used for treating cancer patients.

Abstract:Combination chemotherapy has been a mainstay in cancer treatment for the last 60 years. Although the mechanisms of action and signaling pathways affected by most treatments with single antineoplastic agents might be relatively well understood, most combinations remain poorly understood. This review presents the most common alterations of signaling pathways in response to cytotoxic and targeted anticancer drug treatments, with a discussion of how the knowledge of signaling pathways might support and orient the development of innovative strategies for anticancer combination therapy. The ultimate goal is to highlight possible strategies of chemotherapy combinations based on the signaling pathways associated with the resistance mechanisms against anticancer drugs to maximize the selective induction of cancer cell death. We consider this review an extensive compilation of updated known information on chemotherapy resistance mechanisms to promote new combination therapies to be to discussed and tested.

Abstract:Camptothecin (CPT) derivatives are widely used as small molecule chemotherapeutic agents and have demonstrated efficacy in the treatment of diverse solid tumors. A variety of derivatives have been developed to resolve the drawbacks of poor water solubility, high toxicity and rapid hydrolysis in vivo . Howver, the obstacles, such as acquired resistance and toxicity, still exist. The utilisation of rational drug combinations has the potential to enhance the efficacy and mitigate the toxicity of CPT derivatives. This paper provides an overview of CPT derivatives in combination with other drugs, with a particular focus on cell cycle inhibitors, DNA synthesis inhibitors, anti-metastatic drugs and immunotherapy agents. Concurrently, the mechanisms of antitumor activity of combinations of different classes of drugs and CPT derivatives are elucidated. While the various combination strategies have yielded more favourable therapeutic outcomes, the efficacy and toxicity of the drug combinations are influenced by the inherent properties of the drugs involved. Moreover, a summary of the drug conjugates of CPT derivatives was provided, accompanied by an analysis of the structural activity relationship (SAR). This paves the way for the subsequent developments in drug combinations and delivery modes.

Abstract:OBJECTIVE:Multidrug resistance (MDR) plays an important role in the failure of chemotherapy on malignant tumors. This study was to investigate the biomolecular mechanisms of cyclosporine A (CsA), tetrandrine (Tet) and their combination on multidrug resistance in cell line K562/A02.METHODS:K562/A02 cells were treated with CsA and (or) Tet. The intracellular daunorubicin (DNR) concentration and the expression of P-glycoprotein (P-gp) were observed by flow cytometry assay. The mRNA expression of mudtiding resistance gene (mdr1) was measured by fluorescent semi-quantitative reverse transcriptase polymerase chain reaction (RT-PCR).RESULTS:CsA and Tet (alone or combination) elevated the intracellular DNR concentration in K562/A02 cells (the fluorescence intensity of intracellular DNR in K562/A02 cells was 60%, 65% and 98% respectively of that in K562 cells). The fluorescence intensity of P-gp in K562 and K562/A02 cells was 0.18% and 96.51%. The P-gp expression was down-regulated after treated with CsA, Tet and both (75.32%, 76.86% and 48.61%); mdr1 mRNA was also down-regulated, and the effect of their combination was more obvious.CONCLUSION:MDR can be partially reversed by CsA or Tet, the combination of both drugs shows a great synergistic reversal effect.

Abstract:Curing cancer has always presented a formidable clinical challenge. Among the various treatment strategies for combating tumors, the combination of targeted therapy and immunotherapy has recently assumed significant importance. Regrettably, while targeted drugs demonstrate efficiency in the early stages of cancer treatment, patients inevitably develop drug resistance as treatment progresses, ultimately resulting in treatment failure and death. Currently, effective countermeasures against drug-resistant tumor cells remain limited, and research into the mechanisms of drug resistance continues to garner profound interest. The current understanding of drug resistance primarily focuses on two aspects: intrinsic or primary drug resistance, and acquired or secondary drug resistance. Further explanations delve into molecular mechanisms, including acquired gene mutations, epigenetic modifications, the plasticity of cancer stem cells, and the mediation of exosomes. With the advancement of single-cell analysis, our understanding of these potential mechanisms has become more comprehensive. In this review, we initially explore classical signaling pathways related to tumorigenesis and cancer cell progression. Additionally, we summarize recent findings regarding gene mutations and modifications related to drug resistance in cancer. Finally, we discuss the plasticity of cancer stem cells and the latest research on exosome-mediated tumor drug resistance.
