Value of fetal pulmonary artery/aorta ratio measured on short-axis view in mid-second trimester

Abstract:Objective To establish the reference ranges of fetal pulmonary artery diameter,aortic inner diameters and pulmonary artery diameter/aorta ratio measured on short-axis view in mid-second trimester.Methods 6 76 3 unselected singleton pregnancies were enrolled from September 2008 to October 2010.Routine screening scan of second trimester and fetal echocardiography were performed at 18-23W gestation.Fetal pulmonary artery diameter,aortic inner diameter and pulmonary artery diameter/aorta ratio were measured on short-axis view.Neonatal echocardiography was conducted within three days after birth.Results The 6 763 fetuses had a mean pulmonary artery diameter/aorta ratio of 1.10±0.08(95%CI,1.00-1.27;ranging 0.18 to 1.80).In 6 69 9 fetuses with normal heart,our results showed that fetal pulmonary and aortic inner diameter are highly correlated with gestational age(GA).Furthermore,the best-fit equation of fetal pulmonary artery diameter,aortic inner diameter and GA was transformed into linear regression equation of which the residual showed homogeneity of variance and normal distribution. Following the theory of normal distribution,the age-related reference centiles were calculated and a normal growth chart of fetal pulmonary artery diameter and aortic inner diameter was established for clinical reference using GA as the independent variable,and fetal pulmonary artery diameter and aortic inner diameter as the dependent variable.The best-fit regression equation was Y[aortic mner diameter(cm)]= -1.865+0.238X(GA)(r=0.373,P<0.000 1) and V[pulmonary inner diameter(cm)]=-1.916+0.256X(GA) (r=0.357,P<0.000 1).Conclusion The reference ranges of fetal pulmonary artery diameter,aortic inner diameter and pulmonary artery/aorto ratio measured in the short-axis view for local population in mid-second trimester may helpful for detecting outflow tract anomalies.
