Abstract:Midpalatal corticotomy-assisted rapid maxillary expansion (MCRME) is a minimally invasive treatment of maxillary transverse deficiency (MTD) in young adults. However, the effect of MCRME on respiratory function still needs to be determined. In this study, we evaluated the changes in maxillary morphology and the upper airway following MCRME using computational fluid dynamics (CFD). Twenty patients with MTD (8 males, 12 females; mean age 20.55 years) had cone-beam computed tomography (CBCT) images taken before and after MCRME. The CBCT data were used to construct a three-dimensional (3D) upper airway model. The upper airway flow characteristics were simulated using CFD, and measurements were made based on the CBCT images and CFD. The results showed that the widths of the palatal bone and nasal cavity, and the intermolar width were increased significantly after MCRME. The volume of the nasal cavity and nasopharynx increased significantly, while there were no obvious changes in the volumes of the oropharynx and hypopharynx. CFD simulation of the upper airway showed that the pressure drop and maximum velocity of the upper airway decreased significantly after treatment. Our results suggest that in these young adults with MTD, increasing the maxillary width, upper airway volume, and quantity of airflow by MCRME substantially improved upper airway ventilation.

Preliminary Evaluation of Treatment of Obstructive Sleep Apnea and Hypopnea Syndrome Using Modified Herbst Appliance

Abstract:PURPOSES: To evaluate the effectiveness of modified Herbst oral appliance in treatment of obstructive sleep apnea and hypopnea syndrome(OSAHS) by three-dimensional CT.METHODS: Ten patients with mild or moderate OSAHS by PSG were included in the study.The upper airway was scanned at normal respiration and Muller's maneuver by spiral CT(GE lightspeed ultra).Each patient was treated using Herbst oral appliance and adjusted several times until the subjective symptoms were improved.Three month later,each patient was re-evaluated by PSG and re-scanned by three-dimensional CT in two positions.The cross-section area with its corresponding sagittal and transverse diameters and the volume at retropalatal and retroglossal regions,the site and range of upper airway structure was evaluated.Paired t test was used for data analysis using SPSS 12.0 software package.RESULTS: There were significant differences between pre-treatment and three months after treatment with Herbst appliance subjectively and objectively.The transverse diameter and cross-section area were increased significantly at retro palatal and retro lingual level(P0.05).The volume of upper airway was augmented significantly(P0.05).Muller's test before and after treatment indicated that obstructive sites were decreased obviously.CONCLUSIONS: Modified Herbst appliance is effective for mild or moderate OSAHS.The upper airway three-dimensional CT with Muller's maneuver can be used in evaluating upper airway and the efficiency of treatment with oral appliance.
[Three-Dimensional Model Analysis of Obstructive Sleep Apnea Hyponea Syndrome Patients with Long-Term Treatment of Oral Appliances].

Abstract:PURPOSE:The aim of the study was to investigate the dental changes of patients with obstructive sleep apnea hypopnea syndrome (OSAHS) with long-term treatment of oral appliances, via the method of three-dimensional model analysis.METHODS:Using Geomagic Studio 2014 software, we transferred the dental models, which were from 18 OSAHS patients before and after treatment of oral appliances, into three-dimensional models for digital analysis. Datasets obtained from pre- and after treatment were compared for accuracy via paired t test using SPSS 22.0 software package.RESULTS:Eighteen patients using oral appliances for 6.57±1.98 years, showed significant differences in some dentition values between pre-treatment and after-treatment. The total dentition changes indicated intrusion of upper premolars, buccalization of upper posterior teeth and mesialization of lower posterior teeth. Statistical analysis demonstrated decrease in upper dental arch length, increase in upper posterior arch width and decrease in upper arch depth and dramatic reduction of overjet in anterior teeth. In the same time, other values evaluated showed no significant change before and after treatment of oral appliances.CONCLUSIONS:Long-term wearing oral appliances results in changes in several variables of dental occlusion, which should not be ignored for dentists conducting this treatment plan. However, the side effect of dental occlusion changes is little on a whole, leading to high security in this aspect.
Craniofacial morphological changes induced by a mandibular repositioning oral appliance and their significance

Abstract:Objectives: To identify any craniofacial morphological changes induced by a mandibular-repositioning oral appliance (MRA) and to explore the possibility of predicting the treatment response to MRA by cephalometric analysis in patients with obstructive sleep apnea (OSA). Methods: Seventy OSA patients [male/female: 63/7; age: (50.5±11.6) years; BMI: (27.6±4.6) kg/m2; AI: (34.9±21.3) episodes/hour; and oxygen saturation nadir: (66.3±16.5) %] were enrolled. MRA was fabricated individually for each patient after the consultation by a dentist. Polysomnographic (PSG) examination was repeated with MRA in place 3 months after the initiation of the MRA therapy. For cephalometric analysis, a pair of cephalograms of each patient was obtained, one with and another without MRA. Results and Conclusions: After 3 months' treatment, AI was (156±19.2) episodes/hour, significantly reduced compared with the pre-treatment average AI (34.9±21.3 episodes/hour,P<0.000 1). Oxygen saturation nadir improved from (66.3±16.5)% (pre-treatment) to (74.1±15.5)% (with MRA) (P<0.001). A reduction of AI≥50% was achieved in 42 patients. Insertion of MRA led to anterior shift of the mandible, increase in upper airway width and area and decrease in upper airway length. Those with evident retrognathia and longer anterior upper facial height were more likely to benefit from the MRA management.
Magnetic Resonance Imaging of the Upper Airway in Obstructive Sleep Apnea Before and after Oral Appliance Therapy.

Abstract:OBJECTIVE:To determine the mechanism by which an oral appliance may be used to treat obstructive sleep apnea syndrome (OSAS).METHODS:Eleven OSAS patients (8 males and 3 females) were involved in the study. Mean age was 52.2 +/- 10.6 years; height was 166.6 +/- 7.2 cm, and weight was 75.6 +/- 9.3 kg; body mass index (BMI) was 27.2 +/- 2.9 kg/m2. Each patient underwent magnetic resonance imaging (MRI) and polysomnography before and after oral appliance therapy. Pharyngeal changes were measured and compared with the variation in sleep parameters. Also, Pearson's correlation and multiple linear regression were performed to investigate the relationship of sleep parameters and MRI items.RESULTS:Through oral appliance therapy, the sleep disorder decreased. Apnea/hypopnea index (AHI) decreased from 44.6 +/- 21.5 to 9.6 +/- 6.3 per hour of sleep. Lowest oxygen desaturation rose from 71.4 +/- 15.0% to 82.0 +/- 7.7%. Meanwhile, the upper airway increased at most levels, and especially at oropharynx. As measured by the correlation and regression analysis, the AHI changes had a negative association with tongue volume (R = -0.5730) and a positive association with the area alternation of high oropharynx (R = 0.5823); the change of the lowest oxygen desaturation (SaO2%) was positively associated with whole airway volume (R = 0.6554).CONCLUSION:The oral appliance works by enlarging the upper airway morphology and keeping the airway open, mainly at the back of soft palate. The effect of the oral appliance is associated with the degree of enlargement of the high oropharynx. Those who have a small tongue and a large pharynx may expect to have good results with the use of the oral appliance.
[Three-Dimensional Analysis of the Early Correction of Anterior Crossbite Using Eruption Guidance Appliance].

Abstract:OBJECTIVE:To investigate the clinical effect of eruption guidance appliance in the treatment of anterior cross bite in mixed-dentition children.METHODS:In the study,10 mixed-dentition children with anterior cross bite, totally 12 incisors, were selected. Alginate was used to take upper and lower dentition impression and make a hard plaster model,which served as the eruption guidance appliance for treatment. The pre- and post-operative dental casts were digitized with SmartOptics Activity 880 scanner,and the three-dimensional overlapping models were obtained by reverse engineering software,Geomagic Studio 2012,then the three-dimensional movements of the upper and lower incisors were analyzed using Imageware 13.2 software. The overbite and overjet were analyzed using the same methods. Measurement with copper wire was used to analyze the upper and lower arch length. Space analysis was the result by the sum of crown width minus the arch length. The crown width of unerupted permanent teeth was according to X-ray method to predict. The SPSS 17.0 software was used to analyze the pre- and post-operative measurements of the same child. The normality test of the measured data showed that it conformed to the normal distribution. Therefore,the t test and double side test were used,and the significance level was 0.05.RESULTS:The course of treatment was (5.6±2.7) months. During orthodontic treatment, the upper incisors moved mainly labially (P<0.001) in three-dimensional displacement, and the lower incisors moved mainly the same direction (P=0.025). During the treatment of eruption guidance appliance,the average overbite decreased (1.01±0.9) mm (t=-3.531, P=0.006), and the difference was statistically significant. There was no statistically significant difference between the pre- and post-operative average overjet (t=0.771, P=0.460). The severity of crowding in upper arch decreased (1.9±0.99) mm (t=-6.042, P<0.001),and that in lower arch decreased (1.9±0.74) mm (t=-8.143, P<0.001), both of the differences were statistically significant.CONCLUSION:The anterior cross bite in mixed dentition could be corrected by eruption guidance appliance, and at the same time, the normal overjet and overbite were established, and the teeth were aligned.
Computational Fluid Dynamics Simulation of the Upper Airway of Obstructive Sleep Apnea Syndrome by Muller Maneuver

Abstract:This study aimed to use computer simulation to describe the fluid dynamic characteristics in patients with obstructive sleep apnea syndrome (OSAS) and to evaluate the difference between during quiet respiration and the Muller maneuver (MM). Seven patients with OSAS were involved to perform computed tomographic (CT) scanning during quiet respiration and the MM. CT data in DICOM format were transformed into an anatomically three-dimensional computational fluid dynamics (CFD) model of the upper airway. The velocity magnitude, relative pressure, and flow distribution were obtained. Numerical simulation of airflow was performed to discuss how the MM affected airflow in the upper airway. To measure the discrepancy, the SPSS19.0 software package was utilized for statistic analysis. The results showed that the shape of the upper airway became narrower, and the pressure decreased during the MM. The minimal cross-sectional area (MCSA) of velopharynx was significantly decreased (P<0.05) and the airflow velocity in MCSAs of velopharynx and glossopharynx significantly accelerated (P<0.05) during the MM. This study demonstrated the possibility of CFD model combined with the MM for understanding pharyngeal aerodynamics in the pathophysiology of OSAS.
Analysis of Three-Dimensional Tooth Movement: A Comparative Study Between Digital Dental Models and Craniofacial Models

Abstract:Objective: This study aims to validate the efficacy of using a digital dental model (DM) with reference to the palatal region of interest (PROI) for assessing orthodontic tooth movement (TM) by comparing it with the analysis of a computed tomography (CT) model with reference to the cranial region of interest (CROI). Materials and methods: Thirty-four patients (mean age: 21 years and 11 months) with jaw deformities underwent DM and CT scans before and after presurgical orthognathic treatment. Linear and angular measurements during TM were conducted in three dimensions using both DM and CT to assess reliability. Results: DM analysis with PROI registration exhibited high levels of reproducibility, with minimal standard errors in X, Y, and Z displacements (<0.15 mm) and 0.43 degrees in angular change. CT analysis with CROI registration demonstrates similarly high reproducibility, with standard errors inferior to DM analysis (<0.20 mm). Bland-Altman analysis indicated agreement in linear changes of each X, Y, and Z displacement between DM and CT measurements, with limits of agreement (LOA) below 0.91 mm. Conclusions: The results of this study suggest that the PROI, focusing on the third palatal rugae and the horizontal part of the palatal vault, serves as a reliable reference region for evaluating three-dimensional (3D) tooth movement. Clinical significance: Digital dental models offer distinct advantages including the absence of X-ray exposure, no metal artifacts, and the ability to generate high-resolution 3D models. The methodology demonstrated high precision and reproducibility, supporting its potential clinical utility in orthodontic treatment planning and assessment.
[Evaluation of Maxillary Three-Dimensional Changes in Maxillary Protraction with Alternating Rapid Palatal Expansion and Constriction Based on the Cone-Beam Computed Tomography].

Abstract:OBJECTIVE:To use the cone-beam computed tomography (CBCT) to evaluate the three-dimensional (3D) changes of maxillary landmarks in the maxillary protraction with alternating rapid palatal expansion and constriction and with rapid palatal expansion, and to provide some clinical suggestions for the early treatment of Class III malocclusion.METHODS:A total of 36 maxillary retrusive patients were included and randomized in a 1:1 ratio to either the intervention group (alternating rapid palatal expansion and constriction group, RPE/C) or the control group (rapid palatal expansion group, RPE). Randomization was accomplished with permuted block randomization based on participation sequence. The patients in the RPE/C were treated for 10 weeks (0.5 mm/d) with the repetition of two-week palatal expansion and two-week palatal constriction. The patients in the RPE were taught to complete rapid palatal expansion for 2 weeks (0.5 mm/d ). The patients were instructed to come to the office for the follow-up to ensure the correct procedures. Damaged expanders were repaired (or replaced) and rebanded quickly. Sequential CBCT images including pretreatment (T1), post-expansion (T2) and post-protraction (T3) were required for 3D reconstruction, establishment of landmarks, measurement and analysis by Mimics 10.01.RESULTS:There was significant forward movement of subspinale (A) in the RPE/C after the treatment with (3.06±1.29) mm, compared with RPE (2.16±1.27) mm, P<0.05. There were more symmetrical changes of the landmarks in the RPE/C and there was no statistic significance of the entire treatment time between the two groups. Moreover, the maxillary skeletal landmarks had the following 3D changes of a forward and downward movement during the expansion stages T2-T1, a forward and upward movement during the protraction stages T3-T2 and a forward and downward movement during the total treatments T3-T1 compared with the control group. And the width between the bilateral landmarks increased during the expansion stages T2-T1, narrowed down during the protraction stages T3-T2 and increased during the total treatments T3-T1.CONCLUSION:The maxillary protraction with alternating rapid palatal expansion and constriction provided clinical benefits on maxillary advancement and symmetrical changes in the orthopedic treatment of the patients with maxillary retrognathism and it required further study on the orthodontic analysis and measurements of CBCT.
The Effect Of Gradually Increased Mandibular Advancement On The Efficacy Of An Oral Appliance In The Treatment Of Obstructive Sleep Apnea

Abstract:Study Objectives: To analyze the effect of gradual increments of mandibular advancement on the treatment efficacy of mandibular advancement devices and identify determinants of effective and target protrusion for OSA.Methods: Patients were prospectively recruited. The mandible was titrated from 0 mm with a stepwise increment of 0.5 mm until the AHI was reduced to the lowest level. Rhinospirometry, rhinomanometry, and magnetic resonance imaging were used to observe the change of respiratory function and upper airway morphology.Results: Forty-two patients aged 41.5 +/- 9.0 years participated. There was a dose-dependent relationship between mandibular protrusion and the AHI improvement rate, the success rate, and the normalization rate; the changing curves plateaued after approximately 70% of maximal mandibular protrusion was achieved. The correlation between AHI and mandibular protrusion became stronger as the severity of OSA increased. The target protrusion for patients with mild, moderate, and severe OSA was 3.5 +/- 1.8 mm (38.6 +/- 19.4% maximal mandibular protrusion), 5.8 +/- 1.9 mm (62.9 +/- 18.8% maximal mandibular protrusion), and 5.9 +/- 2.2 mm (68.8 +/- 15.6% maximal mandibular protrusion), respectively. Regression analysis revealed that the factors influencing effective and target protrusion included change of maximal lateral dimension of the total upper airway with mandibular advancement devices, mean lateral dimension of the oropharynx, and soft palate length. Further protrusion brought more lateral expansion of the velopharynx, whereas the change in nasal ventilation was not significant.Conclusions: The dose-dependent effect of mandibular protrusion on reduction of AHI by mandibular advancement devices was nonlinear and became more pronounced with increased severity of OSA. The mandibular protrusion should be more personalized to each patient.
Effects Analysis of A Kind of Anterior Maxillary Traction Device for the Correction of Skeletal Class III Malocclusion at the Mixed Dentition

Abstract:Objective In order to correct the skeletal class III malocclusion in the mixed dentition, a ring shaped orthodontic splint traction appliance has been designed and analyzed. Methods 20 patients with skeletal class III malocclusion at the mixed dentition who had been selected in Department of orthodontics, Xi'an Jiaotong University stomatology hospital from 2008 to 2017, had been required to wear clasp type splint traction appliance for 10 months. Before and after treatment, measurement analysis had been made by using lateral cephalometric radiographs of the patients, and the paired t test through SPSS 20.0 statistical software. Results Clinical results of 20 patients showed that A-Np (mm), OB (mm) were significant(P<0.05). SNA (DEG), ANB (DEG), MxL (mm), Co-A (mm), Wit′ S (mm), LFH (mm), OJ (mm), surface angle (DEG), UL-E (mm) were very significant(P<0.01). Conclusion The authors present that a clasp type splint protraction appliance is simple, economical and practical; the patients with skeletal Class III malocclusion at mixed dentition have obvious curative effects, especially in the correction of maxillary deficiency, anterior teeth overbite and overjet increased, facial convexity improvement, jaw, face towards the normal development. But the lower facial height increased, showed high angle patients with caution.
Three-dimensional Evaluation of the Carriere Motion 3D Appliance in the treatment of Class II malocclusion

Abstract:Introduction: This study aimed to quantify the outcomes of adolescent patients with Class II malocclusion treated with the Carriere Motion 3D Appliance (CMA) combined with full fixed appliances. Methods: Cone-beam computed tomography scans of 22 patients were available before orthodontic treatment (T1), at removal of the CMA (T2), and posttreatment (T3). The average age of the patients was 13.5 ± 1.6 years at T1, 14.1 ± 0.2 years at T2, and 15.6 ± 0.5 years at T3. The 3-dimensional image analysis procedures were performed using ITK-SNAP (version 3.6.0; www.itksnap.org, Hatfield, Pa) and SlicerCMF (version 4.11.0; http://www.slicer.org, Cambridge, Mass); skeletal and dentoalveolar changes relative to cranial base, maxillary, and mandibular regional superimpositions were evaluated. Results: Changes were analyzed with 1 sample t tests using the mean differences during the CMA phase (T1 to T2) and total treatment time (T1 to T3). Significant skeletal changes included a slight reduction of ANB from T1 to T3, mandibular growth (Co-Gn increment of 1.2 mm and 3.3 mm from T1 to T2 and T1 to T3, respectively), inferior displacement of point A, and anterior and inferior displacement of point B. The mandibular plane did not change significantly during treatment. During the CMA treatment, posterior tipping and distal rotation of the maxillary molars, tip back and inferior displacement of the maxillary canines, significant mesial rotation, and superior displacement of the mandibular molars were observed. These movements rebounded during the full fixed appliance phase except for the molar and canine vertical displacements. Clinically significant dental changes during treatment included a reduction in overjet and overbite, Class II correction of the molar and canine relationship, and proclination of the mandibular incisors. Conclusions: The CMA is an effective treatment modality for Class II correction in growing patients because of a combination of mesial movement of the mandibular molar, distal rotation of the maxillary molar, and anterior displacement of the mandible.
[Evaluation of Mastication Function after Three-Dimensional Maxillary Reconstruction].

Abstract:OBJECTIVE:To evaluate the mastication function of the patient whose maxilla was reconstructed with individual titanium mesh and Chinese flap or combined with fibular flap. The superiority of this method on maxillary functional reconstruction was testified.METHODS:Since March of 2001, 10 cases of maxillary defect were reconstructed with individual titanium mesh and Chinese flap or combined with fibular flap, and routine removal partial dentures were fixed after 3 - 6 months postoperatively. Assessment of occlusal force was proceeded by T-Scan II system (Tekscan company, USA).RESULTS:The occlusal force analysis results indicated the asymmetry index of bite force and asymmetry index of occlusal contact area differed significantly between preoperation and postopertion (P < 0.05). The recovery rate of total occlusal force was between 27.05%-74.06%, and the average was 50.15%.CONCLUSION:This new three-dimensional maxillary functional reconstruction method had a satisfactory recovery in both of contour and function recently, especially restored the mastication function effectively.

Abstract:OBJECTIVES The biomechanics in achieving molar distalization may differ between fixed appliances and clear aligners in the control of tooth movement. The objective of this study was to compare the treatment effects between clear aligners (CA) and fixed appliances (FA) in patients treated with miniscrew-assisted molar distalization. METHODS The sample consisted of 46 subjects with mild-to-moderate crowding. A total of 22 patients treated with clear aligners (age, 25.66 ± 6.11 years old) and 24 patients treated with fixed appliances (age, 24.04 ± 4.95 years old) for miniscrew-assisted molar distalization were included in this study. The dental and skeletal changes were evaluated by the pre- and post-treatment lateral cephalograms. RESULTS Significant changes were found with the vertical variables SN-OP angle (2.24 ± 3.22°, P < .05) and SN-MP angle (0.73 ± 1.15°, P < .05) for the FA group when compared with the CA group (SN-OP angle 0.41 ± 2.26° and SN-MP angle -0.21 ± 1.38°, P > .05). Both treatment groups achieved a 2-3 mm. molar distalization with significant intrusion of the upper molars. The CA group showed significantly less distal tipping of molars (U6^PP angle -2.29 ± 3.29° and L6^MP angle -2.92 ± 2.49°, P < .05) compared to the FA group (-5.24 ± 4.28° and -5.53 ± 5.03°, P < .05). In addition, significant retraction and lingual inclination of the upper and lower incisors were found in both groups. LIMITATIONS The changes of tooth position were evaluated by 2D lateral cephalograms, not 3D measurements. CONCLUSIONS Compared to fixed appliances, clear aligners seemed to have better control of vertical dimension and distal tipping of molars in patients treated with miniscrew-assisted molar distalization.

Abstract:Obstructive sleep apnea (OSA) can have many adverse effects on people's health, including cognitive decline and high blood pressure. Typical surgical treatment methods include the commonly performed uvulopalatopharyngoplasty and the highly successful maxillomandibular advancement (MMA). These surgical methods are more effective than non-surgical methods because they widen the airway where a collapse has occurred through direct treatment. However, few studies has shown that moving the upper and lower jaws in a specific manner is the most efficient way to treat OSA during an MMA surgery. In this study, the airway of an OSA patient was reproduced digitally, and computational fluid dynamics analysis was performed on various models with changed airway shapes, including the original model based on an actual CT image and three resizing models of the retropalatal (RP) and retroglossal (RG) regions of the airway. Consequently, it was possible to provide more quantitative predicted flow data, which could be helpful in performing sophisticated OSA surgery. Among the four airway models of the OSA patient, a reduction in the epiglottis regional pressure difference of up to 40.2% was evident in the model with an expanded RG region, and a reduction in the wall shear stress of up to 25.8% was confirmed. The proposed process could be an important aid for surgeons in determining the optimal surgical method suitable for an individual patient's uniquely-shaped airway.

Abstract:Objective To set up the smart adjustable three-dimensional finite element model with high geometric similarity of space closing mechanics using sliding method treated with straight wire appliance rapidly based on the Materialise's interactive medical image control system(Mimics).Method An adult skull was selected for CT scanning and the data were imported into the Mimics for 3-D reconstruction.Firstly,the position of the tooth was adjusted to fully align according to parameters of the Opa-K straight wire appliance system;Secondly,the wire and the bracket were established in the ABAQUS's pre-processing program and were assembled to the tooth by boolean operation.Results The smart adjustable three-dimensional finite element model were constructed with high geometric similarity of space closing mechanics using sliding method treated with straight wire appliance.Lingual tipping displacement was found with lateral and central incisors and the pressure zones were formed in the periodontal membrane of the lingual side of the incisors under 2N load.Conclusions The model is effective and feasible,and it could provide the effective method for further investigation for space closing mechanics using sliding method treated with straight wire appliance.
Effect of bite openings and mandibular protrusion on genioglossus muscle activity in healthy adults with oral appliance

Abstract:Oral appliance (OA) can effectively treat obstructive sleep apnea; however, numerous types of oral appliances and designs are variable and the precise mechanisms behind differences in treatment outcomes are uncertain. The objective of this study was to evaluate the effects of different degrees of mandibular position [4° of bite openings (BO): 2, 4, 8 and 12 mm; and protrusion (P): 0, 50%, MAX], for both the upright and supine positions: BO2 mm_P0%, BO4 mm_P0%, BO4 mm_P50%, BO4 mm_PMAX, BO8 mm_P0%, BO12 mm_P0%; with an OA on the: (1) activity of the genioglossus (GG) muscle by electromyogram, (2) inspiration by airflow sensor, and (3) recording mandibular movements (incisor and mandibular condyle point) in each position. Nine healthy male adults (age 27.5 ± 1.30 years) were recruited. The results show that GG muscle activity increased significantly from BO 4 mm_P0% to BO12 mm_P0% during the supine position, and the strongest signal was found in BO4 mm_PMAX, compared to all of the other positions, and GG muscle activity in BO4 mm_P0% tended to be lower. From supine to upright position the inspiration increased significantly but GG muscle activity did not. These results might be a stimulus to augment a compensatory mechanism of GG muscle induced by OA, however, mainly in protrusion position. The increase of BO (2-12 mm) and even maximum protrusion might not negatively affect the temporomandibular joint.
CBCT combining with plaster models: application in virtual three-dimensional subapical segmental osteotomy to obtain more precise occlusal splint

Abstract:Anterior subapical segmental osteotomy is considered to be an important surgical technique to obtain functional occlusion and improve the facial profile for patients with maxillary and mandibular protrusion or retrusion, and some complications, such as ischemic necrosis of the distal segment, devitalization of the teeth adjacent to the osteotomy site, and inadequate movement space of segment for obtaining a good occlusion or facial profile, usually exist during surgery. Imprecise measurement of root length, interradicular distance, and intertooth distance based on traditional panoramic radiography that demonstrated existing horizontal distortion and vertical distortion may play an important role in resulting in these problems. In addition, the root is invisible for surgical simulation in traditional plaster models. The recently developed cone-beam computed tomography (CBCT) presents a higher spatial resolution with a lower radiation dose, simultaneously with excellent accuracy and without magnification of images. The presented technique was used to obtain a precise occlusal splint in virtual 3D subapical segmental osteotomy by combining CBCT with plaster models that could guarantee the measurement accuracy of root length, interradicular distance, and intertooth distance, followed by the result of fewer tooth root damage and more precise forecasting of available movement space of jaw segment. Combining with other advantages of virtual 3D surgery, such as precise teeth surface of plaster models, soft tissue simulation, genoplasty simulation, and zygoma plasty simulation, this presented technique may offer a preferable method to patients who need subapical segmental osteotomy.
