CT Diagnosis of Superior Vena Cava Syndrome from Lung Cancer

Abstract:Purpose: To study CT appearances of superior vena cava syndrome (SVCS) from lung cancer. Materials and Methods:Fifty-one cases with lung cancer resulting in SVCS confirmed by pathological examination were analyzed retrospectively and statistically with emphasis on CT features of lung cancer, superior vena cava obstruction and secondary lesions of SVCS. Results: (1) Lung cancer resulting in SVCS in right upper pulmonary lobe, right middle pulmonary lobe, right low pulmonary lobe, left upper pulmonary lobe, left low pulmonary lobe and right complete pulmonary lobes was 32 cases, 3 cases, 5 cases, 2 cases, 5 cases and 4 cases respectively. Central type, right upper mediastinal type, peripheral type was 35 cases, 12 cases and 4 cases respectively; (2) CT appearances of lesions around obstructive segment of SVC involved in 11 cases of ringed encasement,5 cases of partial ringed encasement and 5 cases of sandwich-like encasement,10 cases of procession,15 cases of compression toward left and 5 cases of complete obstruction.At different degree of SVC obstruction,probability of the former five of them on CT was of difference (P<0.05);(3) On CT appearances of secondary lesions,cases of simple constitution and opening of collateral pathway,simple swelling of chest wall,both of the two and neither of the two were 14 cases,12 cases,16 cases and 9 cases respectively on CT. With SVC obstruction at different extent,constitution and opening of collateral pathway and/or swelling of chest wall were different in shown ratio on CT (P<0.05). Conclusion: SVCS resulted from lung cancer can be diagnosed by contrast enhanced CT.
