Impact of Graft with or Without Middle Hepatic Vein on Donor's Safety in Right Lobe Living Donor Liver Transplantation

Abstract:Objective:To investigate the safety of right hemi-hepatectomy with or without middle hepatic vein(MHV) principally according to percentage remnant liver volume(%RLV),graft-to-recipient body weight ratio(GRWR)and the venous drainage from segmentⅣ in living donor liver transplantation(LDLT).Methods:Forty-three donors were planed to perform hemi -hepatectomy.When % RLV 35% and GRWR 1.0%,the patients belonged to group B.When % RLV ≥35% and GRWR1.0%,the patients belonged to group A.When % RLV ≥35% and GRWR 1.0%,according to the major venous drainage from segmentⅣ,the patients belonged to group A if the major venous drainage from segmentⅣ to left hepatic vein,and the patients belonged to group B if the major venous drainage from segmentⅣ to MHV.The patients in group A(n=15) were perform hemi-hepatectomy with MHV and group B(n=28) were perform hemi-hepatectomy without MHV.The general state of health,alanine aminotransferase(ALT),aspartate aminotransferase(AST),serum total bilirubin(TBIL),albumin(ALB) and the state of liver regeneration were compared between the two groups.Results:Except the %RLV and GRWR,there were no significant differences in other parameters between the two groups.After operation,there were no significant differences between the maximum values of the ALT,AST,TBIL and the minimum value of ALB in two groups.All of the donors were recovered and discharged in two weeks after operation and returned to their pre-donation lifestylesin two groups.Conclusion:According to this selection algorithm to decide the right hemi -hepatectomy with or without MHV,donor's safety can be ensured.
