Abstract:Objective To discuss the application of endoscopy in neurosurgery. Methods 36 cases of transsphenoidal pituitary operations were performed with endoscope-assisted microsurgery(EAM); 22 obstructive hydrocephalus, 6 colloid cysts, 5 cystic craniopharygioma and l cysticercosis of lateral ventricle were treated by endoscopic neurosurgery (EN) combining with stereotactic technique; l0 cases in cerebellopontine angle (CPA) microvascular decompression were treated with endoscopy-controlled microneurosurgery (ECM). Results 24 cases of pituitary adenomas were resected totally, 12 were resected subtotally. The size of ventricles in 2l of 22 cases with obstructive hydrocephalus were markedly decreased,one case was reoccurred; The cystic cavity of 9 intracrainial cystic lesions were markedly decreased, complications were presented in 2 cases. 10 cases of CPA microvascular decompression were performed with endoscopic technique, postoperative outcome was good. Conclusion During endoscopic transsphenoidal sellar tumor resection, we can get a good operation field and the tumor can be resected totally. Stereotactic biopsy combining with third ventriculostomy can be used at the same time in vision.Intracrainial cyst can be punctured,irrigated, biopsy and penetrated the wall of the cyst. By endoscopic technique, CPA microanatomy during microvascular decompression can be observed from different angles with minimal invasion.
The study of endoscope-assisted microsurgical anatomy on the far-lateral keyhole approach

Abstract:Objective The feasibility of endoscope-assisted microsurgical surgery via the far-lateral partial transcondylar keyhole approach and its endoscopic anatomy are investigated. Methods A total of 8( 16 sides) adult moist heads with connected neck were used to simulate the procedures of the far-lateral partial transcondylar keyhole approach under the microscope and neuroendoscope. Results The application of neuroendoscope could increase the operative view angle and sufficiently expose the ventrolateral medulla oblongata,partial postero-inferior cerebellar artery,whole segment of hypoglossal nerve,the borderline of vertebral artery and postero-inferior cerebellar artery,lower cranial nerves entered jugular foramen,and vestibulofacial nerve. Conclusion Endoscope-assisted microsurgery via the far-lateral partial transcondylar keyhole approach is feasible and the application of neuroendoscope can decrease the surgical injury.

Abstract:Objective To evaluate the application of three kinds of approach to the cerebellopontine angle under neuroendoscope.Methods Suboccipital retrosigmoid extracerebellar keyhole approach,suboccipital retrosigmoid infratentorial-supracerebellar keyhole approach and suboccipital retrosigmoid inferior floccular process keyhole approach were simulated under neuroendoscope respectively,and the surgical routes and anatomical landmarks to the root exit/entry zoon(REZ) of the Ⅴ,Ⅶ,Ⅸ cranial nerves were defined.Results The REZ of the cranial nerves Ⅴ,Ⅶ and Ⅸ were observed through the suboccipital retrosigmoid extracerebellar keyhole approach,and the landmarks covered Meckel cave,internal auditory foramen,jugular foramen and cranial nervesⅤ,Ⅶ,Ⅷ,Ⅸ.The structure from the REZ of cranial nerve Ⅴ to Meckel cave were explored through suboccipital retrosigmoid infratentorial-supracerebellar keyhole approach,and the landmarks covered the petrosal vein,cranial nerve Ⅴ and Meckel cave.The REZ of the cranial nerves Ⅸ and Ⅶ were explored via suboccipital retrosigmoid infrafloccular process keyhole approach,and the landmarks covered the choroid plexus of the fourth ventricle and floccular process.Conclusions Infratentorial-supracerebellar keyhole approach is superior to extracerebellar keyhole approach for microvascular decompression of the cranial nerve Ⅴ,while infrafloccular process keyhole approach is superior to extracerebellar keyhole approach for microvascular decompression of the cranial nerve Ⅶ or Ⅸ.
A Combination Use of Endoscope and Microscope in Cerebral Pontine Angle Surgery

Abstract:Objective To evaluate the application of combination use of microscope and endoscope in cerebral pontine angle ( CPA) surgery.Methods A total of 72 patients undergone lateral skull base surgeries via endoscope under microscopic control from January 2006 to January 2015 was reviewed respectively .The patients including 35 males and 37 females were composed of 22 cases of vestibular swannnomas , 45 cases of cranial neuropathy and 5 cases of CPA chelesteatoma .Twenty cases of vestibular swannnomas , 15 cases of cranial neuropathy and 2 cases of CPA chelesteatoma undergone the surgery via retrosigmoid approach , while other cases undergone the surgery via retrolabyrinthine approach .Surgical procedures were accomplished under the control of microscope and different angular endoscope with imaging fusion.The surgical results were evaluated according to subjective and objective criteria , and all patients were followed up for 15 years.Results The symptoms was disappeared in the cranial neuropathy patients , without facial paralysis , complication of other nerves or hearing loss .Twenty-two patients with vestibular schwannomas got total tumor removal without facial palsy or neurological deficits ; useful hearing was preserved in 16 of 22 patients (72.7%), and no tumor recurrence was found during 1-5 years follow-up. There was also no facial palsy or other complications in 5 cases of CPA chelesteatoma , which gained completely surgical removal;useful hearing was preserved in 3 of 5 cases of these patients and no recurrence was occurred during 1-1.5 years follow-up.Conclusions A combination use of endoscope and microscope could combine advantages and avoid disadvantages of two techniques .It can provide better exposure with minimal invasion in CPA surgery , and is especially applicable in surgery for cranial neuropathy , vestibular schwannoma and CPA cholesteatoma , which should be performed through retrosigmoid approach and retrolabyrinthine approach .
Comparison of microscopic and endoscopic approaches to the cerebellopontine angle

Abstract:Objective: To examine the efficacy of the endoscope as an adjunct to the operating microscope in defining the surgical anatomy of the cerebellopontine angle (CPA). Methods: The surgical anatomy of the CPA was examined in cadaveric CPAs through a retrosigmoid approach. The upper, middle, and lower neurovascular complexes and the individual segments of the cerebellar arteries in the CPA were examined with the surgical microscope and 0° and 45° rigid endoscopes. Results: The microscope provided satisfactory views of the posterior surface of the neural and vascular structures in the central part of the CPA cistern. The endoscope provided superior views of the nerves' junction with the brainstem, their dural exit, and their vascular relationships. The endoscope also provided superior views of the individual segments of the cerebellar arteries. Conclusion: The combination of endoscopic and microsurgical techniques aids in achieving optimal exposure in CPA surgery.

Abstract:Objective: To investigate the clinical indication, key operating, points advantages & disadvantages of endoscopic neurosurgery. Methods: A total of 69 patients with intracranial disorders were treated by endoscopic neurosurgery, 6 suffering from pituitary adenoma, 21 from intracranial arachnoid cysts, 18 from hydrocephalous, 6 from septum pellucidum, 8 from intracranial epidermoid cyst, 9 from angioreticuloma and 1 from intracranial multiple metastatic tumor. Results: Clinical symptoms and signs were relieved in various degrees, and the clinical manifestation and CT and MR image were obviously improved in all patients after operation. Conclusion: Endoscopic neurosurgery is a safe and effective treatment methord for some patients with intracranial disorders. It is characteristic of less operation damage, light patient's reaction and faster rehabilitation for the patients following operation. Therefore, it is now the first choice for the minimally invasive neurosurgery.
The Use of Endoscope in Retrolabyrinthine Approach for Cerebellopontine Angle Region Surgeries

Abstract:Objective To report application of endoscope in the retrolabyrinthine approach of cerebellopontine an-gle surgery. Methods A total of 32 patients undergone endoscopic surgeries for CPA lesions via retrolabytinthine ap-proach from January 2010 to January 2016 were reviewed, including 19 males and 13 females with ages ranging from 25-69 years. Surgery indications included cranial neuropathy (n=27, Meniere's disease n=20, hemifacial spasm n=5 , tri-geminal neuralgia n=2), CPA cholesteatoma (n=3) and vestibular schwannnoma (n=2). Patients were followed up for 1-5 years. Results Symptoms in the 27 cases of cranial neuropathy resolved after surgery. Vestibular neurectomy was performed in the 20 cases of Meniere's disease and the rate of postoperative vertigo control was 100%. Microvascular decompression of the facial nerve was done in the 5 cases of hemifacial spasm with complete remission of facial spasm symptoms after surgery. Trigeminal nerve microvascular decompression in the 2 cases of trigeminal neuralgia also brought symptoms resolution after operation. There were no complications, nor clinical recurrence during the 1-5 years follow-up. Total lesion removal was achieved in all 3 patients with CPA cholesteatoma without facial palsy or other neu-rological deficits. Useful hearing was preserved in two patients, and no matrix recurrence was found during the 1-5 years follow-up. Tumor removal was total without facial palsy or other neurological deficits in the 2 patients with vestib-ular schwannomas, with preservation of useful hearing and no tumor recurrence during the 1-5 years follow-up in both patients. Conclusions Endoscopic assisted retrolabyrinthine approach has good application value in functional cranial nerve surgeries. It provides excellent exposure of the responsible blood vessel and cranial nerves (CN) V-XI. In cerebel-lopontine angle cholesteatoma and acoustic neuroma surgeries, endoscopy can help achieve minimally invasive resec-tion of lesions.
Fully Endoscopic Retrosigmoid Approach for Cerebellopontine Angle Tumors

Abstract:Background: Fully endoscopic or endoscope-controlled approaches are essentially keyhole approaches in which rigid endoscopes are the sole visualization tools used during the whole procedure. At the early attempts of endoscope-assisted cranial surgery, it was noted that rigid endoscopes enabled overcoming the problem of suboptimal visualization when small exposures are used. The technical specifications and design of the currently available rigid endoscopes are associated with a group of unique features that define the endoscopic view and lay the basis for its superiority over the microscopic view during brain surgery. Fully endoscopic retrosigmoid approach for cerebellopontine angle tumors is a minimally invasive approach that is not routinely practiced by neurosurgeons, with few series published so far. Unfamiliarity with the technique, steep learning curve, and concerns about inadequate exposure, neurovascular injury, and decreased visibility may explain this fact. In this chapter we elaborate on the surgical technique and nuances of the fully endoscopic retrosigmoid approach and present an overview of the published series. Methods: From a prospective database of endoscopic procedures maintained by the senior author, clinical data, imaging studies, operative charts, and videos of cases undergoing fully endoscopic retrosigmoid approach for cerebellopontine angle tumors were retrieved and analyzed. The pertinent literature was also reviewed. Results: The surgical technique of the fully endoscopic retrosigmoid approach was formulated. Conclusion: The endoscopic technique has many advantages over the conventional procedures. In our hands, the technique has proven to be feasible, efficient, and minimally invasive with excellent results.

Abstract:INTRODUCTION:The literature reflects a resurgence of interest in endoscopic and keyhole endoscope-assisted neurosurgical approaches as alternatives to conventional microsurgical approaches in carefully selected cases. The aim of this study was to assess the technical challenges of neuroendoscopy, and the scope for technological innovations to overcome these barriers.MATERIALS AND METHODS:All full members of the Society of British Neurosurgeons (SBNS) were electronically invited to participate in an online survey. The open-ended structured survey asked three questions; firstly, whether the surgeon presently utilises or has experience with endoscopic or endoscope-assisted approaches; secondly, what they consider to be the major technical barriers to adopting such approaches; and thirdly, what technological advances they foresee improving safety and efficacy in the field. Responses were subjected to a qualitative research method of multi-rater emergent theme analysis.RESULTS:Three clear themes emerged: 1) surgical approach and better integration with image-guidance systems (20%), 2) intra-operative visualisation and improvements in neuroendoscopy (49%), and 3) surgical manipulation and improvements in instruments (74%).DISCUSSION:The analysis of responses to our open-ended survey revealed that although opinion was varied three major themes could be identified. Emerging technological advances such as augmented reality, high-definition stereo-endoscopy, and robotic joint-wristed instruments may help overcome the technical difficulties associated with neuroendoscopic approaches.CONCLUSIONS:Results of this qualitative survey provide consensus amongst the technology end-user community such that unambiguous goals and priorities may be defined. Systems integrating these advances could improve the safety and efficacy of endoscopic and endoscope-assisted neurosurgical approaches.
[Application of the Neuro-Endoscope to the Intracranial Aneurismal Surgery].

Abstract:OBJECTIVE:To study the role of neuro-endoscope in the intracranial aneurismal surgery, analyzing its benefits, and disadvantages.METHODS:Endoscopy was used as an adjunct in the microsurgical treatment of clipping aneurysms on 88 patients with 89 aneurysms, of which 82 aneurysms in 81 patients were located in the anterior circulation and 7 were located in the posterior circulation. Keyhole approach was performed on all patients, and micro-Doppler ultrasound technique was used before and after clipping. In 84 aneurysms, endoscope-assisted microneurosurgery (EAM) was used in addition to microsurgical dissection and clipping so as to observe the neck anatomic features and perforators and to verify the optimal clipping position. Endoscope-controlled microneurosurgery was used for 5 aneurysms to observe the anatomy around aneurysm, such as the posterior communicating artery and the opposite anterior communicating artery behind the internal carotid artery.RESULTS:Postoperative angiography performed on 86 patients showed satisfactory aneurysm clipping. Operative mortality was 0. There were 7 cases of postoperative complications, with an incidence rate of 7.95%, and none case was directly related to endoscopy.CONCLUSION:Improving observation of regional anatomy because of its ability in magnification, illumination, and looking around dead angle, neuro-endoscopy is very useful for complex aneurysms.
[The Development and Clinical Application of a Multipurpose Neuroendoscope].

Abstract:The multipurpose rigid neuroendoscope MGE-4 has been developed with the application of the self-focussing lens technology. We have been using it to examine and treat 10 neurosurgical patients who suffered from tumours, hydrocephalus, intracerebral haematomas of head injury and hypertensive cerebral haemorrhage. The short-term effect was satisfactory and there was no operative complications. It is considered that Model MGE-4 with the accessories can be used safely for the brain functional subcortical lesion or the deep-seated cystic lesions and haematomas.

Applying the Full Endoscopic Keyhole Technique to CPA Lesions: A Single-Center Study

Abstract:In recent years, endoscopy has become an increasingly common tool used during neurosurgical procedures. However, its application in treating cerebellopontine angle (CPA) lesions has not progressed as rapidly. In this study, the authors present their initial experience with surgically treating CPA lesions using a fully endoscopic keyhole retrosigmoid approach. They conducted a retrospective analysis of clinical data from patients who underwent endoscopic keyhole CPA surgery at their center between May 2017 and April 2022. They provide a comprehensive explanation of this method and an overview of the strategies that have been developed to achieve better clinical outcomes. The study included 107 patients, consisting of 10 cases of vestibular schwannoma, 21 cases of epidermoid cyst, 32 cases of trigeminal neuralgia, and 44 cases of hemifacial spasm. The authors analyzed the clinicodemographic details of the patients. Among the 31 tumor cases, gross total resection was achieved in 25 patients (80.6%), while near-total resection was performed in 6 patients (19.4%). In patients with trigeminal neuralgia, facial pain resolved in 31 out of 32 patients (96.9%). Similarly, facial convulsions disappeared or were relieved in all 44 patients (100%) with hemifacial spasms after the operation. Postoperative complications included facial nerve paresis (n=9, 8.4%), with improvement observed in 6 cases during follow-up, transient facial hypoesthesia (n=3, 2.8%), cerebrospinal fluid rhinorrhea (n=3, 2.8%), transient abducens paresis (n=1, 0.9%), and postoperative hemorrhage (n=1, 0.9%). Endoscopy provides improved deep illumination and, combined with close-up observation, enhances the visualization of structures within the CPA region. The fully endoscopic keyhole technique is a safe and effective method for managing CPA lesions.

Abstract:Neuroendoscope has unique advantages in observing the relationship between intracranial aneurysms and peripheral vessels and nerves during intracranial aneurysm clipping.With the development of neuroendoscope technology,as well as the assistance of endoscopic light and display system,the clipping effects of some intracranial aneurysms have significantly improved.Although there are still the lack of depth of the operation field,the space occupying of the endoscope itself and iatrogenic injury,with the development of endoscopic equipment and operation methods,neuroendoscope will play its role in the surgical treatment of intracranial aneurysms.
Clinical Application of Endoscopic Techniques to Neurosurgery for Intracranial Lesions

Abstract:Objective To evaluate the value of endoscopic technique to microsurgery for the intracranial lesions.Methods The endoscopic neurosurgery,endoscope-controlled neurosurgery,and endoscope-assisted neurosurgery were performed respectively in 122 cases of intracranial lesions including 68 cases of tumors,51 cases of hematomas,2 cases of cysts in the lateral ventricle of cerebrum and 1 cases of hydrocephalus.Results All the hematomas were satisfactorily removed in 51 patients with imtracranial hematomas.Of 68 patients with the intracranial tumors,48 underwent total resection of the tumors,15 subtotal,and 5 greatly partial.The cysts were totally resected in 1 patients and the cyst shrank after the operation in another patients with cyst.The symptom was improved and the ventricle shrank after the operation in 1 patient with hydrocephalus.Conclusions The neuroendoscopic technique may be widely applied to the minimally invasive neurosurgery for the intracranial lesions.

Advancing Education in Endoscopic Spinal Navigation: Novel Methods and Technical Note

Abstract:This report aims to demonstrate how to teach anatomy and understanding of spinal endoscopic vision and navigation using mnemonics. The authors present a new surgical technique for teaching endoscopic spinal navigation in a didactic manner with tips such as the "rule of the hand" and decomposition of the endoscopic navigation movement. We demonstrate how the surgery is seen and illustrate how images are projected onto the screen, then divide the navigation into spatial orientation and self-navigation. The article describes the proper puncture technique, how to introduce the working portal, and how to assimilate this new anatomical vision using the "rule of the hand." The surgeon projects their hand on the video screen to guide themselves when starting the navigation and uses the same technique to localize regions of interest during surgery. Finally, the authors break down the navigational movement into three components: forceps positioning, triangulation, and joystick motion. One of the biggest challenges when learning spinal endoscopic surgery is understanding the anatomy seen through the endoscope. By decomposing movements required for navigation, one can understand how to make proper use of the equipment as well as improve their knowledge of this "new anatomy." The learning methods taught in this article have the potential to decrease the learning curve and radiation exposure to those that are still acquainting themselves to spinal endoscopic navigation. We recommend that further studies measure and quantify the impact of these methods on surgical practice.

Abstract:Background/Aims: Minimally invasive neurosurgery is a growing field, more so in recent decades. The modernization of tools, especially the endoscope, has allowed for critical improvements and crucial advancements in minimally invasive neurosurgery. The current classification scheme for endoscopic procedures needs to be updated to reflect these advancements. Methods: Although the field of neuroendoscopy is continually evolving, the terminologies utilized to describe endoscopic procedures reflect and favor its use as mostly an assisting device complementing the microscope. Even though the classification was adequate for its time, it has become inexact and therefore confusing. We therefore propose a new classification scheme that encompasses the growing independence of endoscopic minimally invasive neurosurgery (EMIN) as well as the changing landscape within EMIN procedures. Results: We have classified our EMIN procedures, since 2002, as either 'intraendoscopic' (IEN) or 'extraendoscopic' (XEN) in relation to the endoscope's axis. Exemplary cases of IEN and a review of the literature are presented as well. Conclusion: Our proposed classification scheme for solely endoscopic procedures is presented. Anatomical study for microsurgery of extended retrosigmoid approach

Abstract:Objective To investigate the improvement of surgical field exposure in cerebellopontine angle area via the extended retrosigmoid approach.Methods Suboccipital retrosigmoid approach was used on 10 adult fresh cadaveric heads(20 sides) and then the extended retrosigmoid approach was simulated by further revealing the transverse sinus and sigmoid sinus.Using internal auditory meatus,the entrance of Meckel's cave and midpoint of the clivus as reference points respectively,the maximum angles and distances between superior and inferior border,lateral and medial border of bone window and the three reference points were measured in two approaches.Then,the exposure of vital nerve and blood vessels in the cerebellopontine angle area via the extended retrosigmoid approach was observed.Results In extended retrosigmoid approach,the distances between midpoint of lateral border of bone window and three reference points were shorter than that in suboccipital retrosigmoid approach(all P<0.05).The maximum angles between both superior and inferior border of bone window and the reference points were increased significantly,and did also the maximum angles between both lateral and medial border of bone window and the reference points in extended retrosigmoid approach(all P<0.05).Conclusions The extended retrosigmoid approach can increase the extent of surgical exposure,expand the visual angle of operation,improve the total resection rate of large tumors,and protect the vital nerves and blood vessels in the cerebellopontine angle area.

Abstract:Neuroendoscopic techniques, with their good momentum of development and constantly emerging hot spots, have become one of the major aspects of minimally invasive neurosurgery. In recent years, in addition to the endoscopic transnasal surgery, endoscopic techniques have been gradually applied in transcranial surgery, which appears to be the most phenomenal progress in the field of endoscopic skull base surgery. With endoscopic transnasal surgery and endoscopic transcranial, skull base lesions can be approached from different views, making endoscopic skull base surgery a more thorough and sensible armoury for neurosurgeons.
