Pulmonary multidrug resistant tuberculosis as demonstrated on thin-slice computed tomography

Abstract:Objective To assess features of multidrug resistant tuberculosis(MDR-TB) compared with drug sensitive tuberculosis(DS-TB) as demonstrated on thin-slice computed tomography(CT). Methods Between from April 2010 and March 2013, thin-section CT findings of primary MDR-TB in 189 patients and of DS-TB in 189 patients were retrospectively reviewed. The frequency and patterns of lung lesions(including micronodule, nodule, mass, tree-in-bud sign, consolidation, ground glass opacity, thick-wall, thin-wall and worm-erode cavity, bronchiectasis, bronchi irregular and regular stenosis, mucus impaction, lymphadenopathy and pleural effusion, number of lobes affected by parenchymal lesions, laterality and number and size of cavities) were compared between the two types of TB. Statistical comparisons were performed with the use of the χ2and t tests for univariate analysis and the cox proportional hazards regression analysis for multivariate analysis. Results Based on univariate analysis, masses, consolidation, thick-wall and worm-erode cavity, mucus impaction, bilateral involvement, 5 or 6 segmental or lobar involvements, more than three cavities and more larger cavities were more frequently seen in MDR TB patients as compared with DS-TB(P<0.05). However, the cox proportional hazards regression analysis demonstrated that only consolidation(HR=7.29, 95% CI 3.11-13.91, P<0.001), 6 segmental or lobar involvement(HR=6.54, 95% CI: 2.81-14.77, P<0.001) and more than three cavities(HR=8.01, 95% CI: 2.97-15.58, P<0.001) were MDR-TB independent prediction factor. Conclusion The presence of MDR-TB as detected on thin-slice CT scan may help the differentiation MDR-TB from DS-TB.
