Abstract:Objective To determine the dose-response relationship of remifentanil inhibiting responses to endotracheal intubation when combined with propofol in severely preeclamptic patients undergoing caesarean section.Methods One hundred severely preeclamptic patients,aged 20-36 yr,weighing 32-36 kg,with body mass index ≤ 35 kg/m2,at 32-36 weeks of gestation,of ASA physical status Ⅰ or Ⅱ,undergoing caesarean section under general anesthesia,were randomly divided into 5 groups (n =20 each) using a random number table:group R1-group R5.Anesthesia was induced with iv injection of remifentanil 0.25 μg/kg (group R1),0.50 μg/kg (group R2),0.75 μg/kg (group R3),1.00 μg/kg (group R4) or 1.25 μg/kg (group R5),propofol 1.5-2.0 mg/kg,and succinylcholine 1.5 mg/kg.The patients were endotracheally intubated and mechanically ventilated.Anesthesia was maintained with inhalation of 2％ sevoflurane before birth,and iv injection of midazolam 30 μg/kg,fentanyl 6 μg/kg,and cis-atracurium 0.2 mg/kg,and continuous iv infusion of propofol 4-7 mg·kg-1 · h-1,and remifentanil 0.2 μg· kg-1 · h-1 after birth.The response to endotracheal intubation was defined as positive when SP≥ 160 mmHg after intubation and maintained at this level for more than 1 min.The ED50,ED95 and 95 ％ confidence interval (CI) of remifentanil inhibiting responses to endotracheal intubation were calculated by probit analysis.Results The ED50 and ED95 (95％ CI) of remifentanil inhibiting responses to endotracheal intubation were 0.65 (0.53-0.77) μg/kg and 1.30 (1.10-1.70) μg/kg,respectively,in severely preeclamptic patients undergoing caesarean section.Conclusion When combined with propofol,the ED50 and ED95 of remifentanil inhibiting responses to endotracheal intubation in severely preeclamptic patients undergoing caesarean section are 0.65 and 1.30μg/kg,respectively.

Abstract:Objective To study the effects of naloxone concentration and exposure time on human sperm motility in vitra. Methods Sperm samples were collected from 20 healthy men by masturbation. technique and divided into the control group(1 aliquot) and the naloxone group(5 aliquots). Sperm motility in vitra were detected using computer-assisted sperm analysis(CASA) under different concentration of naloxone (10 -8~10 -3 mol/L) and exposure time (0.5~5h). Results Sperm motility was gradually increased and then gradually decreased under the concentration of naloxone (10 -8~10 -3 mol/L) and different exposure time (0.5~5h). The change trend showed the characteristic of parabola curve. Under same concentration of naloxone, the efficacy was enhanced in parallel with exposure time. Conclusion Naloxone showed dual effects on sperm motility under lower doses incubation and high doses incubation.
[Effects of Opioid Analgesics on Sperm Motility in Vitro].

Abstract:OBJECTIVE:To observe the effects of short-term exposure to opioid analgesics on human sperm motility. METHODS:Twenty normal semen samples were collected, each divided into 19 groups, one as the control and the others treated in vitro with six opioid analgesics at three different concentrations, respectively, and sperm motility was assessed by computer-assisted sperm analysis at 15 min, 2 h and 4 h. RESULTS:Compared with the control group, fentanyl, alfentanil and sufentanil at 1 x 10(-5), 2 x 10(-3) and 0.05 mg/ml significantly decreased the percentage of grade a + b sperm at 15 min, 2 h and 4 h (P<0.05), and so did butorphanol at 2 x 10(-3) and 0.05 mg/ml (P<0.05) and dezocine at 0.05 and 0.5 mg/ml (P<0.05), but neither showed any remarkable effect at 1 x 10(-5) mg/ml at the three time points (P>0.05). Pentazocine effected no significant difference at 3 x 10(-5) and 0. 05 mg/ml (P>0.05) but a gradual increase in the percentage of grade a + b sperm at 0.5 mg/ml at the three time points (P<0.05). Butorphanol totally inhibited sperm motility at 0.05 mg/ml at 15 min and at 2 x 10(-3) mg/ml at 2 h, and so did dezocine at 0.05 and 0.5 mg/ml, but such inhibitory effect was not observed with fentanil, alfentanil and sulfentanil at 0.05 mg/ml. As for the sperm motility decreasing effect at 0.05 mg/ml at 15 min, sufentanil, butorphanol and dezocine exhibited significant differences (P<0.05) while fentanyl displayed none from alfentanil (P>0.05). CONCLUSION:Given the same length of time of treatment, butorphanol and dezocine totally inhibit sperm motility at a high concentration, but make no significant change at a low concentration. While fentanyl, alfentanil and sufentanil can significantly decrease sperm motility at the same low concentration, and partially inhibit it at all concentrations. On the contrary, a high concentration of pentazocine can promote human sperm motility.

Abstract:Objective To assess the effects of fentanyl (F),alfentanil (A),sufentaril (S),butorphanol (B),dezocine (D) and pentazocine (P) on human sperm motility in vitro.Methods Twenty samples of semen with normal sperm motility were collected.The semen was mixed with HTF liquid culture medium and sperm concentration was adjusted to (40-80) × 106/ml.Then semen suspension 50 μl was added into the culture dishes.The semen suspensions were randomly divided into 3 groups:group Ⅰ control-semen suspension 50 μl + 0.9％ NaCl 50 μl.GroupⅡ single opioid-semen suspension 50 μl + 50 μl of 4 concentrations of F,A,S,B,D and P (10-3,10-4,10-5,10-7 mol/L).Group Ⅲ combined opioid-naloxone-semen suspension 50 μl + 25 μl of 3concentrations of naloxone (2 × 10-8,1 × 10-7,2 × 10-7 mol/L) + 25 μl of 4 concentrations of F,A,S,B and D.After being incubated for 1 h the sperm motility was assessed by sperm analyzer and the amplitude of change in sperm motility was calculated.Median inhibition concentration (IC50) and complete inhibition concentration ( IC100 ) of the 6 opioids were calculated from concentration-response curve.Results ( 1 ) The shape of concentration-response curve for F,A and S was straight line.The maximal decrease in sperm motility was (56 ± 5)％(F),(58±7)％ (A) and(79±:6)％ (S).(2) The shape of concentration-response curve for B,D and P was sigmoid.Low concentrations of B,D and P did not affect sperm motility.Median and high concentrations of B and D could inhibit sperm motility.The maximal inhibition was 100％.High concentration of P increased sperm motility.The maximal increase was (19 ± 6)％.(3) Naloxone shifted the concentration-response curve for F,A,S,B and D to the right,and decreased the maximal inhibitory effect of F,A and S dose-dependently but did not affect the maximal inhibitory effect of B and D.(4) IC50 was in the order of A≈ F ＞ D ＞ S≈- B.IC100 was D ＞ B.Conclusion F,A and S inhibit sperm motility by acting on opioid receptor on human sperms while B and D act through opioid or nonopioid pathways.P can increase sperm motility.
Effect of Remimazolam Supplementation on Propofol Requirements During Hysteroscopy: A Double-Blind, Dose-Response Study.

Abstract:BACKGROUND:Propofol is commonly used for procedural sedation but may increase side effects in a dose-dependent manner. Remimazolam, an ultrashort-acting benzodiazepine, has been approved for procedural sedation but may delay awakening. This study tested the hypothesis that remimazolam as a supplement reduces effect-site propofol concentration (Ceprop) required to suppress response to cervical dilation in patients undergoing hysteroscopy.METHODS:One hundred and fifty patients who were scheduled for hysteroscopy were randomized to receive 0, 0.05, 0.1, 0.15, or 0.2 mg·kg-1 intravenous remimazolam, followed by a bolus of sufentanil 0.15 μg⋅kg-1, and a target-controlled propofol infusion. The initial target Ceprop was 3.5 μg·mL-1 and was increased or decreased in subsequent patients by steps of 0.5 μg·mL-1 according to whether there was loss of response to cervical dilation in the previous patient. We used up-down sequential analysis to determine values of Ceprop that suppressed response to cervical dilation in 50% of patients (EC50).RESULTS:The EC50 of propofol for suppressing response to cervical dilation was lower in patients given 0.1 mg·kg-1 (2.08 [95% confidence interval, CI, 1.88-2.28] μg·mL-1), 0.15 mg⋅kg-1 (1.83 [1.56-2.10] μg·mL-1), and 0.2 mg⋅kg-1 (1.43 [1.27-1.58] μg·mL-1) remimazolam than those given 0 mg⋅kg-1 (3.67 [3.49-3.86] μg·mL-1) or 0.05 mg⋅kg-1 (3.47 [3.28-3.67] μg·mL-1) remimazolam (all were P < .005). Remimazolam at doses of 0.1, 0.15, and 0.2 mg·kg-1 decreased EC50 of propofol by 43.3% (95% CI, 41.3%-45.5%), 50.3% (48.0%-52.8%), and 61.2% (58.7%-63.8%), respectively, from baseline (remimazolam 0 mg⋅kg-1). Propofol consumption was lower in patients given 0.1 mg⋅kg-1 (4.15 [3.51-5.44] mg·kg-1), 0.15 mg⋅kg-1 (3.54 [3.16-4.46] mg·kg-1), and 0.2 mg⋅kg-1 (2.74 [1.73-4.01] mg·kg-1) remimazolam than those given 0 mg⋅kg-1 (6.09 [4.99-7.35] mg·kg-1) remimazolam (all were P < .005). Time to anesthesia emergence did not differ significantly among the 5 groups.CONCLUSIONS:For women undergoing hysteroscopic procedures, remimazolam at doses from 0.1 to 0.2 mg·kg-1 reduced the EC50 of propofol inhibiting response to cervical dilation and the total propofol requirement. Whether the combination could improve perioperative outcomes deserves further investigation.

Abstract:Opioid analgesics are the most common therapeutic analgesic for acute pain. In this study, the toxicological and pharmacological features of a group of opioid analgesics were characterized by the motility of human sperm. Aliquots of sperm were incubated with various concentrations of opioid analgesics in vitro. Computer-assisted sperm analysis was used to assess sperm motility at 15 minutes, 2 hours, and 4 hours after drug addition to the medium. Butorphanol and dezocine showed marked reduction of motility after incubation with sperm for 15 minutes. Butorphanol was more effective than dezocine in immobilizing sperm. Other opioids studied, such as fentanyl, alfentanil, and sufentanil, showed only partial inhibitory activity. Based on the data reported herein, we have found that butorphanol and dezocine exert a sperm-immobilizing effect. However, fentanyl, alfentanil, and sufentanil exhibit only partial inhibition of sperm motility. Given the increasing use of opioids and their potential effect on sperm motility, these findings are greatly relevant to male reproductive health.
Remimazolam versus propofol in combination with esketamine for surgical abortion: A double-blind randomized controlled trial

Abstract:Remimazolam is a new benzodiazepine with a short half-life, good efficacy, and safety profiles in general anesthesia. Combining esketamine with propofol (P + E) could reduce propofol consumption and injection pain. It is, however, unclear if a low dose of remimazolam co-administrated with esketamine (R + E) is comparable to the increasingly used P + E for surgical abortion with general anesthetic. We conducted a double-blind randomized controlled trial to compare the efficacy and safety of R + E and P + E. Two hundred patients scheduled for a surgical abortion were randomized to receive remimazolam 0.3 mg/kg plus esketamine 0.3 mg/kg (R + E), and propofol 2 mg/kg plus esketamine 0.3 mg/kg (P + E). Sedative effectiveness was evaluated by measuring the time to lose consciousness (LOC), recovery time, and successful sedation rate. Safety was assessed by hemodynamics and adverse events during and postoperation. The time to LOC and recovery time in R + E was 5 s shorter and 1 min longer than that in P + E, respectively (both p < 0.001). Success sedation rate did not differ between groups (p = 0.73). Bradycardia incidence and injection site pain were less frequent in the R + E group than that in the P + E group. More rash was observed in the R + E group compared with the P + E group (32% vs. 5%, p < 0.001), but all were mild (only chest rash) and resolved subsequently. Low dose of remimazolam when combined with esketamine has favorable profiles with rapid onset and recovery, but mild hemodynamic side effects and adverse events. It can be used as an alternative for surgical abortion with general anesthetic.

Abstract:Purpose: To determine the anesthetic effect and related indicators following concurrent administration of remifentanil and propofol in cerebral hemorrhage surgery patients.Methods: A total of 88 cerebral hemorrhage patients admitted in Lishui People’s District Hospital, Nanjing, China, from December 2019 to December 2020, were assigned to two groups, viz, control group which received fentanyl and propofol for anesthesia, while study group was administered remientanil combined with propofol for anesthesia There were 44 patients in each group. Hemodynamic index, brain injury marker index, stress response index, awakening condition, propofol dosage, anesthetic effect, and adverse reactions were assessed and recorded.Results: Heart rate (HR), diastolic blood pressure (DBP), and systolic blood pressure (SBP) at T2 and T3 of the two groups were less than those at T1. At T3, the study group’s HR, DBP, and SBP were substantially higher than those of control group (p < 0.05); At 12 and 24 h after operation, brain injury markers and stress response indices in study group were significantly lower compared to control group (p < 0.05), while in comparison to control group (79.55 %), the degree of anesthesia in the study group was higher (95.45 %; p < 0.05). The incidence of adverse reactions in the study group (15.91 %) was lower than in control group (43.18 %; p < 0.05).Conclusion: Remifentanil, when combined with propofol anesthesia, stabilizes hemodynamics, protects against brain injury, and reduces stress reactions in patients undergoing cerebral hemorrhage surgery. The combination is also highly effective and safe. However, validation of these findings in larger clinical trials is required.

Effect of Remifentanil on the Pain after Artificial Abortion under Propofol Infusion

Abstract:Objective To investigate whether hyperalgesia would develop after artificial abortion anesthetized with propofol and remifentanil,and whether co-administration of ketamine would prevent that.Methods One hundred and eight early pregnant women undergoing artificial abortion under target controlled infusion(TCI) of propofol 7.5 μg/ml were randomized to 4 groups of propofol alone(group P),remifentanil(0.6 μg/kg)(group R),ketamine(0.15 mg/kg)(group K),and remifentanil plus ketamine(group KP).VAS scoring was performed and body movement was recorded during operation.Results HR and MAP at uterine neck dilatation is lower in group R than that in group P(P0.05).VAS score at 5 min during operation was significantly higher in group R than that in the other three groups(P0.05).Body movement was seen in 4 cases of group P and in 3 cases of group KP.Conclusion Remifentanil(0.6 μg/kg) cotreated with propofol target concentration infusion increases the VAS score after artificial abortion,suggesting that remifentanil may induce opioid-induced hyperalgesia(OIH) and ketamine could probably prevent OIH.
To Explore Effects of Propofol Plus Remifentanil on Analgesic Artificial Abortion

Abstract:Objective To explore effects of propofol plus remifentanil on analgesic artificial abortion. Method 178 patients undergoing analgesic artificial abortion were randomly divided into two groups: group A and B. Patients in group A receiving propofol (1.5~2.0 mg/kg) intravenously and those in group B receiving propofol (1.5~2.0 mg/kg) and remifentanil (1 μg/kg) instead. Propofol was increased according to the response of patients. The anesthetic effect, response during the operation, dosage and induction time were observed and compared between the two groups.Results The initial and later increased dosage of propofol in group A were markedly higher than those in group B (p0.05). MAP and HR measured at 1, 5, 10 minutes after operation were significantly higher in group A than in group B. Time needed to open eyes, restore consciousness and leave hospital was significantly longer in group A than in group B (p0.05). VAS value was considerably higher io group A than in group B (p0.05). Restlessness was more often occurred in group A than in group B (p0.05). Conclusion The infusion of propofol plus remifentanil in painless artificial abortion was superior to that of propofol alone. It shortened the awaken time, enhanced the anesthetic effects and should be extended in the clinic.
Pretreatment With Oxycodone Simultaneously Reduces Etomidate-Induced Myoclonus And Rocuronium-Induced Withdrawal Movements During Rapidsequence Induction

Abstract:Background: Etomidate and rocuronium are often paired in rapid-sequence anesthesia induction. However, the effect of pretreatment with oxycodone on myoclonic and withdrawal movements has not been previously investigated. The aim of this study was to evaluate the effects of oxycodone on the incidence and severity of etomidate-induced myoclonus and rocuronium-induced nociceptive withdrawal movements during rapid-sequence anesthesia induction.Material/Methods: We randomly divided 120 patients into the saline group (group S) and the oxycodone group (group O) (n=60 in each group). Patients received 0.05 mg/kg oxycodone or saline intravenously 2 min before administration of 0.3 mg/kg etomidate. The occurrence and severity of myoclonus were assessed after administration of etomidate, then rocuronium was injected, followed by evaluation of withdrawal movements.Results: The total frequency of involuntary movements following sequential administration of etomidate and rocuronium was significantly lower in Group O than in Group S (28.3% vs. 90%, p<0.001). The total frequency and grade 3 severity of myoclonus following etomidate injection in Group O was significantly lower than in Group S (25.0% vs. 63.3% for total frequency; 0 vs. 10 for grade 3 severity, P<0.001). The total frequency and grade 3 intensity of withdrawal movements were significantly less in Group O than in Group S (6.7% vs. 73.3% for total frequency; 0 vs. 11 for grade 3 intensity, P<0.001).Conclusions: Oxycodone is effective for simultaneously preventing etomidate-induced myoclonus and rocuronium-induced withdrawal movements during general anesthesia induction.
Inhibition of DOR prevents remifentanil induced postoperative hyperalgesia through regulating the trafficking and function of spinal NMDA receptors in vivo and in vitro

Abstract:Background: Several studies have demonstrated that intraoperative remifentanil infusions have been associated with opioid-induced hyperalgesia (OIH). Activation of delta opioid receptor (DOR) and augmentation of N-methyl-d-aspartate (NMDA) receptor expression and function may play an important role in the development of OIH. The aim of this study was to investigate whether DOR inhibition could prevent remifentanil-induced hyperalgesia via regulating spinal NMDA receptor expression and function in vivo and in vitro. Methods: A rat model of remifentanil-induced postoperative hyperalgesia was performed with the DOR agonist deltorphin-deltorphin II or the DOR antagonist naltrindole injected intrathecally 10 min before remifentanil infusion. Mechanical and thermal hyperalgesia were measured at -24h, 2, 6, 24 and 48 h after remifentanil infusion. Western blot was applied to detect the membrane and total expression of DOR and NMDA receptor subunits (NR1, NR2A and NR2B) in spinal cord L4-L6 segments. In addition, whole-cell patch-clamp recording was used to investigate the effect of DOR inhibition on NMDA receptor-induced current in spinal cord slices in vitro. Results: We found that membrane trafficking of DOR, NR1 and NR2B subunits in the spinal cord increased after remifentanil administration and surgery. The DOR antagonist naltrindole could attenuate mechanical and thermal hyperalgesia without affecting baseline nociceptive threshold, reduce membrane expression of DOR and decrease the membrane and total expressions of NR1 and NR2B subunits. Furthermore, the amplitude and the frequency of NMDA receptor-induced current were significantly increased by remifentanil incubation in neurons of the dorsal horn, which was reversed by the application of naltrindole. Conclusion: The above results indicate that inhibition of DOR could significantly inhibit remifentanil-induced hyperalgesia via modulating the total protein level, membrane trafficking and function of NMDA receptors in the dorsal horn of spinal cord, suggesting that naltrindole could be a potential anti-hyperalgesic agent for treating OIH.

Abstract:Objective To evaluate the efficacy of nalmefene antagonizing postoperative respiratory depression induced by opioids.Methods Two hundred and forty ASA Ⅰ orⅡpatients aged 18-64 yr with body weight fluctuating within 20% of the standard body weight were included in this multicenter,randomized,double-blind,positive drug-controlled study.Anesthesia was induced with etomidate 0.3 mg/kg and TCI of sufentanil(effect-site concentration 0.4.ng/ml).Tracheal intubation was facilitated with vecuronium 0.1 mg/kg or rocuronium 0.6mg/kg.The patients were mechanically ventilated.PETCO2 was maintained at 35-45 mm Hg.Anesthesia was maintained with sevoflurane+ sufentanil TCI(Ce=0.1-0.4 ng/ml).Patients undergoing neurosurgery and liver or kidney operation were excluded.The operation time was within 3 h.The residual effects of muscle relaxants were reversed after operation.The patients were randomly divided into 2 groups(n=120 each):group Ⅰneloxone andgroup Ⅱ nalmefene.Naloxone 0.1 mg or nalmefene 0.25 μg/kg was injected iv over 30 s and was repeated 5 min later if necessary until the respiratory rate＞10 bpm,PETCO2＜45 mm Hg and apnea time＜15 s.The total amount of naloxone was≤0.4 mg while that of nalmefene≤1 μg/kg.BP,HR,SpO2,PETCO2,respiratory rate and apnea time were recorded immediately before and at 2 and 5 min after haloxone/nalmefene administration and then every 5 min until 5 min after extubation.The recovery of spontaneous breathing within 30 min after naloxone/nalmefene administration,extubation time and Ramsay sedation score at 5 min after extubation were recorded.The patients were also observed for adverse reactions.Results Spontaneous breathing recovered within 30 min after naloxone/nalmefene administration in all patients in both groups.The extubation time was significantly shorter in nalmefene group than in naloxone group.There was no significant difference in Ramsay sedation score,BP,HR,SpO2 and incidence of adverse reactions between the 2 groups.Conclusion Nalmefene is better than naloxone in antagonizing opioid-induced postoperative respiratory depression.
Effect of remifentanil on rocuronium-induced neuromuscular block during general anesthesia

Abstract:Objective To investigate the effect of remifentanil (R) given by target-controlled infusion (TCI) on the rocuronium-induced neuromuscular (N-M) block during general anesthesia with propofol or isoflurane. Methods Forty-five ASAⅠorⅡpatients aged 20-65 yrs undergoing elective abdominal surgery under general anesthesia were randomly divided into 3 groups ( n = 15 each) : groupⅠpropofol (P) ; groupⅡpropofol + remifentanil (R) and groupⅢisoflurane (I). The patients were premedicated with intramuscular pethidine 50 mg, promethazine 25 mg and scopolamine 0.3 mg. Epidural catheter was placed at T9/10,10/11 or 11/12 interspace before induction of anesthesia and correct placement was confirmed by a test dose of 2 % lidocaine 3 ml. In groupⅠanesthesia was induced and maintained with TCI of propofol (target plasma concentration at 4 mg·L-1) while in groupⅡwith TCI of propofol (target plasma concentration at 4 mg·L-1) and remifentanil (target plasma concentration at 4μg·L-1) . Rocuronium 0.6 mg·kg-1 was given i.v. to facilitate tracheal intubation. In groupⅢanesthesia was induced with TCI of propofol as in groupⅠand maintained with 0.9% isoflurane. Tracheal intubation was facilitated with succinylcholine 100 mg. Rocuronium 0.6 mg·kg-1 was given at 5 min after T1 returned to 100% of control level before succinylcholine. N-M function was monitored and recorded using acceleromyography (TOF-GUARD, Organon Teknika N. V. ) . The force of thumb adduction in response to single and TOF stimulation of ulnar nerve was recorded. The recovery of spontaneous breathing was also recorded. Results The onset time of N-M block was comparable among the 3 groups ranging from 56 to 58 seconds. The duration of N-M block, the 75% recovery time and the recovery indexes in isoflurane group were significantly longer than those in group P and R (P < 0.05) . There was no significant difference in the above parameters between group P and R. No patient breathed spontaneously in group R during operation while 9 patients in group P and 5 patients in group I showed signs of spontaneous breathing. Conclusion TCI of remifentanil has no effect on rocuronium-induced N-M block.

Abstract:Objective: To investigate the anesthetic effect of dezocine and remifentanil combined with propofol in painless artificial abortion. Methods: 90 cases in total of painless induced abortion in our hospital from May 2017 to May 2020 were retrospectively analyzed, 29 cases of propofol anesthesia (group A), 29 cases of dezocine combined with propofol anesthesia (group B), and 32 cases of remifentanil combined with propofol anesthesia (group C) were compared the anesthesia situation. Results: Compared with the cases in group A, the incidence of pain, body movement and SPO2<90% at the injection site were lower in group B and group C, the use of propofol was reduced, and the VAS scores were lower when the patients begin to awake, and the efficiency was higher than that in group A(P 0.05). Conclusion: Dezocine and remifentanil combined with propofol for painless artificial abortion anesthesia has more significant effect than propofol anesthesia alone, which can relieve patients' pain and reduce the chance of respiratory depression.

Abstract:Purpose: To investigate the effect of target-controlled infusion of remifentanil in combination with propofol on anesthesia and endotracheal intubation response in patients undergoing surgical aneurysm clipping. Methods: The clinical data of 106 patients undergoing surgical aneurysm clipping at the Central Hospital of Wuhan, Wuhan City, China from September 2020 to December 2022 were retrospectively analyzed. Subjects in control group were given fentanyl intravenous infusion anesthesia while study group was treated with target-controlled infusion anesthesia of remifentanil combined with propofol. Hemodynamics and blood-gas indices were evaluated before anesthesia (T1), immediately after endotracheal intubation (T2) and at extubation (T3). The incidence of anesthesia-related complications was determined. Results: In study group, the agitation score was decreased compared to that of control group, while the Ramsay sedation score was increased (p < 0.05). The incidence of cough during endotracheal catheter indwelling and extubation was 7. 27 % in study group, which was less than 21. 57 % in control group (p < 0.05). The cough score of study group was also reduced (p 0.05). Furthermore, the mean arterial pressure at T2 and T3 was upregulated and heart rate (HR) was downregulated in study group (p < 0.05). Recovery time of spontaneous respiration, time of eye-opening and extubation in study group were decreased compared to control group (p < 0.05). Conclusion: Target-controlled infusion of remifentanil in combination with propofol improves anesthetic effect in patients undergoing surgical aneurysm clipping, reduces tracheal intubation reaction, maintains hemodynamic stability and improves recovery quality of patients. Future study will be required to evaluate the efficacy and optimal effective concentration in a diverse population.

Abstract:Objective To observe the patients hemodynamic reactions to the different stimula-tions under different blood concentrations of propofol with remifentanil.Exerting the response surface methodology,the half of the maximum effective concentration of propofol and the interaction index of propofol with remifentanil were calculated.Methods A total of 120 patients for scheduled abdominal surgery,randomly divided into 12 groups(n = 10).TCI 12 different concentration combinations of propofol with remifentanil,also the the hemodynamic reactions to harmful stimulations of intubation and incising were observed.The data were analyzed by responding surface analysis.Results The in-teraction index of propofol and remifentanil under intubation was 0.523 0±0.223 7,that under inci-sing 0.361 3±0.148 4,half of the maximum effective concentration of propofol for intutation and in-cising was (6.878 7 ± 1.832 1 ) μg/ml and (4.41 1 6 ± 0.902 5 ) μg/ml respectively. Conclusion Propofol and remifentanil have a synergistic inhibitory effect on harmful cardiovascular reaction among Chinese.During the endotracheal intubation,the requirements of half of the maximum effective concentration of propofol and intensity of the synergy with remifentanil are greater than those during the incising.

Abstract:Background The purpose of this network meta-analysis (NMA) was to evaluate the efficacy of intravenous opioid μ-receptor analgesics in shortening the duration of mechanical ventilation (MV) in ICU patients. Methods Randomized controlled trials comparing the efficacy of remifentanil, sufentanil, morphine, and fentanyl on the duration of MV in ICU patients were searched in Embase, Cochrane, Pubmed, and Web of Science electronic databases. The primary outcome was MV duration. The Bayesian random-effects framework was used to evaluate relative efficacy. Results In total 20 studies were included in this NMA involving 3,442 patients. Remifentanil was not associated with a reduction in the duration of MV compared with fentanyl (mean difference (MD) -0.16; 95% credible interval (CrI): −4.75 ~ 5.63) and morphine (MD 3.84; 95% CrI: −0.29 ~ 10.68). The secondary outcomes showed that, compared with remifentanil, sufentanil can prolong the duration of extubation. No regimen significantly shortened the ICU length of stay and improved the ICU mortality, efficacy, safety, and drug-related adverse events. Conclusion Among these analgesics, remifentanil did not appear to be associated with a reduction in MV duration. Clinicians should carefully titrate the analgesia of MV patients to prevent a potentially prolonged duration of MV due to excessive or inadequate analgesic therapy. Abstract:An open‐label, randomized, crossover study in healthy volunteers compared the reversal of remifentanil‐induced respiratory depression by intranasal (IN) naloxone hydrochloride (4 mg) to IN nalmefene (2.7 mg) (NCT 04828005). Subjects were administered a hypercapnic gas mixture which produces an elevation in minute ventilation (MV), a result of the ventilatory response to hypercapnia. Subjects breathed a hypercapnic gas mixture through a tight‐fitting mask for an initial period of 46 min prior to a series of mask "holidays" introduced to reduce subject discomfort and encourage study completion. Ten minutes after initiating the hypercapnic gas mixture, a remifentanil bolus was administered, and an infusion continued for the study duration. Subjects were administered either naloxone or nalmefene 15 min after initiating the remifentanil infusion and MV monitored for 21 min followed by a mask holiday. Both nalmefene and naloxone produced a time‐dependent reversal of remifentanil‐induced reductions in MV measured 2.5–20 min post administration. At the primary endpoint (5 min post administration), nalmefene increases in MV (5.75 L/min) were nearly twice that produced by naloxone (3.01 L/min) (P < .0009); the point estimate favors nalmefene, demonstrating non‐inferiority and superiority. In this model of opioid‐induced respiratory depression, nalmefene has a more rapid onset of action than naloxone, which required 20 min to achieve a comparable reversal of respiratory depression. Both nalmefene and naloxone were well tolerated by healthy volunteers. This rapid onset of action may prove particularly valuable in an era when over 90% of fatalities are linked to synthetic opioid overdose.
