Abstract:Introduction: Malignant ascites is a common complication of peritoneal metastatic disease.HES for England suggested that ascites (malignant and non-malignant) resulted in 6107 hospital Consultant episodes in 2002 to 2003, 77% of which required admission with a median length of stay of 6 days.Only 7% of hospital consultant days for ascites were single day episodes.There is no established treatment for malignant ascites in addition to paracentesis that has been used consistently over the last 30 years.There are few randomised trials.Radioisotopes, corynebacterium parvum, chemotherapeutic agents, interferon and most recently catumaxomab have been used.In Middlesbrough we have been using pleurex drains successfully for a number of years.Methods: We retrospectively audited all permanent peritoneal ( pleurex) drains inserted at James Cook University Hospital over a 36 month period to assess the level of complications, relief of symptoms and other procedures needed.Results: 48 patients were identified: 30 females and 18 males.Their age ranged from 31 to 90 median 61.A wide range of malignancies were treated including hepatobiliary 19%, ovary 18%, breast 13%, oesophagogastric 13%, unknown primary 13%.83% of patients had a previous paracentesis.Diuretics and use of other treatments except systemic chemotherapy were uncommon.Patients were admitted to hospital for a median of 5 days (1 day prior to the procedure and 3 days after).Complications were uncommon.6.5% become blocked and an infection occurred in 14.5%. 1 patient died within 24 hours.79% of patients had no complications.92% of patients had symptom relief.Only 1 patient required a further drain due to pocked ascites.Patients lived a median of 30 days post procedure with 49% of patients alive with a functioning drain at one month and 15% at 4 months.Conclusion: Pleurx drains are an effective method of palliating malignant ascites in patients with limited survival.They allow future care to be delivered at home and prevent hospitalisation.
