Abstract:Objective To investigate the sensitivity of Heidelberg Retina Tomography (HRT-Ⅱ)parameters on the early optic nerve change of glaucoma head. Design Cross-sectional study. Participants Twenty-one patients (36 eyes) with big cupped disc(C/ D≥0.7) and twenty-seven patients (31 eyes) with early glaucoma (C/D≥0.7, MD≤3dB) were studied. Methods All patients underwent visual field and HRT-Ⅱ examination to get parameters of optic nerve head. T test was used to compare the parameters of optic disc between big cupped disc and early glaucoma. Main Outcome Measures HRT Ⅱ parameters included disc area, cup area, rim area, cup volume, rim volume, C/D area ratio, mean cup depth, maximum cup depth, cup shape measure, height variation contour, mean RNFL thickness and RNFL cross sectional area. Results Statistics analysis with T test (t=2.247-3.714, P=0.000-0.028) showed there were significant differences of rim area, rim volume, mean retinal nerve fiber layer thickness, sectional area of retinal nerve fiber layer between big cupped disc and early glaucoma. Conclusions We can pay attention to those parameters with HRT- Ⅱ to distinguish patients with big cupped disc and early glaucoma.
Retinal Nerve Fiber Layer in Primary Open-Angle Glaucoma with High Myopia Determined by Optical Coherence Tomography and Scanning Laser Polarimetry.

Abstract:BACKGROUND:Fundus changes associated with high myopia (HM) may mask those associated with primary open-angle glaucoma (POAG). This study aim to determine the characteristics of RNFL thickness changes in patients with both POAG and HM and compare these to changes in patients with only HM. The diagnostic capabilities of both OCT and GDxVCC in this subset of patients are also evaluated.METHODS:Twenty-two eyes with POAG and HM (spherical equivalent (SE) between -6.0 and -12.0 D) were evaluated, and 22 eyes with HM were used for comparison. Characteristic retinal nerve fiber layer (RNFL) thickness profiles in patients with POAG and HM were examined using optical coherence tomography (OCT) and scanning laser polarimetry with variable corneal compensation (GDxVCC), and the diagnostic capabilities of these imaging modalities were compared. RNFL parameters evaluated included superior average (Savg-GDx), inferior average (Iavg-GDx), temporal-superior-nasal- inferior-temporal (TSNIT) average, and nerve fiber indicator (NFI) on GDxVCC and superior average (Savg-OCT), inferior average (Iavg-OCT), nasal average (Navg-OCT), temporal average (Tavg-OCT), and average thickness (AvgThick-OCT) on OCT (fast RNFL scan). Visual field testing was performed and defects were evaluated using mean defect (MD) and pattern standard deviation (PSD).RESULTS:The RNFL parameters (P < 0.05) significantly different between groups included Savg-GDx, Iavg-GDx, TSNIT average, NFI, Savg-OCT, Iavg-OCT, Tavg-OCT, and AvgThick-OCT. Significant correlations existed between TSNIT average and AvgThick-OCT (r = 0.778), TSNIT average and MD (r = 0.749), AvgThick-OCT and MD (r = 0.647), TSNIT average and PSD (r = -0.756), and AvgThick-OCT and PSD (r = -0.784). The area under the receiver operating characteristic curve (AUROC) values of TSNIT average, Savg-GDx, Iavg-GDx, NFI, Savg-OCT, Iavg-OCT, Navg-OCT, Tavg-OCT, and AvgThick-OCT were 0.947, 0.962, 0.973, 0.994, 0.909, 0.917, 0.511, 0.906, and 0.913, respectively. The NFI AUROC was the highest value.CONCLUSIONS:RNFL thickness was significantly lower in all but the nasal quadrant in patients with POAG and HM, compared to patients with only HM. Measurements with OCT and GDxVCC were well-correlated, and both modalities detected RNFL thickness changes. However, GDxVCC was better than OCT in detecting POAG in HM patients.
Correlation Between Optic Disc Perfusion and Glaucomatous Severity in Patients with Open-Angle Glaucoma: an Optical Coherence Tomography Angiography Study

Abstract:PURPOSE:To explore how optic disc perfusion varies in patients with open-angle glaucoma (OAG) and how this correlates with glaucoma severity.METHODS:We performed a prospective and cross-sectional observational study that included 62 eyes from 62 patients with OAG, divided into three groups according to their visual field (VF) results, and 20 eyes from 20 normal control subjects. Optic disc perfusion was studied using optical coherence tomography angiography (angio-OCT), and flow index and vessel density were determined. The VF, mean deviation (MD), pattern standard deviation (PSD), retinal nerve fiber layer (RNFL) thickness, and ganglion cell complex (GCC) thickness were also recorded. The potential associations between disc perfusion and VF defects or structural loss were analyzed.RESULTS:In OAG patients, the disc flow index and vessel density were significantly lower than in normal controls (all p<0.001) and were correlated with the severity of glaucoma. In OAG eyes, the flow index and vessel density were significantly correlated with MD, RNFL, and GCC thickness (all p<0.01), but were not in the normal controls. The receiver operating characteristic (ROC) curve analysis also revealed that disc flow index and vessel density had the power to differentiate normal eyes from eyes with OAG (under the ROC curves: 0.82 and 0.80, respectively).CONCLUSIONS:Angiograms demonstrated a reduced disc flow index and vessel density in glaucoma, and this reduction was closely related to GCC thickness. This indicated that measurement of disc perfusion by angio-OCT might be important for the monitoring of glaucoma.
[The Diagnostic Ability of Heidelberg Retina Tomography to Detect Glaucomatous Optic Neuropathy in Chinese].

Abstract:OBJECTIVE:To evaluate the ability of Heidelberg retina tomography (HRT) in the diagnosis of primary open angle glaucoma (POAG).METHODS:The topography of optic nerve head of 116 normal eyes and 99 eyes with POAG were examined using HRT. The differences of the optic disc parameters between two groups were compared. The topographic parameters of the optic nerve head were undergone stepwised discriminant analysis and the discriminant function was established. The sensitive diagnostic parameters of POAG were screened.RESULTS:The topographic parameters of optic nerve head, including disc area (DA), cup area (CA), cup/disc area ratio (C/DAR), rim area (RA), height variation contour (HVC), cup volume (CV), rim volume (RV), mean cup depth (MCD), maximum cup depth (MxCD), cup shape measure (CSM), mean retinal nerve fiber layer thickness (mRNFLT), retinal nerve fiber layer cross section area (RNFLA), showed significant (P < 0.001) difference between normal subjects and patients with POAG. The sensitive diagnostic parameters of POAG were C/DAR, RA, CSM, CV, MCD. The discriminant function was Y = -2.083-4.833 x C/DAR-20.379 x CSM + 2.035 x RA + 3.955 x CV-3.701 x MxCD. The sensitivity, specificity and diagnostic accuracy were 81.8%, 86.2% and 84.2%, respectively, which were comparable to the automatic analysis program of HRT.CONCLUSIONS:The topographic parameters of optic nerve head in patients with POAG showed significant differences from that in normal people. The power of discriminant function established based on our data was comparable to the HRT automatic analysis program. Our results indicate that HRT automatic analysis program is applicable to Chinese.
Evaluation of optic nerve head microcirculation in open-angle glaucoma patients with unilateral visual field defect

Abstract:Introduction: Microcirculation of optic nerve head (ONH) in open-angle glaucoma (OAG) patients with unilateral visual field (VF) loss has yet to be fully investigated, especially the perimetrically unaffected fellow eyes. Methods: Thirty-eight OAG patients with VF defect in one eye and normal VF in the other eye, and thirty-one healthy participants were analyzed. All participants underwent laser speckle flowgraphy (LSFG), spectral-domain optical coherence tomography (SD-OCT) imaging, and VF test for further analyses. LSFG measurements included mean blur rate in all area of ONH (MA), big vessel area of ONH (MV) and tissue area of ONH (MT). SD-OCT parameters included circumpapillary retinal nerve fiber layer (cpRNFL) thickness and macula thicknesses. The difference of LSFG and SD-OCT indices between glaucoma patients and healthy controls were compared. The diagnostic accuracy was analyzed with the areas under the receiver operating characteristic curves (AROCs). Results: Global cpRNFL thickness and macular thickness in unaffected eyes of OAG patients were higher than their fellow eyes and lower than healthy eyes. MA and MV in healthy eyes and unaffected eyes were significantly higher than in affected eyes. MT in unaffected eyes of OAG patients was higher than in their fellow affected eyes, but lower than in healthy eyes. The AROCs were highest for cpRNFL (0.925), followed by macular thickness (0.838), and MT (0.834). Conclusions: ONH microcirculation in perimetrically unaffected fellow eyes was decreased in OAG patients with unilateral VF loss. LSFG can detect changes of ONH in high-risk eyes before detectable VF damage, which may reflect the vascular pathophysiology for glaucoma.

Optic Nerve Head Morphologic Characteristics in Chronic Angle-Closure Glaucoma and Normal-Tension Glaucoma

Abstract:Purpose: To determine whether the morphometric features of the optic disk in eyes with chronic angle-closure glaucoma (CACG) are different from those with normal-tension glaucoma (NTG).Patients and Methods: This is a retrospective, case-matched study. Patients with CACG or NTG in an early or moderate stage were recruited. On the basis of age, sex, and visual field mean defect, patients with CACG were matched to those with NTG on a case-by-case basis. Heidelberg retina tomography was used to evaluate the optic disk, and parameters were compared between the 2 groups.Results: Sixty-three patients were recruited for the study. Twenty cases (eyes) with CACG were matched to 20 cases (eyes) with NTG. The total, temporal/inferior, nasal/superior, and nasal/inferior rim area of the optic disk were significantly larger (P < 0.05) in the CACG group. In patients with CACG rather than NTG, the cup volume in total and in each sector (except temporal) was significantly smaller (P = 0.007 to 0.041). In patients with CACG rather than NTG, the mean cup depth and cup shape measure of the optic disk in total and in each sector (except temporal and temporal/inferior) were smaller (P = 0.002 to 0.054).Conclusions: The morphometric features of the optic disk in eyes with CACG are different from those with NTG.
Diagnostic Capability of Fourier-Domain Optical Coherence Tomography in Early Primary Open Angle Glaucoma

Abstract:BACKGROUND Optical coherence tomography (OCT) is a high resolution noncontact imaging modality which can quantitatively detect the optic disc and retinal structure. This study was designed to evaluate the diagnostic capability of parameters of the optic disc, retinal nerve fiber layer thickness, and ganglion cell complex (GCC) using a new technology called Fourier-domain OCT (FD-OCT) for early primary open angle glaucoma (POAG) patients. METHODS Two groups of patients, early perimetric damage POAG and normal subjects were included in this observational cross-sectional study. All patients underwent FD-OCT and visual field examination in addition to full ophthalmic examinations. Receiver operating characteristic curves (ROC) were studied for all parameters. The sensitivity and specificity for distinguishing between normal and early glaucomatous eyes, the areas under the receiver operating characteristic curves (AROC) and positive, negative likelihood ratios were evaluated for all the single parameters and selected combined parameters using arbitrary cutoffs. RESULTS Thirty-four eyes of 34 early POAG patients and 42 eyes of 42 normal subjects were analyzed. Cup/disc (C/D) vertical ratio presented the best sensitivity and positive likelihood ratio for selected specificities (95% and 85%) which were 79.4% and 88.2%, 33.4 and 7.4, respectively. Among all single parameters, the C/D vertical ratio demonstrated the highest AROC which was at 0.930. The average thickness of circumpapillary RNFL on 3.45 mm showed the highest AROC among all of the peripapillary RNFL parameters. The sensitivity at selected specificity and AROC of GCC were not as high as C/D vertical ratio and RNFL AT on 3.45 mm. When the C/D vertical ratio, RNFL AT on 3.45 mm, and rim area were combined using a logistical diagnostic model, the AROC was raised to 0.949 but not significantly different from the top single parameter, C/D vertical ratio. CONCLUSIONS The key parameters obtained by FD-OCT were able to show the significant differences of optic discs, thickness of RNFL and GCC between POAG patients and normal subjects. According to sensitivity, specificity, likelihood ratio and AROC, the top three parameters from FD-OCT for early diagnosis of POAG were C/D vertical ratio, RNFL AT on 3.45 mm, and the rim area.
Macular thickness assessment with optical coherence tomography for glaucoma early diagnosis

Abstract:Optical coherence tomography (OCT) is a non-contact,good repeatability technique to obtain fastly high-resolution images from the anterior and posterior segments of the eye,and it has been widely used in the field of ophthalmology.Previous studies have shown that the thickness of circumpapillary retinal nerve fibre layer (cpRNFL) and optic disc assessment with OCT are valuable in diagnosing glaucoma.Traditional time domain (TD) OCT can measure macular nerve epithelium thickness.However,spectral-domain(SD) OCT allows a higher image resolution and can measure the thickness of retinal ganglion cell complex,which has less variablity in normal subjects so to be used as better indicator for early diagnosis in glaucoma.In this paper,we review the macular thickness measurement with OCT in glaucoma early diagnosis.
[Topographic Stereoscopic Measurements of Optic Disc in Early Primary Open Angle Glaucoma].

Abstract:OBJECTIVE To search a sensitive parameter for the early diagnosis of primary open angle glaucoma (POAG). METHOD A system of computerized image analysis was used to acquire images of the optic nerve head of patients with POAG, suspect glaucoma (SG) and of normal persons. Each of these groups contained 31 eyes of 31 cases. The mean relative depths of disc rim and cup at different areas in the optic nerve head were measured. All these patients were followed up for four to six months, and the changes of the mean depths of every sector were compared between each pair of the three groups with statistic analysis. RESULTS The mean depth of the neural rim and cup of each sector and that of total rim and total cup of POAG and SG patients were all deeper than that of normal controls. The mean depths of neural rim of the superior sector, inferior sector and the total rim area of POAG were deeper than that of SG patients. Compared to their first measurements, the changes of mean depths of neural rim of superior sector and total neural rim area of POAG patients were more prominent than that of normal controls and SG. We also compared the relative depth of the total rim area in the different areas in the optic nerve head with other two dimensional optic disc parameters such as cup/disc ratio, etc. CONCLUSION Our study suggests that three-dimensional stereoscopic measurement of the surface of optic nerve head and follow-up be of paramount importance in the early diagnosis of POAG patients.
[A Study on Correlation Between the Structural Parameter of Optic Disc and Visual Field in Primary Open-Angle Glaucoma].

Abstract:OBJECTIVE:To study the consistency between the optic disc neural rim depth (NRD) and visual field defect in patients with primary open angle glaucoma (POAG).METHODS:A system of computerized image analysis was used to measure the relative NRDs of temporal-superior and temporal-inferior area of the optic disc in patients with primary open angle glaucoma. The consistency between the area with relatively larger NRD and the location of severer visual field defect was investigated.RESULTS:In cases with early and middle stage POAG, the value of NRD corresponding to the severer location of the visual field defect was significantly larger than that corresponding to the location without or with mild such defect. Among 54 eyes, there were 46 eyes whose location with larger NRD value was consistent with the position of severer visual field defect, the rate of consistency being 85.2%.CONCLUSION:In cases with early and middle stage POAG, the increase of NRD value at temporal-superior and temporal-inferior area is asymmetrical, and the consistency between the area with larger NRD value and the corresponding position of severer visual field defect is relatively good. NRD is helpful to the early diagnosis of POAG.
Comparison of diagnostic accuracy of Heidelberg Retina Tomograph II and Heidelberg Retina Tomograph 3 to discriminate glaucomatous and nonglaucomatous eyes

Abstract:Purpose: To compare the diagnostic accuracy of the Moorfields regression analysis (MRA), parameters, and glaucoma probability score (GPS) from Heidelberg Retinal Tomograph (HRT) 3 (Heidelberg Engineering, Heidelberg, Germany) with MRA and parameters from HRT II in discriminating glaucomatous and healthy eyes in subjects of African and European ancestry. Design: Case-control institutional setting. Methods: Seventy-eight glaucoma patients (44 of African ancestry, 34 of European ancestry) and 89 age-matched controls (46 of African ancestry, 33 European ancestry), defined by visual fields and self-reported race were included. Imaging was obtained with HRT II, and data were exported to a computer with the HRT 3 software using the same contour line. Area under the receiver operating characteristic (ROC) curves (AUCs), sensitivity, and specificity were evaluated for the entire group, the African ancestry group, and the European ancestry group separately. Mean disk area was compared between correctly and incorrectly diagnosed eyes by each technique. Results: Disk, cup, and rim areas from HRT 3 were lower than HRT II (P < .0001). AUC (sensitivity at 95% specificity) was 0.85 (54%) for vertical cup-to-disk ratio (VCDR) HRT 3, 0.84 (45%) for VCDR HRT II, and 0.81 (44%) for GPS at the temporal sector. MRA HRT 3 showed greater sensitivity but lower specificity than HRT II for the entire group, the African ancestry group, and the European ancestry group. GPS classification had the lowest specificity. Glaucomatous eyes incorrectly classified by GPS had smaller mean disk area (P = .0002); control eyes incorrectly classified had greater mean disk area (P = .015). Conclusions: VCDR from HRT 3 showed higher sensitivity than HRT II and GPS for the entire group and for those of African ancestry and of European ancestry separately. Sensitivity of MRA improved in HRT 3 with some trade-off in specificity compared with MRA of HRT II. GPS yielded erroneous classification associated to optic disk size.
[Comparison of Peripapillary Atrophy in Eyes with Unilateral Visual Field Defect in Patients with Primary Open-Angle Glaucoma and Normal Tension Glaucoma].

Abstract:OBJECTIVE:To evaluate the difference of the peripapillary atrophy between two eyes of unilateral visual field defect in each of patient with primary open-angle glaucoma (POAG) and normal tension glaucoma (NTG).METHODS:This study included 40 eyes with unilateral visual field defect in patients with POAG (20 cases) and NTG (20 cases). Color stereo optic disc photographs were morphometrically evaluated by a video camera and a computer system. The refractive range (spherical equivalent) of all these subjects was between -3.00D and +3.00D.RESULTS:The cup areas (CA) and cup/disc areas (CA/DA) in eyes with visual field defect were significantly larger than in eyes with normal visual field (CA: t=5.332, P<0.01; CA/DA: t=5.126, P<0.01). The alpha zone areas (alphaA) of PPA were significantly enlarged in visual field defect eyes compared with visual field normal eyes (t=3.02, P=0.0045), while there is no significant difference in beta-zone area (betaA) between two eyes. (S=13.5, P=0.426). The frequency of the PPA (alpha-zone and beta-zone) occurrence were also similar in both eyes. (alphaA: chi(2)=0.00, P=1.000; betaA: chi(2)=1.2857, P=0.2568).CONCLUSION:The results suggest that either beta-zone area or the presence of PPA is not closely correlated to the visual field defect in POAG and NTG patients with exclusion of moderate or high myopia.
Intereye and intraeye asymmetry analysis of retinal microvascular and neural structure parameters for diagnosis of primary open-angle glaucoma

Abstract:Purpose To investigate the diagnostic values of intereye or intraeye asymmetry of retinal perfused vessel density and neural structure parameters for detection of glaucoma. Methods In total, 152 healthy subjects and 72 bilateral primary open-angle glaucoma (POAG) patients were enrolled. Total POAG group contains all glaucoma patients. Early to moderate POAG group contains patients whose binocular mean defect values were larger than −12 dB. The retinal perfused vessel densities were acquired using optic coherence tomography angiography. The neural structure parameters include RNFL, GCC thickness and its derivative indices like focal loss volume percentage (FLV%) and global loss volume percentage (GLV%). Intereye asymmetry equaled to the absolute difference of parameters between paired eyes. Intraeye asymmetry was defined as absolute difference between the inferior and superior hemisphere values from one random selected eye. The areas under the receiver operating characteristic curves (AUROCs) were calculated to evaluate diagnostic ability. Results From pairwise comparison analysis of ROC curves, the intereye asymmetric parameters with the largest diagnostic accuracy were FLV and GLV% (AUROC = 0.944), which were significantly superior to the intereye asymmetry of perfused vessel density in peripapillary area and parafoveal area ( P < 0.05). Particularly, the intereye asymmetry of FLV% (AUROC = 0.926) and GLV% (AUROC = 0.950) showed excellent diagnostic precision for detecting early to moderate glaucoma patients. However, the intraeye asymmetry of microvascular parameters and neural structure parameters showed fair diagnostic ability for identifying POAG patients. Conclusions The intereye asymmetry of neural structure parameters, particularly the FLV% and GLV%, outperformed the microvascular parameters for identifying POAG patients.
Challenges In Early Glaucoma Detection

Abstract:Introduction: Glaucoma is the most common optic neuropathy which is characterized by progressive loss of retinal ganglion cells, the excavation of the optic nerve head, associated with defects in the visual field. It is not a disease, but the final result of united and yet completely unidentified cellular and subcellular processes and effects of many factors responsible for changes in retinal ganglion cells leading to their accelerated apoptosis. Methods: This is a prospective-retrospective, comparative, randomized clinical trial that included 150 patients, 97 were female and 53 male. The age of patients ranged from 18 to 80 years. Results: The highest degree of myopia in category of tilted optic discs had patients with large disc (4.05 + -0.65). Values of the degree of myopia have linearly declined in relation to the size of the oblique disc. The analysis of the results revealed that the subjects who had a higher degree of myopia associated with glaucoma had frequent parapapillar atrophy of alpha and beta zones. The highest percentage of subjects with parapapillar changes were in the group of patients who had other than glaucoma and myopia (62%), then in the group of patients with glaucoma only (56%). Conclusion: Previous studies on the relationship between myopia and open-angle glaucoma are based on the results of observational studies. However, according to recent findings, based on the available studies, the systematic approach to estimate the association between myopia and glaucoma does not exist. Disc Damage Likelihood Scale (DDLS) is a new system for assessing glaucomatous damage of the optic disc which strongly correlates with the degree of visual field loss.
Diagnostic Capability of Different Morphological Parameters for Primary Open‐angle Glaucoma in the Chinese Population

Abstract:Background To assess the diagnostic capability of novel Bruch’s membrane opening (BMO)-based disc parameters, the BMO-minimum rim width (BMO-MRW) and the BMO-minimum rim area (BMO-MRA) in the Chinese population and compare them to the retinal nerve fiber layer (RNFL) from optical coherence tomography (OCT) and the rim area (RA) from the Heidelberg retinal tomograph-III (HRT-III). Methods In total, 200 eyes of 77 healthy and 123 primary open-angle glaucoma (POAG) subjects were included in this cross-sectional study. All participants underwent the visual field test and structural measurements by OCT and HRT-III. The areas under the receiver operating characteristic curves (AUCs) of different structural parameters were calculated to assess their diagnostic power and compared using the DeLong test. Results In populations with different characteristics, the BMO-MRW and BMO-MRA had better diagnostic power than the RA. In discriminating between all POAG subjects and healthy controls and between early-stage patients and controls, the global BMO-MRW had comparable AUCs with the RNFL, but the BMO-MRA had lower AUCs than the RNFL. In healthy subjects with macrodiscs, both the global and sectoral BMO-MRW were thinner than those in healthy subjects with normal disc size. The AUCs of BMO-MRA, BMO-MRW and RNFL in subjects with macrodiscs were comparable. Additionally, in the myopic population, the BMO-MRA and BMO-MRW had comparable AUCs with the RNFL. Conclusions The BMO-MRW had comparable diagnostic power with the RNFL, and compared with BMO-MRW, the BMO-MRA might have advantages in certain populations, such as macrodiscs. All OCT-derived parameters exceeded the RA in diagnostic capability.
[Clinical Characteristics of Eyes with Unilateral Disc Hemorrhage in Normal Tension Glaucoma Patients].

Abstract:OBJECTIVE:To investigate the clinical characteristics of the eyes with unilateral disc hemorrhage (DH) in normal tension glaucoma (NTG) patients.METHODS:37 normal-tension glaucoma patients with unilateral DH were studied. Extensive parameters were analyzed to evaluate which parameters differ significantly between the DH eyes and the companion eyes. These parameters are: (1) intraocular pressure (IOP), (2) dioptres of refractive errors, (3) central corneal thickness (CCT), (4) disc area, (5) disc shape including horizontal diameter, vertical diameter and horizontal diameter/vertical diameter, (6) global parameters of optic disc, including rim area (G-RA), rim volume (G-RV), mean retinal never fiber layer thickness (G-MRNFLT), cup shape measure (G-CSM) and height variation contour (G-HVC), (7) sectorial parameters of DH located area, including S-RA, S-RV, S-MRNFLT, S-CSM and S-HVC, (8) parameters of the peripapillary atrophy (PPA) beta zone including the incidence rate, area, extent and maximum width. The global and sectorial parameters of the optic disc were obtained directly from HRT-II. The other parameters, such as optic disc horizontal diameter, vertical diameter and PPA, were obtained by processing the raw optic disc images acquired from HRT-II using computer software Image-pro plus version 6. Univariate and multivariate regression analyses were performed on the acquired parameters.RESULTS:The disc vertical diameter (P < 0.01) and incidence rate (P = 0.031) of zone beta appeared to be significantly larger in the DH eyes than the ones in the contra-lateral eyes, while horizontal diameter/vertical diameter was significantly smaller in the DH eyes. As for the optic disc global parameters, the G-MRNFLT (P = 0.028), G-HVC (P = 0.047) were significantly lower in the DH eyes than in the contra-lateral eyes while the G-CSM was significantly higher in DH eyes (P = 0.047). The differences of G-RA, G-RV between two eyes were not significant. As for the regional parameters, S-RA, S-RV, S-MRNFLT were significantly smaller in the DH eyes than in the contra-lateral eyes with P-values 0.033, 0.01 and 0.008 respectively. S-CSM was significantly larger in DH eyes (P = 0.021). S-HVC did not display significant difference between two groups. There were also no significant differences in CCT, dioptres of refractive errors, IOP, PPA parameters (including area, extent and maximum width) between DH eyes and companion eyes. Multivariate regression analysis demonstrated that contained disc area, dioptres of refractive errors, G-RA, G-MRNFLT, G-CSM, horizontal diameter/vertical diameter and zone beta extent, the horizontal diameter/vertical diameter were the significant factors independently associated with disc hemorrhage (P = 0.032, OR < 0.001: < 0.001 - 0.35). Logistic regression analysis also showed that contained disc area, dioptres of refractive errors, S-RA, S-MRNFLT, S-CSM, horizontal diameter/vertical diameter and zone beta extent, the horizontal diameter/vertical diameter and S-MRNFLT were the significant factors independently associated with disc hemorrhage (S-MRNFLT: P = 0.019, OR < 0.001: < 0.001 - 0.154; horizontal diameter/vertical diameter: P = 0.02, OR < 0.001: < 0.001 - 0.124).CONCLUSION:Our study indicates that horizontal diameter/vertical diameter is significantly smaller in NTG patients with unilateral optic disc hemorrhages. The DH located area also appears to have thinner RNFL thickness. Unilateral optic disc hemorrhages tend to have severe RNFL damage in DH located area and their optic disc shape tend to be longer, therefore we hypothesis that the elongation of the optic disc may be associated with the occurrence of DH.
Optical Coherence Tomography Angiography Analysis of Peripapillary Choroidal Microvascular Density in normal-tension Glaucoma and Primary open-angle Glaucoma

Abstract:Prcis: Glaucoma patients had a reduction in the inner annulus peripapillary choroidal microvascular density (PCMD) that became worse as the glaucoma severity progressed, which might provide new evidence supporting the vascular theory. Purpose: To compare PCMD among normal-tension glaucoma (NTG), primary open-angle glaucoma (POAG), and healthy controls using optical coherence tomography (OCT) angiography (OCTA). Methods: The study included 40 POAG, 25 NTG, and 33 healthy controls. All subjects underwent OCT and OCTA testing. Inner annulus and outer annulus PCMD, as well as peripapillary vessel density (VD), was calculated. One-way analysis of variance was used to compare the vascular parameters of the three groups. Pearson correlation analysis or Spearman correlation test was used to evaluate the correlation between PCMD and glaucomatous severity factors. The spatial positional relationship between PCMD and corresponding peripapillary retinal nerve fiber layer (RNFL) thickness and visual field (VF) mean deviation (MD) was also assessed. Results: The average and four quadrants of the inner annulus PCMD and peripapillary VD in the two glaucomatous groups were significantly lower than in normal eyes (P<0.05). Strong correlations were found between inner annulus PCMD and VF MD, peripapillary VD, and RNFL in POAG patients. Similarly, the inner annulus PCMD in NTG patients was strongly correlated with peripapillary VD and RNFL (all r>0.5). Strong positional correlations were found between inner superior quadrantal PCMD and RNFL thickness in both POAG and NTG patients (r=0.566, r=0.731, respectively). Likewise, inner inferior quadrantal PCMD exhibited a strong correlation with RNFL thickness in POAG patients (r=0.608). Strong positional correlations were also found between inner superior PCMD and VF MD in both POAG and NTG patients (r=0.589, r=0.622, respectively). Inner inferior PCMD exhibited a moderate correlation with VF MD in both POAG and NTG patients (r=0.487, r=0.440, respectively). Conclusion: The study found that the inner annulus PCMD decreased to varying degrees in NTG and POAG patients. The inner annulus PCMD was closely related to the structural and visual function parameters of glaucoma in both NTG and POAG. Furthermore, inner PCMD demonstrated a spatial correlation with corresponding RNFL thickness and VF MD.

Abstract:Introduction: Primary open-angle glaucoma (POAG) is a common type of glaucoma caused by damage to the optic nerve, leading to visual field abnormalities and reduced visual acuity. Understanding the relationships between these variables can improve diagnostic and management procedures, enabling personalized treatment programs that consider each patient's vision and condition. Materials And Methods:A multifaceted approach was used to assess 50 patients diagnosed with POAG at Sree Mookambika Institute of Medical Sciences, Kulasekharam. Data included demographic information, medical history, and ophthalmic history. Visual field testing was performed using the 24-2 protocol, and optic disc morphology was assessed using digital fundus photographs and optical coherence tomography. Statistical analysis was performed using SPSS. Results:The study analyzed 50 patients with moderate field defects, early field defects, severe field defects, advanced field defects, end stage disease, and normal eyes. Results showed a significant percentage of eyes with early field changes, moderate field defects, severe field changes, advanced field defects, end stage disease, and normal eyes. Conclusion: The study investigated the relationship between optic disc morphology in primary open-angle glaucoma (POAG) and visual field abnormalities, visual acuity, and other factors. It found that specific aspects of the optic disc's morphology are correlated with these symptoms.

Abstract:Glaucoma is an irreversible disease which is featured by characteristic optic nerve cupping and corresponding visual field deficits.For the hysteretic nature of visual field,as well as the physiological advantages of macular area,the measurements of inner macular thickness may be superior in the early diagnosis of glaucoma.Thanks to the rapid development of OCT technology which can isolate the inner macular layers,some papers have reported that the inner macular thickness analysis can distinguish early glaucomatous defects from the normal eyes well and may be predictive in the following-up of glaucoma.
