Effect of C-peptide on diabetic nephropathy in GK rats

Abstract:Objective To evaluate the effects of C-peptide on renal pathology and function of GK rats with diabetic nephropathy. Methods Diabetic GK rats with more than 300 mg of 24-h urinary protein were randomly divided into 3 groups:C-peptide group , vehicle group and insulin group. All treatment lasted 12 weeks and normal Wistar rats at the same age were used as normal group. Glycemia, 24-h urinary protein and 24-h urinary albumin were measured before treatment and every 4 weeks during treatment period. After 12 weeks of treatment, kidneys were harvested and renal pathology was detected by PAS staining and electron microscopy. Real-time PCR, Western blot and immunohistochemistry were used to evaluate the expression level of receptor of advanced glycation end products (RAGE), protein kinase C-β (PKC-β) and protein kinase A (PKA) in the glomeruli of diabetic nephropathy. One-way ANOVA were used for statistical analysis. Results Compared with normal group, the blood glucose, 24-h urinary protein and urinary albumin in vehicle group were statistically increased. C-peptide treatment can decrease the 24-h urinary protein and 24-hour urinary albumin without influence on glycemic control. Insulin can decrease blood glucose effectively, but has no influence on the 24-h urinary protein and 24-h urinary albumin. Pathological detection showed that C-peptide improve glomerulosclerosis, podocytes morphology and reduce the thickness of GBM as compared to vehicle group. The result of real-time PCR, Western blot and immunohistology demonstrated that C-peptide downregulated the expression of RAGE and PKC-β, and upregulated the expression of PKA in glomeruli of diabetic rats, and insulin has no significant effect on expression of PKC and PKA. Conclusion Diabetic nephropathy in GK rats is ameliorated by C-peptide through downregulating the expression of RAGE and PKC-β, upregulating the expression of PKA in glomeruli.
