Uncinate process resection first approach in pancreaticoduodenectomy

Abstract:Objective To introduce a new operative technique applied in pancreaticoduodenecto- my.Method When tumor stretches over the neck of the pancreas;the situation of tumor infiltration to the superior mesentery artery is not clear;or when there maybe abnormal origin of hepatic artery, uncinate process resection first approach can effectively avoid the embarrassing situation during pan- creaticoduodenectomy.If no distant or liver metastasis was found after a thorough abdominal explora- tion,gastrocolic ligment was opened and Kocher's manoneur was performed to expose pancreas and the tumor,uncinate process was first resected from the superior mesentery artery.If the tumor infiltration to the superior mesentery artery was found,then the radical operation was abandoned.When there was abnormal origin of liver artery,this approach could effectively avoid the accidental injury of liver artery.If the tumor was stretched over portal region,this technique could guarantee the resection of tumor meanwhile avoid the injury of portal vein.Results From June 2004 to May 2006,9 patients re- ceived pancreaticoduodenectomy according to this technique.Of the 9 patients with an average age of 60.5,6 were male and 3 female.There was no peri-operative mortality.The mean operative duration was 6.5 hours while the traditional pancreaticoduodeneetomy costs 5.2 hours,this technique signifi- cantly prolong the time cost.Post operative complication rate and intraoperative blood loss was not in- creased while using this new technique.Conclusion Uncinate proess resection first approach is a safe operation for experienced pancreatic surgeons.
