Morphological characteristic differences between blastic phase of chronic myelocytic leukemia and acute leukemia

Abstract:Objective: To investigate the morphological characteristic differences between blastic phase of chronic myelocytic leukemia and acute leukemia.Methods: The different morphological characteristics of bone marrow and peripheral blood cells were analyzed of eleven cases of chronic myelocytic leukemia at blastic phase and fifty-two patients of newly diagnosed acute leukemia.Results: Among eleven cases of the blastic phase of chronic myelocytic leukemia patients,two cases were changed to acute lymphocytic leukemia and others were changed to acute non-lymphocytic leukemia.Among fifty-two cases of newly diagnosed acute leukemia,fifteen cases were acute lymphocytic leukemia and thirty-seven cases were acute non-lymphocytic leukemia.In peripheral blood of the CML cases changed to AML,the numbers of eosinophilia and or basophil were above normal values in CML,and platelet number in peripheral blood was greater than 50×109/L.Bone marrows of the CML cases changed to AML were hyperplasia,the early immature myeloid cells were significantly increased,and the eosinophilic and basophilic cells were also increased in bone marrow,and NAP integral was normal.In the blood smear of CML patients changed to ALL,the promyelocytic cell and eosinophil,basophil cells were increased,which was different from the case of primary ALL.In addition,we need a combination of clinical history and cytogenetic examination to identify the blastic phase of CML and AL.Conclusion: For patients with chronic myelocytic leukemia at blastic phase,the eosinophilic or basophilic cells and promyelocytic cells were increased both in blood and bone marrow.We should identify them with combination of NAP,clinical history and cytogenetic examination.
