ASL MRI for Cerebral Hypoperfusion in Patients with Symptomatic Carotid Artery Occlusion

Abstract:Purpose:To evaluate the value of the arterial spin labelling (ASL) magnetic resonance imaging (MRI) for the cerebral hypoperfusion in patients with symptomatic unilateral severe internal carotid artery (ICA) stenosis or occlusion by comparison of relative cerebral blood flow (rCBF) between ASL and dynamic susceptibility contrast -enhanced (DSC) MRI. Materials and Methods: Twelve volunteers were performed ASL and DSC and the rCBF ratios were calculated as right side versus left side and compared. Twelve patients with symptomatic ICA stenosis≥70% in unilateral side were performed brain ASL and DSC MRI. The rCBF ratios from DSC and ASL in the region of interest were calculated as stenosis side versus contralateral side and then compared. Results: In 12 volunteers, the values of the rCBF ratios from DSC and ASL were no significant difference and the normal rCBF ratios were 1. 0012±0. 1030 and 1. 0015±0. 1611 respectively. In all 32 patients, 15 cases with cerebral hypoperfusion were found in DSC. No significant difference was found in the value of rCBF ratios from DSC and ASL. Conclusion:The ASL MRI is capable of detecting the cerebral hypoperfusion in patients with unilateral ICA stenosis or occlusion as a noninvasive manner.
