Abstract:Hepatocellular carcinoma (HCC) is a prominent contributor to cancer-related mortality worldwide. Early detection and diagnosis of liver cancer can significantly improve its prognosis and patient survival. Ultrasound technology, serving has undergone substantial advances as the primary method of HCC surveillance and has broadened its scope in recent years for effective management of HCC. This article is a comprehensive overview of ultrasound technology in the treatment of HCC, encompassing early detection, diagnosis, staging, treatment evaluation, and prognostic assessment. In addition, the authors summarized the application of contrast-enhanced ultrasound in the diagnosis of HCC and assessment of prognosis. Finally, the authors discussed further directions in this field by emphasizing overcoming existing obstacles and integrating cutting-edge technologies.
Application of Breast Ultrasound Imaging Report and Data System classification in diagnosis of special types of breast cancer

Abstract:Objective:To investigate the value of Breast Ultrasound Report and Data System (BI-RADS) classification in diagnosis of special types of breast cancer.Methods:A total of 112 patients with special type of breast cancer (112 breast lesions) confirmed by pathology were analyzed by using BI-RADS ultrasound category in the Second Affiliated Hospital, Zhejiang University School of Medicine from August 2009 to August 2020. All patients underwent ultrasound before surgery. The breast lesions were evaluated by senior attending and junior resident according to BI-RADS ultrasound category respectively. Taking histopathological result as the gold standard, the sensitivity and accuracy of BI-RADS classification in the diagnosis of special types of breast cancer were calculated.The differences between different special types of breast cancer in terms of ultrasound characteristics and pathological features were analyzed. Kappa consistency test was used to evaluated the consistency of the results of two physicians.Results:In the 112 patients, pathological results showed that there were 20 cases of metaplastic carcinoma, 19 cases of invasive carcinoma with medullary features, 16 cases of differentiated carcinoma of apocrine gland, 12 cases of mucinous carcinoma, 12 cases of invasive micropapillary carcinoma, 10 cases of invasive papillary carcinoma, 6 cases of invasive lobular carcinoma and 17 cases of other special types of carcinoma. Among them, 4 cases (3.5%) were BI-RADS 3, 13 cases (11.6%) were BI-RADS 4a, 42 cases (37.5%) were BI-RADS 4b, 47 cases (42.0%) were BI-RADS 4c and 6 cases (5.4%) were BI-RADS 5. The accuracy and sensitivity of BI-RADS classification in diagnosis of special types of breast cancer was 96.43% and 96.43%, respectively. There was significant difference in BI-RADS grade among different special types of breast cancer ( P<0.05). Most lesions were characterized by hypoechoic with irregular shape and angular or microlobulated margin. The nodule size, boundary, echo and posterior echo in breast cancer with different special types showed significant differences (all P<0.05). There was a good consistency between the two physicians (Kappa=0.789). Conclusions:The ultrasonography features of different special types of breast cancer are different. BI-RADS classification has great value in diagnosis of special types of breast cancer.

Abstract:Frequent advances in transducer design, electronics, computers, and signal processing have improved the quality of ultrasound images to the extent that sonography is now a major mode of imaging for the clinical diagnosis of breast cancer. Breast ultrasound is routinely used for differentiating cysts and solid nodules with high specificity. In combination with mammography, ultrasound is used to characterize solid masses as benign or malignant. There is growing interest in using Doppler ultrasound and contrast agents for measuring tumor blood flow and for imaging tumor vascularity. Ease of use and real-time imaging capability make breast ultrasound a method of choice for guiding breast biopsies and other interventional procedures. Breast ultrasound is used in many forms. B-mode is the most common form of imaging for the breast, although compound imaging and harmonic imaging are being increasingly applied to better visualize breast lesions and to reduce image artifacts. These developments, together with the formulation of a standardized lexicon of solid mass features, have improved the diagnostic performance of breast ultrasound. Several approaches that are currently being investigated to further improve performance include: (1) computer-aided-diagnosis; (2) the assessment of tumor vascularity and tumor blood flow with Doppler ultrasound and contrast agents; and (3) tissue elasticity imaging. In the future, ultrasound will play a greater role in differentiating benign from malignant masses and in the diagnosis of breast cancer.

Abstract:Objective With the advance of imaging technology,ultrasound has been used for the screening examination of breast cancer.This study aimed to investigate the value of ultrasound-guided automatic biopsy(USGAB) in the diagnosis of breast tumors.Methods We compared the results of pathological examination and USGAB with those obtained by surgery in 60 cases of breast tumors.ResultsThe results of USGAB conformed to those of pathological examination in 58(96.7%) of the cases,of which 50 were confirmed to be breast cancer by molecular biology,and the other 8 diagnosed as benign masses.Conclusion USGAB,as a safe,accurate and efficient method,has a significant clinical application value in the diagnosis and differential diagnosis of breast tumors.

Abstract:Breast cancer is the most common cancer in women worldwide. Providing accurate and efficient diagnosis, risk stratification and timely adjustment of treatment strategies are essential steps in achieving precision medicine before, during and after treatment. Radiomics provides image information that cannot be recognized by the naked eye through deep mining of medical images. Several studies have shown that radiomics, as a second reader of medical images, can assist physicians not only in the detection and diagnosis of breast lesions but also in the assessment of risk stratification and prediction of treatment response. Recently, more and more studies have focused on the application of ultrasound radiomics in breast management. We summarized recent research advances in ultrasound radiomics for the diagnosis of benign and malignant breast lesions, prediction of molecular subtype, assessment of lymph node status, prediction of neoadjuvant chemotherapy response, and prediction of survival. In addition, we discuss the current challenges and future prospects of ultrasound radiomics.

Abstract:Objective To investigate the availability of real time enhanced-ultrasonography(SonoVue) in diagno-sing breast tumor.Methods34 patients with breast tumors(16 benign, 18 gmalignant) were examined by real time enhanced-ultrasonography, and SonoVue and CnTI Techs were applied in the examination.To observe the type of Wash-in and Wash-out Curve, measure acoustic quantified parameters, compare and analyze the characteristics and changes of the curves in different groups(benign and malignant).ResultsThe curve appeared slow-up and quick-down type in benign group, quick-up and slow-down type in malignant group either.Ultrasongraphic quantified parameters:There were no significance in peak time of contrast between the groups(P 0.05).The peak intensity and average intensity of peak-60s of SonoVue in malignant group were significantly higher than those in benign group(P 0.05).The obliquity rate and areas under peak-60s of the Wash-in and Wash-out curve in malignant group were highly significantly bigger than those in benign group(P 0.01).ConclusionsIt was capable to image the difference of benign and malignant breast tumor in Wash-in and Wash-out Curve by real time enhanced-ultra-sonography(SonoVue), which will improve the accuracy of the diagnosis of breast tumor.

Abstract:Breast cancer is the most common female cancer, and the second cause of cancer-related mortality of women in our society. Mammography is the gold-standard method of breast imaging. However it is not an optimal screening tool, especially in cases of dense breast parenchyma. Even when optimally performed, its sensitivity ranges between 69 and 90%. Ultrasound represents an additional diagnostic tool that raises the detection rate of benign and malignant breast lesions. It is the method of choice for differentiating solid from cystic lesions, for further characterizing mammographic findings and better appreciating palpable breast lesions. B-mode ultrasonography is used in every day practice. Harmonic imaging and compound imaging can be used to ameliorate the image contrast and resolution. Colour Doppler is used for studying lesion vascularization however there is no consensus as to whether it really permits to differentiate malignancies from benign lesions. New technical developments such as breast elastography, 3D ultrasound and dedicated ultrasound computed aided diagnosis (CAD) are promising methods for the future.

Abstract:We evaluated the clinical significance of the combined use of mammography + ultrasound as a new breast screening strategy. The inclusion criteria were: (1) females aged > 40yrs; (2) breast cancer diagnosis by the breast image screening personnel at FUSCC; (3) screening by both ultrasound and mammography; (4) mammographic/sonographic images analyzed independently by different radiologists; and (5) follow-up for at least 1 year. Fifty-four women were enrolled. The analysis included diagnostic sensitivity of mammography, ultrasound, and mammography + ultrasound. The sensitivities of mammography and mammography + ultrasound were compared overall as well as among different age groups/breast densities. The screening sensitivity of mammography, ultrasound, and mammography + ultrasound was 79.6, 57.4, and 92.6 %, respectively. The difference between mammography and mammography + ultrasound was significant (P < 0.05). The benefit of including ultrasound with mammography as a new breast image screening strategy was found statistically significant in patients with dense breast on mammogram while it was non-significant in younger (< 50 years) women. We, therefore, concluded that mammography + ultrasound increased the diagnostic sensitivity of breast screening; hence it was more desirable for women with dense breast on mammography.

Abstract:The mammary gland is an important human organ that secretes milk and feeds offspring, while breast tumors are benign or malignant tumors that occur in the breast tissue. There are many causes of breast cancer, and the incidence continues to rise, which is an important killer that threatens women's health. In recent years, a large number of researchers have been interested in the study of AI diagnosis of breast cancer. Artificial intelligence uses a specific algorithm to intelligently process ultrasound images, and develops a high-precision and high-efficiency breast cancer recognition model through training and optimization of the algorithm. At present, the application of computer-aided detection methods in breast cancer ultrasound has been gradually promoted, and the combined application of artificial intelligence has played an advantageous role in the field of breast disease ultrasound diagnosis, such as shortening the examination time, effectively improving the detection rate and diagnostic accuracy rate. The main reasons are: the accuracy of traditional AI diagnosis model of breast cancer based on machine learning is not high; The aim of this paper is to improve the accuracy of early diagnosis of breast diseases effectively and reduce the misdiagnosis rate caused by overwork of doctors on the basis of clear medical images and computer-aided diagnosis technology.

Abstract:In recent years, automated volume scanner system (AVSS) ultrasonography is widely applied in superifcial tissue diseases besides breast diseases, which is designed for diagnosis of breast diseases originally. The characteristic images in the coronal plane of AVSS offer more details for the diagnosis of superifcial tissue diseases including blood vessels, nerves, superifcial soft tissue masses, hernia and so on. Here, the imaging characteristics, research progress and limitation of AVSS are reviewed.

Abstract:1. Ultrasound radiomics is in the preliminary research stage to predict the prognosis of breast cancer. 2. Multimodal prediction models that combine data from patients' clinical data, pathological results, gene expression, multiple images, and other data are the future research directions. 3. High‐quality studies can advance the clinical application of radiomics in the accurate diagnosis and treatment of breast cancer. Breast cancer is the most common malignant tumor and the leading cause of cancer‐related deaths in women worldwide. Effective means of predicting the prognosis of breast cancer are very helpful in guiding treatment and improving patients' survival. Features extracted by radiomics reflect the genetic and molecular characteristics of a tumor and are related to its biological behavior and the patient's prognosis. Thus, radiomics provides a new approach to noninvasive assessment of breast cancer prognosis. Ultrasound is one of the commonest clinical means of examining breast cancer. In recent years, some results of research into ultrasound radiomics for diagnosing breast cancer, predicting lymph node status, treatment response, recurrence and survival times, and other aspects, have been published. In this article, we review the current research status and technical challenges of ultrasound radiomics for predicting breast cancer prognosis. We aim to provide a reference for radiomics researchers, promote the development of ultrasound radiomics, and advance its clinical application.

Ultrasound of the Breast, Including Interventions: An Update

Abstract:The quality of breast ultrasound (US) has constantly improved over recent decades. New transducer technology using high frequencies between 7 and 18 MHz are the physical base for a high spatial resolution. Advances in imaging techniques eliminate speckles and artefacts and improve contrast resolution. International standards are in place for examination technique, description of findings and assessment categories. Future automated volume scanning promises to reduce the problems of operator dependence. US is the breast imaging method of choice for most patients with clinical signs and symptoms. The sensitivity of standard US for breast cancer ranges from 55% to 95%. It transfers an additional diagnostic yield of 30–40% in comparison with mammography to patients with radiodense breasts. US improves detection and characterisation of lesions in preoperative staging and in patients under surveillance after breast cancer. US is inferior to mammography and magnetic resonance imaging (MRI) in the primary detection of ductal carcinoma in situ (DCIS) and does not add information compared with MRI in familiar breast cancer. To date, US has no role in population- based screening due to a lack of prospective and randomised data. US-guided core needle biopsy is the minimally invasive biopsy method of choice for all breast lesions that correlate with findings of other imaging modalities. Sensitivity of US-guided large-core needle biopsy is 93–98%; specificity ranges from 95% to 100%.
Towards a more sophisticated use of breast ultrasound

Abstract:The diagnostic role of breast ultrasound has been expanded along with the improvement of high-frequency transducers and digital technology. Vascular assessment has progressed enough to depict normal vascular anatomy of the breast and the lymph nodes. Pathologic vessels are seen in almost all the tumors, thus improving US sensitivity for nonpalpable carcinomas. New contrast agents will recirculate enough to search for vascular foci during a thorough investigation of both breasts and nodal stations. The US role in screening might be now revised. Many factors are now in favor of targeted US screening in dense and complex breasts and in high-risk patients. Screening sensitivity is significantly increased. Most of these US-detected tumors are small enough to be curable. Mammography and sonography together are a unique problem-solving and cost-effective tool. They can easily guide fine aspirations or larger biopsies reducing the cost of unnecessary surgical procedures. Accurate US investigations facilitate the surgical approach to a very conservative and cosmetic operation. High-resolution sonography can demonstrate the intraductal spread of tumors and their multiple foci more easily than mammography, but US diagnosis is less sensitive than magnetic resonance mammography in the evaluation of the real tumoral extent. Ductal branching has a complex pattern; therefore, intraductal spread and multifocal nodes are better demonstrated by multiplanar analysis of 3D ultrasound data volumes. Sonography can easily explore the different nodal chains. Metastatic disease is indicated by an enlarged and round shape and the absence of the echogenic hilum. Irregularities in the cortex are a very useful sign in metastatic nodes without total replacement of lymphoid tissue by neoplastic cells. These signs are very specific. A time-consuming, radiation-emitting and costly sentinel biopsy may be avoided in one of every five clinically node-negative patients. But preoperative US assessment is also important as sonography is very sensitive in patients with extensive nodal involvement that might result negative at the sentinel node procedure. New technologies and contrast agents allow perfusional studies that enhance the contrast resolution and will increase the sensitivity of US for small nodal metastases.

Abstract:Technological advances are opening new fields of investigation for breast ultrasound. Specificity and color Doppler, staging and extended clinical roles in benign conditions are reviewed according to the state-of-the-art technology. Annular arrays are the best technical choice but new broadband linear arrays approach their quality and allow to visualize very slow flows within the breast. Still limitations exist and ultrasound is not suitable for screening. Providing better definition of normal as well as abnormal features, high-resolution sonography improves the specificity of the diagnosis for the majority of nodules and allows a better definition of both local and regional staging in nodular and diffuse conditions. Color Doppler and contrast media are increasing ultrasound specificity and are particularly useful in evaluating vascularity during therapies. Impressive results are achieved in the study of multifocal and multicentric carcinomas, in determining the degree of tumoral invasion of the surrounding tissues and of the ducts. Local and regional staging are greatly improved; the information given to the surgeon is more precise and allows easier therapeutic decisions. The diagnostic role of high-frequency ultrasound is also convincing in most benign conditions like inflammations, traumas and duct ectasia. In most of these conditions sonography is the best imaging modality to study the disease type and extent. The ability to monitor treatment efficacy or the natural outcome makes ultrasound and color Doppler a gold standard for breast inflammations.

Abstract:Objective:To investigate the value of ultrasonography and fine needle aspiration biopsy(FNAB) in the diagnosis of breast cancer.Methods: Total of 128 patients with palpable breast mass(< 2 cm in diameter) were examined by ultrasonography and FNAB respectively,and the results were compared with the result of pathology.Results: Among the 128 patients,48 were confirmed to be breast cancer by pathology.The sensitivity of ultrasonography in diagnosis of cancer was 61.8%(21/34)and specificity was 80.8%(59/73).The sensitivity of FNAB in diagnosis of cancer was 95.6%(43/45)and specificity was 87.1%(27/31).Though there was no significant difference in specificity between ultrasonography and FNAB(P> 0.05),the sensitivity of FNAB in diagnosis of cancer was higher than ultrasonography(P<0.05).Conclusion: FNAB can give a quite reliable diagnostic value in breast cancer.Fine needle aspiration cytology is a useful and convenient diagnosis method for breast cancer,which also can supply reliable foudation for operation and other treatments.

Abstract:Objective To investigate the diagnostic value of automated breast volume scanner in benign and malignant breast lesions of different types of ultrasound.Methods Data from 196 breast lesions confirmed pathologically were analyzed.Every patient accepted examination by ABVS.According to the proportion of the breasts,they were divided into four types:glandular type,glandular fibro type Ⅰ,glandular fibro type Ⅱ and fatty type.Based on pathological results,the ROC curve was drawn to analyze the diagnostic value of ABVS for four types of breast lesions,and the diagnostic performance of ABVS for lesions of four types was compared.Results Area under ROC curve of ABVS in differentiating benign and malignant lesions of four ultrasound types were 0.950 (glandular type),0.983 (glandular fibro type Ⅰ),o.907 (glandular fibro type Ⅱ) and 0.941 (fatty type) respectively.Comparing the area under the curve,there was no significant difference between the diagnostic performance of ABVS for lesions of four types(P＞0.05).Conclusions ABVS has a high diagnostic value in differentiating benign and malignant lesions of different breast types of ultrasound.
Value of Combined Application of Ultrasonic Elastography and Conventional Ultrasonography in the Differen tial Diagnosis of Breast Lesions

Abstract:Objective:To explore the value of combined application of ultrasonic elastography and conventional ultrasonography in the differential diagnosis of breast lesions.Methods:A total of 330 breast lesions in270 patients proved by surgical-pathology from Jan 2009 to Dec 2010 were examined with conventional ultrasonography and ultrasonic elastography.The sonograms and elastograms were analyzed.The results were compared with pathology.Results:The sensitivities of conventional ultrasonography,ultrasonic elastography and combined application of the two techniques in the differential diagnosis of malignant breast lesions were 86.3%(101/117),70.9%(83/117)and94.0%(110/117),respectively;the corresponding specificities were 92.0%(196/213),89.2%(190/213)and 94.8%(202/213),respectively;the corresponding accuracies were 90.0%(297/330),82.7%(273/330)and 94.5%(312/330),respectively.Combined application of the two techniques could significantly improve the sensitivity,specificity and accuracy,compared with conventional ultrasonography and ultrasonic elastography.Conclusions:Combined application of conventional ultrasonography and ultrasonic elastography can improve differential diagnostic ability in breast lesions.
