Abstract:Objectives Adenomyosis is a common and refractory disease in gynecology. Preserving the uterus during treatment for adenomyosis remains a problem. High-intensity focused ultrasound (HIFU) is widely used in treatment of solid tumors. This study aimed to analyze patients with adenomyosis who were treated by HIFU and to preliminarily examine the characteristics of patients who are more suitable for HIFU to treat adenomyosis with reliable efficacy. Methods Over 2 years, 67 women who were diagnosed with adenomyosis and treated with HIFU at our gynecology department were included in this study. We investigated outcomes of their symptoms (dysmenorrhea and hypermenorrhea) and the volume of their uterine lesions. We also compared the patients’ clinical profiles. Results The women had a mean follow-up duration of 11.6 ± 0.46 months. In the numerical rating scale, used to assess the degree of dysmenorrhea, the score was significantly lower (mean difference: −1.94, 95% confidence interval: −2.704 to −1.176) 3 months after HIFU treatment compared with before treatment, then it remained stable for 3 to 12 months. Hypermenorrhea was reduced to a certain degree, with a mean difference of −0.54 (−1.01–0.02). Conclusions HIFU is a new noninvasive treatment method for adenomyosis that may help relieve dysmenorrhea.

Abstract:Refractory infertility is a relatively intractable issue regarding its treatment at present,In recent years,acupuncture-moxibustion therapy participate increasingly in treatment of refractory infertility and has achieved a certain therapeutic effect.It is discovered that acupuncture-moxibustion effectively improves ovarian function,promotes ovulation and improves ovum quality and endomentrial receptivity.Hence,this therapy has been widely applied to the treatment of infertility induced by primary ovarian insufficiency (POI),polycystic ovarian syndrome (PCOS),endometriosis (EM),intrauterine adhesion (IUA),etc..In the paper,the research status of this disease treated with acupuncture-moxibustion is reviewed.It is anticipated to determine the direction and lay the foundation for further research.
Effect of individualized treatment with sexual behavior instruction on hemospermia (report of 89 cases)

Abstract:Objective To investigate the effect of individualized treatment with sexual behavior instruction on nontumorous hemospermia. Methods From Feb of 2005 to Apr of 2008, 89 patients with nontumorous hemospermia were divided randomly into conventional therapy group (32 cases) and individualized treatment group (57 cases). In conventional therapy groups, the patients were treated with antibiotics, carbazochrome and vitamin C. Patients in individualized treatment groups were further subdivided into infection group (39 cases) and non-infection group (18 cases) based on the results of routine analysis and bacterial culture of prostatic fluid/semen. Then patients in individualized treatment group received sexual behavior instruction. The effect of treatment was followed up. Results The recovery rate and recurrence rate were 95.7% and 3.5% in individualized treatment groups, and 60.9% and 9.4% in conventional therapy groups, respectively. A significant difference in recovery rate or recurrence rate was found between individualized treatment group and conventional therapy group (P<0.05). Conclusion Individualized treatment with sexual behavior instruction showed great advantage in treatment of nontumorous hemospermia and promising value in clinic practice.

Abstract:Objective: To describe the aetiology and efficacy of the treatment of persistent and recurrent hemospermia as a novel technique of transurethral seminal vesiculoscopy. Methods: The clinical data of 290 patients with persistent and recurrent hemospermia (a course of hemospermia persisting more than 6 months) from multiple medical center of South China were analyzed retrospectively. The age ranged from 16 to 69 years (mean 31 years). The definite etiologies of persistent and recurrent hemospermia were confirmed by physical examination including digital rectal examination (DRE), tailored investigations such as blood PSA and clotting time, transrectal ultrasound (TRUS), CT or MRI. Of those with persistent and recurrent hemospermia, 269 patients were successfully performed by transurethral seminal vesiculoscopy since July 2008. The aetiology of the other 21 cases with persistent hemospermia were attributable to cystadenoma of the seminal vesicle [1], tuberculosis of tractus genitalis [1], bleeding risk secondary to liver cirrhosis [3], benign prostatic hyperplasia [12] and prostate cancer [4] confirmed by urogenital instrumentation or prostate biopsy. Results: All the patients with persistent and recurrent hemospermia were confirmed by transurethral seminal vesiculoscopy (162 seminal vesiculitis and 108 seminal stone secondary to them, 91 ejaculatory ducts obstruction incompletely, 42 Mullerian cyst, 16 cysts of seminal vesicle, 3 cysts of ejaculatory duct and 12 benign prostatic hyperplasia). The mean operative time was 21 min (range, 5–90 min). There were no complications including injury of urethra and seminal vesicle and postoperative discomforts in the perineal region. The mean follow-up period was 24 months (range, 3–72 months). In those 269 cases, 11 patients were out of follow-up. Hematospermia in 235 cases disappeared and 23 patients respectively recurred in 5 to 60 months after receiving transurethral seminal vesiculoscopy. Of those 23 cases with postoperatively recurrent hemospermia, 12 patients were cured by re-transurethral seminal vesiculoscopy. Conclusions: The aetiologies of persistent and recurrent hemospermia are mostly associated with seminal vesiculitis and seminal stone secondary to vesiculitis or ejaculatory ducts obstruction incompletely. Transurethral seminal vesiculoscopy could be an effective diagnostic and therapeutic procedure for it.
The Diagnosis and Treatment of Etiology of Hemospermia

Abstract:Purpose:To investigate the diagnosis and treatment for etiology of hemospermia.Methods:42 patients with hemospermia were treated between April 1988 and January 2001. Of them 41 cases underwent transperineal bilateral seminal veseculography under ultrasound guidance, aspirated seminal vesicle fluid and other examinations. 1 case underwent digital rectal examination, CT and transrectal ultrasound. Many kinds of methods were adopted for treatment of etiology of hemospermia.Results:The etiology of hemospermia was determined in all patients. Of them the etiology was prostatic carcinoma in 1, seminal vesicle carcinoma in 1, posturethritis in 1, polyps of verumontanum in 4, seminal vesiculitis in 18 and prostatovesiculitis in 17. Radical prostate and seminal vesiculectomy was performed in 2 patients. Continuous transcatheter antibiotic drugs instillation therapy was adopted for 35 patients with urogenital inflammation and cure rate was 91.4 %. Corresponding treatments were performed in 5 patients with polyps of verumontanum and posturethritis.Conclusions:Hemospermia is mainly caused of lower urogenital infections, especially with prostatovesiculitis. Transperineal seminal vesicle puncture under ultrasound guidance and continuous transcatheter antibiotic drugs instillation was an effective method for diagnosis and treatment of hemospermia owing to urogenital inflammation.

Abstract:Objectives To describe the aetiology and therapeutic procedures for persistent and recurrent hemospermia as a novel technique of transurethral seminal vesiculoscopy. Methods The clinical data of 25 patients with a course of 0.5 to 17 years’ (mean 2.5 years) hemospermia in our single center were analyzed retrospectively and literatures were reviewed. The age ranged from 16 to 67 years (mean 38 ys). All the patients with hemospermia were not cured by medical treatment or any other physical therapy. Of those with persistent and recurrent hemospermia, 21 patients were successfully treated by transurethral seminal vesiculoscopy since 2008. The aetiology of the other 4 cases with persistent hemospermia were attributable to tuberculosis of seminal vesicle (1), bleeding risk secondary to liver cirrhosis (1) and prostate cancer (2) by urogenital instrumentation or prostate biopsy. The ejaculatory duct and seminal vesicle were conducted to observe under direct vision through the distal seminal tracts using a 4.5/6.5 F rigid ureteroscope. Results Twenty-one patients with persistent and recurrent hemospermia were confirmed by transurethral seminal vesiculoscopy (12 seminal vesiculitis and 9 seminal stone secondary to them, 4 ejaculatory duct dilation, 3 cysts of seminal vesicle and 2 ejaculatory ducts obstruction). The mean operative time was 40 min (15-120 min). There was no complications such as injury of urethra and seminal vesicle and postoperative discomforts in the perineal region. The mean follow-up period was 15 months (range 3-42 months). Hematospermia in 16 cases disappeared and 2 patients respectively recurred in 5 and 9 months after receiving transurethral seminal vesiculoscopy. Another one patient with recurrent hemospermia was cured by vesiculoscopy and transurethral resection of seminal vesicle cyst. Conclusions The aetiologies of persistent and recurrent hemospermia are mostly associated with seminal vesiculitis and seminal stone secondary to vesiculitis or incomplete ejaculatory ducts obstruction. Transurethral seminal vesiculoscopy with little invasion could be an effective diagnostic and therapeutic procedure for it.
Transurethral Seminal Vesiculoscopy in the Diagnosis and Treatment of Persistent or Recurrent Hemospermia: a Single-Institution Experience.

Abstract:The diagnosis and treatment of hemospermia presents significant difficulty, especially if it persists or recurs. In this retrospective study, we assessed whether transurethral seminal vesiculoscopy is feasible and effective in the diagnosis and treatment of hemospermia. To address this complex condition, we report our experience in a population of patients treated with transurethral seminal vesiculoscopy. From February 2006 to July 2008, 72 hemospermic patients underwent transurethral seminal vesiculoscopy examination and treatment at our urology center. Transurethral seminal vesiculoscopy was performed by a 7-F or 8-F rigid ureteroscope. The endoscopic procedure was conducted through the normal anatomic route of the seminal tracts. In this series, the mean follow-up period was 21.7 months. Definite diagnosis was made for 93.1% patients, and 94.4% patients were cured or showed a decrease in their symptoms. Postoperative complications were not observed in the study. Our study proves that transurethral seminal vesiculoscopy is effective in the diagnosis and treatment of hemospermia with minimal complication.
Etiology and Management of Persistent Hematospermia: a Pilot Study

Abstract:To study the etiology of persistent hematospermia and to evaluate the efficacy of transrectal ultrasonography (TRUS)-guided transperineal needle aspiration and irrigation for diagnosis and treatment of persistent hematospermia.Twelve patients were included in the study, with a mean age of (36.4 +/- 10.8) years old, and a mean duration of the disease of (13.9 +/- 6.4) months. After the expressed prostatic secretion (EPS) by prostatic massage was cultured, patients with recurrent hematospermia received TRUS-guided transperineal needle aspiration for seminal vesicle fluid (SVF), which was sent for bacteriological and cytological examination. If the EPS culture were positive, certain antibiotics according to the drug sensitivity assay were injected into the abnormal seminal vesicle(s) via TRUS-guided transperineal needle puncture. The treatment would be repeated one month later if the patients still had hematospermia. The patients were followed up every three months.Abnormal images were found in left seminal vesicle (SV) in 4 cases, right in 3 cases, bilateral in 2 cases, and no abnormal findings in 3 cases. The abnormal findings included: 7 cases of SV and/or ejaculatory duct dilation, 3 cases of thickening SV wall, 3 cases of calcification or calculi of SV, and 1 case of Müllerian duct cyst. SVF cultures were positive in 7 cases: methicillin-resistant Staphylococcus aureus (MRSA) 4 cases, methicillin-resistant coagulase-negative Staphylococcus (MRCNS), E. Coli, Proteus mirabilis 1 case, respectively. In five of these 7 cases, bacteriological cultures of SVF and EPS showed the same results. All patients were treated by TRUS-guided transperineal injection of certain antibiotics into SV. Seven cases were injected once, 5 cases twice. The mean follow-up period of 10 patients was (16.7 +/- 5.9) months. Hematospermia disappeared in 6 cases.SV infection of bacteria, especially infection of the drug resistant strains was one of the main causes of persistent hematospermia. The difficulties in treatment of persistent hematospermia were due to infection of drug resistant bacteria, calcification or calculi of SV, obstruction of ejaculatory duct. TRUS-guided transperineal aspiration of SVF was helpful to the etiologic diagnosis of persistent hematospermia.

Hematospermia disappeared in 3 cases and relapsed at least once in the other 7. The last follow-up showed hematospermia recurrence in 2 cases.CONCLUSION:TRUS-guided transperineal irrigation of antibiotics into SV is effective to a certain extent for persistent hematospermia.
TRUS-Guided Interventional Therapy on Persistent Hematospermia

Abstract:Objective:To evaluate the value of transperineal seminal vesicle puncture under TRUS guidance and continuous transcatheter antibiotics instillation for the treatment of persistent hematospermia.Methods:Twenty-five patients with persistent hematospermia were included in the study,aged 43.2±21.3 years,and with a course of the disease 23.2±25.3 months.After the expressed prostatic secretion by prostatic massage was cultured,all 25 patients were detected by TRUS and treated by transperineal seminal vesicle puncture under TRUS guidance with continuous transcatheter antibiotics instillation for 7 days.Results:Abnormal images were found in the seminal vesicle and/or ejaculatory duct in 18 cases,including seminal vesicle and/or ejaculatory duct dilation,calculi of seminal vesicle,seminal vesicle wall thickening or calcification and ejaculatory duct cyst.All patients were successfully catheterized,23 patients completed the treatment for one week,two cases had their catheter extubated early.All patients were followed up for 6~24 months.Hematospermia is disappeared in 23 cases(92.0%)and relapsed in the other 2.Conclusions:TRUS-guided Transperineal seminal vesicle puncture and continuous transcatheter antibiotics instillation was an effective method for the treatment of persistent hematospermia.
Clinical research of Compound Xuanju Capsule treating oligoathenospermia due to spleen and kidney Yang deficiency

Abstract:Objective To investigate the efficacy of Compound Xuanju Capsule for treating oligoathenospermia due to spleen and kidney Yang deficiency. Methods Between Jun, 2008 and Mar, 2009, 33 male infertile patients in our andrology department were given Compound Xuanju Capsule for a period of 24 weeks and came for followed-up at the 4th, 8th, 12th, 16th, 20th and 24th weekend for receiving clinical examinations and safety evaluation. Observation items include: semen analysis, sperm morphology analysis, seminal plasma biochemical analysis and serum sex hormone. Results After treatment for 24 weeks. The sperm density, sperm motility, a class and a+b class sperm percentage increased significantly when (P<0.05). Compared with those before treatment, 2 patients' partners conceived during treating period, the total effective rates was 53.34%.Fructose density and α-glycosidase also increased, but no statistical significance. Serum sex hormone of two groups did not vary before and after treatment(P>0.05). Conclusion Compound Xuanju Capsule for treating oligoathenospermia due to spleen and kidney Yang deficiency is effective.
Seminal Vesiculoscopy in the Treatment of Refractory Hemospermia and Ejaculatory Duct Obstruction

Abstract:Objective:To summarize the experience in the diagnosis and treatment of refractory hematospermia and ejaculatory duct obstruction by seminal vesiculoscopy.METHODS:We retrospectively analyzed the clinical data about 42 cases of refractory hematospermia and 6 cases of ejaculatory duct obstruction with azoospermia. We investigated the diagnosis, treatment, and prognosis of the diseases.RESULTS:All the patients underwent pelvic MRI and seminal vesiculoscopy. MRI for the 42 refractory hematospermia patients showed that 21 (50.0%) had cystic dilatation in the uni- or bilateral seminal vesicles, 25 (59.5%) had abnormal internal signal intensity in the uni- or bilateral seminal vesicles, 12 (28.6%) had both the problems above, and 4 (9.52%) had no obvious abnormality in the seminal vesicle area. The bilateral seminal vesicles were <1 cm in width in 3 of the 6 cases of ejaculatory duct obstruction, and obviously enlarged in the other 3, but without abnormal internal signals. No recurrence was found during the 3－36 months follow-up.CONCLUSIONS:The history and physical examination play important roles in the diagnosis of refractory hemospermia, and MRI is more valuable than TRUS in the diagnosis of seminal vesicle diseases. Seminal vesiculoscopy is an effective option for the management of persistent hematospermia and ejaculatory duct obstruction.

Abstract:Objective To investigate the efficacy of huangmai mixture treating oligoathenospermia due to spleen and kidney yang deficiency.Methods Sixty-six male infertility patients were randomized into experimental group and control group,given with huangmai mixture and compound xuanju capsule,respectively.All patients recruited went through a treating period of 24 weeks and came for follow-up visits at the 4th,8th,12th,16th,20th and 24th weekend,receiving clinical examinations and safety evaluation.Observation items included semen analysis,sperm morphology analysis,antisperm antibody,seminal plasma biochemical analysis and serum sex hormone.Results After treatment for 24 weeks,the total effective rates of experimental group and control group were 63.33% and 53.33%,respectively.The total effective rates between groups were not statistically significant(P0.05).In total,6 patients' partners conceived during treating period(4 in experimental group and 2 in control group).Compared with those before treatment,the sperm density,sperm motility,a class sperm percentage,a+b class sperm percentage and sperm activity in the treated group and control group increased significantly(P0.05).Fructose density and α-glycosidase also increased,but the increasing degree showed no statistical significance.Serum sex hormone of two groups did not vary before and after treatment(P0.05).Statistical significance was not observed in percentage increased between groups(P0.01).Conclusion Huangmai mixture for treating oligoathenospermia due to spleen and kidney yang deficiency has the same efficacy and safety compared with compound xuanju capsule,with a relatively low cost,huangmai mixture is more suitable for clinical application.

Abstract:OBJECTIVE To observe clinical therapeutic effects of dissolving phlegm and nourishing yin treatment on human semen hyperviscosity(SHV) and to explore its mechanism and acting targets.METHODS 88 SHV patients were randomly divided into treatment group(G1),in which patients were given Chinese medicine of dissolving phlegm and nourishing yin based on syndrome differentiation,and control group(G2),in which patients were administrated by vitamin C,E and Zinc-Selenium Tablets for 3 courses.4 weeks was 1 course.Besides,CASA was adopted to analyze semen's quality and detect prostate specific antigen in seminal plasma,three prostate indexes(zinc,acid phosphatase and citric acid) before and after treatment.RESULTS The indicators including hyperviscosity,quality and prostate specific antigen in seminal plasma of two groups after treatment improved(P0.05),and G1 was superior to G2(P0.05).CONCLUSION TCM treatment of dissolving phlegm and nourishing yin is of great clinical effects,whose mechanism and targets may lie in its regulation of the disordered secreting function of prostate gland.

Abstract:Introduction Hematospermia can be a challenging medical condition to treat for the urologist. While almost universally benign, it causes considerable patient stress and anxiety. There are no standardized treatments and while many cases may resolve on their own the duration can be quite variable. Tranexamic acid (TXA), an antifibrinolytic, has been used to control bleeding in various settings including heavy menstrual bleeding, traumas, and intraoperatively to prevent and control bleeding. Objective We sought to investigate the safety and efficacy of tranexamic acid, an oral anti-fibrinolytic for the treatment of idiopathic hematospermia. Methods A cohort of patients presenting to urology clinic within the past year for primary complaint of hematospermia without other clear etiology (trauma, radiation, prior surgery, prostatitis) were prescribed TXA 1300 mg BID for 7 days. Patients were followed as part of routine care and resolution of hematospermia and side effects were observed and documented. Results Eight patients (median age 42.1 years) met criteria and were treated with the TXA protocol for idiopathic hematospermia. All had immediate resolution at completion of the 7-day course. Two patients had a recurrent episode within follow-up period (Median follow-up of 5.4 months) which resolved with a second treatment course. No adverse events were noted, and all patients were able to tolerate TXA treatment. Conclusions TXA appears to be a safe and efficacious treatment for idiopathic hematospermia where often the main goal is to rule out other etiologies and expeditious resolution to relieve patients' stress and anxiety. Transrectal Ultrasound-Guided Seminal Vesicle Catheterization with Continuous Antibiotic Infusion for the Treatment of Refractory Hematospermia

Abstract:The aim of the present study was to describe transrectal ultrasound (TRUS)-guided seminal vesicle catheterizations with continuous antibiotic infusion in patients with persistent hematospermia. A retrospective record review of 45 patients with refractory hematospermia treated with TRUS-guided seminal vesicle catheterization between 2010 and 2017 was performed. Seminal vesicle puncture and catheterization was performed under TRUS guidance for all patients. Antibiotic irrigation was used to rinse the seminal vesicle until the outflow fluid was clear. The trocar sleeve was left in situ and fixed on the skin of the perineum at the end of the procedure. All patients underwent a 24-h continuous infusion of antibiotic solution through the catheter. The patients were followed up to 3 years for the presence of hematospermia. The duration of refractory hematospermia was between 6 months and 9 years. A total of 14 patients exhibited consecutive hematospermia, while the remaining patients exhibited intermittent episodes. On TRUS, 15 cases of ejaculatory duct cyst, 7 cases of ejaculatory duct expansion, 3 cases of ejaculatory duct stones, 6 cases of seminal vesicle expansion, 8 cases of seminal vesicle stones and 5 cases of seminal vesicle wall or ejaculation wall calcification were diagnosed. A total of 41 patients completed the scheduled treatment plan; however, the catheter was dissociated on the 3rd or 4th day of catheterization in 4 patients. After a 1-3 year follow-up, hematospermia was not observed in 42 patients (93.33%) with recurrence in the remaining 3 patients. In conclusion, TRUS-guided seminal vesicle catheterization with continuous antibiotic infusion appeared to be a safe and effective method for the treatment of hematospermia.

Abstract:Background and Aims: Cryptozoospermia is an extreme oligozoospermia with an unsatisfactory treatment effect, with an incidence rate of approximately 8.73% in male infertility, whose effective solution has become the call of the times. Western Medicine has achieved certain effects through drugs, surgery, and assisted reproductive therapy, but this is still not ideal. Traditional Chinese medicine (TCM) has made many achievements in other disciplines; however, there is still a lack of evidence-based medical evidence to improve sperm production.Methods: The relevant literatures from the China National Knowledge Internet (CNKI) and PubMed in the past 10 years were collected in this article, of which the mechanisms, advantages, or current controversies of various treatment methods of Western Medicine and TCM were analyzed, to find new treatment methods and research directions.Results: With the development of modern science and technology, medical treatments for cryptozoospermia have become increasingly abundant; however, there is still no universally recognized unified and effective guiding plan. Although TCM has not been fully verified by evidence-based medicine, most TCM combined with Western Medicine can achieve unexpected results.Conclusion: The combination of TCM and Western Medicine may become a bane for cryptozoospermia and bring good news to infertile men worldwide.
Seminal Plasma Mir-192A: a Biomarker Predicting Successful Resolution of Nonobstructive Azoospermia Following Varicocele Repair

Abstract:This study was performed to investigate a potential marker for the presence of spermatozoa in the ejaculate following varicocelectomy in Chinese men with nonobstructive azoospermia and varicoceles. The micro-RNA (miR)-192a levels in seminal plasma and testicular tissue were evaluated by quantitative real-time polymerase chain reaction from 60 men with nonobstructive azoospermia and varicoceles (Group A: 27 men with spermatozoa found in the ejaculate after surgery; Group B: 33 men without spermatozoa found in the ejaculate after surgery) and 30 controls. The seminal plasma and testicular tissue miR-192a levels were higher in Group B than in Group A and the controls (P < 0.001), and there was no significant difference between Group A and the controls (P > 0.05). Apoptosis and proliferation assays with miR mimics and inhibitors showed that miR-192a induced GC-2 cell apoptosis through the activation of Caspase-3 protein. Thus, seminal plasma miR-192a appears to be a potential marker for successfully indicating spermatozoa in the ejaculate following microsurgical varicocelectomy in men with nonobstructive azoospermia and varicoceles. Seminal plasma miR-192a may be a useful clinical marker for prescreening to determine which patients with nonobstructive azoospermia and varicoceles would benefit from varicocelectomy.
