Abstract:Although immunotherapy in combination with anti-angiogenesis therapy has made a breakthrough in the first-line treatment of cancer, considering the low responder rate and the adverse events, it is vital to propose a new combination modality. In this study, we report single encapsulated mesoporous silica coated gold nanoparticles that synergize sensitizing radiotherapy with the current combination therapy. Distinguished from simply combining two treatments, the nanoparticle-mediated "trident" therapy resolved the problem of matching the dose between radiation and drug, which determines the outcome since drug demand rises with immunosuppression from increased sensitivity to radiotherapy. The nanomedicine produced energy depositions when radiation was introduced, and released the loaded toripalimab and bevacizumab, exhibiting significant anti-tumor properties. In vitro tumor cell viability results indicated the highest inhibition by the "trident" therapy and in vivo animal models also revealed the earliest decrease in tumor tissue volume. As a result, the "trident" therapy is expected to further improve the anti-tumor benefits of the combination of immunotherapy and anti-angiogenesis therapy and provides a versatile perspective on cancer treatment.

Abstract:Increasing evidence implicates the tumor microbiota as a factor that can influence cancer progression. In patients with colorectal cancer (CRC), we found that pre-resection antibiotics targeting anaerobic bacteria substantially improved disease-free survival by 25.5%. For mouse studies, we designed an antibiotic silver-tinidazole complex encapsulated in liposomes (LipoAgTNZ) to eliminate tumor-associated bacteria in the primary tumor and liver metastases without causing gut microbiome dysbiosis. Mouse CRC models colonized by tumor-promoting bacteria ( Fusobacterium nucleatum spp.) or probiotics ( Escherichia coli Nissle spp.) responded to LipoAgTNZ therapy, which enabled more than 70% long-term survival in two F. nucleatum -infected CRC models. The antibiotic treatment generated microbial neoantigens that elicited anti-tumor CD8 + T cells. Heterologous and homologous bacterial epitopes contributed to the immunogenicity, priming T cells to recognize both infected and uninfected tumors. Our strategy targets tumor-associated bacteria to elicit anti-tumoral immunity, paving the way for microbiome–immunotherapy interventions.

Abstract:Bacteria-based immunotherapy has become a promising strategy to induce innate and adaptive responses for fighting cancer. The advantages of bacteriolytic tumor therapy mainly lie in stimulation of innate immunity and colonization of some bacteria targeting the tumor microenvironment (TME). These bacteria have cytotoxic proteins and immune modulating factors that can effectively restrain tumor growth. However, cancer is a multifactorial disease and single therapy is typically unable to eradicate tumors. Rapid progress has been made in combining bacteria with nanotechnology. Using the nanomolecular properties of bacterial products for tumor treatment preserves many features from the original bacteria while providing some unique advantages. Nano-bacterial therapy can enhance permeability and retention of drugs, increase the tolerability of the targeted drugs, promote the release of immune cell mediators, and induce immunogenic cell death pathways. In addition, combining nano-bacterial mediated antitumor therapeutic systems with modern therapy is an effective strategy for overcoming existing barriers in antitumor treatment and can achieve satisfactory therapeutic efficacy. Overall, exploring the immune antitumor characteristics of adjuvant clinical treatment with bacteria can provide potential efficacious treatment strategies for combatting cancer.

Abstract:Induction of immunogenic cell death (ICD) by hyperthermia can initiate adaptive immune responses, emerging as an attractive strategy for tumor immunotherapy. However, ICD can induce proinflammatory factor interferon-γ (IFN-γ) production, leading to indoleamine 2,3-dioxygenase 1 (IDO-1) activation and immunosuppressive tumor microenvironment, which dramatically reduces the ICD-triggered immunotherapeutic efficacy. Herein, we developed a bacteria-nanomaterial hybrid system (CuSVNP20009NB) to systematically modulate tumor immune microenvironment and improve tumor immunotherapy. Attenuated Salmonella typhimurium (VNP20009) that can chemotactically migrate to hypoxic area of tumor and repolarize tumor associated macrophages (TAMs) was employed to intracellularly biosynthesize copper sulfide nanomaterials (CuS NMs) and extracellularly hitchhike NLG919-embedded and glutathione (GSH)-responsive albumin nanoparticles (NB NPs), forming CuSVNP20009NB. After intravenous injection into B16F1 tumor-bearing mice, CuSVNP20009NB could accumulate in tumor tissues and repolarize TAMs from immunosuppressive M2 to immunostimulatory M1 phenotype, and release NLG919 from extracellular NB NPs to inhibit IDO-1 activity. Under further near infrared laser irradiation, intracellular CuS NMs of CuSVNP20009NB could photothermally induce ICD including calreticulin (CRT) expression and high mobility group box 1 (HMGB-1) release, promoting intratumoral infiltration of cytotoxic T lymphocytes. Finally, CuSVNP20009NB with excellent biocompatibility could systematically augment immune responses and significantly inhibit tumor growth, holding great promise for tumor therapy.

Abstract:Bacterial therapy, which targets the tumor site and aims at exerting an antitumor immune response, has displayed a great potential against malignant tumors. However, failure of the phase I clinical trial of Salmonella strain VNP20009 alone demonstrates that bacterial treatment alone can unsatisfy the requirements of high efficiency and biosafety. Herein, a strategy of both-in-one hybrid bacteria is proposed, wherein the chemotherapeutic drug doxorubicin (DOX) is integrated onto the surface of glucose dehydrogenase (GDH)-overexpressed non-pathogenic Escherichia coli (E. coli) strain, to potentiate the antitumor efficacy. Nicotinamide adenine dinucleotide phosphate (NADPH), which is produced by GDH from E. coli, promotes the generation of toxic reactive oxygen species (ROS) within the tumor, and ROS is then catalyzed by the DOX-activated NADPH oxidases. Importantly, the hybrid bacteria enhance stimulated systemic antitumor immune responses, thereby leading to effective tumor eradication. When this strategy is applied in four different tumor models, the hybrid bacteria significantly inhibited tumor metastasis, postsurgical regrowth, and primary/distal tumor relapse. The both-in-one ROS-immunity-boosted hybrid bacteria strategy provides knowledge for the rational design of bacteria-based synergistic cancer therapy.

Abstract:As an important component of the tumor microenvironment, the intratumoral bacteria can significantly reduce the chemotherapeutic response, in which Fusobacterium nucleatum has been established as the representative intratumoral bacteria in colorectal cancer, promoting chemotherapy resistance by activating autophagy pathways. Herein, the concept of antibacterial-augmented antitumor nanomedicine for pathogen-related cancer therapy was elaborated by designing antibacterial-antineoplastic nanoagents that modulate intratumoral bacteria to augment anticancer efficiency. A generic coordination-redox strategy was used for homogeneous in situ growth of antibacterial ultrasmall silver nanoparticles into the dendritic mesoporous silica nanoparticles , which were further loaded with chemotherapeutic drugs. The antibacterial-antineoplastic agents exhibited excellent antitumor efficacy by scavenging Fusobacterium nucleatum to modulate autophagy, thereby overcoming chemoresistance to heighten the therapeutic effect. This work proposes a conceptual framework of antibacterial-augmented antitumor nanomedicine for refined cancer-therapeutic efficacy by modulating intratumoral bacteria to enhance therapeutic responses of chemotherapy, opening new horizons on nanomedicine design for more effective tumor therapy.

Engineered bacteria breach tumor physical barriers to enhance radio-immunotherapy

Abstract:Radiotherapy widely applied for local tumor therapy in clinic has been recently reinvigorated by the discovery that radiotherapy could activate systematic antitumor immune response. Nonetheless, the endogenous radio-immune effect is still incapable of radical tumor elimination due to the prevention of immune cell infiltration by the physical barrier in tumor microenvironment (TME). Herein, an engineered Salmonella secreting nattokinase (VNPNKase) is developed to synergistically modulate the physical and immune characteristics of TME to enhance radio-immunotherapy of colon tumors. The facultative anaerobic VNPNKase enriches at the tumor site after systemic administration, continuously secreting abundant NKase to degrade fibronectin, dredge the extracellular matrix (ECM), and inactivate cancer-associated fibroblasts (CAFs). The VNPNKase- dredged TME facilitates the infiltration of CD103+ dendritic cells (DCs) and thus the presentation of tumor-associated antigens (TAAs) after radiotherapy, recruiting sufficient CD8+ T lymphocytes to specifically eradicate localized tumors. Moreover, the pre-treatment of VNPNKase before radiotherapy amplifies the abscopal effect and achieves a long-term immune memory effect, preventing the metastasis and recurrence of tumors. Our research suggests that this strategy using engineered bacteria to breach tumor physical barrier for promoting immune cell infiltration possesses great promise as a translational strategy to enhance the effectiveness of radio-immunotherapy in treating solid tumors.

Abstract:Clinical evidence has indicated that tumor‐colonizing bacteria would be closely related to the tumor development and therapeutic responses. Selectively eliminating bacteria within tumors may be an attractive approach to enhance cancer treatment without additional side effects. Herein, we found that owing to the high affinity between the membrane protein Fap‐2 on Fusobacterium nucleatum (F. nucleatum) and D‐galactose‐β (1‐3)‐N‐acetyl‐D‐galactosamine (Gal‐GalNAc) overexpressed on colorectal tumor cells, F. nucleatum would colonize in colorectal tumors, as evidenced by both clinical samples and animal tumor models. Notably, F. nucleatum colonized in colorectal tumors would lead to immunosuppressive tumor microenvironment, greatly reducing their responses to immune checkpoint blockade (ICB) therapy. Inspired by this finding, we designed a F. nucleatum‐mimetic nanomedicine by fusing F. nucleatum cytoplasmic membrane (FM) with Colistin loaded liposomes to achieve selectively killing tumor‐colonizing F. nucleatum without affecting gut microbes. As the results, the therapeutic responses of F. nucleatum‐colonized tumors to ICB therapies could be successfully restored, as demonstrated in F. nucleatum‐infected subcutaneous CT‐26 tumor model, chemical‐induced spontaneous colorectal cancer models and MC‐38 tumor model. In summary, our work presented a F. nucleatum‐Mimicking nanomedicine which could selectively eliminate tumor‐colonized bacteria, promising for enhancing the responses of cancer immunotherapy against F. nucleatum‐colonized colorectal cancer. Entirely Synthetic Bacterial Nanomimics for Highly-Effective Tumor Suppression and Immune Elicitation

Abstract:Bacteria are considered to be "perfect" anticancer agents on account of their unique capabilities of specific tumor targeting, active intratumoral penetration and controllable cytotoxicity induction, but so far there is no great success in clinical translations due to many challenging issues including intrinsic bacterial toxicity, potential immunogenicity and safety concerns. To address this dilemma, an artificial bacterial nanomimic (BNM) consisted of biocompatible multifunctional peptide dendrimers and a polysaccharide was exploited to imitate the structure and function of bacteria. The resulted BNMs could recognize and bind with the sialic acid (SA) moieties on cell surface, thereby achieving improved cellular uptake and intratumoral penetration. Light activated BNMs caused distinct tumor cell apoptosis and necrosis through greatly up-regulating the intracellular ROS level, evidenced with oxidative damage of lysosomes, cathepsin release and cell death. Meanwhile, both the BNMs and antigens releasing from dying cells significantly promoted the maturation of dendritic cells and secretion of pro-inflammatory cytokines, which further improved the antitumor effect. Encouragingly, light activated BNMs displayed excellent tumor suppression capacity and completely eliminated two tumors, and significantly prolonged the survival time. This study provides a new strategy for engineering therapeutic agents in a bioinspired fashion.

Abstract:Recent discoveries in commensal microbiota demonstrate the great promise of intratumoral bacteria as attractive molecular targets of tumors in improving cancer treatment. However, direct leveraging of in vivo antibacterial strategies such as antibiotics to potentiate cancer therapy often leads to uncertain effectiveness, mainly due to poor selectivity and potential adverse effects. Here, building from the clinical discovery that patients with breast cancer featured rich commensal bacteria, we developed an activatable biointerface by encapsulating commensal bacteria-derived extracellular vesicles (BEV) with a responsive nanocloak to potentiate immunoreactivity against intratumoral bacteria and breast cancer. We show that the interfacially cloaked BEV (cBEV) not only overcame serious systemic side responses but also demonstrated heightened immunogenicity by intercellular responsive immunogenicity, facilitating dendritic cell maturation through activating the cGAS-STING pathway. As a preventive measure, vaccination with nanocloaked cBEVs achieved strong protection against bacterial infection, largely providing prophylactic efficiency against tumor challenges. When treated in conjunction with immune checkpoint inhibitor anti-PD-L1 antibodies, the combined approach elicited a potent tumor-specific immune response, synergistically inhibiting tumor progression and mitigating lung metastases.
Bacteria colonization in tumor microenvironment creates a favorable niche for immunogenic chemotherapy

Abstract:Abstract The tumor microenvironment (TME) presents differential selective pressure (DSP) that favors the growth of cancer cells, and monovalent therapy is often inadequate in reversing the cancer cell dominance in the TME. In this work, we introduce bacteria as a foreign species to the TME and explore combinatorial treatment strategies to alter DSP for tumor eradication. We show that cancer-selective chemotherapeutic agents and fasting can provide a strong selection pressure against tumor growth in the presence of bacteria. Moreover, we show that an immunogenic drug (oxaliplatin), but not a non-immunogenic one (5-FU), synergizes with the bacteria to activate both the innate and adaptive immunity in the TME, resulting in complete tumor remission and a sustained anti-tumor immunological memory in mice. The combination of oxaliplatin and bacteria greatly enhances the co-stimulatory and antigen-presenting molecules on antigen-presenting cells, which in turn bridge the cytotoxic T cells for cancer-cell killing. Our findings indicate that rational combination of bacterial therapy and immunogenic chemotherapy can promote anticancer immunity against the immunosuppressive TME.

Abstract:Abstract Tumor-associated bacteria (TAB) play a critically important role in regulating the microenvironment of a tumor, which consequently greatly deteriorates the therapeutic effects by chemo- and radiotherapy deactivation and, more considerably, leads to substantial immunosuppression. On the contrary, herein we propose a nanocatalytic tumor-immunotherapeutic modality based on the bacteria disintegration by bacteria-specific oxidative damage under magnetic hyperthermia for highly effective immune response activation-promoted tumor regression. A monodispersed and superparamagnetic nanocatalytic medicine modified by arginyl-glycyl-aspartic acid (RGD) and (3-carboxypropyl)triphenylphosphonium bromide (TPP), named as MNP-RGD-TPP herein, has been synthesized, which features selective accumulation at the TAB by the electrostatic affinity, enabling effective TAB disintegration by the nanocatalytic Fenton reaction producing abundant cytotoxic hydroxyl radicals in situ under alternating magnetic field-induced hyperthermia. More importantly, the lipopolysaccharide has been metabolically secreted from the destructed TAB as pathogen-associated molecular patterns (PAMPs) to M1-polarize tumor-associated macrophages (TAMs) and promote the maturation of dendritic cells (DCs) for innate immuno-response activation of TAMs, followed by cytotoxic T lymphocytes awakening under the PAMPs presentation by the mature DCs against tumor cells. The integrated innate and adaptive immunity activations based on this TAB-promoted nanocatalytic immunomedicine, instead of magnetic heating-induced hyperthermia or the released Fe2+/Fe3+ Fenton agent, has been found to achieve excellent therapeutic efficacy in an orthotopic colorectal cancer model, demonstrating the great potential of such an integrated immunity strategy in clinical tumor immunotherapy.

Abstract:Rationale: Gut microbiota plays a crucial role in cancer development and treatment. Studies show that although the gut microbiota is able to promote tumor growth, its presence also improves the efficacy of cancer treatment such as immunotherapy. To date, understanding of the potential impact of the gut microbiota on other treatment modalities such as cancer nanomedicine is still limited. In this study, we aimed to establish the relationship between gut microbiota and cancer nanomedicine, which can potentially open a new path in cancer treatment that combines gut microbiota modulation along with nanotherapeutics. Methods: Mice bearing 4T1 triple-negative breast cancer cells were subjected to gut microbiota modulation by antibiotics (ABX) treatment in the drinking water. Mice given normal water was used for control. The effects of ABX treatment towards gut bacteria was studied by RT-qPCR and 16S next generation sequencing of fecal samples. The mice were then subjected to liposomal doxorubicin (LipoDox) treatment and the amount of nanotherapeutics that accumulated in the tumors was quantified. For therapeutic efficacy, the mice were subjected to ABX treatment and given three injections of LipoDox or saline, while the tumor growth was monitored throughout. Results: Analysis of fecal bacterial content showed that ABX treatment resulted in depletion of gut microbiota. Quantification of LipoDox content revealed significantly increased accumulation in ABX tumor compared to control. Compared to LipoDox treatment alone, we found that combined gut microbiota depletion and LipoDox treatment resulted in augmented long-term anti-tumor efficacy and significantly improved median survival compared to LipoDox only (control vs ABX = 58.5 vs 74 days, p = 0.0002, n = 10 for both groups), with two mice surviving until the end of the experimental end point without experiencing relapse. We also identified the increase in vascular permeability of ABX-treated tumors correlated to for improved therapeutic efficacy and outcome. Conclusion: We showed that gut microbiota depletion led to enhanced tumor vascular permeability, which allowed a larger amount of LipoDox nanoparticles to accumulate in the tumor, leading to better long-term effects. Our results suggest that gut microbiota modulation may be exploited in combination with available nanomedicine-based therapeutics to improve cancer diagnosis, therapeutic efficacy and outcome.

Abstract:Transforming growth factor β (TGF–β), a versatile immunosuppressive cytokine, has gained increasing attention as a potential target for cancer immunotherapy. However, current strategies are constrained by tumor heterogeneity and drug resistance. Therapeutic probiotics, such as Escherichia coli Nissle1917 ( EcN ), not only regulate the gut microbiota to increase beneficial bacteria with anti-tumor effects, but also modulate immune factors within the body, thereby enhancing immunity. In this study, we developed an oral microgel delivery system of EcN @(CS–SA) 2 by electrostatic interaction between chitosan (CS) and sodium alginate (SA), aiming to enhance its bioavailability in the gastrointestinal tract (GIT). Notably, EcN @(CS–SA) 2 microgel showed a synergistic enhancement of the anti-tumor efficacy of Galunisertib (Gal, a TGF–β inhibitor) by inducing apoptosis and immunogenic cell death (ICD) in tumor cells, as well as promoting increased infiltration of CD8 + T cells into the tumor microenvironment (TME).

Abstract:Tumor-targeting attenuated Salmonella could induce certain antitumor therapeutic effect through its proliferation characteristic and the consequent activated immune response, while host defense cells represented by neutrophils would trap and eliminate these invading bacteria via producing excess hydrogen peroxide (H2O2)-including reactive oxygen species in the bacteria-infected tumor, thereby impairing the efficacy of the bacteria treatment of tumor. Herein, we attempt to combine bacteria treatment and oxygen-dependent radioimmunotherapy of tumor through injection of neutrophil-targeted nano-catalase into the bacteria-treated mice for perfect tumor treatment outcome. Denatured albumin is used to coat catalase and deliver it to the neutrophils infiltrated in bacteria-infected tumor tissue. Taking advantage of the generating H2O2 by neutrophils, easily-diffused oxygen is produced and spread the whole tumor under the catalysis of nano-enzyme, leading to enhanced radiotherapy of hypoxic tumor cells. Moreover, the optimized tumor microenvironment, synergistically caused by potent immune-stimulation of bacteria, generating oxygen and tumor radiotherapy, would boost the antitumor immunity. This novel combination therapy strategy holds great promise to provide new ideas for future clinical cancer treatment.
Neutrophil hitchhiking nanoparticles enhance bacteria-mediated cancer therapy via NETosis reprogramming

Abstract:Bacteria have shown great potential in anti-tumor treatment, and an attenuated strain of Salmonella named VNP20009 has been shown to be safe in clinical trials. However, colonized bacteria recruit neutrophils into the tumor, which release NETs to capture and eliminate bacteria, compromising bacterial-based tumor treatment. In this study, we report a neutrophil hitchhiking nanoparticles (SPPS) that block the formation of NET to enhance bacteria-mediated tumor therapy. In the 4 T1 tumor-bearing mouse model, following 24 h of bacterial therapy, there was an approximately 3.0-fold increase in the number of neutrophils in the bloodstream, while the amount of SPPS homing to tumor tissue through neutrophil hitchhiking increased approximately 2.0-fold. It is worth noting that the NETs in tumors significantly decreased by approximately 2.0-fold through an intracellular ROS scavenging-mediated NETosis reprogramming, thereby increasing bacterial vitality by 1.9-fold in tumors. More importantly, the gene drug (siBcl-2) loaded in SPPS can be re-encapsulated in apoptotic bodies by reprogramming neutrophils from NETosis to apoptosis, and enable the redelivery of drugs to tumor cells, further boosting the antitumor efficacy with a synergistic effect, resulting in about 98% tumor inhibition rate and 90% survival rate.

Abstract:Toll-like receptors (TLRs), found on antigen-presenting cells including macrophages and dendritic cells, are essential for identifying infections and initiating adaptive immunity. Therefore, among the many strategies for cancer immunotherapy, methods based on TLR agonists have been one of the most pursued directions. In addition to these immunologic activation functions, TLR agonists also contribute to reshaping immunosuppressive “cold” tumors into “hot” tumors. Despite their remarkable immunomodulatory properties, TLR agonists exhibit a low therapeutic index. Non-specific stimulation of various immune cells may produce excessive levels of inflammatory cytokines, leading to systemic side effects. The continuous development in nanotechnology has presented the possibility of innovative therapies to improve the efficacy and safety of TLR agonist-based immunotherapy. In this review, we describe the functions of TLRs in the tumor microenvironment, as well as, the pathways that activate immune responses, in addition to summarizing and discussing recent developments in nanotechnology for immunomodulation with TLR agonists, including platforms such as nanocapsules, micelles, liposomes, nanogels, and others. Nanotechnology can effectively improve the efficacy of TLR agonist-based immunotherapy, while playing a pivotal role in solving the limitations of the current treatment.

Abstract:Adverse effects and multidrug resistance remain significant obstacles in conventional cancer therapy. Nanomedicines, with their intrinsic properties such as nano-sized dimensions and tunable surface characteristics, have the potential to mitigate the side effects of traditional cancer treatments. While nanomaterials have been widely applied in cancer treatment, challenges such as low targeting efficiency and poor tumor penetration persist. Recent research has shown that anaerobic bacteria exhibit high selectivity for primary tumors and metastatic cancers, offering good safety and superior tumor penetration capabilities. This suggests that combining nanomaterials with bacteria could complement their respective limitations, opening vast potential applications in cancer therapy. The use of bacteria in combination with nanomaterials for anticancer treatments, including chemotherapy, radiotherapy, and photothermal/photodynamic therapy, has contributed to the rapid development of the field of bacterial oncology treatments. This review explores the mechanisms of bacterial tumor targeting and summarizes strategies for synthesizing bacterial-nanomaterial and their application in cancer therapy. The combination of bacterial-nanomaterial hybrids with modern therapeutic approaches represents a promising avenue for future cancer treatment research, with the potential to improve treatment outcomes for cancer patients.

Abstract:Immunotherapy using immune checkpoint inhibitors (ICIs) has emerged as one of the most potent cancer therapies. However, low response rates to ICI therapeutics, particularly in solid tumors, have been observed in clinical practice because of several factors, with the poor immunogenic condition of the tumor microenvironment (TME) being considered a key feature. In this study, we flamed up the TME of solid tumors by using LTX-315 (LTX), a first-in-class oncolytic cationic peptide, and an RNA drug, poly(I:C) (IC), Toll-like receptor (TLR) 3 agonist, to boost the therapeutic effect of anti-PD-1 (aPD1) therapy. Intelligent hybrid nanoparticles (NPs) were designed and fabricated with the decoration of CD47 mimicking peptide (CD47p) (LTX/IC-NP@CD47p) on their surfaces to suppress phagocytosis of phagocytes and enhance the persistence of NPs in the circulatory system, followed by improved accumulation in tumor tissue and a higher cellular uptake capacity of cancer cells. The results demonstrated that LTX/IC-NP@CD47p effectively triggered immunogenic cell death (ICD) in vitro and in vivo characterized by abundant damage-associated molecular pattern (DAMP) levels and robustly induced type I interferon release, which enhanced the immune condition of solid tumors with increased antitumor effector immune cells, supporting the therapeutic effect of aPD1 therapy. Moreover, the combination of LTX/IC-NP@CD47p treatment and aPD1 therapy inhibited tumor metastasis by reducing the epithelial-mesenchymal transition (EMT) in primary tumors, which prolonged the survival rate of tumor-bearing mice in the in vivo model. Thus, our study suggests a potent strategy for improving therapeutic efficacy of ICIs by combining an ICD inducer and TLR agonist to induce a highly immunogenic TME.
