Abstract:Objective. Pain sensitivity decreases following isometric exercise. It is not clear whether this exercise-induced hypoalgesia (EIH) occurs to the same extent in men and women. It is also unclear if the effect is systemic or local to the exercised musculature. The aim of our study was to investigate whether fatiguing isometric exercise of the spinal and hip extensors would result in increased pressure pain threshold (PPT) at sites local to and remote from the exercised muscles in healthy men and women and whether there is a relationship between central sensitization, psychosocial factors, and PPT. Subjects. 35 healthy adults (age 27.1 ± 4.5 years, 22 women). Methods. This was a within-subjects cohort study. Participants completed questionnaires quantifying central sensitization, pain catastrophizing, sleepiness/insomnia, anxiety, and depression. PPT was assessed at the lumbar and thoracic paraspinals, hamstrings, gastrocnemius, wrist, and third digit before and immediately after participants performed the Biering–Sorensen test to failure. Results. PPT increased postexercise in the thoracic paraspinals, hamstrings, and gastrocnemius in men and women and in the lumbar paraspinals in men only but did not change at the wrist and digit sites. A lower average PPT at baseline was associated with a higher central sensitization scores. A greater increase in average PPT postfatigue was significantly associated with higher average PPT at baseline. Conclusions. Exercise-induced hypoalgesia occurs at sites overlying the muscles involved in fatiguing exercise, but not at remote sites, and is more evident in males than females. The magnitude of EIH depends upon baseline PPT. Even in healthy individuals, greater central sensitization is associated with lower baseline PPT.

The effects of a 15-week physical exercise intervention on pain modulation in fibromyalgia: Increased pain-related processing within the cortico-striatal- occipital networks, but no improvement of exercise-induced hypoalgesia

Abstract:Dysfunctional top-down pain modulation is a hallmark of fibromyalgia (FM) and physical exercise is a cornerstone in FM treatment. The aim of this study was to explore the effects of a 15-week intervention of strengthening exercises, twice per week, supervised by a physiotherapist, on exercise-induced hypoalgesia (EIH) and cerebral pain processing in FM patients and healthy controls (HC). FM patients (n = 59) and HC (n = 39) who completed the exercise intervention as part of a multicenter study were examined at baseline and following the intervention. Following the exercise intervention, FM patients reported a reduction of pain intensity, fibromyalgia severity and depression. Reduced EIH was seen in FM patients compared to HC at baseline and no improvement of EIH was seen following the 15-week resistance exercise intervention in either group. Furthermore, a subsample (Stockholm site: FM n = 18; HC n = 19) was also examined with functional magnetic resonance imaging (fMRI) during subjectively calibrated thumbnail pressure pain stimulations at baseline and following intervention. A significant main effect of exercise (post > pre) was observed both in FM patients and HC, in pain-related brain activation within left dorsolateral prefrontal cortex and caudate, as well as increased functional connectivity between caudate and occipital lobe bordering cerebellum (driven by the FM patients). In conclusion, the results indicate that 15-week resistance exercise affect pain-related processing within the cortico-striatal-occipital networks (involved in motor control and cognition), rather than directly influencing top-down descending pain inhibition. In alignment with this, exercise-induced hypoalgesia remained unaltered.

Abstract:Abstract Purpose Muscle pain can impair exercise performance but the mechanisms for this are unknown. This study examined the effects of muscle pain on neuromuscular fatigue during an endurance task. Methods On separate visits, twelve participants completed an isometric time-to-task failure (TTF) exercise of the right knee extensors at ~ 20% of maximum force following an intramuscular injection of isotonic saline (CTRL) or hypertonic saline (HYP) into the vastus lateralis. Measures of neuromuscular fatigue were taken before, during and after the TTF using transcranial magnetic stimulation (TMS) and peripheral nerve stimulation. Results The mean pain intensity was 57 ± 10 in HYP compared to 38 ± 18 in CTRL ( P 0.05). The TMS silent period was 9% longer at 100 s during the TTF in HYP compared to CTRL ( P = 0.026). Conclusion Muscle pain reduces exercise performance through the excacerbation of neuromuscular fatigue that is central in origin. This appears to be from inhibitory feedback from group III/IV nociceptors which acts to reduce central motor output.

Abstract:Purpose: The purpose of this article is to provide a holistic understanding of likely mechanisms of fatigue during high-intensity sports lasting 1–10 min. Methods: The contributions from skeletal muscle, central nervous system (motor drive), and fatigue sensations to lower performance during intense cycling, running, rowing, repeated sprints, or knee extensions were evaluated. Results: At the end of exercise, reductions of external power, force, or velocity and peak isometric forces are moderate (10–30%), motor drive falls by <8%, and fatigue sensations are extreme, e.g., Borg's rating of perceived exertion >18. Multiple putative fatigue factors can change simultaneously to interactively lower performance and/or provide protection. Major detrimental factors include raised intramuscular inorganic phosphate, severe extra- and intracellular acidosis, cerebral hypoxemia, large reduction of trans-sarcolemmal K+-gradient (also Na+- and Cl−-gradients), and reduced muscle glycogen levels. Factors resisting fatigue include elevated adrenaline, extracellular lactate, interstitial [H+] and [K+], and a normal trans-sarcolemmal Cl−-gradient. Conclusions: Raised inorganic phosphate interacting with intracellular H+, and a large rundown of trans-sarcolemmal ion gradients are likely to cause peripheral fatigue. Severe arterial hypoxemia (due to plasma acidosis, raised core temperature, lowered PaO2), and factors released from glycogen lowered muscles, e.g., interleukin-6, may cause central fatigue and strong fatigue sensations.

Abstract:Exertional leg pain is a relatively common condition that can negatively impact athletic performance and may pose a diagnostic and management challenge for the sports medicine physician. A comprehensive clinical evaluation is important to narrow the broad differential associated with exertional leg pain and focus the selection of the most appropriate diagnostic tests and treatment modalities in the care of these individuals. This article will provide an update on recent literature regarding the etiologies, pathophysiology, clinical presentation, workup/diagnosis, and treatment of exertional leg pain in athletes with a focus on vascular etiologies, chronic exertional compartment syndrome, and bone stress injury.

Abstract:Background/Objectives: There is a subset of patients with pain who become worse after exercise. To explore this, we examined the responses of people with chronic primary pain to a standardized high intensity exercise protocol used to induce delayed onset muscle soreness (DOMS). Methods: Ten participants with a diagnosis of chronic widespread muscle pain (CWMP) were matched by age and reported gender to ten participants without muscle pain (i.e., no pain (NP)). Participants completed a standardized DOMS protocol. Pain intensity in the arm at rest and with movement was assessed using daily electronic diaries. Peak pain, the timing of peak pain, and the time to recovery were compared between groups. Associations of pain variables with the functional connectivity of the sensorimotor (SMN), cerebellum, frontoparietal control (FPN), and default mode network (DMN) both within network nodes and the rest of the brain was assessed. Results: Significant differences in peak pain, the time to peak pain, and the time to recovery were noted between groups for both pain at rest and pain with movement after controlling for catastrophizing and pain resilience. Connectivity across the SMN, FPN, and DMN was associated with all pain-related variables. Significant group differences were identified between groups. Conclusions: A standardized muscle "injury" protocol resulted in more pain, a longer time to peak pain, and a longer time to resolve pain in the patient group compared to the NP group. These differences were associated with differences in connectivity across brain regions related to sensorimotor integration and appraisal. These findings provide preliminary evidence of the dysregulation of responses to muscle (micro)trauma in people with chronic pain.

Abstract:Lewis1 postulated that pain in ischemic, active muscles was brought about by the accumulation in the tissue spaces of a metabolite which he designated as "Factor P." He also indicated that Factor P was the stimulus responsible for the pain of intermittent claudication and coronary occlusion. Abstract:Exercise-Induced Hypoalgesia (EIH) refers to an acute reduced pain perception after exercise. This systematic review and meta-analysis investigated the effect of a single aerobic exercise session on local and remote EIH in healthy individuals, examining the role of exercise duration, intensity, and modality. Pressure pain thresholds (PPT) are used as the main measure, applying the Cochrane risk of bias tool and GRADE approach for certainty of evidence assessment. Mean differences (MD; Newton/cm2) for EIH effects were analysed. Thirteen studies with 23 exercises and 14 control interventions are included (498 participants). Most studies used bicycling, with only two including running/walking and one including rowing. EIH occurred both locally (MD = 3.1) and remotely (MD = 1.8), with high-intensity exercise having the largest effect (local: MD = 7.5; remote: MD = 3.0) followed by moderate intensity (local: MD = 3.1; remote: MD = 3.0). Low-intensity exercise had minimal impact. Neither long nor short exercise duration induced EIH. Bicycling was found to be effective in eliciting EIH, in contrast to the limited research observed in other modalities. The overall evidence quality was moderate with many studies showing unclear risk biases.

Abstract:Objective: Myalgic encephalomyelitis / chronic fatigue syndrome (ME/CFS) and fibromyalgia (FM) are two debilitating, moderately comorbid illnesses in which chronic musculoskeletal pain symptoms are prevalent. These individuals can experience post-exertional malaise (PEM), a phenomenon in which symptom severity is worsened for 24 hours or longer after physical stress, but the pain-related component of PEM is not well characterized. Design: Systematic review and meta-analysis. Methods: Case-control studies involving adults with ME/CFS or FM and measuring pain symptoms before and after exposure to a standardized aerobic exercise test were included. Hedges' d effect sizes were aggregated with random-effects models, and potential moderators were explored with meta-regression analysis. Results were adjusted for nesting effects with three-level modeling. Results: Forty-five effects were extracted from 15 studies involving 306 patients and 292 healthy controls. After adjusting for nesting effects, we observed a small to moderate effect indicating higher post-exercise pain in patients than in controls (Hedges' d = 0.42; 95% confidence interval [CI]: 0.16-0.67). The mean effect was significantly moderated by pain measurement time point (b = -0.19, z = -2.57, P = 0.01), such that studies measuring pain 8-72 hours after exercise showed larger effects (d = 0.71, 95% CI = 0.28-1.14) than did those measuring pain 0-2 hours after exercise (d = 0.32, 95% CI = 0.10-0.53). Conclusions: People with ME/CFS and FM experience small to moderate increases in pain severity after exercise, which confirms pain as a component of PEM and emphasizes its debilitating impact in ME/CFS and FM. Future directions include determining mechanisms of pain-related PEM and developing exercise prescriptions that minimize symptom exacerbation in these illnesses.

Abstract:Applied Physiology, Nutrition, and Metabolism, Ahead of Print. Physical activity acutely alters pain processing known as exercise-induced hypoalgesia (EIH). This randomized controlled crossover study investigated the effects of two different rowing exercises on EIH and to explore whether possible EIH effects are related to individual rowing specific performance. Fifty male experienced rowers conducted two rowing sessions (submaximal: 30 min of moderate rowing (70% of maximum heart rate); maximal: 350 m in an all-out fashion) and a control session. Pre and post exercise pain sensitivity was measured bilaterally using pressure pain thresholds (PPT; Newton (N)) at the elbow, knee, ankle, sternum, and forehead. Individual performance was determined as maximum watt/kg and was tested for correlations with changes in PPT. Higher PPT values were observed after maximal exercise at all landmarks with a mean change ranging from 2.5 ± 7.8 N (right elbow; p = 0.027; dz = 0.323) to 10.0 ± 12.2 N (left knee; p ≤ 0.001; dz = 0.818). The submaximal (range from −1.6 ± 8.8 N (Sternum; p = 0.205; dz = 0.182) to 2.0 ± 10.3 N (right ankle; p = 0.176; dz = 0.194)) and control session (range from −0.5 ± 7.6 N (left elbow; p = 0.627; dz = 0.069) to 2.6 ± 9.1 N (right ankle; p = 0.054; dz = 0.279)) did not induce changes. Relative performance levels were not correlated to EIH (range from: r = −0.129 (p = 0.373) at sternum to r = 0.176 (p = 0.221) at left knee). EIH occurred globally after a short maximal rowing exercise while no effects occurred after rowing for 30 min at submaximal intensity. EIH cannot be explained by rowing specific performance levels in experienced rowers. However, the sample may lack sufficient heterogeneity in performance levels to draw final conclusions.

Abstract:The purpose of this study was to investigate the effect of experimentally induced muscle pain on the motor-control strategies of synergistic muscles during submaximal fatiguing isometric contractions. The root mean square (RMS) and median frequency (MF) of the surface electromyographic (EMG) signal from synergistic plantarflexors and dorsiflexors were assessed to exhaustion. Ten subjects performed sustained dorsiflexions and plantarflexions at two contraction levels, 50% and 80% of maximum voluntary contraction, with or without muscle pain, induced by injection of 6% hypertonic saline in one synergist. In the painful contractions, the RMS of the EMG signal was decreased compared to the control condition in the initial phase of the contraction, in the muscles where pain was induced as well as in the nonpainful synergists. Moreover, the EMG signal MF decreased faster during muscle pain than in the control condition. The endurance time was shorter during muscle pain, and some of the nonpainful synergists showed increased compensatory activity at the end of the contractions to maintain the target force. The decreased EMG activation during pain was coupled with significantly decreased torque levels during the painful condition that would partly explain the results. However, the ratio between force and EMG amplitude was decreased for both the painful and nonpainful synergists, so other mechanisms might explain the present findings. This study shows that localized muscle pain can reorganize the EMG activity of synergists where no pain is present. These findings may have implications for the understanding of manifestations seen in relation to painful musculoskeletal disorders.

Abstract:<h3 class="article-section__sub-title section1"> Background</h3><p>Patellofemoral pain (PFP) is a common knee pain condition where hip and knee exercises help improve treatment outcomes. This study compared the acute effect of hip versus knee exercises on anti‐nociceptive and pro‐nociceptive mechanisms in young females with long‐standing PFP.</p><h3 class="article-section__sub-title section1"> Methods</h3><p>In this randomized cross‐over study, 29 females with PFP performed hip and knee exercises in randomized order during a single day. Pressure pain thresholds (PPTs) were assessed by handheld pressure algometry at the patella, the tibialis anterior muscle, and the contralateral elbow. Cuff pressure algometry at the lower legs was used to assess pain detection threshold (cPDT) and tolerance (cPTT) as well as conditioned pain modulation (CPM: change in cPDT during contralateral cuff pain conditioning) and temporal summation of pain (TSP: ten painful cuff stimulations assessed on a visual analogue scale [VAS]).</p><h3 class="article-section__sub-title section1"> Results</h3><p>PPTs at the tibialis anterior muscle but not at the patella increased compared with baseline following both exercises (<i>p</i> < .002). Compared with baseline, the cPDTs and cPTTs increased after both types of exercises (<i>p</i> < .001). The cPTTs increased more after knee compared to hip exercises (<i>p </i>< .007). VAS scores for TSP were increased following hip exercises (<i>p</i> < .001) although the rate of VAS increase over repeated stimulations was not significantly affected by exercises. The CPM‐effect was reduced after both types of exercises (<i>p</i> < .001). </p><h3 class="article-section__sub-title section1"> Conclusion</h3><p>A general hypoalgesic response to slowly increasing pressure stimuli was observed following both hip and knee exercises as well as decreased conditioned pain modulation, potentially indicating an attenuated ability from exercise to inhibit pain.</p>

Popular Gym Fitness Sport: An Analysis of 1387 Recreational Athletes Regarding Prone to Pain Exercises and the Corresponding Localisations

Abstract:Background: Recreational fitness sports are popular worldwide and rank first among organised sports. This study aims to bridge a knowledge gap by examining which exercises are most prone to causing pain symptoms, as a possible precursor for injury, and analysing the body regions that are most frequently affected. Methods: Using an online questionnaire, 20 demographic and training-specific items and 49 sport-specific exercises were recorded. Frequent exercises as well as the incidence and distribution of pain symptoms that the athletes experienced during or in relation to their training were evaluated. Results: The study assessed common exercises and documented the frequency and distribution of pain symptoms experienced by athletes during or in relation to their training. A total of 1387 respondents were included in this study. Of these, 732 (53.1%) experienced pain during their fitness training, with 333 (24.2%) being female and 397 (22.3%) being male. The method of creating a training plan showed a significant influence (p < 0.001): athletes who devised their own plans reported pain or instability more frequently than those in the comparison groups. Guided exercises on machines resulted in the lowest frequency of pain (11.54%), while exercises with free weights were associated with the highest pain rate among respondents (19.94%). Specifically, exercises such as the back squat, deadlift, bench press, and triceps dips were identified as the exercises most commonly associated with pain. The most frequently reported pain region was the shoulder, followed by the lower back and knees. Conclusion: The findings reveal a significant number of unreported pain symptoms. The disparity between rigorous training volumes and the absence of professional care frequently leads to injuries and pain. It is incumbent upon sports medicine to investigate the root causes of these complaints (pain or instability) to implement preventive measures against potential injuries.
Response to experimental cold-induced pain discloses a resistant category among endurance athletes, with a distinct profile of pain-related behavior and GABAergic EEG markers: a case–control preliminary study

Abstract:Pain is a major public health problem worldwide, with a high rate of treatment failure. Among promising non-pharmacological therapies, physical exercise is an attractive, cheap, accessible and innocuous method; beyond other health benefits. However, its highly variable therapeutic effect and incompletely understood underlying mechanisms (plausibly involving the GABAergic neurotransmission) require further research. This case–control study aimed to investigate the impact of long-lasting intensive endurance sport practice (≥7 h/week for the last 6 months at the time of the experiment) on the response to experimental cold-induced pain (as a suitable chronic pain model), assuming that highly trained individual would better resist to pain, develop advantageous pain-copying strategies and enhance their GABAergic signaling. For this purpose, clinical pain-related data, response to a cold-pressor test and high-density EEG high (Hβ) and low beta (Lβ) oscillations were documented. Among 27 athletes and 27 age-adjusted non-trained controls (right-handed males), a category of highly pain-resistant participants (mostly athletes, 48.1%) was identified, displaying lower fear of pain, compared to non-resistant non-athletes. Furthermore, they tolerated longer cold-water immersion and perceived lower maximal sensory pain. However, while having similar Hβ and Lβ powers at baseline, they exhibited a reduction between cold and pain perceptions and between pain threshold and tolerance (respectively −60% and − 6.6%; −179.5% and − 5.9%; normalized differences), in contrast to the increase noticed in non-resistant non-athletes (+21% and + 14%; +23.3% and + 13.6% respectively). Our results suggest a beneficial effect of long-lasting physical exercise on resistance to pain and pain-related behaviors, and a modification in brain GABAergic signaling. In light of the current knowledge, we propose that the GABAergic neurotransmission could display multifaceted changes to be differently interpreted, depending on the training profile and on the homeostatic setting (e.g., in pain-free versus chronic pain conditions). Despite limitations related to the sample size and to absence of direct observations under acute physical exercise, this precursory study brings into light the unique profile of resistant individuals (probably favored by training) allowing highly informative observation on physical exercise-induced analgesia and paving the way for future clinical translation. Further characterizing pain-resistant individuals would open avenues for a targeted and physiologically informed pain management.

Abstract:Introduction Exercise produces an immediate lessening of pain sensitivity (Exercise-Induced Hypoalgesia (EIH)) in healthy individuals at local and distant sites, possibly through a shared mechanism with conditioned pain modulation (CPM). Dynamic resistance exercise is a recommended type of exercise to reduce pain, yet limited research has examined the effects of intensity on EIH during this type of exercise. Therefore, the primary purpose of this study is to compare changes in PPT at a local and distant site during a leg extension exercise at a high intensity, a low intensity, or a quiet rest condition. A secondary purpose is to examine if CPM changes after each intervention. The final purpose is to examine if baseline pain sensitivity measures are correlated with response to each intervention. Methods In a randomized controlled trial of 60 healthy participants, participants completed baseline pain sensitivity testing (heat pain threshold, temporal summation, a cold pressor test as measure of CPM) and were randomly assigned to complete a knee extension exercise at: 1) high intensity (75% of a 1 Repetition Maximum (RM), 2) low intensity (30% 1RM), or 3) Quiet Rest. PPT was measured between each set at a local (quadriceps) and distant (trapezius) site during the intervention. CPM was then repeated after the intervention. To test the first purpose of the study, a three-way ANOVA examined for time x site x intervention interaction effects. To examine for changes in CPM by group, a mixed-model ANOVA was performed. Finally, a Pearson Correlation examined the association between baseline pain sensitivity and response to each intervention. Results Time x site x intervention interaction effects were not significant (F(5.3, 150.97) = 0.87, p = 0.51, partial eta 2 = 0.03). CPM did not significantly change after the interventions (time x intervention F(1,38) = 0.81, p = 0.37, partial eta 2 = 0.02. EIH effects at the quadriceps displayed a significant, positive moderate association with baseline HPT applied over the trapezius (r = 0.61, p<0.01) and TS (r = 0.46, p = 0.04). Discussion In healthy participants, PPT and CPM did not significantly differ after a leg extension exercise performed at a high intensity, low intensity, or quiet rest condition. It is possible pre-intervention CPM testing with a noxious stimuli may have impaired inhibitory effects frequently observed during exercise but future research would need to examine this hypothesis.

Abstract:Jens Agerström, Kent Stening, Olof Axman Department of Psychology, Linnaeus University, 391 82 Kalmar, Sweden Background: The ability to traverse psychological distance by going beyond the experienced reality of the self, here and now, is fundamental for effective human functioning. Yet, little is known about how physical pain affects transcendence of psychological distance. Using a construal level theory framework of psychological distance, the current research examines the hypothesis that pain impairs people's ability to traverse any kind of psychological distance whether it be temporal, social, and spatial distance, or the hypothetical. Methods: Using the cold pressor test, 151 participants participated in an experiment where they were either induced with acute pain (treatment group) or no pain (control group) while completing a battery of questions measuring to what extent their current thoughts were transcending psychological distance. Results: The results were largely consistent with the hypothesis. Relative to the control group, pain induced participants showed significantly less transcendence of past temporal distance, social distance, spatial distance, and the hypothetical. Furthermore, greater self-reported pain intensity was significantly associated with less transcendence of temporal (past and future), social, and spatial distance. Conclusion: Physical pain impairs the ability to traverse psychological distance. The research has practical implications for the pain clinic and for pain-afflicted individuals in everyday life. Abstract:BackgroundLocomotor muscle fatigue (LMMF) and exercise-induced muscle damage (EIMD) are common conditions experienced during long-distance running due to the pooled effect of mechanical and metabolic strain on the locomotor muscles. However, little is known about the instant effects of combined LMMF and EIMD on pacing behaviour and performance during the decisive final stages of ‘real-world’ long-distance running events.MethodsTwenty-two highly trained runners (11 females) completed two maximal self-paced 20-km treadmill time trials in a counterbalanced crossover design: (A) in a tapered condition and (B) with LMMF and EIMD. Indicators of muscle damage, muscle metabolic strain, and endocrinological stress were assessed to investigate the physiological effects, and a three-dimensional framework of perceived fatigability was applied to investigate the perceptual effects of running with LMMF and EIMD on performance fatigability.ResultsLMMF and EIMD caused restrictions in work capacity and medium increases in blood leucocyte and neutrophil count, interleukin-6, and cortisol concentrations, collectively constituting a physiological milieu likely not conducive to high performance. LMMF and EIMD further caused large increases in perceived physical strain and large decreases in valence as well as large increases and decreases in action crisis and flow state, respectively.ConclusionsUnder the constraint of amplified physical duress, findings are suggestive of heuristic and rational antecedents in the goal disengagement process. Dynamic changes in physiological and perceptual effects of LMMF and EIMD are hypothesised to underpin the observed alterations in pacing behaviour and performance fatigability during long-distance running. The applied three-dimensional framework provides a more comprehensive understanding of strain-perception-thinking-action coupling in centrally regulated and goal-directed exercise behaviour.

It was proven that the solution set of a linear program under uncertainty is convex and derived explicit expressions for this set for some particular cases. The main result of this paper is to show that the solution set is not only convex but also polyhedral.
