Abstract:Eccrine hidrocystoma (EH) is a benign tumor derived from the cystic dilatation of the eccrine sweat ducts caused by the retention of eccrine secretions. EH is rare and is found most often in middle-aged women, especially those who work or stay for a long duration in hot and humid environments. The lesions usually involve periocular and malar areas, manifesting as multiple translucent skin-colored to bluish cysts. The sizes and numbers of the lesions can fluctuate according to the season, with exacerbation in warm weather and alleviation in cold weather. EH is histopathologically characterized by unilocular dermal cysts lined by two layers of cuboidal to flattened epithelial cells.

Abstract:A case of eccrine epithelioma is reported. A 82-year-old woman had a blackish ulcerated nodule of 5mm×10mm in size on her right lower eyelid. The nodule had increased in size for these three years. She visited our office on March 24, 1998. Since clinical diagnosis was basal cell epithelioma, the lesion was totally excised with more than 5mm margin including orbicularis oculi muscle. Histopathological findings were similar to solid type basal cell epithelioma. But there were some difference; tumor cells connected directly with eccrine glands and were composed of two cell types; dark elongated nuclei with condensed cytoplasm and large clear round uuclei with abundant cytoplasm. Thus our case was suggested to be eccrine epithelioma. [Skin Cancer (Japan) 2000 ;15: 31-34]

Abstract:Eccrine porocarcinoma is an extremely rare skin adnexal malignant neoplasia with highly invasive and metastatic potential. We report an additional case of eccrine porocarcinoma with intracranial metastases. This case is characterized by a complete record of the progress of eccrine porocarcinoma, its immunohistochemistry after three operations showed a progressive increase in the level of Ki-67 index. We herein report a case of a 37-year-old-male with eccrine carcinoma occurring on the left posterior occipital scalp which invaded the skull and dura, presenting with progressive headache. This patient has performed three surgeries in total. During the last hospitalization, he underwent an extended surgical resection, lymphadenectomy, myocutaneous flap transplantation and vascular anastomosis in our institution. After surgery, he was treating with radiotherapy at 200 Gray in 12 fractions. But one year after the operation, he developed chest tightness, imaging examination and biopsy puncture revealed pulmonary metastasis. Intracranial metastasis of eccrine porocarcinoma is a late event with poor prognosis. This case emphases on that progressively increased level of Ki-67 index may predict more chance to occur the intracranial metastasis of scalp eccrine porocarcinoma, long-term follow-up and appropriately dense follow-up interval is necessary.

Abstract:A 69 year-old female patient presented with a reddish-brown, enlarging lesion on her right lower eyelid. Physical exam revealed a 4-5mm, oval-shaped, reddish-brown, shiny, exophytic papule on the right lower eyelid. Shave biopsy of the lower eyelid papule was performed, which revealed nests of cells with prominent cribriform arrangements, focally surrounded by small pools of blue stringy mucinous material and papillary architecture. Cells stained positive for EMA, synaptophysin, and chromogranin, which confirmed the diagnosis of endocrine mucin-producing sweat gland carcinoma (EMPSGC). The lesion was fully removed with negative margins via Mohs micrographic surgery. Endocrine mucin-producing sweat gland carcinoma (EMPSGC) is a rare, low-grade, cutaneous adenocarcinoma that typically arises on the eyelids of elderly women. EMPSGC may be associated with invasive mucinous carcinoma.

Abstract:A 29-year-old male patient presented with erythema and papules along with itching more than 2 months, and the symptoms aggravated for 3 days. He did not respond to regular antihistamines treatment, but clinical symptoms were relieved by intramuscular injection of compound betamethasone. Skin biopsy showed dermis infiltrated by masses of eosinophils and distinctive "flame figures". Then, diagnosis of eosinophilic cellulitis was made and resolved by glucocorticoid treatment.

Abstract:We describe a 41-year-old Asian male who presented with a left upper eyelid mass for one year associated with multiple pigmented maculopapules on his face, who was found to have Epidermodysplasia verruciformis (EV). The ophthalmic examination was unremarkable. The patient spontaneously started to have disseminated, bean-sized, pinkish flat papules on his forehead since childhood and had history of multiple non-melanoma skin carcinoma. He had been treated as possible xeroderma pigmentosum (XP) by dermatologists. The oculoplastic surgeon resected the lesion and submitted tissue for histopathological and genetic analysis. Eyelid biopsy was consistent with a squamous cell carcinoma (SCC). Next-generation Sequencing (Sanger sequencing) suggested the homozygous mutation of TMC8 gene which is indicative of EV. Our case is a rare report in eyelid SCC in the setting of EV. This patient highlights the importance of considering systemic genodermatosis rather than individual eyelid malignancy and the function of molecular diagnosis in clinical challenge case.
Erdheim-Chester Disease Presenting as Bilateral Facial Masses: A case report and review of literature

Abstract:Erdheim-Chester disease (ECD) is a rare form of non-Langerhans cell histiocytosis with unknown aetiology. It is recently recognised to be neoplastic with genetic mutations affecting the mitogen-activating protein kinase pathway. We report a 49-year-old female patient who initially presented in 2012 to a tertiary care centre in Muscat, Oman, with bilateral facial masses. These were removed but later recurred over a period of 10 years. She then presented with xanthelasmas, bone lesions, secondary infertility due to hypothalamic hypogonadism, diabetes insipidus and Hashimoto's hypothyroidism. The facial masses were biopsied and they showed classic morphological features in the form of diffuse infiltration by foamy histiocytes with scattered Touton type of giant cells, patchy lymphocytic infiltrates and dense fibrosis. The patient is stable and is being followed-up. The presented ECD case is particularly interesting due to the recurrent bilateral facial masses. To the best of the authors' knowledge, this is the first documented case in Oman.

Abstract:BACKGROUND Eccrine porocarcinoma (EPC) is a rare skin tumor that mainly affects the elderly population. Tumors often present with slow growth and a good prognosis. EPCs are usually distinguished from other skin tumors using histopathology and immunohistochemistry. However, surgical management alone may be inadequate if the tumor has metastasized. However, currently, surgical resection is the most commonly used treatment modality. CASE SUMMARY A seventy-four-year-old woman presented with a slow-growing nodule in her left temporal area, with no obvious itching or pain, for more than four months. Histopathological examination showed small columnar and short spindle-shaped cells; thus, basal cell carcinoma was suspected. However, immunohistochemical analysis revealed the expression of cytokeratin 5/6, p63 protein, p16 protein, and Ki-67 antigen (40%), and EPC was taken into consideration. The skin biopsy was repeated, and hematoxylin and eosin staining revealed ductal differentiation in some cells. Finally, the patient was diagnosed with EPC, and Mohs micrographic surgery was performed. We adapted follow-up visits in a year and not found any recurrence of nodules. CONCLUSION This case report emphasizes the diagnosis and differentiation of EPC.

Here we discuss a case of massive bilateral multiple EIC of the breast presenting seven years after reduction mammaplasty. Abstract:BACKGROUND Gigantic epidermal cysts (GECs) are rare benign skin appendicular tumours also known as keratinocysts. GECs have a high incidence and their wall is made up of epidermis. Epidermal cysts can occur in any part of the skin; clinical manifestations include skin colour hemispherical swelling; cystic; mobile; 0.5 cm to several centimetres in diameter; and slow growth. CASE SUMMARY Herein, we report a case involving a 56-year-old female with a GEC in the occipitalia. On July 25, 2023, a patient with a GEC was admitted to the neurosurgery Department of the Second Affiliated Hospital of Xi'an Medical University. The phyma was shown to be a solid mass during the operation and was confirmed to be a GEC based on pathological examination. CONCLUSION Epidermal cysts are common cystic nodules on the surface of the body, the aetiology is unclear, the clinical manifestations can vary, and the misdiagnosis rate is high. However, giant epidermal cysts are rare. In most cases, however, the prognosis is satisfactory. This paper analyses and summarizes the population, location, clinical and pathological characteristics and pathogenesis of the disease to strengthen the understanding of this disease and improve the accuracy of clinical diagnosis.

Abstract:All the patients were children,3 were male and 2 were female.Skin lesion showed a dome-shaped transparent,cystic papule on face with no subjective symptoms.Histological findings revealed that a single dilated cystic cavity composed of 2 layers cells in dermis.Laser therapy was used to treat the five cases.One case′skin lesion was near complete resolution.The other′s curative effects will follow up.

Abstract:Pachydermoperiostosis is a rare genetic disease known as primary or idiopathic hypertrophic osteoarthropathy (HOA)/Touraine-Solente-Gole syndrome. It is an autosomal dominant or recessive disorder comprising digital clubbing, periostosis, hyperhidrosis, and pachydermia (thickening of facial skin). Ocular manifestations are uncommon; however, blepharoptosis may occur. This case presented with severe bilateral ptosis due to the disease progression. A large 20 mm upper lid resection with levator advancement was performed to improve his ability to see. This is the first reported case of pachydermoperiostosis (PDP) in Jamaica. We present a rare case of pachydermoperiostosis with severe blepharoptosis, who attained a good result with surgical intervention.

Abstract:Apocrine hidrocystomas are benign cystic tumors resulting from apocrine sweat glands' proliferation. They typically present as solitary, slow-growing nodules at the head and neck, especially in the periorbital cutaneous region. We present a case of periorbital apocrine hidrocystoma in a 22-year-old woman that was treated as chalazion previously. Besides the hallmark histopathological findings of apocrine hidrocystoma, IgG4 plasma cell infiltration of the cystic wall was also found. The ratio of IgG4-to-IgG-positive plasma cells was high, whereas serum IgG4 was within the standard limit. This is, to date, the only probable IgG4-related ophthalmic disease associated with apocrine hidrocystoma.

Abstract:The characteristics of the pulmonary cysts on the high-resolution computed tomography (HRCT) chest images are an important diagnostic clue to distinguish among cystic lung diseases. The diagnostic accuracy of HRCT was reported to be as high as 90% by experienced pulmonologists and radiologists. Herein, we report the case of an elderly woman with Birt-Hogg-Dubé syndrome (BHDS) whose HRCT images displayed lymphangioleiomyomatosis-like features of the pulmonary cysts, rendering it difficult for us to diagnose BHDS. This case illustrates the significance of a thorough anamnesis, physical examination, and skin biopsy of facial papules to establish an accurate diganosis.

Abstract:Purpose: To describe the clinical features of a conjunctival apocrine hidrocystoma that developed 6 decades after a strabismus surgery and review existing literature on apocrine hidrocystomas of the conjunctiva and caruncle. Methods: Case report and review of literature on conjunctival apocrine hidrocystomas. Results: A 71-year-old man with a history of strabismus surgery as a child presented with a cystic lesion on the nasal conjunctiva and caruncle for 1 year. Excision of the lesion showed a unilocular cavity lined by a double layer of cells with the hallmark finding of apical decapitations, confirming a diagnosis of apocrine hidrocystoma. Seven additional cases of conjunctival and caruncular apocrine hidrocystomas were reviewed. All cases presented after 50 year of age. Most cases presented nasally or within the caruncle and had a pigmented appearance. No other reported cases had a history of trauma or surgery. All cases were treated with surgical excision without recurrence. Conclusions: History of strabismus surgery or conjunctival trauma may lead to ectopic deposition of apocrine glands that may contribute to the formation of an apocrine hidrocystoma.

Abstract:Introduction: Sebaceous gland hyperplasia of the eyelids, known as adenomatoid or pseudoadenomatous hyperplasia, is a rare benign condition. Optimal management strategies for this specific type of eyelid tumor require further investigation. Case presentation: The patient presented with a 21-year history of a progressively enlarged mass in the right lower eyelid. Previous treatments, including laser photocoagulation and surgical excision, have failed to prevent recurrence. The mass, characterized by a firm texture and low mobility, has raised concerns regarding malignancy. However, histopathological examination following surgical excision identified the mass as sebaceous gland hyperplasia. The patient's medical history was notable for benign gastrointestinal and intestinal polyps with no evidence of malignancy. Conclusions: A final diagnosis of eyelid sebaceous gland hyperplasia was established after surgical excision and comprehensive histopathological analyses. The patient's successful recovery without recurrence over a three-month follow-up period post-surgery highlights the efficacy of the surgical approach and the use of intraoperative frozen section pathological examination.
A Rare Case of Eccrine Carcinoma Demonstrating Intracranial Invasion in a Patient With Advanced Hepatocellular Carcinoma

Abstract:Eccrine carcinoma (EC) is a rare intraepidermal carcinoma of the eccrine sweat glands. Even more rare are instances of EC exhibiting intracranial invasion. Here, we describe the case of a metachronous EC mass demonstrating intracranial invasion in a patient with advanced-stage hepatocellular carcinoma (HCC), reporting CT head findings of a left frontal skull expansile destructive mass with soft tissue density and immunostain findings of the following: CEA: positive, granular, EMA: positive, AE1/AE3: positive, CK7: strongly positive, CK20: negative, GCDFP: negative, and HEPAR: negative. The only recommended treatment for EC is surgical excision with tumor-free margins, and no chemotherapy protocols currently exist. Due to socioeconomic factors, our patient was unable to receive adequate treatment for her HCC, nor surgical excision for her EC. However, the unique presentation of a rare intracranial EC tumor causing no neurological deficits in a patient with untreated HCC merits the need for a more thorough identification of secondary tumors via biopsy in patients with HCC to identify possible associations between these two tumors in future patients.

Abstract:Hypereosinophilic syndrome (HES) is a rare haematologic disorder characterised by unexplained, persistent eosinophilia with organ involvement. We report the case of a 74-year-old patient who presented with a pemphigoid-like bullous rash as the only manifestation of an HES. This report highlights the importance of the dermatological manifestations of this potentially life-threating condition that may initially present as a skin-limited disease. Bullous pemphigoid (BP) is a very common autoimmune blistering dermatosis that may share common clinical and histological features or associate with an HES. HES should be considered when patients diagnosed with BP demonstrate atypical course under corticosteroids and/or recalcitrant blood eosinophilia.
Eyelid Metastatic Thyroid Papillary Carcinoma

Abstract:Purpose: The aim of this study was to report the clinical and histopathological features of an eyelid metastasis in a 70-year-old male with no prior medical history. Methods: A 70-year-male presented with a painless, hard, red nodular growth in his left lower medial eyelid of 6 months duration, involving the canaliculus and punctum. He did not complain of any other ocular or systemic abnormality. The clinical differential diagnosis included lymphoma or a primary adnexal tumour. The lesion was excised under local anaesthetic cover and sent for routine histopathological analysis. Results: The histology revealed a carcinoma involving the posterior and anterior lamella of the lower lid. It was composed of papillary and follicular areas. The tumour nuclei were optically clear, overlapping, had nuclear grooves and were associated with eosinophilic colloid material. The tumour cells were strongly positive for nuclear thyroid transcription factor-1 and thyroglobulin (which also stained some of the eosinophilic colloid material). The histological and immunohistochemical features were those of metastatic papillary carcinoma of the thyroid. Unfortunately, the patient's thyroid has yet to be examined because of loss to follow-up. Conclusion: To the best of our knowledge, this is the first report of thyroid papillary carcinoma metastasizing to the eyelid as the presenting feature of the disease. A review of the literature of metastatic thyroid carcinoma to the eyelids is presented, to place the findings of this case report in context.
