Abstract:BackgroundBreathlessness is a common symptom in people with advanced disease. The most effective treatments are aimed at treating the underlying cause of the breathlessness but this may not be possible and symptomatic treatment is often necessary. Strategies for the symptomatic treatment of breathlessness have never been systematically evaluated. Opioids are commonly used to treat breathlessness: the mechanisms underlying their effectiveness are not completely clear and there have been few good-sized trials in this area.ObjectivesTo determine the effectiveness of opioid drugs given by any route in relieving the symptom of breathlessness in patients who are being treated palliatively.

Abstract:Chronic breathlessness at rest or on minimal exertion despite optimal treatment of the underlying chronic cause(s) is termed chronic refractory breathlessness. This is prevalent across the community and is an independent indicator of poor prognosis. This narrative review focuses on the palliation of chronic refractory breathlessness in people predominantly with non-cancer diagnoses. Breathlessness is a complex sensation with at least three dimensions—intensity, distress/unpleasantness and its impact on function. It is the conscious representation of a mismatch between central ventilatory drive (the demand to breathe) and the responding respiratory output (the ability to breathe). Measurement relies on subjective reports by patients using a choice of uni- and multi-variable tools; the minimal clinically important difference is the smallest change conceived as clinically meaningful by the patients. Exogenous and endogenous opioids work centrally to reduce the sensation of breathlessness, with morphine as a mu opioid receptor agonist the most widely studied. Regular, low doses of sustained-release morphine have been shown to safely reduce breathlessness in this setting without evidence of respiratory depression nor obtundation. Patients should be initiated at a dosage of 10 mg/24 h and titrated by 10 mg if there is no benefit once in steady state. The highest dosage in the only dose-ranging study published to date was only 30 mg/24 h. Predictors of response to opioids for chronic refractory breathlessness include younger people with more severe breathlessness at baseline. Future research should address whether upward titration delivers further clinical benefit and whether all underlying aetiologies respond as predictably to opioids.

Abstract:Background: Chronic breathlessness is a recognized clinical syndrome that severely impacts patients and carers, who become increasingly restricted in their daily activities. Often, patients become reliant on their carers, who are required to provide constant support. Although individual experiences of breathlessness have been previously investigated, there are few studies exploring contemporaneous experiences of breathlessness of the patient and their carer. This study aimed to understand the experience of severe chronic breathlessness in people with chronic obstructive pulmonary disease (COPD) from the perspective of the patient and carer unit. Methods: A qualitative study embedded in a randomised, placebo-controlled effectiveness study (RCT) of regular, low-dose (≤32 mg/day), sustained-release morphine for chronic breathlessness associated with COPD. Recruitment occurred between July 2017 and November 2018 in one respiratory and palliative care services, in South Australia. Participants were community-dwelling patients with COPD and severe breathlessness (modified Medical Research Council scale 3 or 4) and their carers. Separate semi-structured interviews were conducted with patients and carers, recorded and transcribed verbatim. Analysis was informed by grounded theory using a constant comparative approach. Results: From the 26 patients with a carer recruited for the RCT in South Australia, nine were interviewed in their homes. Six patients were men, median age 77 years. Carers were mostly women, who were their wives (n = 6), median age 70. Five themes emerged from the data: (1) shrinking world; (2) mutual adaptation; (3) co-management; (4) emotional coping; and (5) meaning in the face of death. Conclusion: Chronic breathlessness is a systemic condition that permeates all aspects of the patient's and carer's lives. Working as a team, patients and carers manage chronic breathlessness to achieve maximal function and well-being. Patients and carers share many aspects of the experience of breathlessness, but the carer seems particularly susceptible to emotional distress. Future chronic breathlessness interventions should target the patient and the carer, both together and separately to address their common and individual needs. Trial registration: The main trial is registered (registration no. NCT02720822; posted March 28, 2016).

Abstract:Chronic Obstructive Pulmonary Disease (COPD) is a complex condition with significant impact on prognosis, especially in advanced stages where symptom burden becomes critical. Breathlessness affects patients' quality of life, and despite various therapeutic strategies, the role of opioids in palliative care for COPD remains under investigation. The acceptance of a therapeutic trial of different types of opioids is increasing not only in end-of-life situations but also for stable COPD patients experiencing intolerable refractory breathlessness despite optimal conventional therapy. Recent clinical trials have raised questions about the overall clinical benefit of opioids in addressing breathlessness in COPD, prompting the need to clarify inconsistencies and identify specific subgroups that may benefit from opioid therapy. In the clinical setting, it is crucial to understand the attributes of patients who exhibit positive responses to opioids and what type of opioids could have a positive impact. This research paper aims to offer an update of the most recent evidence of opioid treatment in managing breathlessness among individuals with COPD with a head-to-head evaluation of the supporting and opposing proof in the medical literature.

Breathlessness in cancer and chronic obstructive pulmonary disease: using a qualitative approach to describe the experience of patients and carers

Abstract:Objective: To investigate and document the effects of breathlessness on the everyday lives of patients with cancer and COPD and their carers. This subject has been little researched, although dyspnoea is recognized as a disabling, distressing symptom. The number of breathless people is increasing as patients with all types of cardio-respiratory disease live longer. Methods: Patients with severe COPD and cancer and their carers were interviewed at home using a semistructured format to record their perceptions of the impact of breathlessness, the help they had received from medical and caring services, and their ideas on how these could be improved. Results: 10 patients with COPD (6 male) and 10 with cancer (6 male) and their spouses were interviewed. All patients found breathlessness frightening, disabling, and restricting. Patients developed a stoical, philosophical approach in order to live with dyspnoea and the difficulties it imposed: this was also an important way of reducing the emotional impact of breathlessness. Patients' spouses suffered significantly, experiencing severe anxiety and helplessness as they witnessed their partners' suffering and felt powerless to reduce it. The restrictions imposed by breathlessness affected their lives profoundly. Support of all kinds, practical, medical, and psychosocial was highly valued but was provided inconsistently and sporadically. Where help was given it came most frequently from general practitioners (GPs, family physicians) and specialist respiratory nurses. Significance of results: This study is the first to document the psychosocial needs of carers, which are not adequately recognized or addressed at present. Patients and carers may feel most isolated and need support outside the working hours of most services and future provision needs to reflect this. Patients with cancer experience a more rapid onset of breathlessness. More clinicians need to be educated in the management of chronic breathlessness so known helpful strategies are more widely employed.

Abstract:Introduction: Individuals with pulmonary fibrosis can have complex symptoms. Early palliative care (PC) is recommended, though with little evidence on the balance of support between specialist respiratory and PC teams to deliver this in progressive disease. Aim: Assess delivery of symptom management in patients with severe lung function impairment at a UK ILD centre. Delivery of nurse support is examined against disease trajectory. Symptom management interventions are examined including support delivered via specialist PC teams. Methodology: A retrospective analysis of 71 patients initiating antifibrotic treatment with a baseline TLco <50%. Palliative care need was compared with disease trajectory based on stable or deteriorating TLco over a year. Data was derived from clinical records of the Oxford ILD service. Results: 41% of patients had support for breathlessness and cough, 32% breathlessness and 11% cough alone delivered by ILD nurses. 33% with deteriorating disease were referred to specialist PC. Referral rationale reflected broader patient need; initial PC plans focused on benefits advice, future planning, and occupational therapy. Specialist PC support enabled patient choice of place of death. Conclusion: Initial specialist PC focused on areas which transcend specific symptoms. Community PC support has a positive impact through to end-of-life. A joint ILD/PC nurse post and MDT will be created to enable specialist PC input at an earlier stage.

Holistic services for chronic breathlessness in people with advanced disease: a systematic review and meta-analysis

Abstract:Background: Breathlessness is a common, distressing symptom in people with advanced disease. Holistic services that draw on integrated palliative care have been developed for this group. Aim: To examine the components, outcomes, and recipients’ experiences of these services. Methods: Systematic review, searching nine databases to June 2017. Eligibility and quality were independently assessed by two authors. Data on service models, health and cost outcomes were synthesised, using meta-analyses as indicated. Data on patient/carer experiences were synthesised thematically. Results: Of 3,239 records identified, 37 studies of 18 different services were included. Most services enrolled people with thoracic cancer, involved palliative care staff, and comprised 4-6 contacts over 4-6 weeks. Common interventions were psychological support, and breathing and relaxation techniques. Meta-analyses demonstrated reductions in distress due to breathlessness (n=324; mean difference (MD) -2.30, 95%CI -4.43 to -0.16; p=0.03) and depression (n=408, MD -1.67, 95%CI -2.52 to 0.81, p<0.001) favouring the intervention. Statistically non-significant effects were observed for breathlessness mastery (n=282, MD 0.25, 95%CI -0.07 to -0.56, p=0.12) and anxiety (n=552, MD -1.59, 95%CI -3.22 to 0.05, p=0.06). There was no effect on health status or quality of life. Services were highly valued by recipients. Conclusion: Holistic services for chronic breathlessness in people with advanced disease lead to improvements in psychological aspects of breathlessness and health, and are acceptable to patients and carers. Services are heterogeneous: the key therapeutic components should be identified.

Abstract:Globally, engagement of patients in the self management of disease and symptom problems has become a health policy priority to improve health outcomes in cancer. Unfortunately, little attention has been focused on the provision of self-management support (SMS)in cancer and specifically for complex cancer symptoms such as breathlessness. Current management of breathlessness, which includes treatment of underlying disease, pharmacological agents to address comorbidities and opiates and anxiolytics to change perception and reduce the sense of breathing effort, is inadequate. In this perspective paper, we review the rationale and evidence for a structured, multicomponent SMS program in breathlessness including four components: breathing retraining, enhancing positive coping skills, optimizing exertional capacity and reducing symptom burden and health risks. The integration of SMS in routine lung cancer care is essential to improve breathlessness, reduce psychological distress, suffering and improve quality of life.

Abstract:Palliative Medicine, Ahead of Print. Background:Severe and refractory chronic breathlessness is a common and burdensome symptom in patients with advanced life-limiting disease. Its clinical management is challenging because of the lack of effective interventions.Aim:To provide practice recommendations on the safe use of pharmacological therapies for severe chronic breathlessness.Design:Scoping review of (inter)national guidelines and systematic reviews. We additionally searched for primary studies where no systematic review could be identified. Consensus on the recommendations was reached by 75% approval within an international expert panel.Data sources:Searches in MEDLINE, Cochrane Library and Guideline International Network until March 2023. Inclusion of publications on the use of antidepressants, benzodiazepines, opioids or corticosteroids for chronic breathlessness in adults with cancer, chronic obstructive pulmonary disease, interstitial lung disease or chronic heart failure.Results:Overall, the evidence from eight guidelines, 14 systematic reviews and 3 randomised controlled trials (RCTs) on antidepressants is limited. There is low quality evidence favouring opioids in patients with chronic obstructive pulmonary disease, cancer and interstitial lung disease. For chronic heart failure, evidence is inconclusive. Benzodiazepines should only be considered for anxiety associated with severe breathlessness. Antidepressants and corticosteroids should not be used.Conclusion:Management of breathlessness remains challenging with only few pharmacological options with limited and partially conflicting evidence. Therefore, pharmacological treatment should be reserved for patients with advanced disease under monitoring of side effects, after optimisation of the underlying condition and use of evidence-based non-pharmacological interventions as first-line treatment.

Abstract:BACKGROUND:Breathlessness is a common and distressing symptom, which increases in many diseases as they progress and is difficult to manage. We assessed the effectiveness of early palliative care integrated with respiratory services for patients with advanced disease and refractory breathlessness.METHODS:In this single-blind randomised trial, we enrolled consecutive adults with refractory breathlessness and advanced disease from three large teaching hospitals and via general practitioners in South London. We randomly allocated (1:1) patients to receive either a breathlessness support service or usual care. Randomisation was computer generated centrally by the independent Clinical Trials Unit in a 1:1 ratio, by minimisation to balance four potential confounders: cancer versus non-cancer, breathlessness severity, presence of an informal caregiver, and ethnicity. The breathlessness support service was a short-term, single point of access service integrating palliative care, respiratory medicine, physiotherapy, and occupational therapy. Research interviewers were masked as to which patients were in the treatment group. Our primary outcome was patient-reported breathlessness mastery, a quality of life domain in the Chronic Respiratory Disease Questionnaire, at 6 weeks. All analyses were by intention to treat. Survival was a safety endpoint. This trial is registered with ClinicalTrials.gov, number NCT01165034.FINDINGS:Between Oct 22, 2010 and Sept 28, 2012, 105 consenting patients were randomly assigned (53 to breathlessness support service and 52 to usual care). 83 of 105 (78%) patients completed the assessment at week 6. Mastery in the breathlessness support service group improved compared with the control (mean difference 0·58, 95% CI 0·01-1·15, p=0·048; effect size 0·44). Sensitivity analysis found similar results. Survival rate from randomisation to 6 months was better in the breathlessness support service group than in the control group (50 of 53 [94%] vs 39 of 52 [75%]) and in overall survival (generalised Wilcoxon 3·90, p=0·048). Survival differences were significant for patients with chronic obstructive pulmonary disease and interstitial lung disease but not cancer.INTERPRETATION:The breathlessness support service improved breathlessness mastery. Our findings provide robust evidence to support the early integration of palliative care for patients with diseases other than cancer and breathlessness as well as those with cancer. "It's like a forgotten issue sometimes …": Qualitative study of individuals living and caring for people with chronic breathlessness

Abstract:Introduction: This study aims to explore the perspectives of patients and carers with chronic breathlessness on current provision of care, care expectations, and self-management needs to develop relevant health services and resources to improve clinical outcomes. Methods: In-depth semistructured interviews were conducted on patients living with chronic breathlessness and carers. Results: Thirteen patients (cardiac, respiratory, and noncardiorespiratory) and two carers were interviewed (mean age 57 years, 47% female, median duration with breathlessness 5 years). Four main themes were identified: (1) living with breathlessness, (2) diagnosis delays, misdiagnosis, and knowledge gaps, (3) beyond curing disease: symptom relief and improving quality of life, and (4) self-management and limited support for it. Conclusion: Breathlessness has a high personal impact but remains a neglected condition in Australia. Patients suffer from lack of personal, community, and provider awareness, discontinuity of care, and too few clinical and self-management options.
Understanding facilitators and barriers to oxygen therapy for patients with interstitial lung disease

Abstract:Background: Interstitial lung disease (ILD) is comprised of a heterogeneous group of pulmonary diseases. Oxygen therapy is used in patients with advanced lung disease; however, there are challenges associated with initiation of oxygen therapy specific to individuals with ILD. The key objectives of this study were to create a common understanding of the facilitators and barriers to oxygen therapy for patients with ILD, and healthcare professionals (HCP) caring for patients with ILD. Methods: This qualitative study included 1 hour semistructured focus groups/interviews. An iterative and concurrent process was used for data collection and analysis to allow for supplementary development of themes and concepts generated. Data analysis used a three-phase approach: coding, categorising and development of themes. Results: A total of 20 patients and/or caregivers and 31 HCP took part in 34 focus groups/interviews held over 3 months (November 2022-January 2023). Facilitators to oxygen therapy were identified including support from HCP and support groups, the perseverance and self-advocacy of patients, a straightforward administrative process and vendors/private industry that expedite access to oxygen therapy. There were also several barriers to accessing oxygen therapy for patients with ILD. The themes identified include rural disparity, testing requirements and qualifying for funding and the need for ILD-specific evidence base for oxygen therapy. Conclusion: Further research is needed to facilitate development of specific exertional oxygen criteria for patients with ILD, to create supports for oxygen use and monitoring and to enable providers to tailor therapy to patients. Oxygen therapy education for ILD should address the benefits and risks of oxygen therapy.

Abstract:Dyspnea is a common and distressing symptom in patients with advanced malignant and non-malignant diseases. It is a subjective experience that can only be described by the patients themselves and can be associated with a massive reduction in quality of life, including social isolation and wish to hasten death. Often there is an affective component such as anxiety or panic. Objective parameters do not necessarily correlate with the subjective experience. Health professionals often underestimate and inadequately treat the burden of dyspnea. The introduction of the concept of chronic breathlessness syndrome or acute-on-chronic-breathlessness aims to illustrate the nature of the condition and facilitate the identification and access to appropriate treatment. The management of dyspnea is complex, and for effective treatment, a combination of general, non-pharmacological, and pharmacological measures is usually advisable. Opioids should be offered to patients with incurable cancer and refractory dyspnea for symptom relief. They can be supplemented with benzodiazepines in cases of concomitant anxiety. The administration of oxygen is only indicated in cases of hypoxemia. Key measures include education, self-management skills acquisition and advance care planning for emergency situations.
Multidisciplinary Care and Prognosis in Patients With COPD and Interstitial Lung Disease Prescribed Long-Term Oxygen Therapy

Abstract:BACKGROUND:Home oxygen therapy is prescribed for patients with advanced lung disease based on the criteria established in landmark trials in subjects with COPD. In clinical practice, its use has been extrapolated to other diseases, including interstitial lung disease (ILD). Patients with COPD and ILD experience a high symptom burden and require access to specialized multidisciplinary care. We aimed to evaluate the health-related outcomes and supportive care needs of patients with COPD and ILD receiving home oxygen therapy.METHODS:This was a retrospective cohort study using the oxygen database of a quaternary metropolitan teaching hospital. Patients with a diagnosis of COPD or ILD who were prescribed home oxygen therapy between January 2012–December 2018 were identified. Demographic information, results of physiologic testing, comorbidities, hospitalizations, and mortality data were collected.RESULTS:Three hundred and eighty-four subjects were included for analysis, of whom 56% were male. The median age was 75 y. The majority (59%) had a diagnosis of COPD. Long-term oxygen therapy (LTOT) was prescribed for 187 (48.7%), with no significant demographic differences between those with COPD or ILD. Another 187 were prescribed ambulatory oxygen alone, with 55 transitioning to LTOT during the study period. Most subjects (65.4%) were referred for pulmonary rehabilitation; however, palliative care referrals were generally low (22.9%). Referrals to other medical specialties and allied health were common (82%). Transplant-free survival after commencement of LTOT was poor, with 38% of subjects surviving at 5 y. The 5-y survival of subjects with ILD after commencing on LTOT was 10% compared to 52% for those with COPD. Multivariable Cox regression analyses showed that the only predictor of survival after commencing LTOT was the principal respiratory diagnosis.CONCLUSIONS:This study found that subjects prescribed LTOT had poor transplant-free survival after initiation, which was significantly worse for those with ILD compared to those with COPD. Despite their poor overall survival, worse than many cancers, only a minority were referred for palliative care input. Referrals to pulmonary rehabilitation were also suboptimal. This patient population had complex care needs requiring multidisciplinary management. Appropriate and early referrals to palliative care and improved care coordination for this complex group of patients are key areas for improvement in clinical practice.

Abstract:Breathlessness diminishes the physical, mental and social wellbeing of people living long-term with this disabling symptom [1]. Identifying its impacts on patients and their families helps to inform appropriate non-pharmacological and pharmacological management [2, 3]. A randomised controlled trial suggests that clinicians are less likely to identify or manage chronic breathlessness than chronic pain [4]. Previous population studies estimate 9.5% of adults experience breathlessness [5], with one in 100 individuals being seriously impacted daily [6]. We conducted a population study aimed at identifying the proportion of people with breathlessness who report this symptom in clinical consultations. If discussed, we explored whether patients or clinicians (physicians; nurses) initiated the conversation and, if not discussed, whether patients would welcome such discussions.

Abstract:The genesis of dyspnea involves the activation of several mechanisms that are mediated and perceived depending on previous experiences, values, emotions, and beliefs. Breathlessness may become unbearable, especially in patients who are terminally ill, whether afflicted by respiratory-, cardiac-, or cancer-related disorders, because of a final stage of a chronic process, an acute event, or both. Compared with pain, palliation of dyspnea has received relatively little attention in clinical practice and the medical literature. This is particularly true when the breathlessness is associated with acute respiratory failure because most of the studies on pharmacologic and nonpharmacologic treatments of respiratory distress have excluded such patients. Assessments of the quality of dying for patients in an ICU consistently show that few patients are considered by family members to breathe comfortably at the end of their life. This review focuses on the management of dyspnea in patients with advanced terminal illness, summarizing clinical trial evidence on pharmacologic and nonpharmacologic interventions available for these patients.

Abstract:Background Breathlessness is a common and distressing symptom of many advanced diseases, affecting around 2 million people in the UK. Breathlessness increases with disease progression and often becomes chronic or refractory. Breathlessness-triggered services that integrate holistic assessment and specialist palliative care input as part of a multiprofessional approach have been developed for this group, offering tailored interventions to support self-management and reduce distress. Objectives The aim was to synthesise evidence on holistic breathlessness services for people with advanced disease and chronic or refractory breathlessness. The objectives were to describe the structure, organisation and delivery of services, determine clinical effectiveness, cost-effectiveness and acceptability, identify predictors of treatment response, and elicit stakeholders’ evidence-based priorities for clinical practice, policy and research. Design The mixed-methods evidence synthesis comprised three components: (1) a systematic review to determine the clinical effectiveness, cost-effectiveness and acceptability of holistic breathlessness services; (2) a secondary analysis of pooled individual data from three trials to determine predictors of clinical response; and (3) a transparent expert consultation (TEC), comprising a stakeholder workshop and an online consensus survey, to identify stakeholders’ priorities. Results Thirty-seven papers reporting on 18 holistic breathlessness services were included in the systematic review. Most studies enrolled people with thoracic cancer, were delivered over 4–6 weeks, and included breathing training, relaxation techniques and psychological support. Meta-analysis demonstrated significant reductions in the Numeric Rating Scale (NRS) distress due to breathlessness, significant reductions in the Hospital Anxiety and Depressions Scale (HADS) depression scores, and non-significant reductions in the Chronic Respiratory Disease Questionnaire (CRQ) mastery and HADS anxiety, favouring the intervention. Recipients valued education, self-management interventions, and expertise of the staff in breathlessness and person-centred care. Evidence for cost-effectiveness was limited and inconclusive. The responder analysis (n = 259) revealed baseline CRQ mastery and NRS distress to be strong predictors of the response to breathlessness services assessed by these same measures, and no significant influence from baseline breathlessness intensity, patient diagnosis, lung function, health status, anxiety or depression. The TEC elicited 34 priorities from stakeholders. Seven priorities received high agreement and consensus, reflecting stakeholders’ (n = 74) views that services should be person-centred and multiprofessional, share their breathlessness management skills with others, and recognise the roles and support needs of informal carers. Limitations The evidence synthesis draws predominantly from UK services and may not be generalisable to other settings. Some meta-analyses were restricted by reporting biases and statistical heterogeneity. Conclusions Despite heterogeneity in composition and delivery, holistic breathlessness services are highly valued by recipients and can lead to significant improvements in the distress caused by breathlessness and depression. Outcomes of improved mastery and reduced distress caused by breathlessness are not influenced by patient diagnosis, lung function or health status. Stakeholders highlighted the need for improved access to person-centred, multiprofessional breathlessness services and support for informal carers. Future work Our research suggests that key therapeutic components of holistic breathlessness services be considered in clinical practice and models of delivery and educational strategies to address stakeholders’ priorities tested. Abstract:In healthy people, breathing is the most natural thing in the world. No need to think about it. No need to be concerned about it. It is not even the object of conscious perception. But when breathing becomes difficult, when it produces suffering, nothing else mattersa. Life discolours and shrinks around an act of breathing that has become elusive and uncertain, but pervasive. Disability ensues, which adds “a variety of adverse psychosocial, spiritual, or other consequences” to the respiratory-related physical limitations [1]. Respiratory suffering, be it called dyspnoea, breathlessness or by any other name, is therefore a major (and probably often the main) driver of impaired quality of life in patients afflicted with chronic respiratory diseases (and also cardiac diseases, neuromuscular diseases and severe obesity). To put things more bluntly, not being able to breathe freely is probably the worst thing that can happen to a human being. Dyspnoea has long been compared to pain [2] and has a lot of neurophysiological similarities with it [3, 4]. Yet in many ways dyspnoea is probably worse than pain. Indeed, acute dyspnoea goes hand in hand with fear, the fear of dying, which is not systematically the case with pain. And, not being a universal experience like pain, dyspnoea might be less susceptible than pain to induce reactions and empathy from those who witness it. Trained healthcare professionals dealing with respiratory distress on a daily basis fail to correctly evaluate the dyspnoea of their patients [5], and even though recent evidence suggests that vicarious dyspnoea does exist in a manner that resembles vicarious pain [6], the dyspnoea of chronic diseases tends to become invisible to caregivers [7]. This is perhaps because medical responses to dyspnoea are less codified and less efficient than responses to pain, but this phenomenon can only amplify the negative consequences of dyspnoea on the psychology of those experiencing it [7, 8]. Yet, as emphasised by Başoğlu [8] in a recent editorial, failure to enquire about, assess and properly treat breathlessness as outlined in specialist clinical guidelines is a breach of clinicians' ethical and legal duties to patients (see also [9]). On top of this clinical importance, respiratory suffering is the point of convergence and the final pathway of an array of diseases that at times have little in common and of which the specialists can have trouble understanding each other. In other words, dyspnoea is the “unifier” of respiratory medicine with all its diversity. For all these reasons, dyspnoea should be a foremost concern for all healthcare professionals, a primary criterion in clinical research, and the focus of specific multidisciplinary research efforts. Fortunately there are indications that this is becoming the case worldwide, and the European Respiratory Society (ERS) plays a significant role in this movement as attested to by a number of publications of all types in the European Respiratory Journal [1, 8, 10–14] and the European Respiratory Monograph series [15], and the endorsement by the ERS of the Dyspnea 2016 meeting organised by the International Dyspnea Society (www.dyspnea2016inparis.fr/sponsors/endorsement).
