Abstract:<h3>Background</h3><p>The broad spectrum of uveitis disorders requires a multimodal imaging approach in the daily practice of an ophthalmologist. As inflammatory conditions, they have in common an alteration in leukocyte migration. In this context, optical coherence tomography angiography (OCTA) might be of great value for diagnosing or following up patients with these disorders. To date, OCTA has rather been used as an additional tool besides the well-established diagnostic imaging tools, but its complementary diagnostic features become increasingly relevant, to follow disease activity and treatment response and for the understanding of pathomechanisms of various uveitis types.</p><p>This review summarizes the possible applications of OCTA and its advantages and disadvantages as opposed to dye-based angiographies in uveitic diseases.</p><h3>Main body</h3><p>Hitherto gold standards in the diagnostic workup of posterior or intermediate uveitis have been angiography on a dye-based method, which is fluorescein or indocyanine green. It gives information about the status of the blood-retinal barrier and the retinal and choroidal vasculature by visualizing diffuse leakage as a state of inflammation or complications as an ischemia or choroidal neovascularization. As noninvasive methods, fundus autofluorescence depicts the status of metabolic activity of the retinal pigment epithelium and OCT or enhanced depth imaging OCT, respectively, as a depth-resolving imaging method can supply additional information.</p><p>OCTA as a non-invasive, depth-resolution imaging tool of retinal and choroidal vessels adds detailed qualitative and quantitative information of the status of retinal and choroidal vessels and bridges the gap between the mentioned conventional diagnostic tools used in uveitis.</p><p>It is important, though, to be aware of its limitations, such as its susceptibility to motion artifacts, limited comparability among different devices, and restricted contribution of information regarding the grade of disease activity.</p><h3>Conclusion</h3><p>OCTA as a non-invasive, depth-resolution imaging tool can give qualitative and quantitative information about the status of retinal and choroidal vessels, but also has certain limitations. Employing OCTA as a complementary rather than exclusive tool, it can give important additional information about the macro- and microvasculature under inflammatory circumstances. Thereby, it also contributes to the understanding of the pathophysiology of various uveitis entities.</p>

Abstract:Ocular inflammatory diseases account for a significant number of blind registrations worldwide, involving predominantly young patients. Differential diagnosis of uveitis cases maybe challenging as they are infrequent and share common features. With the evolution of multimodal imaging several new signs of inflammation have been described facilitating prompt and effective diagnosis. Non‐invasive modalities including OCT, OCT‐A and Autofluorescence along with traditional diagnostic techniques such as fluoresceine and indocyanine green angiography provide plausible evidence of activity or remission of inflammation which can be used to determine further management and follow‐up. This lecture will highlight imaging features essential in efficacious monitoring of uveitis.

Abstract:Computer vision, understood as the area of science that trains computers to interpret digital images through both artificial intelligence (AI) and classical algorithms, has significantly advanced the analysis and interpretation of optical coherence tomography (OCT) in retina research. The aim of this review is to summarise the recent advances of computer vision in imaging processing in uveitis, with a particular focus in optical coherence tomography images.Literature review.The development of computer vision to assist uveitis diagnosis and prognosis is still undergoing, but important efforts have been made in the field.The automatising of image processing in uveitis could be fundamental to establish objective and standardised outcomes for future clinical trials. In addition, it could help to better understand the disease and its progression.

[The Application of Optical Coherence Tomography in Uveitis Patients].

Abstract:Uveitis is an inflammatory disease which involves the uvea, retina, retinal vessel and vitreous body. The disease mainly affects middle age and young people with a variety of clinical manifestations and is often recurrent. If not treated properly, it will cause visual impairment and even blindness. Spectral domain optical coherence tomography (SD-OCT) is a noninvasive examination which provides clear images quickly without contacting the eyes. As the development of OCT, we can obtain the sectional images with similar resolution as histological examination. Especially, when the enhanced depth image OCT (EDI-OCT) was used in clinical, the structures of choroid could also be detected. Therefore, OCT was used more and more in uveitis patients. Anterior segment OCT (AS-OCT) is used to evaluate the inflammation in anterior chamber. It also can provide the changes of iris and corneal structures during acute phase and recovery period. SD-OCT can clearly display many kinds of retinopathy in patients with uveitis including macular edema and the changes of retinal thickness. EDI-OCT can be used to observe the changes of retina and choroid at the same time. In particular, the measurement of choroidal thickness can be used to evaluate the choroidal changes and the degree of the response to treatment. Therefore, SD-OCT helps to diagnose uveitis and is easily used in the disease follow-up and treatment effect observation.

Abstract:Background: With an increased number of papers on how to interpret optical coherence tomography angiography (OCTA) findings in uveitis, the aim of this review is to assess its efficacy for the quantitative monitoring of structural and functional changes in inflamed conjunctiva and iris vessels in patients with acute anterior uveitis and iris neovascularization. Main body: OCTA, currently designed as a retinal vascular imaging system, has been recently adapted for anterior segment and showed good potential for successful imaging of the conjunctiva, the cornea, and the iris. OCTA can successfully delineate corneal vessels with substantial image quality. At the same time, it can detect changes in conjunctival and limbal vascularization and thus can be applied to pseudo-inflammatory conditions such as pterygium. Anterior segment OCTA allows analysis of iris vasculature and 3-D reconstruction of the normal iris vessels. OCTA can determined iris vessel filling defects or their flow increase, when present, secondary to inflammatory conditions. In addition, OCTA gives qualitative vessel density values that can be compared pre- and post-anti-inflammatory treatment. OCTA for imaging of the iris vasculature in health and disease is highly dependent on iris pigmentation. In both OCTA and fluorescein angiography, iris pigmentation causes vasculature imaging blockage, but OCTA provides more detailed iris vasculature images. Fine, clinically invisible iris vessels can be visualized by OCTA in the very early stages as well as in the regressed stage of NVI. Additional studies including different iris pathologies are needed to determine the most optimal scanning parameters in OCTA of the anterior segment. Conclusions: This review aims to establish the current application of OCTA to anterior segment disorders of the eye, with an emphasis on exploring its use in iris vessel dilation seen in various forms of iritis, as a predictive factor for further episodes of inflammation. In addition, OCTA can depict neovascularization of the iris secondary to proliferative diabetic retinopathy.

Abstract:Uveitis cases can be challenging in diagnosis, management, monitoring the treatment response and prognosis. The Multimodal imaging can provide us with key features findings in diagnostic dilemma cases, treatment response and prognosis response in severe refractive posterior uveitis cases. Fluorescein and Indocyanine Angiography have been the Gold Standard for many years along with the OCT expanding role in Uveitis in recent years (ex‐Bacillary Detachment, Acute Macular Neuropathy, White Dot Syndromes etc.). Fundus Autofluorescence (FAF), Enhanced Deep Imaging of the Choroid (EDI), OCT –Angiography inclusive Wide Field imaging play also a crucial role in the management of challenging Uveitis cases . Challenging Uveitis cases combined with complex Uveitic treatment highlight the importance of Multimodal imaging key role for all Uveitis specialist.

Quantitative optical coherence tomography angiography: A review

Abstract:As a new optical coherence tomography (OCT) modality, OCT angiography (OCTA) provides a noninvasive method to detect microvascular distortions correlated with eye conditions. By providing unparalleled capability to differentiate individual plexus layers in the retina, OCTA has demonstrated its excellence in clinical management of diabetic retinopathy, glaucoma, sickle cell retinopathy, diabetic macular edema, and other eye diseases. Quantitative OCTA analysis of retinal and choroidal vasculatures is essential to standardize objective interpretations of clinical outcome. Quantitative features, including blood vessel tortuosity, blood vessel caliber, blood vessel density, vessel perimeter index, fovea avascular zone area, fovea avascular zone contour irregularity, vessel branching coefficient, vessel branching angle, branching width ratio, and choroidal vascular analysis have been established for objective OCTA assessment. Moreover, differential artery–vein analysis has been recently demonstrated to improve OCTA performance for objective detection and classification of eye diseases. In this review, technical rationales and clinical applications of these quantitative OCTA features are summarized, and future prospects for using these quantitative OCTA features for artificial intelligence classification of eye conditions are discussed. Abstract:Analyses of images is of great importance to clinical diagnoses.This paper presents an image analysis method based on statistics.The analysis method has directions and can alter its power parameter to give prominence to emphases on demands.The analysis on the statistics parameters of OCT images of the burning and transition parts of artificial skins and its self-concrescence process states that the image analysis method based on statistics is completely feasible to describe the samples′ features.The description method based on statistics for OCT images is completely viable.

Abstract:Clinical imaging provided by optical coherence tomography (OCT) and its variant, OCT-angiography (OCT-A), has revolutionised eyecare practice. The imaging techniques allow for the identification and quantification of ocular structures, supporting the diagnosis and prognosis of eye disease. In this review, an overview of the usefulness of OCT-A imaging in the diagnosis and management of a range of ocular conditions is provided when used in isolation or in combination with other imaging modalities and measures of visual function (visual field results). OCT-A imaging has the capacity to identify and quantify ocular vasculature non-invasively, thereby assisting the clinician in the diagnosis or to determine the efficacy of intervention in ocular conditions impacting retinal vasculature. Thus, additional clinically useful information can be obtained in eye diseases involving conditions such as those impacting retinal vessel occlusion, in diabetic retinopathy, inherited retinal dystrophy, age-related macular degeneration, choroidal neovascularisation and optic nerve disorders. Through a clinical case series, various ocular conditions are reviewed, and the impact of OCT-A imaging is discussed. Although OCT-A imaging has great promise and is already used in clinical management, there is a lack of set standards to characterise altered vascular features in disease and consequently for prognostication, primarily due to a lack of large-scale clinical trials and variability in OCT-A algorithms when generating quantitative parameters.

Abstract:Noninfectious uveitis refers to a category of inflammatory diseases involving the uvea,vitreous,optic disk and retina,with the exception of infectious factors or masquerade syndrome.These kind of blinding diseases are frequently recurrent,and the diagnosis and follow-up require fundus imaging techniques.OCT angiography (OCTA) is a rapid,noninvasive and quantifiable blood flow imaging modality that provides a depiction of the microvasculature morphology of the retinal and choroidal through different segmentation and detects the abnormal blood perfusion as well as the neovascularization.OCTA plays an important role in the diagnosis,assessment and follow-up for anterior uveitis,posterior uveitis and pan-uveitis such as Vogt-Koyanagi-Harada disease,Beh(c)et's disease,ocular sarcoidosis,birdshot chorioretinopathy,serpiginous choroiditis,multifocal choroiditis,punctate inner choroidopathy,acute zonal occult outer retinopathy,acute posterior multifocal placoid pigment epitheliopathy,multiple evanescent white dot syndrome,and also provides clue about their pathophysiologic mechanisms.
OCTA-ReVA: an open-source toolbox for comprehensive retinal vessel feature analysis in optical coherence tomography angiography

Abstract:Optical coherence tomography angiography (OCTA) has significantly advanced the study and diagnosis of eye diseases. However, current clinical OCTA systems and software tools lack comprehensive quantitative analysis capabilities, limiting their full clinical utility. This paper introduces the OCTA Retinal Vessel Analyzer (OCTA-ReVA), a versatile open-source platform featuring a user-friendly graphical interface designed for the automated extraction and quantitative analysis of OCTA features. OCTA-ReVA includes traditional established OCTA features based on binary vascular image processing, such as blood vessel density (BVD), foveal avascular zone area (FAZ-A), blood vessel tortuosity (BVT), and blood vessel caliber (BVC). Additionally, it introduces new features based on blood perfusion intensity processing, such as perfusion intensity density (PID), vessel area flux (VAF), and normalized blood flow index (NBFI), which provide deeper insights into retinal perfusion conditions. These additional capabilities are crucial for the early detection and monitoring of retinal diseases. OCTA-ReVA demystifies the intricate task of retinal vasculature quantification, offering a robust tool for researchers and clinicians to objectively evaluate eye diseases and enhance the precision of retinal health assessments.
Quantitative Analysis of Anterior Chamber Inflammation Using the Novel CASIA2 Optical Coherence Tomography

Abstract:PURPOSE: We evaluated the clinical utility of a novel anterior segment optical coherence tomography (AS-OCT) device, CASIA2, to evaluate parameters indicative of anterior chamber (AC) inflammation severity in uveitis, including AC cell number, flare, and keratic precipitates (KPs). DESIGN: Prospective evaluation of a diagnostic device. METHODS: Uveitis eyes were classified into active and inactive groups. The number of hyperreflective dots representing AC cells and optical density ratio (aqueous-to-air relative intensity [ARI] index) for flare qualification were calculated from AS-OCT images. In addition, a program was designed to quantify the posterior corneal surface smoothness (PCSS) of each image for KPs evaluation. The maximum, minimum, and average PCSS values were calculated from 128 images per eye and compared among active uveitis, inactive uveitis, and control eyes. Correlations between Standardization of Uveitis Nomenclature grade and both hyperreflective dot number and ARI index were evaluated. Receiver operating characteristic (ROC) curves were constructed to test the values of these indicators for uveitis diagnosis. RESULTS: AC hyperreflective dot count, ARI index, and maximum and average PCSS values were all significantly higher in the active uveitis group than in the inactive and control groups. Hyperreflective dot count and ARI index were associated with Standardization of Uveitis Nomenclature cell and flare grade. According to ROC curve analysis, maximum PCSS was the best indicator for the diagnosis of uveitis involving the anterior segment, meanwhile the hyperreflective dot number was the best to identify active AC inflammation from the inactive. CONCLUSIONS: Quantification of AC cell number, flare, and KPs using the CASIA2 device is a promising strategy for the objective assessment of AC inflammation. (C) 2020 Elsevier Inc. Structural Imaging of Benign Conjunctival Proliferative Diseases Using Optical Coherence Tomography

Abstract:Objective: To use anterior segment optical coherence tomography (AS-OCT) for structural imaging and accurate measurements of pinguecula and pterygium in order to understand the structural features of these lesions. Methods: This study was a prospective parallel control study. Lateral bulbar conjunctiva of 25 cases (30 eyes) that included pinguecula (10 eyes of 8 cases), pterygium (10 eyes of 9 cases), and normal eyes (10 eyes of 8 cases), were enrolled from the Department of Ophthalmology of the Fourth Affiliated Hospital of Zhejiang Medical University from January to April 2018. The anterior segment module of an Optovue optical coherence tomography angiography (OCTA) was used for measurements. The thickness of the bulbar conjunctiva epithelium and propria tissues at sites at a distance of 0.0 mm, 0.5 mm, 1.0 mm, 1.5 mm, 2.0 mm and 2.5 mm from the scleral process were stratified within the 2.5 mm range, were measured, compared and further analyzed. The data were described by means of mean±standard deviation. Repeated measurement analysis of variance was used for comparison of each point in the group, one-way analysis of variance was used for comparison between groups. Results: Compared with normal eyes and pinguecula eyes, pterygium showed thickened epithelium in 0.5 mm point (P<0.05). In the 2.5 mm range on the nasal side, pterygium and pinguecula showed thickened subepithelium tissues (P<0.05), especially for pterygium, compared to normal eye tissue. Conclusions: AS-OCT can image microstructures in benign conjunctival proliferative diseases, resulting in accurate quantitative diagnosis and evaluation. The thickness of subepithelium tissues increases in pinguecula and pterygium.

Progress in Retinal and Eye Research

Abstract:The recent introduction of optical coherence tomography angiography (OCTA) has remarkably expanded our knowledge of the choroid through in vivo investigation of the anatomical and pathological features of this important vascular layer. New insights elucidating the morphological features of the choroid, in both physiological and pathological conditions, indicate that this vascular structure plays a crucial role in many chorioretinal disorders. In this article, a review of the salient histological and anatomical features of the choroid, essential for the proper interpretation of in vivo imaging, is followed by a discussion of the fundamental principles of OCTA and the application of this advanced imaging modality to study and understand the choroid. The current limitations of OCTA and potential advancements that may improve the widespread adoption of this tool are also discussed. A detailed review of the OCTA features of the choroid in the healthy eye is followed by relevant findings in major chorioretinal diseases, including age-related macular degeneration, central serous chorioretinopathy, uveitis, and inherited retinal disorders.

Abstract:Optical coherence tomography angiography (OCTA) is a revolutionary method in the visualization of the vascular system in different retinal and choroidal layers. During the last 4 years since the commercial availability of different OCTA devices, attempts have been made to utilize this technology in various aspects of ocular oncology from the differentiation of benign and malignant lesions to assisting in evaluation of post-treatment complications, such as radiation retinopathy. However, current OCTA technology is restricted by various artefacts and inherent limitations, some of which are more pronounced in the presence of elevated tumoural lesions. Imminent advancements in OCTA systems and image acquisition processes promise a great potential for application of OCTA in ocular oncology.

Abstract:Optical Coherence Tomography (OCT) is one of the most emerging imaging modalities that has been used widely in the field of biomedical imaging. From its emergence in 1990's, plenty of hardware and software improvements have been made. Its applications range from ophthalmology to dermatology to coronary imaging etc. Here, the focus is on applications of OCT in ophthalmology and retinal imaging. OCT is able to non-invasively produce cross-sectional volume images of the tissues which are further used for analysis of the tissue structure and its properties. Due to the underlying physics, OCT images usually suffer from a granular pattern, called speckle noise, which restricts the process of interpretation, hence requiring specialized noise reduction techniques to remove the noise while preserving image details. Also, given the fact that OCT images are in the $\mu m$ -level, further analysis in needed to distinguish between the different structures in the imaged volume. Therefore the use of different segmentation techniques are of high importance. The movement of the tissue under imaging or the progression of disease in the tissue also imposes further implications both on the quality and the proper interpretation of the acquired images. Thus, use of image registration techniques can be very helpful. In this work, an overview of such image analysis techniques will be given.

Abstract:Similar to ultrasound adapting soundwaves to depict the inner structures and tissues, optical coherence tomography (OCT) utilizes low coherence light waves to assess characteristics in the eye. Compared to the previous gold standard diagnostic imaging fluorescein angiography, OCT is a noninvasive imaging modality that generates images of ocular tissues at a rapid speed. Two commonly used iterations of OCT include spectral-domain (SD) and swept-source (SS). Each comes with different wavelengths and tissue penetration capacities. OCT angiography (OCTA) is a functional extension of the OCT. It generates a large number of pixels to capture the tissue and underlying blood flow. This allows OCTA to measure ischemia and demarcation of the vasculature in a wide range of conditions. This review focused on the study of four commonly encountered diseases involving the retina including age-related macular degeneration (AMD), diabetic retinopathy (DR), central serous chorioretinopathy (CSC), and macular telangiectasia (MacTel). Modern imaging techniques including SD-OCT, TD-OCT, SS-OCT, and OCTA assist with understanding the disease pathogenesis and natural history of disease progression, in addition to routine diagnosis and management in the clinical setting. Finally, this review compares each imaging technique's limitations and potential refinements.

Vitreoretinal Interface Cells Correlate In Vivo With Uveitis Activity and Decrease With Anti-Inflammatory Treatment

Abstract:Purpose: To highlight the utility of en face swept-source optical coherence tomography angiography (SS-OCTA) in assessing vitreoretinal interface cells (VRICs) of patients with active uveitis and their dynamics. Methods: In this prospective, single-center study, 20 eyes from patients with active uveitis were analyzed using six 6 × 6-mm macular scans at three time points: active inflammation (baseline), clinically improving (T1), and resolved inflammation (T2). VRICs were visualized using 3-µm en face OCT slabs on the inner limiting membrane. The variation of VRIC number, density, and size over time was assessed, and VRIC measurements were compared with clinical grading. Results: At baseline, the VRIC count was significantly higher (552.5 VRICs) than that of the healthy controls (478.2 VRICs), with a density of 15.3 cells/mm2. VRIC number decreased significantly to 394.8 (P = 0.007) at T1, with a density of 10.9 cells/mm2 (P = 0.007). VRIC size reduced from 6.8 µm to 6.3 µm at T1 (P = 0.009) and remained stable at T2 (P = 0.3). Correlation coefficients between inflammatory parameters (anterior chamber cells and National Eye Institute vitreous haze), and VRIC count indicated a positive correlation at baseline (r = 0.53), weakening at T1 (r = 0.36), and becoming negative at T2 (r = -0.24). Conclusions: En face SS-OCTA revealed increased VRIC number and size in active uveitis, likely due to monocyte recruitment. Post-inflammation control, VRIC number, size, and density significantly decreased, returning to normal despite residual anterior chamber cells or vitreous haze. Translational relevance: Visualization of VRICs by in vivo OCT opens up new opportunities for therapeutic targets.
