Abstract:Three-dimensional information is essential for a proper understanding of the healing potential of the menisci and their overall role in the knee joint. However, to date, the study of meniscal vascularity has relied primarily on two-dimensional imaging techniques. Here we present a method to elucidate the intricate 3D meniscal vascular network, revealing its spatial arrangement, connectivity and density. A polymerizing contrast agent was injected into the femoral artery of human cadaver legs, and the meniscal microvasculature was examined using micro-computed tomography at different levels of detail and resolution. The 3D vascular network was quantitatively assessed in a zone-base analysis using parameters such as diameter, length, tortuosity, and branching patterns. The results of this study revealed distinct vascular patterns within the meniscus, with the highest vascular volume found in the outer perimeniscal zone. Variations in vascular parameters were found between the different circumferential and radial meniscal zones. Moreover, through state-of-the-art 3D visualization using micro-CT, this study highlighted the importance of spatial resolution in accurately characterizing the vascular network. These findings, both from this study and from future research using this technique, improve our understanding of microvascular distribution, which may lead to improved therapeutic strategies.

What problem does this paper attempt to address?
The problem that this paper attempts to solve is to reveal the complexity of the microvascular network of the knee meniscus through advanced 3D imaging techniques, especially its spatial arrangement, connectivity and density. Specifically, the researchers hope: 1. **Develop a method**: Use polymeric contrast agents and micro - computer tomography (micro - CT) technology to perform detailed imaging and analysis of the meniscus microvascular network in human cadaver legs. 2. **Quantitative analysis**: Reveal the vascular distribution characteristics within the meniscus by quantitatively evaluating parameters such as blood vessel diameter, length, tortuosity and branching pattern in different regions. 3. **Improve understanding**: Accurately describe the structure of the vascular network through high - resolution 3D visualization technology, so as to better understand the microvascular distribution of the meniscus, which may help improve treatment strategies. ### Background - **Importance of the meniscus**: The meniscus plays a crucial role in maintaining the health and function of the knee joint. Its blood supply is essential for the regenerative capacity of the meniscus. - **Limitations of existing research**: Current research on meniscus blood vessels mainly relies on two - dimensional imaging techniques, which cannot fully reveal the three - dimensional structure of blood vessels. - **Differences in vascular distribution**: Different regions of the meniscus (such as the red - red zone, red - white zone and white - white zone) have different vascular densities and distribution characteristics, and these differences affect the healing potential of the meniscus. ### Research objectives - **Explore feasibility**: Verify the feasibility of using advanced 3D imaging techniques (such as micro - CT) to map and visualize the human meniscus microvascular network. - **Quantitative analysis**: Analyze the regional characteristics of the 3D meniscus microvascular network by defining different quantitative parameters (such as vascular density, branching pattern, average diameter, length and tortuosity). ### Methods - **Sample preparation**: Use Thiel - fixed cadaver leg samples and inject the polymeric contrast agent µAngiofil through the femoral artery. - **Imaging technology**: Use the EasyTom XL Ultra 230–160 micro/nano - CT scanner for imaging, and scan the samples at low, medium and high resolutions respectively. - **Image processing**: Use Fiji and Avizo software for image segmentation and 3D modeling, extract the center line of the vascular network, and calculate various vascular parameters. ### Results - **Vascular segmentation**: The vascular network was successfully segmented by combining the maximum entropy algorithm and the white top - hat operation. - **3D imaging**: The detailed structure of the meniscus and its vascular network was demonstrated through 3D visualization technology. - **Analysis of vascular parameters**: It was found that the perimeniscal zone of the meniscus has the highest contribution to the vascular volume, while the vascular density in the medial region is lower. - **Statistical analysis**: The significant differences in vascular parameters in different regions were verified by one - way ANOVA and Tukey - Kramer test. ### Significance - **Improve understanding**: The microvascular distribution of the meniscus was more accurately described through high - resolution 3D imaging technology, providing an important reference for future clinical treatment. - **Potential applications**: These findings may help develop new treatment strategies, especially in young patients, to prevent or delay the occurrence of osteoarthritis by promoting meniscus regeneration. In conclusion, this paper systematically reveals the complexity of the meniscus microvascular network through advanced 3D imaging techniques, providing a new perspective for understanding the physiological and pathological mechanisms of the meniscus.
