Abstract:Objective To discuss and summarize the clinical presentation,pathological fea-ture,diagnosis and treatment of primary renal synovial sarcoma. Methods We report one case of primary renal synovial sarcoma,and relevant literature was reviewed. Results A 50-year-old woman presented with backache and abdominal distension for one day,the CT scan showed a large mass of approximately 22 cm × 21 cm × 15 cm in the abdomen and retroperitoneum which was considered as tumor rupture.Subsequently this patient received the emergency exploratory laparoto-my and left radical nephrectomy,a total of 2 600 ml blood was removed during the surgery.On his-topathological examination,spindle cell was observed and finally it was diagnosed with monophasic primary renal synovial sarcoma. Conclusions Primary renal synovial sarcoma is a rare tumor with poor prognosis and difficult preoperative diagnosis.The definitive diagnosis depends on immunohisto-chemistry and genetic analysis.Surgery is the mainstay of treatment,adj uvant chemotherapy and ra-diotherapy were still controversial.

Abstract:Objective To investigate the diagnosis and treatment of primary renal synovial sarcoma.Methods The data of 2 patients with primary renal synovial sarcoma were retrospectively analyzed,and the related literature in Pubmed and CBMdisc was retrieved.Results The diagnosis of primary renal synovial sarcoma was confirmed by postoperative pathology.Immunohistochemical staining showed that for the first patient,the markers Vim,CK7,CK(Pan),CK(Low MW),EMA,CD99 and Bcl-2 were positive in the tumor cells,while Des was negative.For another patient,the marker Vim was positive,CK,Des and CD99 were partial positive,while Myod1,CD34,Melanoma and CD68 were negative.Both cases died of tumor metastasis 7 or 10 months after the operation.Retrieved literature showed that of the 33 cases followed up,11 cases died and 14 cases had tumor metastasis or recurrence.Conclusions Primary renal synovial sarcoma is a highly malignant and rarely seen tumor with poor prognosis.Its diagnosis depends on pathological and immunohistochemical examinations.

Abstract:This report describes the clinical, histological and immunohistochemical features of two patients with primary pulmonary synovial sarcoma in the context of the literature. Chest pain, cough, haemoptysis and an enlarging pleural-based mass are the main clinical manifestations. Diagnosis depends on identifying epithelioid or spindle cells microscopically and on immunohistochemistry showing positivity for cytokeratin and vimentin and epithelial membrane antigen stains. Surgical excision is the main treatment approach.

Abstract:Synovial sarcoma is a soft tissue malignancy of unknown origin which usually occurs around the joints of the extremities, but rarely detected in the kidneys. A case of primary synovial sarcoma of the kidney was reported, which was preoperatively diagnosed as a malignant tumor of the right kidney for intermittent gross hematuria. After the laparoscopic radical nephrectomy, primary renal synovial sarcoma was confirmed by pathological examination. The patient refused further treatment, and there was no recurrence or metastasis during the 13 months of follow-up.

Abstract:OBJECTIVE:To investigate the clinical characteristics,prognosis, survival and diagnosis of primary renalsarcoma. METHODS:In the study,17 cases of renal sarcoma were reviewed and analyzed, the incidence, clinical manifestations, treatments and prognosis of renal sarcoma discussed and analyzed, and the correlated literature also reviewed. RESULTS:Radical nephrectomy was the main treatment for 16 cases, one case was punctured and pathologized. The distributions of pathology were leiomyosarcoma, malignant fibrous histiocytoma, rhabdomyosarcoma, synoviosarcoma, liposarcoma, and clear-cell sarcoma. Thirteen of the subjects were followed-up, and the time of survival varied from 7 to 132 months. CONCLUSION:Renal sarcomas are rare and highly malignant and the prognosis is poor. Early diagnosis and radical nephrectomy can prolong the patient's life.
Renal rhabdoid synovial sarcoma: report of a case with literature review

Abstract:Purpose To report a rare case of renal rhabdoid synovial sarcoma and review the literature,in order to improve the realization for this disease and reduce misdiagnosis.Method The clinicopathological data of 1 case rhabdoid renal synovial sarcoma were retrospectively analyzed.The tumors were examined by immunohistochemical of EnVision two-step staining and FISH,the related literatures were reviewed.Result A 31-year-old male patient accepted the right kidney radical operation in November 2014 after imaging examination of right kidney tumor.Microscopically,the tumor cells showed short spindle cells with rich cytoplasm and eosinophilic bodies in the cytoplasm.The pathological diagnosis is the renal rhabdoid tumor for this time.The patient was found a tumor between the liver and the diaphragm by imaging examination in October 2015.The second operation was carried out successfully.Microscopically,the tumor cells were spindle with little cytoplasm and without eosinophilic bodies in the cytoplasm.It was a typical synovial sarcoma in morphology for this time.Immunohistochemical staining showed positive for vimentin,EMA,CD56,and TLE1,SS18SSX fusion gene was disclosed in the primary and recurrent tumor cells,it was therefore corrected as rhabdoid synovial sarcoma for the primary tumor.Conclusion Renal rhabdoid synovial sarcoma is rare.Renal primary rhabdoid synovial sarcoma is easily misdiagnosed as renal rhabdoid tumor.The renal rhabdoid synovial sarcoma has broadened the differential diagnosis of renal rhabdoid tumors spectrum.Even for a tumor with typical rhabdoid morphology,molecular biology method for differential diagnosis is needed.SS18-SSX fusion gene is the basis for diagnosis of synovial sarcoma.

Abstract:To assess epidemiology, clinical presentation, treatment and overall survival of adult patients with renal sarcomas, the 2004–2016 SEER and NCDB databases were queried for adult patients diagnosed with renal sarcoma, calculating average annual age-adjusted incidence rates (AAIR) and average annual percentage change (AAPC) as well as overall survival (OS). In n = 1279 included renal sarcoma patients, AAIR remained constant over the study period (average 0.53 cases/1million; AAPC = 0.7, p = 0.6). Leiomyosarcoma (AAIR 0.14 cases/1 million) and malignant rhabdoid tumors (0.06 cases/1 million) were most common. Sarcoma histiotypes demonstrated considerable heterogeneity regarding demographic and cancer-related variables. Patients presented with advanced local extent (T3 33.3%; T4 14.2%) or distant metastases (29.1%) and commonly underwent surgical resection (81.6%). Longer OS was independently associated with younger age, female sex, lower comorbidity index, low T stage, negative surgical margins, absence of tumor necrosis or distant metastases and leiomyosarcoma histiotype (multivariable p < 0.05 each). Treatment efficacy varied according to sarcoma histiotype (interaction p < 0.001). Accounting for 0.25% of renal malignancies, renal sarcomas include 43 histiotypes with distinct epidemiology, clinical presentation, outcomes and sensitivity to systemic therapy, thereby reflecting soft-tissue sarcoma behavior. Renal sarcoma treatment patterns follow recommendations by renal cancer guidelines with surgical resection as the cornerstone of therapy.

Abstract:Synovial cell sarcoma is a soft-tissue tumor with a unique t (X; 18) (p11.2; q11.2) chromosomal translocation. Generally, the tumor occurs in the proximity of joints of young adults with a poor prognosis. Primary synovial cell sarcoma arising from the kidney is extremely rare. Here we report 2 patients who were initially considered as renal cell carcinoma but were finally diagnosed as primary renal synovial cell sarcoma by histopathology and molecular studies. Urologists should be aware of the possibility of malignancy in cystic renal masses and consider a diagnosis of renal synovial cell sarcoma. The differential diagnosis is difficult, and molecular or genetic analysis should be applied for the diagnosis.
Primary Synovial Sarcoma Of The Kidney: A Rare And Easily Misinterpreted Renal Entity

Abstract:Primary synovial sarcomas of kidney are very rare, present a diagnostic dilemma. In this study, we reported clinicopathologic, immunohistochemical, and genetic characteristics of 12 cases of primary renal synovial sarcoma (SS). The ages of the 12 patients ranged from 21-52 years. Microscopically, 8 cases were monophasic type and 4 cases were biphasic type. The epithelial components of biphasic type SS were positive for AE1/AE3 (4/4) and EMA (4/4). Monophasic synovial sarcomas were focally positive for AE1/AE3 (3/8) and EMA (5/8). In all 12 cases, the spindle cell components were diffusely positive for vimentin (100.0%), BCL2 (83.3%), and CD99 (33.3%). CD10, CD34, SMA, and desmin were negative in all cases. The SS18 gene was rearranged in 10 cases. The SS18-SSX fusion gene was identified in another 2 cases. Clinical outcome data were available for 8/12 (66.7%) patients. Follow-up revealed that 5 patients (62.5%) died of the disease within 11 to 37 months after diagnosis (mean, 20.6 mo). Three patients (37.5%) were alive without evidence of disease 1 to 23 months after diagnosis (mean, 9 mo). We concluded that primary renal synovial sarcoma is a rare, aggressive tumor with poor prognosis that occurs over a wide age range but primarily in young adults and is morphologically similar to embryonal and mesenchymal neoplasms of the kidney. Adequate sampling, immunohistochemical staining, and specific fusion gene detection are essential to establish an accurate diagnosis.

Abstract:We experienced two cases of renal primary synovial sarcoma. Case 1: A 29-year-old man underwent laparoscopic radical nephrectomy and was originally diagnosed with renal cell carcinoma. Case 2: A 25-year-old man was treated by open radical nephrectomy since radiographical findings indicated tumor invasion to the ureter causing hydronephrosis. Both cases were pathologically diagnosed as renal synovial sarcomas, and were followed using computed tomography. Recurrence was observed within a year in both cases.

Abstract:Synovial sarcomas (SS) represent a unique subset of soft tissue sarcomas (STS) and account for 5–10% of all STS. Synovial sarcoma differs from other STS by the relatively young age at diagnosis and clinical presentation. Synovial sarcomas have unique genomic characteristics and are driven by a pathognomonic t(X;18) chromosomal translocation and subsequent formation of the SS18:SSX fusion oncogenes. Similar to other STS, diagnosis can be obtained from a combination of history, physical examination, magnetic resonance imaging, biopsy and subsequent pathology, immunohistochemistry and molecular analysis. Increasing size, age and tumor grade have been demonstrated to be negative predictive factors for both local disease recurrence and metastasis. Wide surgical excision remains the standard of care for definitive treatment with adjuvant radiation utilized for larger and deeper lesions. There remains controversy surrounding the role of chemotherapy in the treatment of SS and there appears to be survival benefit in certain populations. As the understanding of the molecular and immunologic characteristics of SS evolve, several potential systematic therapies have been proposed.

Abstract:Synovial sarcoma is a rare sarcoma with an incidence of 3/10e6/yr affecting all ages. Synovial sarcomas present with translocations affecting SS18 and SSX1, SSX2, SSX4 genes, whose protein products affects the transcriptional program of the cancer cell. Treatment of synovial sarcoma in localized phase follows the usual rules of sarcoma treatment, with uncertainties on the value of systemic treatment. In advanced phase, the disease is rarely curable, with current systemic treatments. Immunotherapy with autologous modified T cell recognizing MAGE-A4 or NY-ESO-1 have provided high response rates in recent reports. Synovial sarcoma (SS) is a rare and aggressive disease that accounts for 5%-10% of all soft tissue sarcomas. Although it can occur at any age, it typically affects younger adults and children, with a peak incidence in the fourth decade of life. In >95% of cases, the oncogenic driver is a translocation between chromosomes X and 18 that leads to the formation of the SS18::SSX fusion oncogenes. Early and accurate diagnosis is often a challenge; optimal outcomes are achieved by referral to a specialist center for diagnosis and management by a multidisciplinary team as soon as SS is suspected. Surgery with or without radiotherapy and/or chemotherapy can be effective in localized disease, especially in children. However, the prognosis in the advanced stages is poor, with treatment strategies that have relied heavily on traditional cytotoxic chemotherapies. Therefore, there is an unmet need for novel effective management strategies for advanced disease. An improved understanding of disease pathology and its molecular basis has paved the way for novel targeted agents and immunotherapies that are being investigated in clinical trials. This review provides an overview of the epidemiology and characteristics of SS in children and adults, as well as the patient journey from diagnosis to treatment. Current and future management strategies, focusing particularly on the potential of immunotherapies to improve clinical outcomes, are also summarized.

Abstract:Objective To investigate the clinical manifestations,imaging and pathological features and treatment prognosis of primary renal sarcoma in adults.Methods A retrospective analysis was performed on the clinical data of 48 patients with primary renal sarcoma from January 2009 to December 2018 in the first affiliated hospital and cancer center of Sun Yat-Sen university.There were 30 males and 18 females.Their aged ranged from 27 to 76 yrs with an average age of 50 yrs.A total of 24 patients presented with lumbar and abdominal pain.Abdominal mass was found in 2 cases.Gross hematuria was noticed in 4 cases.Febrile was recorded in 2 cases and 21 cases were diagnosed by physical examination.31 tumors located in the left kidney and 17 tumors located in the right kidney.The tumor diameter ranged from 3 to 16 cm with an average diameter of 8 cm.All patients underwent ultrasound or CT/MRI examination of the urinary system before surgery.The ultrasound showed the undistributed echo inside the tumor with the undistinguished border.The image of necrosis and liquefaction could be seen in some cases.The CT/MRI examination showed the lesion site with the necrosis,liquefaction or cystic changing.The mass exhibited the unregular enhancement with undistinguished border line.The mean diameter of liposcarcoma was 10.2 cm.CT scan demonstrated the relative low density of tumor,which was hard to be identified with AML.The average diameter of leiomyosarcoma was 6.5 cm.The enhanced CT scan showed the low density of tumor,compared with renal parenchyma.46 patients underwent radical nephrectomy,2 patients underwent renal tumor biopsy.And postoperative follow-up was performed.Results Pathological diagnosis revealed that 19 cases with liposarcoma,9 cases with leiomyosarcoma and 4 cases with synovial sarcoma,especially 4 cases with Ewing' s sarcoma and 12 cases with other sarcoma.36 cases were followed up and survived for 4 to 64 months.The average survival time was 28 months.The longest mean survival time was seen in patients with liposarcoma,which was 32 months (ranging 11 to 64 months).The mean survival time of synovial sarcoma group was 25 months (ranging 5-58 months).The mean survival time of Ewing's sarcoma group was 22 months(ranging 12-46 months).and the survival time of leiomyosarcoma group was the shortest 20 months (ranging 4-36 months).Conclusion Renal sarcoma is rare and highly malignant.It needs to be diagnosed with clinical manifestations,imaging and pathological data together.

Abstract:While the overall prognosis is generally quite satisfactory in children, adolescents and young adults with localised synovial sarcoma at first diagnosis, the outcome remains poor for patients after relapse. Conversely to the front-line standardised treatment options, patients with relapse generally have an individualised approach and to date, there is still a lack of consensus regarding standard treatment approaches. Studies on relapsed synovial sarcoma were able to identify some prognostic variables that influence post-relapse survival, in order to plan risk-adapted salvage protocols. Treatment proposals must consider previous first-line treatments, potential toxicities, and the possibility of achieving an adequate local treatment by new surgery and/or re-irradiation. Effective second-line drug therapies are urgently needed. Notably, experimental treatments such as adoptive engineered TCR-T cell immunotherapy seem promising in adults and are currently under validation also in paediatric patients.

Abstract:Objective:To raise awareness of primary retroperitoneal synovial sarcoma by investigating the chni cal manifestations,pathological characteristics and treatment.Method:We described a retrospective study of a case undergoing diagnostic evaluation and surgery for primary retroperitoneal synovial sarcoma with pregnancy.And a clinical analysis was performed with literature.Result:The patient was a seven-month pregnant woman,and color doppler ultrasound showed mixed echo clumps in retroperitoneal space.MRI showed mixed long T1 and long T2 signal.Then the patient underwent radical resection.By the naked eyes we could see that kidney and adrenal gland were surrounded by tumor,and the kidney capsule was intact.We see microscopically spindle cell tumor hyperplasia with mucous degeneration.Immunohistochemistry showed Bcl2,CD99,WT1,CD56,CD57 were positive.Molecular pathology detected t(18q,v)SYT translocation genetic abnormalities.According to microscopy,immunohistochemical,and molecular neuropathology the pathological diagnosis was primary retroperitoneal synovial sarcoma.TOMO therapy and MAID chemotherapy were performed after hospital discharge.At 18 months postoperatively,she showed no evidence of residual disease or distant metastases.Conclusion:Primary retroperitoneal synovial sarcoma is a very rare,highly malignant sarcoma with quick progression,common recurrence and metastasis,poor prognosis and short survival.Early diagnosis and intraoperative complete resection with postoperative chemoradiotherapy will benefit patients.

Abstract:Primary synovial sarcoma of the kidney is rare and difficult to diagnose with 100% accuracy without the use of up-to-date histopathologic methods. Immunohistochemical procedures are well established and are continuously expanding and improving. Currently, these methods are successful in up to 90% of tumor identification. The remaining cases will ultimately benefit by combining immunohistochemistry with tumor-specific genetic marker identifiers, the latter of which are increasing in availability for tumor diagnosis. Abstract:Two cases of primary prostatic synovial sarcoma presenting as a prostatic mass are presented in patients aged 44 and 46 years. Histologically, both tumors were mainly composed of uniform spindle cells forming interlacing fascicles. Clusters of immature epithelioid cells were also observed among the spindle cells in case 1. Immunohistochemically, the tumor cells of both cases were strongly positive for vimentin, bcl-2, CD99, and E-cadherin, as well as focally positive for cytokeratin. However, they were negative for prostate-specific antigen, S-100 protein, CD34, CD117, muscle-specific actin, desmin, and calretinin. The presence of an SYT-SSX gene fusion resulting from t(X;18) was demonstrated from paraffin blocks by reverse transcriptase polymerase chain reaction in both cases. To the authors' knowledge, these represent the fifth and sixth reported cases of prostatic synovial sarcoma. Accurate diagnosis depends on morphologic and immunohistochemical examination and proper molecular analysis.
