Pharmacological inhibition of IL-6-trans-signaling protects from vascular remodeling and pulmonary hypertension in two independent models of perinatal inflammation

Abstract:Background: Perinatal insults lead to aberrant lung and vascular development and are risk factors for pulmonary hypertension (PH). Our previous studies showed that different injuries, e.g. high oxygen concentrations, malnutrition or perinatal obesity, induce perinatal inflammation with high IL-6 and disrupt lung structural maturation. In two models of perinatal inflammation we now tested, if inhibition of IL-6-trans and cis-signaling protects from vascular remodeling and PH. Methods: Neonatal mouse models: (i) hyperoxia-(85% O2, HYX) based bronchopulmonary dysplasia; (ii) high-fat diet-induced maternal and perinatal obesity (HFD). In both models, we tested IL-6 null mice or pharmacological inhibition of IL-6 cis- and trans-signaling by anti-IL-6 monoclonal antibody or soluble gp130Fc, respectively. Results: Blockade of IL-6-trans-signaling and IL-6 null mice preserved alveolar formation and protected from vascular remodeling and muscularization after HYX and HFD. These findings were linked to reduced STAT3 activity, cytoplasmatic sequestration of FoxO1, and ultimately reduced proliferation of lung vascular smooth muscle cells. Assessment of right ventricular (RV) hypertrophy and cardiac function by echocardiography or hemodynamic measurements showed preserved RV function after inhibition of IL-6 trans-signaling in HYX and HFD. Discussion : In two perinatal models, our data demonstrate that IL-6 is central in lung SMC proliferation and causes vascular remodeling with characteristics of PH early in life. Pharmacological inhibition of IL-6 trans-signaling offers a novel therapeutic strategy for early origins of PH.
