Abstract:We read with pleasure the comment by Lee et al. [1] in response to our article titled 'Bioengineered dermal matrix (Integra®) reduces donor site morbidity in total phallic construction with radial artery forearm free-flap' [2]. We share with the authors' the enthusiasm towards the progressive improvement of donor site reconstruction techniques after radial forearm free flap (RAFFF) harvesting and we totally agree in highlighting the need to further researches in this field to improve the donor site-related outcomes. As a matter of fact, the goal would be to minimize donor site morbidity to allow a full functional recovery of the forearm while ensuring an adequate amount of tissue to address total phallic construction (TPC). In the specific context of TPC in transgender men a secondary endpoint would be to minimize the peculiar stigma represented by donor site scar. We totally understand the relevant considerations made by Lee et al. [1] regarding our article. Indeed, although the surgical evaluation of donor site reconstruction was lacking an objective assessment tool, functional outcomes were assessed throughout the use of validated tools such as the Patient and Observer Scar Assessment Scale v2.0 (POSAS), the Vancouver Scar Scale and the Skin Pinch Test [3, 4]. It must be said that, to date, the objective assessment of the quality of a scar still represents a challenge due to the lack of a unique and shared methodology [5]. On this regard, ongoing researches are exploring the use of devices that may help to get a standardized evaluation of donor site morbidity. When available, it will be of primary importance to apply them in the context of multicentric prospective studies to finally understand which type of graft may guarantee better outcomes of donor site reconstruction after RAFFF harvesting. Among the existing devices, the Minolta Chromameter CR-300 (Minolta, Japan) was found to be the most reliable for skin color evaluation whilst the Primos (Canfield Industries, USA) demonstrated excellent intra and interobserver reliability for skin texture evaluation [5]. However, none of the devices stood out for evaluating skin elasticity. The authors concluded that unfortunately none of the devices can be designated as adequate for complete skin evaluation since content and criteria validity, responsiveness, and interpretability were missing. Regarding the use of acellular dermal matrix (ADM), it offers several benefits. ADM is a dermal allograft free of immunogenic components. It can augment or replace damaged dermis, closely resembling normal skin. Its composition includes collagen, elastin (for tensile strength and elasticity), proteoglycans(inducing angiogenesis), laminin (binding with connective tissues), and basement membrane (composed of collagen type IV) [6]. Specifically, Integra® (Integra® Life Sciences, USA) is an ADM composed of a matrix of purified collagen from bovine tendon crosslinked with glycosaminoglycan (GAG) obtained from shark cartilage [7]. Besides providing a scaffold for functional dermal reconstruction, it is worth emphasizing that ADM can lead to remodeling of cutaneous tissue without causing contraction or scarring. Scar tissue is distinguishable from normal skin by its color, texture, thickness, and firmness. This can result in both cosmetic and functional consequences.
