Abstract:To the Editor: By controlling multiple confounding factors, the important article of Qureshi and colleagues implies that a causal link exists between posttraumatic stress disorder (PTSD) and late-life dementia.1,2 This letter reviews the authors' response to it and explores possible cerebral functional and structural evidence to support a relationship between the two diseases. Establishing the relationship between PTSD and dementia heavily relies on cerebral functional abnormalities and cerebral atrophy. It would provide an improvement in clinic decision-making, such as early prevention, better prediction of progress, and prognosis in dementia. However, mainly based on a twin study,3 a recent opinion claimed that no causal relationship seemed to exist between PTSD and dementia.4 Because the opinion and the article of Qureshi and colleagues ignore the PTSD-induced functional abnormalities that are commonly accepted and some debate still exists regarding the structural studies, it is important to separate the two abnormalities and provide some critical opinions and experiments related to the twin study to reveal the underlying link between the two diseases. Much previous research has found robust evidence for PTSD-induced functional abnormalities, including hyperresponsive amygdala, hypoactive medial prefrontal cortex (MPFC), and anterior cingulate cortex (ACC) dysfunction.5,6 Emotional Stroop research on PTSD first highlighted diminished medial prefrontal and anterior cingulate activation, which neuroimaging research on patients with PTSD exposed to trauma-related pictures, sounds, or traumatic imagery scripts further supported. Because abnormalities of these areas can predict the onset and progress of dementia, these studies suggest that a link exists between the two diseases. Poorer functional activity is not always associated with lower volume that is closely related to dementia, so more evidence from structural imaging studies is needed, but some debate still exists on PTSD-induced neurotoxicity, and the limitations of these experiments are obvious. For example, genetic studies require large sample sizes to detect small to medium gene effects, but subject numbers in twin neuroimaging studies are always small.7 Moreover, the present main drawback of voxel-based morphometry analysis is the potential poor sensitivity to detect regional changes. A great number of animal studies strongly support the hypothesis that stress can decrease dendritic branching and neurogenesis in MPFC volume and damage hippocampal formation.8 MPFC and pregenual ACC are highly interconnected with the amygdala, and structural or functional abnormalities in these regions have been reported to markedly impair fear extinction in human. Some researchers emphasize that not only has research in humans found that ACC volumes in PTSD are smaller than in trauma-exposed individuals without PTSD, but has also found that ACC volume correlates with severity of PTSD symptoms.5 One of the most important types of genetic neuroimaging studies in human is twin studies. One twin study revealed that combat veterans with PTSD and their identical twins who did not have PTSD had smaller hippocampus than combat veterans without PTSD, suggesting that a small hippocampus is a risk factor for rather than a result of PTSD.2 Nevertheless, some scientists criticized the twin study and called for a replication of this research. The same group reported inconsistent results in different papers and the reason is unclear. In addition, a small hippocampus was not an extraordinarily strong predictor for the enormous overlap in the volume data. They believed that hippocampus volume and stress could affect each other.9 In contrast to the previous twin study, another twin study emphasized that there was no evidence of regional differences related to genetic risk for PTSD and that most regional changes correlated with PTSD. Meanwhile, it suggested that pregenual ACC is also significantly associated with PTSD and combat exposure.10 In conclusion, correlation, even post hoc, does not equal causation, but a large number of consistent studies indicate that causation is likely to exist between the two diseases. Further genetic neuroimaging studies and pathological research on the frontal amygdala circuit and hippocampus shall contribute to the causation.
