Abstract:Creutzfeldt-Jakob disease is a spongiform encephalopathy with rapidly progressing neurological symptoms. Always is there a fatal outcome, usually within 1 year of symptom onset. Cortical diffusion restriction is an early diagnostic finding in the disease. In a patient with multiple sclerosis, we found cortical diffusion restriction 27 months prior to symptom onset. It is the second report of the existence of such a long preclinical MRI-positive phase in sCJD. This case report might change the perspective on a generally accepted extremely rapid progressive disease and could even imply a window of opportunity for possible future treatment.

Abstract:Objective To understand the incidence,epidemiological and clinical characteristics of Creutzfeldt-Jakob disease(CJD) in China.Methods The analysis was conducted on the clinical and epidemiological data of suspected CJD cases collected through the surveillance in China in 2010.Cerebrospinal fluid(CSF) and blood samples were taken from the patients to detect PrPSc in brain tissue and 14-3-3 protein in CSF by using Western blot assay and analyze the polymorphism of 129 amino acid and mutation of PRNP gene extracted from blood samples by using PCR and sequencing assay.Results Totally 1 laboratory confirmed cases,46 clinical diagnosed and 25 suspected cases of CJD,3 fatal familial insomnia cases(FFI) and 5 genetic CJD cases were detected,including 1 R208H,1 P102L and 3 T188K mutation cases No seasonal,geographic and population clustering of the disease were observed.The mean age of clinical diagnosed patients was 62 years old,the male to female ratio was 1.30∶1,the mean age of suspected patients was 57 years old,the male to female ratio was 2.14∶1.Rapidly progressive dementia was the mostly common symptom,occurring firstly in 54.93% of the CJD patients.More typical clinical manifestations were presented in clinical diagnosed patients than in suspected patients.Conclusion CJD occurred sporadically in China in 2010,which was demonstrated by the time,geographic,population,gender and age distributions of the cases.

Abstract:Eight cases of rare genetic Creutzfeldt-Jakob disease (gCJD) with a mutation T188K in PRNP have been identified and diagnosed genetically in China since 2006. Among the eight cases, the median age of disease onset was 58years old (ranging from 39 to 76years old). Progressive dementia and pyramidal or extrapyramidal dysfunction appeared in all cases and lasted during the entire clinical course. Myoclonus and visual or cerebellar disturbances were also frequently observed. The median duration of disease was 3months. Cerebral MRI findings revealed high caudate and putamen signals in four out of eight cases. CSF in six out of eight patients tested positive for the 14-3-3 protein. Only one case showed periodic sharp-waves (PSW) in EEG. Most cases lacked a family history of associated diseases, though one patient's mother died of a neurologic disorder without a definite diagnosis. Our data reveal that Chinese T188K gCJD cases have clinical characteristics similar to that of sporadic CJD (sCJD). Compared with other inherited prion disease-associated mutations in China, the genetic frequencies of T188K in PRNP of Han-Chinese are relatively high.

Abstract:Genetic human prion diseases include genetic or familial Creutzfeldt-Jakob disease (gCJD or fCJD), fatal familial insomnia (FFI) and Gerstmann-Straussler-Scheinker syndrome (GSS). T188K gCJD was first described in Germany in 2000.1 The first Chinese patient with T188K gCJD was identified in 2009.2 By March 2016, 30 Chinese cases with T188K gCJD were identified and genetically diagnosed via national CJD surveillance programme. T188K gCJD is now the second-most frequent genetic prion disease among Chinese (next to FFI),3 however, is rarely reported in the other countries besides China.All cases with 30 T188K gCJD in this report were Han-Chinese without sibship. The gender ratio (male to female) was 1:0.76. The onset ages of the patients varied from 40 to 85 years old, with the median age of 59 years. More patients (13 cases) were in the group of 50–59 years old, followed by those of 60–69 and 70–79 years old.The interval from onset to reporting to CJD surveillance centre differed from 1 to 9 months (median: 2.5 months). All patients appeared more than one symptom at onset. Progressive dementia was noticed in 2/3 patients (20/30), followed by cerebellum symptoms and walk unstable (12/30), mental (10/30) and extrapyramidal (9/10) symptoms. In the whole clinical courses, progressive dementia was described in 93.3% (28/30) cases. Abstract:Genetic Creutzfeldt-Jakob disease (gCJD) is caused by a range of mutations in the prion protein gene (PRNP) and account for approximately 10-15% of overall human prion diseases worldwide. They are different with disease onset, disease duration, clinical signs and diagnostic findings. Here we reported a 71 year-old female with an E196K mutation in one PRNP allele, while the codon 129 was a methionine homozygous genotype. The patient started with non-specific symptoms, but displayed rapidly progressive disturbances of speech, memory, cognitive and physical movement. No periodic activity was recorded at electroencephalography (EEG) during the entire disease course. Retrospective investigation of her family members did not reveal similar neurological disorders. Total clinical course was about seven months.
Negative Diffusion Weighted Imaging on Magnetic Resonance Imaging of the Brain in Creutzfeldt-Jakob Disease

Abstract:A 76-year-old Caucasian woman initially presented to the Duke Memory Disorders clinic with a 9-month history of a rapid decline in cognitive, motor, and neuropsychiatric function. On initial presentation, the patient required assistance with activities of daily living. On neurological examination, she was found to have Gerstmann's syndrome along with appendicular apraxia. A positional tremor was noted without myoclonus or fasciculations. She had a paucity of speech and was unable to write her own name. Snout and grasp reflexes were present. Episodes of inappropriate laughter were noted during the exam. She was admitted to the inpatient neurology service for further evaluation. The Diffusion Weighted Imaging sequence on Magnetic Resonance Imaging of the brain was negative for restricted diffusion. An electroencephalogram was unremarkable. Cerebrospinal fluid analysis for Real-Time Quaking-Induced Conversion assay was positive along with an elevated 14-3-3 and increased total Tau protein levels. There was no family history of Creutzfeldt-Jakob disease. The cerebral spinal fluid results were consistent with a diagnosis of Creutzfeldt-Jakob disease, despite the negative MRI brain findings.

Abstract:Introduction: A definitive diagnosis of Creutzfeldt-Jakob disease (CJD) can only be made by neuropathologic examination and demonstration of typical pathologic changes and the pathologic prion protein in central nervous tissues. This study investigated the diagnostic sensitivity and specificity of the microtubule-association protein tau in cerebrospinal fluid (CSF) from Chinese patients with sporadic CJD. Methods: Two hundred two CSF samples from clinically suspected patients with sporadic CJD were analyzed for tau protein by enzyme-linked immunosorbent assay and for the signal transduction regulatory protein 14-3-3 protein by immunoblot. Results: Remarkably increased levels of tau protein and increased incidence of 14-3-3 positivity were observed in probable CJD, when compared with possible CJD and others. With a threshold of 1400 pg/mL, tau determination showed a sensitivity of 90% and a specificity of 94% for the diagnosis of probable CJD. The combination of raised tau and positive 14-3-3 increased the specificity but slightly reduced the sensitivity. Statistical analysis indicated that the raised level of tau positively correlated with the presence of 14-3-3 in CSF but not with other main clinical features, eg, age, gender, clinical manifestations and sampling time. Conclusions: These data suggest that Chinese patients with probable CJD have similar increased levels of tau in the CSF as in Caucasian patients. Measurement of CSF tau will be another potential technique for antemortem CJD diagnosis.

The cerebrospinal fluid (CSF) was positive for 14-3-3 protein. Increased bilateral signal intensity in the frontal and parietal lobes was seen on diffusion-weighted imaging (DWI); periodic activity was recorded on an electroencephalogram (EEG). There was no family history of similar symptoms. The total clinical course was approximately two months.

Abstract:OBJECTIVE:To describe the epidemiological and clinical characteristics of Creutzfeldt-Jakob disease (CJD) in China.METHODS:Clinical and epidemical data on patients from China CJD surveillance network was analyzed. Blood and cerebral spinal fluid (CSF) specimens from these patients were collected. Western blot assay was used to detect 14-3-3 protein in CSF, PCR and sequencing assay were used for analyzing the polymorphism of 129 amino acid and mutation of PRNP gene.RESULTS:A total number of 31 probable and 11 possible sporadic CJD patients were identified. Additionally,one patient with Gerstmann-Straussler-Scheinker syndrome (GSS) and 2 familial CJD cases were identified. No geographic- or occupational-related events were observed among these cases. The mean age of onset on the probable or possible CJD patients were 56.7 and 57.4 years old, with sex ratios of the probable CJD patients as 8:9 and the possible one as 5:6 respectively. Rapid progressive dementia was the main foremost symptom, presenting in 33.3% of the CJD patients. Probable CJD patients showed more clinical manifestations than those possible ones.CONCLUSION:Geography distribution, occupation, ratio of gender and the mean onset age of the CJD cases in 2008 were consistent with the characteristics of the sporadic CJD.

Abstract:A case of Creutzfeldt-Jakob disease (CJD) with a rare mutation of the prion protein (PrP) gene (PRNP) at codon 208 (R208H), while the codon 129 was a methionine homozygous genotype is reported. The patient initial displayed hand tremor, dizziness and progressive cognitive dysfunction. Subsequently, other symptoms gradually appeared, including cerebellar ataxia and mental disorder. No periodic activity was recorded at electroencephalography (EEG) and 14-3-3 protein in cerebrospinal fluid was negative. Total clinical course was about 4 months. Retrospective investigation of this family across seven generations did not figure out clear family history. However, genetic analyses revealed six first-degree family members with the R208H allele.

Abstract:Objective Two different mutations at codon 196, namely E196A and E196K, have been reported to be related to genetic Creutzfeldt-Jakob disease (CJD). We aimed to comparatively analyse the features of Chinese patients with these two mutations from the CJD surveillance system in China. Design and setting Comparative analysis of patients identified via the Chinese National CJD Surveillance System during the period 2006-2020. Participants 16 Chinese patients with genetic CJD with E196A mutation and 5 with E196K mutation. Methods Neurological examination, EEG and MRI, western blot, gene sequence, and RT-QuIC. Results The age of onset of E196K genetic CJD cases (median of 61 years) was older than the E196A cases (median of 67 years). Generally, these two subtypes of genetic CJD were more like sporadic Creutzfeldt-Jakob disease (sCJD) clinically. The E196A cases showed more major symptoms, while those of E196K cases were restricted to dementia and mental problems. During progression, more sCJD-associated symptoms and signs gradually appeared, but none of the E196K cases showed cerebellum and visual disturbances. Typical periodic sharp wave complexes on MRI were recorded in 25% of E196A cases but not in E196K cases. sCJD-associated abnormalities on MRI, positive cerebrospinal fluid (CSF) 14-3-3 and increased CSF total tau were observed frequently, ranging from two out of three cases to four out of five cases, without a difference. Positive CSF RT-QuIC was detected in 37.5% (6 of 16) of E196A cases and 60% (3 of 5) of E196K cases. The duration of survival of E196K cases (median of 4.5 months) was shorter than the E196A cases (median of 6.5 months). Moreover, female cases and cases with young age of onset (<60 years) in E196A displayed longer survival time than male patients and cases with older age of onset (>= 60 years). Conclusions This is the largest comprehensive report of genetic CJD with mutations at codon 196 to date, describing the similarity and diversity in clinical and laboratory tests between patients with E196A and with E196K mutations.

Abstract:Objective To investigate epidemiological, clinical and genetic features of the first Chinese case of Creutzfeldt-Jakob disease (CID) with mutation of E200K in PRNP. Methods The general epidemiological and clinical data were collected; CSF 14-3-3 protein was analyzed by Western blot; The PRNP was amplified by PCR and analyzed. Results A missense mutation in codon 200 (E200K) of the PRNP was identified in this patient; CSF 14-3-3 protein was positive; sleep disturbance was the initial sign and the other symptoms gradually appeared, including memory loss, dizziness and ataxia. Conclusion The CJD patient who was first reported in China has a missense mutation in codon 200 (E200K) of the PRNP, and the codon 129 is a methionine homozygous genotype.

Abstract:Patients with Creutzfeldt‐Jakob disease (CJD) show less atrophy in the primary cortex than patients with Alzheimer's disease (AD). Patients with CJD show more severe functional disruption than patients with AD. Aims Creutzfeldt‐Jakob disease (CJD) is a lethal neurodegenerative disorder, which leads to a rapidly progressive dementia. This study aimed to examine the cortical alterations in CJD, changes in these brain characteristics over time, and the differences between CJD and Alzheimer's disease (AD) that show similar clinical manifestations. Methods To obtain reliable, subject‐specific functional measures, we acquired 24 min of resting‐state fMRI data from each subject. We applied an individual‐based approach to characterize the functional brain organization of 10 patients with CJD, 8 matched patients with AD, and 8 normal controls. We measured cortical atrophy as well as disruption in resting‐state functional connectivity (rsFC) and then investigated longitudinal brain changes in a subset of CJD patients. Results CJD was associated with widespread cortical thinning and weakened rsFC. Compared with AD, CJD showed distinct atrophy patterns and greater disruptions in rsFC. Moreover, the longitudinal data demonstrated that the progressive cortical thinning and disruption in rsFC mainly affected the association rather than the primary cortex in CJD. Conclusions CJD shows unique anatomical and functional disruptions in the cerebral cortex, distinct from AD. Rapid progression of CJD affects both the cortical thickness and rsFC in the association cortex.

Abstract:Human genetic Creutzfeldt-Jakob disease (gCJD; one of the prion diseases) is caused by point mutations and insertions in the prion protein gene (PRNP). Previously we have reported a Chinese gCJD case with a substitution of valine (V) for glycine (G) at codon 114. To investigate the detailed pathogenic and pathologic characteristics of G114V gCJD, 10 different brain regions were thoroughly analyzed. PrP-specific Western blots and immunohistochemical (IHC) assays identified larger amounts of PrP(Sc) in the regions of brain cortex. Assays of the transcriptions of PrP-specific mRNA by RT-PCR and real-time PCR showed comparable levels in 10 brain regions. In line with the distribution of PrP(Sc) , typical vacuolations in brains, markedly in four cortex regions, were detected. Contrast to the distributing features of spongiform and of PrP(Sc) , massive gliosis was detected in all brain regions by GFAP-specific IHC tests. Moreover, two-dimensional gel immunoblots found three major sets of PrP(Sc) spots, indicating that PrP(Sc) in brain tissues was a mixture of molecules with different biochemical properties. The data here provide the pathogenic and neuropathological features of G114V gCJD.

Abstract:OBJECTIVE:To describe the incidence condition and characteristic of Creutzfeldt-Jakob disease (CJD) in China.METHODS:The clinical and epidemical information of patients from China CJD surveillance network was analyzed. Blood and cerebrospinal fluid (CSF) specimens from these patients were differently collected to be used to detect the 14-3-3 protein in the CSF and to analyze the PRNP gene.RESULTS:Ten possible and 8 probable clinically diagnosed CJD patients were found. These patients had sporadic CJD. There were no geographic clustering and occupational risk with these patients. The mean age at onset of disease was approximately 60 years. The male to female ratio was approximately 1:1; rapidly progressive dementia was the main early symptom, which was present in 44% of patients.CONCLUSION:The geographic distribution, occupation, the ratio of male to female and the mean age of onset were consistent with the characteristics of sporadic CJD.

Abstract:Objective To describe epidemiological and clinical characteristics of Creutzfeldt-Jakob disease(CJD)patients in China.Methods The clinical and epidemical information of patients from China CJD surveillance network was analyzed.Blood and cerebral spinal fluid(CSF) specimens from these patients were collected.Western blot assay was used for detecting PrPSc in brain tissue and 14-3-3 protein in CSF,PCR and sequencing assay were used for was used for detecting PrPSc in brain tissue and 14-3-3 protein in CSF,PCR and sequencing assay were used for analyzing the polymorphism of 129 amino acid and mutation of PRNP gene.Results Totally 1 definite CJD patient,31 probable and 31 possible sporadic CJD patients were identified.Additionally,3 FFI cases and 3 genetic CJD cases including G114V,E196K,T188K were definitely diagnosed.No period-,geographic-or occupational-related events were observed among these cases.The mean onset age of probably and possibly diagnosed CJD patients were 60 and 52 years old,respectively.The male to female ratio of the probable CJD patients was 12∶ 19 while that of the possible CJD patients was 14∶ 17.Rapidly progressive dementia was the main foremost symptom,presenting in 48.4% of the CJD patients.Probable CJD patients showed more clinical manifestations than that of the possible one.Conclusion The period and geography distribution,occupation,the ratio of gender and the mean onset age of the CJD cases in 2009 were consistent with the general characteristics of sporadic CJD in the world.

Abstract:Objectives Currently, no established biomarkers exist for presymptomatic sporadic Creutzfeldt-Jakob disease (sCJD). The purpose of this study was to raise awareness about sCJD cases showing abnormalities on brain MRI diffusion-weighted imaging (DWI) before symptom onset and demonstrate temporal changes in DWI abnormalities during the preclinical period. Methods We described the clinical presentation including the results of MRI—performed multiple times in the preclinical period—and the diagnostic workup of a middle-aged man with sCJD. Results MRI of the brain performed 27 months before symptom onset revealed an extremely localized lesion on DWI in the right occipital cortex. Follow-up MRI scans showed propagation of DWI abnormalities along the cortices without the appearance of neurologic symptoms/signs. After symptom onset, the patient9s neuropsychiatric condition rapidly deteriorated. Elevated total tau protein levels and positive 14-3-3 protein were observed in the CSF, and periodic synchronous discharges using electroencephalography resulted in the diagnosis of sCJD. Discussion CJD should be considered in differential diagnoses when localized DWI signal abnormalities propagate along the cortices over time, even in the absence of typical CJD symptoms. DWI signal abnormalities on brain MRI scans may be highly sensitive diagnostic markers for CJD, even in the preclinical stage.
